Abstract
An amusement ride failed when a component in the ride parted, permitting it to fly apart. The ride consisted of a central shaft supporting a spider of three arms, each of which was equipped with an AISI 1040 steel secondary shaft about which a circular platform rotated. The main shaft rotated at about 12 rpm and the platforms at a speed of 20 rpm. The accident occurred when one of the secondary shafts on the amusement ride broke. The point of fracture was adjacent to a weld that attached the shaft to a 16 mm thick plate, which in turn bore the platform support arms. Investigation (visual inspection, 0.4x magnification, and stress analysis) supported the conclusion that a likely cause for the fatigue failure was the combination of residual stresses generated in welding and centrifugal service stresses from operation that were accentuated by areas of stress concentration at the undercut locations. Without the excessive residual stress, the shaft dimensions appeared ample for the service load. Recommendations included applying the fillet weld with more care to avoid undercutting. The residual stresses could be minimized by pre-weld and post-weld heat application.

Abstract
Several fractures occurred in flange studs used for remote handling of radioactive equipment. The studs, of quenched-and-tempered type 414 stainless steel, fractured in the HAZs produced in the studs during the circumferential welding that joined the studs to the flanges. The weld deposits were of type 347 stainless steel, and the flanges were type 304 stainless steel. Metallographic examination of the failed studs revealed that the HAZs contained regions of martensite and that intergranular cracks, which initiated at the stud surfaces during welding, propagated to complete separation under subsequent loading. The studs fractured under service loads as a result of intergranular crack propagation in the HAZ. Rapid heating and cooling during attachment welding produced a martensitic structure in the HAZ of the stud, which cracked circumferentially from the combination of thermal-gradient and phase-change stresses. Joining the studs to the flanges by welding should be discontinued. They should be attached by screw threads, using a key and keyway to prevent turning in service.

Abstract
The failure mode of through-wall cracking of a butt weld in a 5083-O aluminum alloy piping system in an ethylene plant was identified as mercury liquid metal embrittlement. As a result of this finding, 226 of the more than 400 butt welds in the system were ultrasonically inspected for cracking. One additional weld was found that had been degraded by mercury. A welding team experienced in repairing mercury contaminated piping was recruited to make the repairs. Corrective action included the installation of a sulfur-impregnated charcoal mercury-removal bed and replacement of the aluminum equipment that was in operation prior to the installation of the mercury-removal bed.

Abstract
This article offers an overview of fatigue fundamentals, common fatigue terminology, and examples of damage morphology. It presents a summary of relevant engineering mechanics, cyclic plasticity principles, and perspective on the modern design by analysis (DBA) techniques. The article reviews fatigue assessment methods incorporated in international design and post construction codes and standards, with special emphasis on evaluating welds. Specifically, the stress-life approach, the strain-life approach, and the fracture mechanics (crack growth) approach are described. An overview of high-cycle welded fatigue methods, cycle-counting techniques, and a discussion on ratcheting are also offered. A historical synopsis of fatigue technology advancements and commentary on component design and fabrication strategies to mitigate fatigue damage and improve damage tolerance are provided. Finally, the article presents practical fatigue assessment case studies of in-service equipment (pressure vessels) that employ DBA methods.

Abstract
The structural collapse of an iron-ore bucket-wheel stacker reclaimer at the beginning of operation was investigated by means of mechanical tests, microstructural characterization, and computational structural analysis. The mechanical failure was a consequence of a brittle fracture by cleavage. The crack followed the heat-affected zone of a welded joint connecting a rectangular hollow section member and a plate flange. The main factors contributing to failure were related with a combination of design-in and manufacturing-in factors like high load-strength ratio at the point of failure, local stress concentration as a result of geometry restrictions, and weld defects. This particular section was responsible for the load transfer between the front tie member and the boom extremity, and its failure was the main cause of the catastrophic failure of the equipment.

Abstract
Although field corrosion tests had indicated that type 316L stainless steel would be a suitable material for neutralization tanks, the vessels suffered severe corrosion when placed in service. Welded coupons of type 316L had been tested along with similar Alloy 20Cb® (UNS NO8020) specimens in a lead-lined tank equipped with copper coils that had served in this function prior to construction of the new tanks. Both materials exhibited virtually no corrosion and no preferential weld attack. Type 316L was selected for the project. The subsequent corrosion was the result of the borderline passivity of type 316L in hot dilute sulfuric acid (about 0.1%). Inaccuracy of the testing was attributed to the presence of cupric ions in the lead-lined vessel fluids, which had been released by corrosion of the copper coils. Careful control of both temperature and pH was recommended to reduce the corrosion to an acceptable limit.

Abstract
The phenomenon of on-load corrosion is directly associated with the production of magnetite on the water-side surface of boiler tubes. On-load corrosion may first be manifested by the sudden, violent rupture of a boiler tube, such failures being found to occur predominantly on the fire-side surface of tubes situated in zones exposed to radiant heat where high rates of heat transfer pertain. In most instances, a large number of adjacent tubes are found to have suffered, the affected zone frequently extending in a horizontal band across the boiler. In some instances, pronounced local attack has taken place at butt welds in water-wall tubes, particularly those situated in zones of high heat flux. To prevent on-load corrosion an adequate flow of water must occur within the tubes in the susceptible regions of a boiler. Corrosion products and suspended matter from the pre-boiler equipment should be prevented from entering the boiler itself. Also, it is good practice to reduce as far as possible the intrusion of weld flash and other impedances to smooth flow within the boiler tubes.

Abstract
Nondestructive metallographic examination of materials frequently must be performed on-site when the component in question cannot be moved or destructively examined. Often, it is imperative that specific microstructural information (i.e., material type, heat treatment condition, homogeneity, etc.) be obtained either before initial use of a component, or before the use of a component can be safely resumed. In this paper, the use of standard metallurgical laboratory equipment, and the procedures required to conduct nondestructive on-site metallographic analyses of engineering materials, is presented. As an example, the materials and metallographic techniques employed in an actual on-site investigation of a gas tungsten-arc weldment joining two large diameter Ti-6Al-4V alloy cylinders are discussed in depth to illustrate what can be accomplished.

A detailed fracture mechanics evaluation is the most accurate and reliable prediction of process equipment susceptibility to brittle fracture. This article provides an overview and discussion on brittle fracture. The discussion covers the reasons to evaluate brittle fracture, provides a brief summary of historical failures that were found to be a result of brittle fracture, and describes key components that drive susceptibility to a brittle fracture failure, namely stress, material toughness, and cracklike defect. It also presents industry codes and standards that assess susceptibility to brittle fracture. Additionally, a series of case study examples are presented that demonstrate assessment procedures used to mitigate the risk of brittle fracture in process equipment.

Abstract
After some 87,000 h of operation, failure took place in the bend of a steam pipe connecting a coil of the third superheater of a steam generator to the outlet steam collector. The unit operated at 538 deg C and 135 kPa, producing 400 t/h of steam. The 2.25Cr-1Mo steel pipe in which failure took place was 50.8 mm in diam with a nominal wall thickness of 8 mm. It connected to the AISI 321 superheater tube by means of a butt weld and was one of 46 such parallel connecting tubes. The Cr-Mo tubing was situated outside the heat transfer zone of the superheater. The overall sequence of failure involved overheating of the Cr-Mo outlet tubes, heavy oxidation, oxide cracking on thermal cycling, thermal fatigue cracking plus oxidation, creep-controlled crack growth, and rapid plastic deformation and rupture. This failure was indicative of excess temperature of the steam coming from the heat transfer zone of the coil. It showed that many damage mechanisms may combine in the transition from fracture initiation to final failure. The presence of grain boundary sliding as an indication of creep damage was useful in the characterization of the stress level as high and showed that the process of creep was not operative throughout the life of the equipment.

Abstract
Graphitization, the formation of graphite nodules in carbon and low alloy steels, contributes to many failures in high-temperature environments. Three such failures in power-generating systems were analyzed to demonstrate the unpredictable nature of this failure mechanism and its effect on material properties and structures. In general, the more randomly distributed the nodules, the less effect they have on structural integrity. In the cases examined, the nodules were found to be organized in planar arrays, indicating they might have an effect on material properties. Closer inspection, however, revealed that the magnitude of the effect depends on the relative orientation of the planar arrangement and principle tensile stress. For normal orientation, the effect of embrittlement tends to be most severe. Conversely, when the orientation is parallel, the nodules have little or no effect. The cases examined show that knowledge is incomplete in regard to graphitization, and the prediction of its occurrence is not yet possible.

This article primarily deals with many of these metal components of lifting equipment in three categories: cranes and bridges, attachments used for direct lifting, and built-in members of lifting equipment. It first reviews the mechanisms, origins, and investigation of failures. Then the article describes the materials used for lifting equipment, followed by a section explaining the failure analysis of wire ropes and the failure of wire ropes due to corrosion, a common cause of wire-rope failure. Abstract
After about four years of service, cracks appeared on the internal or process-side surfaces of four evaporator pans in a sugar concentrator. The pans consisted of a Mo stabilized austenitic stainless steel inner vessel surrounded by a mild steel steam jacket. Corrosion of the external surface had taken place in the form of confluent pitting over a band adjacent to the fillet weld which attached the pan to the blocking ring. Numerous cracks were present in this corroded zone. Microscopical examination of several specimens cut from the sample revealed that the internal cracks in the pan itself originated from the external side of the plate, i.e. from the region covered by the shrouding ring. They were predominantly of the transgranular type. Because the cracks were not of the intergranular type as usually found with weld decay, they were considered to be indicative of stress-corrosion cracking. Stresses responsible for the cracking resulted from weld contraction. A sample tube was removed from a reformer furnace for life assessment after 69,000 h of service. Sections were cut from the tube, which was a spindle cast A297 Grade HK 40 (25 Cr, 20 Ni, 0.4 C) austenitic steel of 122.5 mm OD and 10.5 mm nominal wall thickness. They were examined metallographically on transverse sections and on longitudinal sections through the butt welds joining the separate cast segments of the tube. Creep damage was mainly concentrated within the inner one third of the wall thickness. The use of damage assessment parameters in evaluating the reformer tube remaining life showed the welds to be inadequate, and to have a strength and creep resistance below those of the base metal.

Abstract
Nuclear power plants typically experience two or three high-cycle fatigue failures of stainless steel socket-welded connections in small bore piping during each plant-year of operation. This paper discusses fatigue-induced failure in socket-welded joints and the strategy Texas Utilities Electric Company (TU Electric) has implemented in response to these failures. High-cycle fatigue is invisible to proven commercial nondestructive evaluation (NDE) methods during crack initiation and the initial phases of crack growth. Under a constant applied stress, cracks grow at accelerating rates, which means cracks extend from a detectable size to a through-wall crack in a relatively short time. When fatigue cracks grow large enough to be visible to NDE, it is likely that the component is near the end of its useful life. TU Electric has determined that an inspection program designed to detect a crack prior to the component leaking would involve frequent inspections at a given location and that the cost of the inspection program would far exceed the benefits of avoiding a leak. Instead, TU Electric locates these cracks by visually monitoring for leaks. Field experience with fatigue-induced cracks in socket-welded joints has confirmed that visual monitoring does detect cracks in a timely manner, that these cracks do not result in catastrophic failures, and that the plant can be safely shut down in spite of a leaking socket-welded joint in a small bore pipe. Historical data from TU Electric and Southwest Research Institute are presented regarding the frequency of failures, failure locations, and the potential causes. The topics addressed include 1) metallurgical and fractographic features of fatigue cracks at the weld toe and weld root; 2) factors that are associated with fatigue, such as mechanical vibration, internal pulsation, joint design, and welding workmanship; and 3) implications of a leaking crack on plant safety. TU Electric has implemented the use of modified welding techniques for the fabrication of socket-welded joints that are expected to improve their ability to tolerate fatigue.

Holland, R.W. Vande Kamp, and J.B. Elder, were performed from the contaminated, annular space of the 46 year old, inactive, 1.03 million gallon waste storage tank. A steerable, magnetic wheel wall crawler was inserted into the annular space through small (6 in., or 150 mm, diam) holes/risers in the tank top. The crawler carried the equipment used to simultaneously collect data with up to four UT transducers and two cameras. The purpose of this inspection was to verify corrosion models and to investigate the possibility of previously unidentified corrosion sites or mechanisms. The inspections included evaluation of previously identified leak sites, thickness mapping, and crack detection scans on specified areas of the tank. No indications of reportable wall loss or pitting were detected. All thickness readings were above minimum design tank-wall thickness, although several small indications of thinning were noted. The crack detection and sizing examinations revealed five previously undetected indications, four of which were only partially through-wall. The cracks that were examined were found to be slightly longer than expected but still well within the flaw size criteria used to evaluate tank structural integrity.

This article discusses pressure vessels, piping, and associated pressure-boundary items of the types used in nuclear and conventional power plants, refineries, and chemical-processing plants. It begins by explaining the necessity of conducting a failure analysis, followed by the objectives of a failure analysis. Then, the article discusses the processes involved in failure analysis, including codes and standards. Next, fabrication flaws that can develop into failures of in-service pressure vessels and piping are covered. This is followed by sections discussing in-service mechanical and metallurgical failures, environment-assisted cracking failures, and other damage mechanisms that induce cracking failures. Finally, the article provides information on inspection practices.

Abstract
Life assessment of structural components is used to avoid catastrophic failures and to maintain safe and reliable functioning of equipment. The failure investigator's input is essential for the meaningful life assessment of structural components. This article provides an overview of the structural design process, the failure analysis process, the failure investigator's role, and how failure analysis of structural components integrates into the determination of remaining life, fitness-for-service, and other life assessment concerns. The topics discussed include industry perspectives on failure and life assessment of components, structural design philosophies, the role of the failure analyst in life assessment, and the role of nondestructive inspection.
