Abstract
A deformed steel tube was received for failure analysis after buckling during a heat-treat operation. The tube was subjected to various metallurgical tests as well as nondestructive testing to confirm the presence of residual stresses. The microstructure of the tube was found to be homogenous and had no banded structure. However, x-ray diffraction analysis confirmed the presence of up to 6% retained austenite which likely caused the tube to buckle during the 910 °C heat treating procedure.

Abstract
A sleeve-shaped fire shield that operates inside one of two burner trains in an oil and gas processing unit ruptured after 15 y of service. A detailed analysis was conducted to determine how and why the sleeve failed. The investigation included visual inspection, chemical and gas analysis, mechanical property testing, stereomicroscopy, and metallographic examination. The fire sleeves are fabricated from 3-mm thick plate made of Incoloy 800 rolled into 540-mm diam sections welded along the seam. Three such sections are joined together by circumferential welds to form a single 2.8 m sleeve. The findings from the investigation indicated that internal oxidation corrosion, driven by high temperatures, was the primary cause of failure. Prolonged exposure to temperatures up to 760 °C resulted in sensitization of the material, making it vulnerable to grain boundary attack. This led to significant deterioration of the grain boundaries, causing extensive grain loss (grain dropping) and the subsequent thinning of sleeve walls. Prior to failure, some portions of the sleeve were only 1.6 mm thick, nearly half their original thickness.

A type 304 stainless steel tube that failed in a boiler stack economizer was analyzed to determine the cause. The investigation consisted of visual, SEM/EDS, and metallographic analysis. Several degradation mechanisms appeared to be at work, including pitting corrosion, chloride stress corrosion cracking, and fatigue fracture. Investigators concluded that the primary failure mechanism was fatigue fracture, although either of the other mechanisms may have eventually caused the tube to fail in the absence of fatigue.

A back wall riser tube in a high pressure boiler failed, interrupting operations in a cogeneration plant. The failure occurred in a tube facing the furnace, causing eight ruptured openings over a 1.8 m section. The investigation consisted of an on-site visual inspection, nondestructive testing, energy dispersive x-ray analysis, and inductively coupled plasma mass spectrometry. The tube was made from SA 210A1 carbon steel that had been compromised by wall thinning and the accumulation of fire and water-side scale deposits. Investigators determined that the tube failed due to prolonged caustic attack that led to ruptures in areas of high stress. The escaping steam eroded the outer surface of the tube causing heavy loss of metal around the rupture points.

An E-Brite /Ferralium explosively bonded tube sheet in a nitric acid condenser was removed from service because of corrosion. Visual and metallographic examination of tube sheet samples revealed severe cracking in the heat-affected zone between the outer tubes and the weld joining the tube sheet to the floating skirt. Cracks penetrated deep into the tube sheet, and occasionally into the tube walls. The microstructures of both alloys and of the weld appeared normal. Intergranular corrosion characteristic of end-grain attack was apparent. A low dead spot at the skirt / tube sheet joint allowed the Nox to condense and subsequently reboil. This, coupled with repeated repair welding in the area, reduced resistance to acid attack. Intergranular corrosion continued until failure. Recommendations included changing operating parameter inlet to prevent HNO3 condensation outside the inlet and replacement of the floating skirt with virgin material (i.e., material unaffected by weld repairs).

Three radiant heating element tubes from an aluminum holding furnace failed after a few months of service. One side of each of the tubes had disintegrated, leaving large holes and thinned cross sections. Microstructural analysis showed that the surface of the tube had been oxidized along the grain boundaries and had extensive precipitation inside the grains. Chemical analysis indicated that the steel used for the tubes was AISI type 316 stainless steel Specifications for the tubes had called for AISI type 310S to be used. It was recommended that other tubes made from the same batch of steel sheet be checked.

A 44.5 mm (1.75 in.) diam type 321 stainless steel seamless tube in a power-generating turbine failed after 19,000 h in service. The tube was used to carry a mixture of approximately 25% steam and 75% hot air. Three fractured pieces and part of the tube containing the mating fracture surface were examined. Both fractographic and metallographic features revealed that the failure was by thermal fatigue caused by the presence of biaxial thermal stresses on the inner surface of the tube. It was recommended that the steam and air be thoroughly mixed prior to entering the tube to decrease the temperature fluctuations of the inner surface.

Radiant tubes that failed prematurely in an ethylene cracking furnace were analyzed to determine the cause of their early demise. The tubes were made from austenitic heat-resistant steel and cracked along their longitudinal axis. New and used tubes were compared using scanning electron microscopy, energy dispersive x-ray spectrometry, and mechanical property testing. This provided critical information and revealed that improper coking and decoking had removed the protective oxide layer (Cr 2 O 3 ) that normally prevents coke deposits from forming on exposed surfaces. Without this layer, coke readily accumulates on the surface of the tubes, fueling carbon diffusion into the metal and a corresponding degradation in microstructure and loss of ductility at high temperatures.

A bent Ni-Cu Monel 400 alloy tube, which operated as part of a pipeline in a petrochemical distillery, failed by through-thickness cracking. The pipeline was used to carry a stream of gaseous hydrocarbons containing hydrochloric acid (HCl) into a reaction tower. The tower provided a caustic solution (NaOH) to remove HCl from the stream, before the latter was directed to a burner. Metallographic examination showed that the cracks were intergranular and were frequently branched. Although nominal chemical composition of the component was found within the specified range, energy dispersive x-ray analysis (EDXA) indicated significant segregation of sulfur and chlorine along the grain boundaries. Failure was attributed to hypochlorous-acid (HClO)-induced stress-corrosion cracking (SCC). The HClO was formed by the reaction of HCl with atmospheric O 2 that entered the tube during shutdowns and startups. Residual stresses, originating from in situ bend forming of the tube during assembly of the line, provided a driving force for crack growth, and the segregation of sulfur on grain boundaries made the material more susceptible to cracking.

Abstract
A pipe in the lateral wall of a boiler powering an aircraft carrier flat-top boat failed during a test at sea. The pipe was made from ASTM 192 steel, an adequate material for the application. Microstructural analysis along with equipment operating records provided valuable insight into what caused the pipe to rupture. Although the pipe had been replaced just 50 h before the accident, the analysis revealed incrustations and corrosion pits on the inner walls and oxidation on the outer walls. Microstructural changes were also observed, indicating that the steel was exposed to high temperatures. The combined effect of pitting, incrustations, and phase transformations caused the pipe to rupture.

An arsenical admiralty brass (UNS C44300) finned tube in a generator air cooler unit at a hydroelectric power station failed. The unit had been in operation for approximately 49,000 h. Stereomicroscopic examination revealed two small transverse cracks that were within a few millimeters of the tube end, with one being a through-wall crack. Metallographic examination of sections containing the cracks showed branching secondary cracks and a transgranular cracking mode. The cracks appeared to initiate in pits. EDS analysis of a friable deposit found on the inside diameter of the tube and XRD analysis of crystalline compounds in the deposit indicated the possible presence of ammonia. Failure was attributed to stress-corrosion cracking resulting from ammonia in the cooling water. It was recommended that an alternate tube material, such as a 70Cu-30Ni alloy or a titanium alloy, be used.

Abstract
Tube failures occurred in quick succession in two boiler units from a bank of six boilers in a refinery. The failures were confined to the SAE 192 carbon steel horizontal support tubes of the superheater pack. In both cases, the failure was by perforation adjacent to the welded fin on the crown of the top tubes and located in an area near the upward bend of the tube. The inside of all the tubes were covered with a loosely adherent, black, alkaline, powdery deposit comprised mainly of magnetite. The corroded areas, however, had relatively less deposit. The morphology of the corrosion damage was typical of alkaline corrosion and confirmed that the boiler tubes failed as a result of steam blanketing that concentrated phosphate salts. The severe alkaline conditions developed most probably because of the decomposition of trisodium phosphate, which was used as a water treatment chemical for the boiler feed water.

A failed SAE-192 carbon steel tube from a 6.2-MPa (900-psig), 200-Mg/h (180-ton/h) capacity refinery boiler was analyzed to determine its failure mode. Optical and SEM examination results were combined with knowledge of the boiler operating conditions to conclude that the failure was hydrogen-induced. The hydrogen was probably generated by the steam-iron reaction. The source of steam on the flue gas side could be traced to a cracked fillet weld in the boiler The failure mode was unusual in that the attack was found to originate from the flue gas side of the tube rather than the steam side.

Original carbon steel and subsequent replacement austenitic stainless steel superheater tube U-bend failures occurred in a waste heat boiler. The carbon steel tubes had experienced metal wastage in the form of caustic corrosion gouging, while the stainless steel tubes failed by caustic-induced stress-corrosion cracking. Sodium was detected by EDS in the internal deposits and the base of a gouge in a carbon steel tube and in the internal deposits of the stainless steel tube. The sodium probably formed sodium hydroxide with carryover moisture and caused the gouging, which was further aggravated by the presence of silicon and sulfur (silicates and sulfates). It was recommended that the tubes be replaced with Inconel 600 or 601, as a practical option until the carryover problem could be solved.

Tube 3 from a utility boiler in service for 13 years under operating conditions of 540 deg C (1005 deg F), 13.7 MPa (1990 psi) and 1,189,320 kg/h (2,662,000 lb/h) incurred a longitudinal rupture near its 90 deg bend while Tube 4 from the same boiler exhibited deformation near its bend. Metallographic examination revealed creep voids near the rupture in addition to graphite nodules. Exposure of the SA209 Grade T1A steel tubing to a calculated mean operating temperature of 530 deg C (983 deg F) for the 13 years resulted in graphitization and subsequent creep failure in Tube 3. The deformation in Tube 4 was likely the result of steam washing from the Tube 3 failure. Graphitization observed remote from the rupture in Tube 3 and in Tube 4 indicated that adjacent tubing also was susceptible to creep failure. In-situ metallography identified other graphitized tubes to be replaced during a scheduled outage.

A low-carbon steel (St35.8) tube in a phthalic anhydride reactor system failed. Visual and stereomicroscopic examination of fracture surfaces revealed heavy oxide/deposits on the outer surface of the tube, tube wall thinning in the area of the fracture, and discolorations and oxides/deposits on the inner surface. Cross sections from the fracture surface were metallographically examined, and the deposits were analyzed. It was determined that the tube had thinned from the inner surface because of a localized overheating condition (probably resulting from a runaway chemical reaction within the tube) and then fractured, which allowed molten salt to flow into the tube.

Leaks developed at random locations in aluminum brass condenser tubes within the first year of operation of a steam condenser in a nuclear power plant. One failed tube underwent scanning electron microscopy surface examination and optical microscope metallography. It was determined that the tube failed from crevice corrosion under seawater deposits that had formed on the inner surface. Mechanical cleaning of the condenser tubes every 6 months and installation of intake screens of smaller mesh size were recommended.

The causes of cracking of an as-drawn 90-10 cupronickel tube during mechanical working were investigated to determine the source of embrittlement. Embrittlement was sporadic, but when present was typically noted after the first process anneal. Microstructural and chemical analyses were performed on an embrittled section and on a section from a different lot that did not crack during forming. The failed section showed an intergranular fracture path. Examination of the fracture surfaces revealed the presence of tellurium at the grain boundaries. The source of the tellurium was thought to be contamination occurring in the casting process that became concentrated in the recycled skimmings. It was recommended that future material specifications for skimmings and for externally obtained scrap copper include a trace analysis for tellurium.

A carbon-molybdenum (ASTM A209 Grade T1) steel superheater tube section in an 8.6 MPa (1250 psig) boiler cracked because of long-term overheating damage that resulted from prolonged exposure to metal temperatures between 482 deg C (900 deg F) and 551 deg C (1025 deg F). The outer diameter of the tube exhibited a crack (fissure) oriented approximately 45 deg to the longitudinal axis and 3.8 cm (1.5 in.) long. The inner diameter surface showed a fissure in the same location and orientation. Microstructure at the failure near the outer diameter surface exhibited evidence of creep cracking and creep void formation at the fissure. A nearly continuous band of graphite nodules was observed on the surface of the fissure. In addition to the graphite band formation, the microstructure near the failure exhibited carbide spheroidization from long-term overheating in all the tube regions examined. It was concluded that preferential nucleations of graphite nodules in a series of bands weakened the steel locally, producing preferred fracture paths. Formation of these graphite bands probably expedited the creep failure of the tube. Future failures may be avoided by using low-alloy steels with chromium additions such as ASTM A213 Grade T11 or T22, which are resistant to graphitization damage.

Abstract
An SB407 alloy 800H tube failed at a 100 deg bend shortly after startup of a new steam superheater. Three bends failed and one bend remote from the failure area was examined. Visual examination showed that the fracture started on the outside surface along the inside radius of the bend and propagated in a brittle, intergranular fashion. Chemical analysis revealed that lead contamination was a significant factor in the failure and phosphorus may have contributed. The localized nature of the cracks and minimum secondary cracking suggested a distinct, synergistic effect of applied tensile stress with the contamination. Stress analysis found that stress alone was not enough to cause failure; however the operating stresses in the 100 deg bends were higher than at most other locations in the superheater Reduced creep ductility may be another possible cause of failure. Remedial actions included reducing the tube temperature, replacing the Schedule 40 100 deg bends with Schedule 80 pipe, and solution annealing the pipe after bending.
