Abstract
A 150 mm (6 in.) schedule 80S type 304 stainless steel pipe (11 mm, or 0.432 in., wall thickness), which had served as an equalizer line in the primary loop of a pressurized-water reactor, was found to contain several circumferential cracks 50 to 100 mm (2 to 4 in.) long. Two of these cracks, which had penetrated the pipe wall, were responsible for leaks detected in a hydrostatic test performed during a general inspection after seven years of service. Investigation (visual inspection, visual and ultrasonic weld examination, water analysis, and chemical analysis) supported the conclusion that the failure was caused by SCC due to stress, sensitization, and environment. Recommendations included replacing all pipe sections and installing them using low-heat-input, multiple-pass welding procedures.

Abstract
The AISI 1080 steel crankshaft of a large-capacity double-action stamping press broke in service and was repair welded. Shortly after the crankshaft was returned to service, the repair weld fractured. The repair-weld fracture was examined ultrasonically which revealed many internal reflectors, indicating the presence of slag inclusions and porosity. A low-carbon steel flux-cored filler metal was used in repair welding the crankshaft, without any preweld or postweld heating. This resulted in the formation of martensite in the HAZ. The repair weld failed by brittle fracture, which was attributed to the combination of weld porosity, many slag inclusions and the formation of brittle martensite in the HAZ. A new repair weld was made using an E312 stainless steel electrode, which provides a weld deposit that contains considerable ferrite to prevent hot cracking. Before welding, the crankshaft was preheated to a temperature above which martensite would form. After completion, the weld was covered with an asbestos blanket, and heating was continued for 24 h. During the next 24 h, the temperature was slowly lowered. The result was a crack-free weld.

Abstract
Welds in two CMo steel catalytic gas-oil desulfurizer reactors cracked under hydrogen pressure-temperature conditions that would not have been predicted by the June 1977 revision of the Nelson Curve for that material. Evidence of severe cracking was found in five weld-joint areas during examination of a naphtha desulfurizer by ultrasonic shear wave techniques. Defect indications were found in longitudinal and circumferential seam welds of the ASTM A204, grade A, steel sheet. The vessel was found to have a type 405 stainless steel liner for corrosion protection that was spot welded to the base metal and all vessel welds were found to be overlaid with type 309 stainless steel. Long longitudinal cracks in the weld metal, as well as transverse cracks were exposed after the weld overlay was ground off. A decarburized region on either side of the crack was revealed by metallurgical examination of a cross section of a longitudinal crack. It was concluded that the damage was caused by a form of hydrogen attack. Installation of a used Cr-Mo steel vessel with a type 347 stainless steel weld overlay was suggested as a corrective action.

Abstract
Two leaks were discovered at a sulfur recovery unit in a refinery. The leaks were at pipe-to-elbow welds in a 152 mm (6 in.) (NPS 6) diam line, operating in lean amine service at 50 deg C (120 deg F) and 2.9 MPa (425 psig). Thickness measurements indicated negligible loss of metal, and the leaks were clamped. A year later, 15 additional leaks were discovered, again at pipe-to-elbow welds in lean amine lines. Further nondestructive testing located other cracks, giving a total of 35. These lines had been in service for approximately eight years. Investigation (visual inspection, hardness testing, and micrographic cross-sections) supported the conclusion that the failure was caused by lean amine SCC. It was considered unlikely that these pipe welds had received such a postweld heat treatment, although it is industry practice to postweld stress relieve piping and pressure vessels in lean amine service if the temperature is expected to be above 95 deg C (200 deg F). Recommendations included inspecting all welds using shear wave ultrasonic testing and postweld heat treating all welds in lean amine service.

Abstract
The link which failed was a special long one connecting a grab chain to a swivel. It was made from En 14A steel and in continuous use for two years. On one of the fracture faces the chisel edge weld preparation was clearly visible and the crack progression markings present indicated failure was due to fatigue. Macro-etched cross sections showed a lack of penetration and fusion in the weld. Fatigue cracks developed and slowly progressed through the weld metal. Fracture occurred when the remaining area of sound metal was insufficient to support the load. Lack of penetration of this magnitude could be revealed by radiography or ultrasonics but it would be difficult to detect the presence of cracks in course of development from the defects. It would be more prudent to ensure that welded links of this type were free from internal cavities before being put into service.

Abstract
A system of carbon steel headers, handling superheated water of 188 deg C (370 deg F) at 2 MPa (300 psi) for automobile-tire curing presses, developed a number of leaks within about four months after two to three years of leak-free service. All the leaks were in shielded metal arc butt welds joining 200 mm (8 in.) diam 90 deg elbows and pipe to 200 mm (8 in.) diam welding-neck flanges. A flange-elbow-flange assembly and a flange-pipe assembly that had leaked were removed for examination. Investigation (visual inspection, hardness testing, chemical analysis, magnetic-particle testing, radiographic inspection, and 2% nital etched 1.7x views) showed varying IDs on the assemblies and supported the conclusions that the failures of the butt welds were the result of fatigue cracks caused by cyclic thermal stresses that initiated at stress-concentrating notches at the toes of the interior fillet welds on the surfaces of the flanges. Recommendations included using ultrasonic testing to identify the appropriate joints and then replacing them. Special attention to accuracy of fit-up in the replacement joints was also recommended to achieve smooth, notch-free contours on the interior surfaces.

Abstract
The failure mode of through-wall cracking of a butt weld in a 5083-O aluminum alloy piping system in an ethylene plant was identified as mercury liquid metal embrittlement. As a result of this finding, 226 of the more than 400 butt welds in the system were ultrasonically inspected for cracking. One additional weld was found that had been degraded by mercury. A welding team experienced in repairing mercury contaminated piping was recruited to make the repairs. Corrective action included the installation of a sulfur-impregnated charcoal mercury-removal bed and replacement of the aluminum equipment that was in operation prior to the installation of the mercury-removal bed.

Cracking occurred in an ASME SB166 Inconel 600 safe-end forging on a nuclear reactor coolant water recirculation nozzle while it was in service. The safe-end was welded to a stainless-steel-clad carbon steel nozzle and a type 316 stainless steel transition metal pipe segment. An Inconel 600 thermal sleeve was welded to the safe-end, and a repair weld had obviously been made on the outside surface of the safe-end to correct a machining error. Initial visual examination of the safe-end disclosed that the cracking extended over approximately 85 deg of the circular circumference of the piece. Investigation (visual inspection, on-site radiographic inspection, limited ultrasonic inspection, chemical analysis, 53x metallographic cross sections and SEM images etched in 8:1 phosphoric acid) supported the conclusion that the cracking mechanism was intergranular SCC. No recommendations were made.

Abstract
After ten years of satisfactory operation, economizer-tube failures occurred in a large black liquor recovery boiler for a paper mill. The economizer contained 1320 finned tubes. Two fins ran longitudinally for most of the tube length and were attached by fillet welding on one side. The economizer tube leaks occurred at the end of the fin near the bottom of the economizer. A sample from a tube that had not failed showed heavy pitting attack on the inside of the tube, probably due to excess oxygen in the feedwater. Penetrant testing revealed numerous longitudinal cracks on the inside in the area of the fin tip. Cracking at the end of the fin-to-tube fillet weld was noted. The results indicate the failures were due to corrosion fatigue whose stresses were primarily thermally induced. A temporary solution included inspecting all tubes with shear-wave ultrasonics. Tubes with the most severe cracking were ground and repair welded. The square corners of the fins were trimmed back with a gradual taper so that expansion strains would be more gradually transferred to the tube surface. Water chemistry was closely evaluated and monitored, especially with regard to oxygen content.

Abstract
A welded natural gas line of 400 mm OD and 9 mm wall thickness made of unalloyed steel with 0.22C had to be removed from service after four months because of a pipe burst. Metallographic examination showed the pipe section located next to the gas entrance was permeated by cracks or blisters almost over its entire perimeter in agreement with the ultrasonic test results. Only the weld seam and a strip on each side of it were crack-free. Based on this investigation, the pipeline was taken out of service and reconstructed. To avoid such failures in the future, two preventative measures may be considered. One is to desulfurize the gas. Based on tests, however, the desulfurization would have to be carried very far to be successful. The second possibility is to dry the gas to such an extent as to prevent condensate, and this corrosion, from forming no matter how low winter temperatures may drop. This measure was ultimately recommended, deemed more effective and cheaper.

This paper reviews several fatigue failures from the waterwall, superheater, and economizer portions of the boiler, their causes and how they were mitigated and monitored. Some cases required simple field modifications by cutting or welding, repair of existing controls, and/or changes in maintenance. Nondestructive inspections by visual, magnetic particle, ultrasonic, and radiographic methods for detecting and monitoring damage are discussed. These failures are presented to provide hindsight that will help others in increasing the success rate for anticipating and analyzing the remaining life of other units.

Abstract
A 460 mm (18 in.) diam suction line to the main feed water pump for a nuclear power plant failed in a violent, catastrophic manner. Samples of pipe, elbow, and weld materials (ASTM A106 grade B carbon steel, ASTM A234 grade WPB carbon steel, and E7018 carbon steel electrode, respectively) from the suction line were analyzed. Evidence of overall thinning of the elbow and pipe material and ductile tearing of fractures indicated that the feed water pipe failed as a result of an erosion corrosion mechanism, which thinned the wall sufficiently to cause rapid, ductile tearing of the material after its design stress had been exceeded. It was recommended that steel with a higher chromium content be used to mitigate the erosion corrosion potential in the lines and that more rigorous nondestructive (ultrasonic) examinations be performed.

Abstract
This paper describes the remote ultrasonic (UT) examinations of a high-level radioactive waste storage tank at the Savannah River Site in South Carolina. The inspections, carried out by E.R. Holland, R.W. Vande Kamp, and J.B. Elder, were performed from the contaminated, annular space of the 46 year old, inactive, 1.03 million gallon waste storage tank. A steerable, magnetic wheel wall crawler was inserted into the annular space through small (6 in., or 150 mm, diam) holes/risers in the tank top. The crawler carried the equipment used to simultaneously collect data with up to four UT transducers and two cameras. The purpose of this inspection was to verify corrosion models and to investigate the possibility of previously unidentified corrosion sites or mechanisms. The inspections included evaluation of previously identified leak sites, thickness mapping, and crack detection scans on specified areas of the tank. No indications of reportable wall loss or pitting were detected. All thickness readings were above minimum design tank-wall thickness, although several small indications of thinning were noted. The crack detection and sizing examinations revealed five previously undetected indications, four of which were only partially through-wall. The cracks that were examined were found to be slightly longer than expected but still well within the flaw size criteria used to evaluate tank structural integrity.

Abstract
Of the many different nondestructive evaluation (NDE) techniques, ultrasonic inspection continues to be the leading nondestructive method for inspecting composite materials, because measurements can be quantitative and the typical defect geometries and orientations lend themselves to detection and characterization. This article focuses on the three common methods for ultrasonic nondestructive inspection of plastics, namely pitch-catch, through-transmission, and pulse-echo, as well as the three basic types of ultrasonic NDE scans: the A-scan, B-scan, and C-scan. The discussion includes the linear and phased array systems that are sometimes used for large-scale inspection tasks to reduce scan times, the various gating and image processing techniques, and how ultrasonic data are interpreted and presented. A brief section on future trends in ultrasonic inspection is presented at the end of the article.

Abstract
Tie bars of a dragline excavator each consisted of a rectangular section steel bar to which eye-pieces, to facilitate anchorage, were attached by butt-welds. Failure of one weld in each bar after seven years of service allowed the boom to fall and become extensively damaged. The appearance of the fracture faces of the two welds showed partial-penetration joints. Failure in each bar had taken place through the weld metal. The presence of built-in cracks introduced zones of stress concentration and the fluctuating loads to which the ties were subjected in service served to initiate fatigue cracks. While the partial-penetration type of weld may be tolerated in a component subjected to bending stresses it is undesirable in one that is required to withstand fluctuating tensile stresses.
