Abstract
The principal task of a failure analyst during a physical-cause investigation is to identify the sequence of events involved in the failure. Technical skills and tools are required for such identification, but the analyst also needs a mental organizational framework that helps evaluate the significance of observations. This article discusses the processes involved in the characterization and identification of damage and damage mechanisms. It describes the relationships between damage causes, mechanisms, and modes with examples. In addition, some of the more prevalent and encompassing characterization approaches and categorization methods of damage mechanism are also covered.

Abstract
This article is a pictorial guide to forms of corrosion that draws attention to common pitfalls or situations that have caused premature corrosion, sometimes with expensive consequences. The examples used are not exhaustive; they highlight the necessity to fully examine materials, conditions, and specific circumstances that together can reduce the anticipated service life of a component or plant. The color images in this article are categorized according to the type of corrosion following the general order that is adopted in Volume 13A of ASM Handbook. The first table of the article provides a categorization of the forms of corrosion. It also provides a reference to articles or sections of articles in Volume 13A that detail the particular corrosion form or mechanism. The second table is a guide listing the figures in this article by material and by the corrosion form or mechanism illustrated.

Examination of a damaged component involves a chain of activities that, first and foremost, requires good observation and documentation. Following receipt and documentation, the features of damage can be recorded and their cause(s) investigated, as this article briefly describes, for typical types of damage experienced for metallic components. This article discusses the processes involved in visual or macroscopic examination of damaged material; the interpretation of fracture features, corrosion, and wear damage features; and the analysis of base material composition. It covers the processes involved in the selection of metallurgical samples, the preparation and examination of metallographic specimens in failure analysis, and the analysis and interpretation of microstructures. Examination and evaluation of polymers and ceramic materials in failure analysis are also briefly discussed.

Hydrogen damage is a term used to designate a number of processes in metals by which the load-carrying capacity of the metal is reduced due to the presence of hydrogen. This article introduces the general forms of hydrogen damage and provides an overview of the different types of hydrogen damage in all the major commercial alloy systems. It covers the broader topic of hydrogen damage, which can be quite complex and technical in nature. The article focuses on failure analysis where hydrogen embrittlement of a steel component is suspected. It provides practical advice for the failure analysis practitioner or for someone who is contemplating procurement of a cost-effective failure analysis of commodity-grade components suspected of hydrogen embrittlement. Some prevention strategies for design and manufacturing problem-induced hydrogen embrittlement are also provided.

Abstract
This article discusses the concepts of quality control (QC) and quality assurance (QA), and clarifies the differences and similarities in the roles and responsibilities of QC and QA personnel. It describes the inspection procedures used to verify proper surface preparation and installation of the protective coating/lining system. Prior to beginning surface-preparation operations, many specifications will require a presurface-preparation inspection to verify the correction of fabrication defects and removal of surface contamination such as grease, oil, cutting compounds, lubricants, and chemical contaminants. When inspecting concrete prior to coating installation, three areas of concern exist: surface roughness, moisture content in concrete, and acidity/alkalinity of the surface. The article provides information on the industry standards for assessing surface cleanliness. It details postcoating application quality requirements, including measuring of dry-film thickness, assessing intercoat cleanliness, verifying minimum and maximum recoat intervals, performing holiday/pinhole detection, conducting cure/hardness testing, and assessing adhesion of the applied coating system.

Polymeric floor coatings refer to flooring materials composed of multicomponent thermoset resins formulated with various fillers and pigments that are installed in situ, usually over concrete substrates. Polymeric flooring systems, specified for all industrial and commercial environments, use a variety of polymer chemistries and are constructed in a variety of methods and designs. This article provides a description of the service conditions for the polymeric flooring systems. It provides information on polymeric flooring systems, including thin-film coatings, self-leveling systems, membrane systems, broadcast systems, troweled systems, and terrazzo. The article also focuses on properties, applications, testing, and factors and requirements to be considered during the installation of polymeric floor coatings. It concludes with a discussion about coating failures, including bonding, cracking, chemical attack, and moisture that affect the polymeric floor coatings on concrete.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

Abstract
This article describes the features, benefits and limitations of petrolatum and microcrystalline wax. It provides a detailed discussion on the steps to be followed before applying the various forms of the wax-based coatings. The wax-based coating forms include petrolatum and microcrystalline tapes, marine petrolatum-based pile systems; cold-applied petrolatum-based paste coating systems; hot-applied microcrystalline wax flood coating systems; wax-based dips, brushons, and sprays; and wax-impregnated fabrics and wax-coated papers. The article also discusses the applications and limitations of these wax-based coatings. It concludes by highlighting the steps involved in the installation of wax-based casing fillers.

Quenchant agitation can be obtained by circulating quenchant in a quench tank through pumps and impellers. The selection of the agitation method depends on the tank design, type and volume of the quenchant, part design, and the severity of quench required. The goal of using nondestructive evaluation (NDE) in conjunction with failure analysis is to obtain the most comprehensive set of data in order to characterize the details of the damage and determine the factors that allowed the damage to occur. The NDE results can be used to determine optimal areas upon which to focus for sectioning and metallography in order to further investigate the condition of the component. This article provides information on the inspection method available for failure analysis, including standard methods such as visual testing, penetrant testing, and magnetic particle testing. It covers the effects of various factors on the properties of the part that may impact failure analysis, describes the characterization of damage modes and crack sizes, and finally discusses the processes involved in application of NDE results to failure analysis.

Fatigue failures may occur in components subjected to fluctuating (time-dependent) loading as a result of progressive localized permanent damage described by the stages of crack initiation, cyclic crack propagation, and subsequent final fracture after a given number of load fluctuations. This article begins with an overview of fatigue properties and design life. This is followed by a description of the two approaches to fatigue, namely infinite-life criterion and finite-life criterion, along with information on damage tolerance criterion. The article then discusses the characteristics of fatigue fractures followed by a discussion on the effects of loading and stress distribution, and material condition on the microstructure of the material. In addition, general prevention and characteristics of corrosion fatigue, contact fatigue, and thermal fatigue are also presented.

Tensile testing of fiber-reinforced composite materials is performed to determine uniaxial tensile strength, Young’s modulus, and Poisson’s ratio relative to principal material directions, and helps in the prediction of the properties of laminates. Beginning with an overview of the fundamentals of tensile testing of fiber-reinforced composites, this article describes environmental exposures that can occur during specimen preparation and testing. These include exposures during specimen preparation, and planned exposure such as moisture, damage (impact), and thermal cycling. The article also discusses the test methods of the four major types of mechanical testing of polymer-matrix composites: tensile, compression, flexural, and shear.

Erosion of a solid surface can be brought about by liquid droplet impingement (LDI), which is defined as "progressive loss of original material from a solid surface due to continued exposure to erosion by liquid droplets." In this article, the emphasis is placed on the damage mechanism of LDI erosion under the influence of a liquid film and surface roughness and on the prediction of LDI erosion. This article presents information regarding the use of protective coatings in municipal potable water systems, including raw water collection and transmission, water treatment plants, and treated water distribution. It provides useful guidance for the selection and use of protective coatings in these municipal water systems. The most commonplace corrosion-damage mechanisms are highlighted. The article describes the most common materials of construction found in municipal water systems, namely, cast iron, ductile iron, carbon steel, precast concrete cylinder pipe and reinforced concrete pipe, prestressed concrete tanks, and stainless steel. It provides information on the most common generic coating systems used for new steel tanks and water storage tanks. It concludes with a discussion of quality watch-outs when selecting or using protective coatings in municipal water systems.

This article provides information on the municipal wastewater system components such as piping, pump stations, headworks, clarifiers, aeration structures, digesters, biosolids dewatering equipment, and sludge stabilization. It explains the major corrosion damage mechanisms to which those component parts of the system are exposed. It presents useful guidelines for selecting and using protective coatings in municipal sewerage collection systems and water reclamation facilities in wastewater treatment plants. The article includes annotated flow diagrams of a wastewater collection system, wastewater treatment plants, and spreadsheets listing the most widely used generic coating systems by structure and substrate material. It concludes with a section on quality watchouts when selecting or using protective coatings in municipal wastewater systems.

Friction and wear are important when considering the operation and efficiency of components and mechanical systems. Among the different types and mechanisms of wear, adhesive wear is very serious. Adhesion results in a high coefficient of friction as well as in serious damage to the contacting surfaces. In extreme cases, it may lead to complete prevention of sliding; as such, adhesive wear represents one of the fundamental causes of failure for most metal sliding contacts, accounting for approximately 70% of typical component failures. This article discusses the mechanism and failure modes of adhesive wear including scoring, scuffing, seizure, and galling, and describes the processes involved in classic laboratory-type and standardized tests for the evaluation of adhesive wear. It includes information on standardized galling tests, twist compression, slider-on-flat-surface, load-scanning, and scratch tests. After a discussion on gear scuffing, information on the material-dependent adhesive wear and factors preventing adhesive wear is provided.

Nondestructive testing (NDT), also known as nondestructive evaluation (NDE), includes various techniques to characterize materials without damage. This article focuses on the typical NDE techniques that may be considered when conducting a failure investigation. The article begins with discussion about the concept of the probability of detection (POD), on which the statistical reliability of crack detection is based. The coverage includes the various methods of surface inspection, including visual-examination tools, scanning technology in dimensional metrology, and the common methods of detecting surface discontinuities by magnetic-particle inspection, liquid penetrant inspection, and eddy-current testing.
