This article provides information on the application of nondestructive examination (NDE) technologies to tube and pipe products. These include modeling and simulation methods, eddy-current methods, magnetic methods, acoustic methods, and physical methods. A summary of nondestructive examination methods based on flaw type and product stage is presented in a table. The article also discusses in-service inspection of tubular products and presents an example that illustrates the importance of nondestructive testing (NDT) for welds in austenitic stainless steel tubing.

This article describes some of the welding discontinuities and flaws characterized by nondestructive examinations. It focuses on nondestructive inspection methods used in the welding industry. The sources of weld discontinuities and defects as they relate to service failures or rejection in new construction inspection are also discussed. The article discusses the types of base metal cracks and metallurgical weld cracking. The article discusses the processes involved in the analysis of in-service weld failures. It briefly reviews the general types of process-related discontinuities of arc welds. Mechanical and environmental failure origins related to other types of welding processes are also described. The article explains the cause and effects of process-related discontinuities including weld porosity, inclusions, incomplete fusion, and incomplete penetration. Different fitness-for-service assessment methodologies for calculating allowable or critical flaw sizes are also discussed.

This article focuses on the analysis of materials and mechanical- (or biomechanical-) based medical device failures. It reviews the failure analysis practices, including evidence receipt, cleaning, nondestructive examination, destructive examination, exemplars analysis, and device redesign. The article examines the common failure modes, such as overload, fatigue, corrosion, hydrogen embrittlement, and fretting, of medical devices. The failure analysis of orthopedic implants, such as permanent prostheses and internal fixation devices, is described. The article reviews the failure mechanisms in some of the more common medical device materials, namely, stainless steels, titanium alloys, cobalt-base alloys, and nitinol. It presents case histories with examples for failure analysis.

Nonlinear ultrasonic nondestructive examination (NDE) techniques are based on nonlinear interaction of ultrasonic waves with the material to be characterized and defects to be detected. This article introduces the basic principles of nonlinear material-wave interaction, the origin of intrinsic nonlinearity in intact solids, and the main mechanisms of excess nonlinearity in damaged metals. It describes the measurement methods for nonlinear ultrasonic materials characterization and flaw-detection. The article schematically illustrates the instrumentation used for measurements of longitudinal wave and Rayleigh surface acoustic waves. It concludes with information on the applications of nonlinear ultrasonics.

It explains the basic concept of flaw detection and evaluation and probability of detection. The article provides an overview of NDE methods with their applications, limitations, and advantages. Nondestructive testing (NDT), also known as nondestructive evaluation (NDE), includes various techniques to characterize materials without damage. This article focuses on the typical NDE techniques that may be considered when conducting a failure investigation. The article begins with discussion about the concept of the probability of detection (POD), on which the statistical reliability of crack detection is based. The coverage includes the various methods of surface inspection, including visual-examination tools, scanning technology in dimensional metrology, and the common methods of detecting surface discontinuities by magnetic-particle inspection, liquid penetrant inspection, and eddy-current testing. The major NDE methods for internal (volumetric) inspection in failure analysis also are described.

Additive manufacturing (AM) is the process of joining materials to make parts from three-dimensional (3D) model data, usually layer upon layer, as opposed to subtractive manufacturing and formative manufacturing methodologies. This article discusses various defects in AM components, such as porosity, inclusions, cracking, and residual stress, that can be avoided by using vendor recommended process parameters and approved materials. It describes the development of process-structure-property-performance modeling. The article explains the practical considerations in nondestructive evaluation for additively manufactured metallic parts. It also examines nondestructive testing (NDT) inspection and characterization methods for each of the manufacturing stages in their natural order. The article provides information on various inspection techniques for completed AM manufactured parts. The various electromagnetic and eddy current techniques that can be used to detect changes to nearsurface geometric anomalies or other defects are also discussed. These include ultrasonic techniques, radiographic techniques, and neutron imaging.

Both nondestructive testing (NDT) and nondestructive evaluation (NDE) use noninvasive measurement techniques to gain information about defects and various properties of materials, components, and structures. This article begins with a discussion on the historical development of quantitative measurement techniques, evaluation reliability, and quantitative interpretation of nondestructive inspection methods. The common nondestructive evaluation methods, along with their uses and limitations, are summarized in a table. The article conceptually illustrates the interplay of NDE and fracture mechanics in the damage tolerant approach. It concludes with information on pressure vessel applications that can be separated into three protocols used by military nuclear power, commercial nuclear power, and non-nuclear pressure vessels and/or fired boilers.

Pitting is a form of localized corrosion that is often a concern in applications involving passivating metals and alloys in aggressive environments. This article describes the test methods for pitting corrosion. These methods include ASTM G 48, ASTM F 746, ASTM G 61, ASTM G 100, and electrochemical noise measurements. The visual examination, metallographic examination, and nondestructive inspection of pits are discussed. Abstract
Visual inspection (VI) is the oldest inspection technique man has used as a quality-control tool to evaluate products, assess their final form in terms of fabrication accuracy and external features based on experience, and decide on their acceptance or rejection. This article discusses the basic principles of visual inspection in terms of direct visual examination and indirect visual examination as well as advantages and limitations of visual inspection. It reviews the factors affecting the effectiveness of VI as a nondestructive testing (NDT): lighting conditions of observation, condition of surface under inspection, physical state/condition of inspector, proper training of personnel and level of expertise, and knowledge of applicable standards. The article provides schematic illustrations of rigid borescopes, fiberscopes, and videoscopes. It concludes with a discussion on automated optical inspection systems.

This article focuses on the general methods and approaches from the perspective of a reconstruction analyst and includes discussions relevant to materials failure analysts at the incident scene. The elements of accident reconstruction are described. These have conceptual similarity with the principles for failure analysis of material incidents that are less complex than a large-scale accident. The article provides a brief review of some general concepts on the use of modeling which can be a very powerful tool for information pertaining to the reconstruction of an accident where the model can be a physical, mathematical, or logical representation of a physical system or process.

Failure analysis is generally defined as the investigation and analysis of parts or structures that have failed or appeared to have failed to perform their intended duty. Methods of field inspection and initial examination are also critical factors for both reconstruction analysts and materials failure analysts. This article focuses on the general methods and approaches from the perspective of a reconstruction analyst. It describes the elements of accident reconstruction, which have conceptual similarity with the principles for failure analysis of material incidents that are less complex than a large-scale accident. The approach presented is that the analysis and reconstruction is based on the physical evidence. The article provides a brief review of some general concepts on the use and limitations of advanced data acquisition tools and computer modeling. Legal implications of destructive testing are discussed in detail.

Abstract
Failure analysis is the process used to determine the cause of a failure. There is no definitive method for performing a failure analysis, and the method chosen is dependent upon the type of failure, the availability of background information, the tools available to perform the analysis, and the skills of the analyst. The information outlined in this article focuses on the general methodology while allowing for case-specific techniques to be utilized along the way. It covers the causes of failure, why a failure analysis is performed, the failure analysis process, the planning of failure analysis investigation, recommendations to prevent the need for a failure analysis, the implementation of product reviews, and forensic standards.
