This appendix focuses on procedures, techniques, and precautions associated with the investigation and analysis of metallurgical failures that occur in service. It describes the steps of an orderly failure analysis from collecting and examining samples to performing mechanical and nondestructive tests, preparing and examining fractographs and micrographs, determining failure mode, writing the report, and developing follow-up recommendations. It also examines the fundamental mechanisms of failure, why they occur, and how to identify them by their characteristic features.

Abstract
A systematic procedure for minimizing risks involved in heat treated steel components requires a combination of metallurgical failure analysis and fitness for service with respect to safety and reliability based on risk analysis. This chapter begins with an overview of heat treat processing of steels. This is followed by sections on various aspects of heat treatment design and heat treating practices for minimizing distortion. Influence of design, steel grade, and condition is then illustrated in the examples of failures due to heat treatment. A procedure is analyzed to improve the performance of the design process of a component. A heat-transfer model, coupling with a phase transformation model, a thermomechanical model, and a thermochemical model, is also considered. The chapter further provides information on the failure aspects of and heat treatment procedures applied to welded components. It ends with a section on risk-based approach applicable to heat treated steel components.

This chapter summarizes the various kinds of gear wear and failure and how gear life in service is estimated and discusses the kinds of flaws in material that may lead to premature gear fatigue failure. The topics covered are alignment, gear tooth, surface durability and breakage of gear tooth, life determined by contact stress and bending stress, analysis of gear tooth failure by breakage after pitting, and metallurgical flaws that reduce the life of gears. The chapter briefly reviews some components in the design and structure of each gear and/or gear train that must be considered in conjunction with the teeth to enhance fatigue life.

Abstract
Many companies conduct only metallurgical evaluations in the wake of failures, discovering nothing more than the physical mechanism by which the failure occurred. The origin of failures, however, is often complex, involving not only physical mechanisms, but also human behavior and latent factors. Failures may also involve multiple parts, entire machines, or processes of any size and shape. The chapter examines the unique aspects of many failures and explains how they can sometimes be traced to systemic issues. It also covers the reasons why products fail, including improper service or operation, improper maintenance, improper testing, assembly errors, fabrication or manufacturing errors, and design errors. The case of the Tacoma Narrows Bridge collapse is presented to illustrate the consequence of overlooked factors, in this case, wind dynamics, and the importance of identifying root causes to prevent repeat failures.

Abstract
Elevated-temperature failures are the most complex type of failure because all of the modes of failures can occur at elevated temperatures (with the obvious exception of low-temperature brittle fracture). Elevated-temperature problems are real concerns in industrial applications. The principal types of elevated-temperature failure mechanisms discussed in this chapter are creep, stress rupture, overheating failure, elevated-temperature fatigue, thermal fatigue, metallurgical instabilities, and environmentally induced failure. The causes, features, and effects of these failures are discussed. The cooling techniques for preventing elevated-temperature failures are also covered.

This report describes the failure of a gas turbine in a combined-cycle power plant and the examination and tests that were conducted to determine the cause. Based on microstructural analysis, hardness measurements, and tensile tests, the failure was attributed to inadequate clearances in the seal land region between two stages in the compressor section of the rotor. The report also recommends changes to remediate the problem.

Abstract
Two filtration components installed on a developmental aircraft cracked during pressure impulse testing. Both parts were made from an aluminum alloy, solutionized and aged, and cracked due to fatigue. In both cases, the crack initiated at a transition region on an inner surface and progressed circumferentially outward. Based on these observations and the results of SEM fractography and microstructural analysis, the fatigue cracking can be traced to insufficient fillet radius at the transition zone.
