The case histories in this volume were compiled from their original sources and adapted to be published in the ASM Failure Analysis Center in 2004 and 2005. In 2019, the case histories were organized into the present volume.

The case histories were initially converted to electronic files in 2004 and 2005. In 2019, the digital markup format was updated to XML and additional subject tagging was added.
