Abstract
Weld-decay and stress-corrosion cracking developed in several similar all-welded vessels fabricated from austenitic stainless steel. During a periodic examination cracks were revealed at the external surface of one of the vessels. External patch welds had been applied at these and several other corresponding locations. Cracks visible on the external surface developed from the inside in a region close to the toe of the internal fillet weld to the deflector plate, and another deep crack associated with a weld cavity is visible slightly to the right of the main fissure. Microscopic examination revealed that precipitation of carbides at the grain boundaries had taken place in the vicinity of the cracks, but that the paths of the cracks were not wholly intergranular. Conditions present in the vicinity of the internal fillet weld must have been such as to favor both inter- and transgranular cracking. It is probable that the heating associated with the repair welds made from time to time also contributed to the trouble. The transgranular cracks, however, were indicative of stress-corrosion cracking.

Abstract
Two AISI type 316 stainless steel dished ends failed through the formation of intergranular stress-corrosion cracks (IGSCC) within a few months of service. The dished ends failed in the straight portions near the circumferential welds that joined the ends to the cylindrical portions of the vessel. Both dished ends were manufactured from the same batch and were supplied by the same manufacturer One of the dished ends had been exposed to sodium at 550 deg C (1020 deg F) for 500 h before failure due to sodium leakage was detected. The other dished end was used to fabricate a second vessel that was kept in storage for 1 year Clear evidence of sensitization was found in areas where IGSCC occurred. Sensitization was extensive in the dished end that had been exposed to sodium at high temperature, and it occurred in a narrow band similar to that typical of weld decay in the dished end that had been kept in storage. Solution annealing was recommended to relieve residual stress, thereby reducing the probability of failure. It was also recommended that the carbon content of the steel be lowered, i.e., that a 316L grade be used.

Abstract
Two aircraft-engine tailpipes of 19-9 DL stainless steel (AISI type 651) developed cracks along longitudinal gas tungsten arc butt welds after being in service for more than 1000 h. Binocular-microscope examination of the cracks in both tailpipes revealed granular, brittle-appearing surfaces confined to the HAZs of the welds. Microscopic examination of sections transverse to the weld cracks showed severe intergranular corrosion in the HAZ. The fractures appeared to be caused by loss of corrosion resistance due to sensitization, that could have been induced by the temperatures attained during gas tungsten arc welding. Tests demonstrated the presence of sensitization in the HAZ of the gas tungsten arc weld. The aircraft engine tailpipe failures were due to intergranular corrosion in service of the sensitized structure of the HAZs produced during gas tungsten arc welding. All gas tungsten arc welded tailpipes should be postweld annealed by re-solution treatment to redissolve all particles of carbide in the HAZ. Also, it was suggested that resistance seam welding be used, because there would be no corrosion problem with the faster cooling rate characteristic of this technique.

Abstract
Type 316L stainless steel pipes carrying brine at 120 deg C (250 deg F) and at a pH of about 7, failed by perforation at or near circumferential butt-weld seams. The failure was examined optically and radiographically in the field. Specimens were removed and examined metallographically and with a SEM in the laboratory. The examinations revealed a combination of failure mechanisms. The pitting failure of the welds was attributed to localized attack of an activated surface, in which anodic pits corroded rapidly. Additionally, SCC driven by residual welding stresses occurred in the base metal adjacent to the welds. Use of highly stressed austenitic stainless steels in high-chloride environments having a temperature above 65 deg C (150 deg F) should be discouraged. Solution annealing or shot peening to reduce residual stresses may be advisable. If heat treatment is not feasible after welding, the substitution of a more corrosion-resistant alloy, such as Incoloy 800 or 825, may be necessary.

Abstract
Corrosion in a closed-loop cooling water system constructed of austenitic stainless steel occurred during an extended lay up of the system with biologically contaminated water. The characteristics of the failure were those of microbiologically influenced corrosion (MIC). The corrosion occurred at welds and consisted of large subsurface void formations with pinhole penetrations of the surfaces. Corrosive attack initiated in the heat affected zones of the welds, usually immediately adjacent to fusion lines. Stepwise grinding, polishing, and etching through the affected areas revealed that voids generally grew in the wrought material by uniform general corrosion. Tunneling or worm-holing was also observed, whereby void extension occurred by initiating daughter voids probably at flaws or other inhomogeneities. Selective attack occurred within the fusion zone, i.e., within the cast two-phase structure of the weld filler itself. The result was a void wall which consisted of a rough and porous ferritic material, a consequence of preferential attack of the austenitic phase and slightly lower rate of corrosive attack of the ferrite phase. The three-dimensional spongy surface was studied optically and with the scanning electron microscope.

Abstract
The defects observed along weldings of stainless steel pipelines employed in marine environments were evidenced by metallographic and electrochemical examination. A compilation of cases on the effect of defective weldings, in addition to improper choice of stainless steel for water pipelines, lead to the conclusion that intercrystalline corrosion in steels involved precipitation of a surplus phase at grain boundaries. Intercrystalline corrosion in austenitic stainless steels due to precipitation of chromium carbides during conditions generated due to welding and ways to avoid the precipitation (including reduction of carbon content, appropriate heat treatment, cold work of steel, reduction of austenitic grain size and stabilizing elements) were described. The presence of microcracks due to highly localized heat concentrations with consequent thermal expansion and considerable shrinkages during cooling was investigated. The specimens were taken from various sources including transverse and longitudinal welding seam, sensitized areas and it was concluded appropriate material selection with respect to medium could control some corrosion processes.

Abstract
Several tubes in a tube bundle in an evaporator used to concentrate an acid nitrate solution failed by leakage. The feed to the evaporator contained about 6% nitrate, and the discharge about 60% nitrate. The tube bundle was comprised of type 309S (Nb) stainless steel drawn-and-welded tubes expanded and welded into two type 304L stainless steel tube sheets. The tubes failed by crevice corrosion. The failed tubes were defective as-received, and the establishment of concentration cells within the longitudinal cracks in the seam welds led to ultimate corrosive penetration of the wall. There was no evidence of crevice corrosion or any localized penetration of tubes that had sound welds. The leaking type 309S (Nb) welded tubes should be replaced with seamless tubes of type 304L stainless steel to minimize the areas requiring welding and to provide maximum weldability for the tube-sheet joints.

Abstract
The onset of leakage adjacent to two butt welds in a 2 in. bore pipe was traced to the development of fine cracks. The pipe carried 40% sodium hydroxide solution. The actual temperature was not known, but the pipeline was steam traced at a pressure of 30 psi, equivalent to a temperature of 130 deg C (266 deg F). Magnetic crack detection revealed circumferential crack-like indications situated a short distance from the butt weld. Cracking originated on the bore surfaces of the tube and was of an intergranular nature reminiscent of caustic cracking in steam boilers. The strength of the solution of caustic soda and possibly the temperature also were in the range known to produce stress-corrosion cracking of mild steels in the presence of stresses of sufficient magnitude. In this instance the location of the cracking suggested that residual stresses from welding, which approach yield point magnitude, were responsible. As all other welds were suspect, the remedy was to remove the joints and to reweld followed by local stress relief.

Abstract
A nozzle in a wastewater vaporizer began leaking after approximately three years of service with acetic and formic acid wastewaters at 105 deg C (225 deg F) and 414 kPa (60 psig). The shell of the vessel was weld fabricated from 6.4 mm (0.25 in.) E-Brite stainless steel plate and measured 1.5 m (58 in.) in diameter and 8.5 m (28 ft) in length. Investigation (visual inspection, chemical analysis, radiography, dye-penetrant inspection, and hydrostatic testing of all E-Brite welds, 4x images, 100x/200x images electrolytically etched with 10% oxalic acid, and V-notch Charpy testing) supported the conclusion that failure of the nozzle weld was the result of intergranular corrosion caused by the pickup of interstitial elements and subsequent precipitation of chromium carbides and nitrides. Carbon pickup was believed to have been caused by inadequate joint cleaning prior to welding. The increase in the weld nitrogen level was a direct result of inadequate argon gas shielding of the molten weld puddle. Two areas of inadequate shielding were identified: improper gas flow rate for a 19 mm (0.75 in.) diam gas lens nozzle, and contamination of the manifold gas system. Recommendations included changes in the cleaning and welding process.

Abstract
The welds joining the liner and shell of a fluid catalytic cracking unit failed. The shell was made of ASTM A515 carbon steel welded with E7018 filler metal. The liner was made of type 405 stainless steel and was plug welded to the shell using ER309 and ER310 stainless steel filler metal. Fine cracks starting inside the weld zone and spreading outward through the weld and toward the surface were observed during examination. Decarburization and graphitization of the carbon steel at the interface was noted. The high carbon level was found to allow martensite to form eventually. The structure was found to be austenitic in the area where the grain-boundary precipitates appeared heaviest. The composition of the precipitates was analyzed using an electron microprobe to reveal presence of sulfur. Microstructural changes in the weld alloy at the interface were interpreted to be caused by dilution of the alloy and the presence of sulfur caused hot shortness. The necessary internal stress to produce extensive cracking was produced by the differential thermal expansion of the carbon and stainless steels. Periodic careful gouging of the affected areas followed by repair welding was recommended.

Abstract
A 150 mm (6 in.) schedule 80S type 304 stainless steel pipe (11 mm, or 0.432 in., wall thickness), which had served as an equalizer line in the primary loop of a pressurized-water reactor, was found to contain several circumferential cracks 50 to 100 mm (2 to 4 in.) long. Two of these cracks, which had penetrated the pipe wall, were responsible for leaks detected in a hydrostatic test performed during a general inspection after seven years of service. Investigation (visual inspection, visual and ultrasonic weld examination, water analysis, and chemical analysis) supported the conclusion that the failure was caused by SCC due to stress, sensitization, and environment. Recommendations included replacing all pipe sections and installing them using low-heat-input, multiple-pass welding procedures.

Abstract
This article discusses the failure analysis of several steel transmission pipeline failures, describes the causes and characteristics of specific pipeline failure modes, and introduces pipeline failure prevention and integrity management practices and methodologies. In addition, it covers the use of transmission pipeline in North America, discusses the procedures in pipeline failure analysis investigation, and provides a brief background on the most commonly observed pipeline flaws and degradation mechanisms. A case study related to hydrogen cracking and a hard spot is also presented.
