Abstract
This chapter provides an overview of the possible mechanisms of failure for heat treated steel components and discusses the techniques for examining fractures, ductile and brittle failures, intergranular failure mechanisms, and fatigue. It begins with a description of the general sources of component failure. This is followed by a section on the stages of a failure analysis, which can proceed one after the other or occur at the same time. These stages of analysis are collection of background data, preliminary visual examination, nondestructive testing, selection and preservation of specimens, mechanical testing, macroexamination, microexamination, metallographic examination, determination of the fracture mechanism, chemical analysis, exemplar testing, and analysis and writing the report. The chapter ends with a discussion on various processes involved in the determination of the fracture mechanism.

This chapter discusses the failure of an aircraft control cable and the investigation that followed. It explains how visual examination showed that the failure occurred in a heavily worn section that runs over a pulley. Further examination under a stereoscope and in a SEM found that 40 out of 49 wires had thinned and broke apart, and that the remaining nine wires failed in tension evidenced by cup and cone fracture and the presence of dimples on the fracture surface. Based on their findings, investigators concluded that cable snapped because of excessive thinning due to contact wear.

Abstract
This chapter describes an investigation following an aircraft accident in which the main undercarriage struts had failed. Visual examination revealed that the starboard strut fractured about 13 cm from the end nearest the underside of the wing. A close-up view of the fracture surface indicated that cracking initiated at the outer periphery of the strut and propagated inward until overload fracture occurred. SEM imaging revealed fatigue striations along the outer periphery and dimples elsewhere, indicative of tensile overload. Based on these observations, investigators concluded that the starboard strut failed by fatigue, which overloaded the port side strut as evidenced by its slant type fracture pattern.

This chapter describes an investigation that was conducted to determine why quill shafts were failing prematurely in gear boxes on aircraft engines. The investigation focused on the splines in a splined bore. Visual examination showed that the splines were heavily worn and covered with red powder on one end. Investigators also observed blueing, an effect of overheating. Based on these observations and the results of SEM imaging, it was concluded that the splines in the spline bore were wearing out for lack of lubrication.

The aluminum alloy skin on the main rotor blade of a helicopter tore off in flight, and an investigation was subsequently conducted to find the cause. Visual examination and SEM fractography revealed that a fatigue crack originated on the underside of a rivet hole at the trailing edge of the blade. The crack then propagated through the outer skin toward the leading edge of the blade. Once the fatigue crack reached critical length, the sheet metal fractured catastrophically, tearing away from the blade.

Abstract
An aircraft went down over water some 30 minutes into a flight. The wreckage was retrieved and the elevator linkage components were dismantled, cleaned, and reassembled. As the chapter explains, both the port and starboard hinge pins had fractured at a tack welded joint along a flange. Based on visual examination, SEM fractography, and chemical analysis, investigators concluded that the hinge pins were not made from the specified steel and were not properly treated after cadmium plating. The pins failed due to hydrogen embrittlement, which may have been aggravated by welding. The chapter provides several recommendations to avoid such failures in the future.

Abstract
Two compressor rotors of similar design and construction were severely damaged during operation. In one rotor, all the blades in the third and fourth stages had been sheared off and some had lifted from the dovetail portion of the drum. The damage in the other rotor was more extensive. Most of the blades in the first four stages had sheared off and many lifted from the dovetail region, particularly in the first two stages where several mounting dovetails had also fractured. Based on visual examination and the results of SEM fractography, metallography, and chemical analysis, investigators concluded that the compressor rotors failed due to stress-corrosion cracking in the dovetail mountings. They also provided recommendations to prevent or mitigate future occurrences.

A test flight was cut short after a fire warning came on indicating a problem with one of the four engines on an aircraft. A visual examination following the precautionary landing revealed several burned hoses, a melted bolt, and fuel leaking from the base of the main burner. The fuel nozzle was also damaged, and based on its microstructure, came very close to melting. Investigators determined that the burner was mounted backwards, facing the compressor rather than the turbine. They also recommended a redesign to prevent the fuel nozzle from being reversed.

A low-pressure turbine rotor blade failed during a test run, causing extensive damage to an aircraft engine. Visual examination showed that the nickel-base superalloy blade broke above the root platform in the airfoil section, leaving a fracture surface with two distinct regions, one characteristic of fatigue, the other, overload. Two dents were also visible on the leading edge, near the origin of the fracture. Based on these observations and the results of SEM fractography, investigators concluded that the blade failed due to fatigue aided by cracks in the surface coating caused by mechanical damage.

Abstract
This article reviews various analytical techniques most commonly used in plastic component failure analysis. The description of the techniques is intended to make the reader familiar with the general principles and benefits of the methodologies. The descriptions of the analytical techniques are supplemented by a series of case studies that include pertinent visual examination results and the corresponding images that aided in the characterization of the failures. The techniques covered include Fourier transform infrared spectroscopy, differential scanning calorimetry, thermogravimetric analysis, thermomechanical analysis, and dynamic mechanical analysis. The article also discusses various analytical methods used to characterize the molecular weight distribution of a polymeric material. It provides information on a wide range of mechanical tests that are available to evaluate plastics and polymers, covering the various considerations in the selection and use of test methods.

Abstract
This chapter explains how investigators determined that a stabilizer link rod fractured due to overload, possibly by a combination of tension and bending forces that occurred during an accident.
