Abstract
This article reviews the most common reasons for failures and the purpose of a failure investigation. It discusses the nine steps for the organization of a good failure investigation. The three basic tools that are helpful in any failure investigation, namely, a fault tree, a failure mode assessment chart, and a technical plan for resolution chart, are reviewed. The article briefly describes failure investigation pitfalls and concludes with information on the other common tools used for failure investigation and root cause determination.

Risk and hazard analysis can be effectively used during design reviews to provide valuable feedback to the design to avoid failures. This article discusses the types of risks, namely, real risk, statistical risk, predicted risk, and perceived risk. It describes the principle and technical methods of risk/hazard analysis practiced in the industry to identify possible hazards and the resources necessary to avoid or reduce risks. Abstract
This article discusses the organization required at the outset of a failure investigation and provides a methodology with some organizational tools. It focuses on the use of problem-solving tools such as a fault tree analysis combined with critical thinking. The discussion covers nine steps to organize a good failure investigation. They are as follows: understand and negotiate goals of the investigation, obtain a clear understanding of the failure, identify all possible root causes, objectively evaluate the likelihood of each root cause, converge on the most likely root cause(s), objectively and clearly identify all possible corrective actions, objectively evaluate each corrective action, select optimal corrective action(s), and evaluate effectiveness of selected corrective action(s). Common problems detrimental to a failure investigation are also covered.

Abstract
Reliability-centered maintenance (RCM) is a systematic methodology for preventing failures. This article begins by discussing the history of RCM and uses Society of Automotive Engineers (SAE) all-industry standard JA1011 as its model to describe the key characteristics of an RCM process. It then expands on questions involved in RCM process, offering definitions when necessary. Next, the article describes the approach of RCM to failure modes and effects analysis (FMEA), the failure management policies available under RCM, and the criteria of RCM for deciding when a specific failure management policy is technically feasible. Then, after discussing the ways that RCM classifies failure effects in terms of consequences, it describes how RCM uses failure consequences to identify the best failure management policy for each failure mode. Next, the building blocks of RCM are put together to create a failure management program. The article ends with a discussion on some practical issues pertaining to RCM that lie outside the scope of SAE JA1011.

Abstract

This article begins with a review of the purposes of mechanical characterization tests and the general considerations related to the mechanical properties of anisotropic systems, specimen fabrication, equipment and fixturing, environmental conditioning, and analysis of test results. It provides information on the specimen preparation, instrumentation, and procedures for various mechanical test methods of fiber-reinforced composites. These include the compression test, flexure test, shear test, open hole tension test, and compression after impact test. The article describes three distinct fracture modes, namely, crack opening mode, shearing mode, and tearing mode. It presents an overview of fatigue testing and fatigue damage mechanisms of composite materials and reviews the types of mechanical measurements that can be made during the course of testing to assess fatigue damage. This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. Abstract
This article describes design factors for products used in engineering applications. The article groups these factors into three categories: functional requirements, analysis of total life cycle, and other major factors. These categories intersect and overlap, constituting a major challenge in engineering design. Performance specifications, risk and hazard analysis, design process, design for manufacture and assembly, design for quality, reliability in design, and redesign are considered for functional requirements. Life-cycle analysis considers raw-material extraction from the earth and product manufacture, use, recycling (including design for recycling), and disposal. The other major factors considered include evaluation of the current state of the art for a given design, designing to codes and standards, and human factors/ergonomics.
