Abstract
The lower receiver of the M16 rifle is an anodized forging of aluminum alloy 7075-T6. Degradation of the receivers was observed after three years of service in a hot, humid atmosphere. The affected areas were those in frequent contact with the user's hands. There was no question that the material failed as a result of exfoliation corrosion, so an investigation was undertaken, centered around the study of thermal treatments that would increase the exfoliation resistance and still develop the required 448 MPa (65 ksi) yield strength. The results of the study concluded that rolled bar stock should be preferred to extruded bar stock. Differences in grain structure of the forgings, as induced by differences in thermal-mechanical history of the forged material, can have a significant effect on susceptibility to exfoliation corrosion. Regarding thermal treatment, the results show conclusively that large changes in strength and exfoliation characteristics of 7075 forgings can be induced by changes in temperature or time of thermal treatment. With regard to the effect of quenching rate on exfoliation characteristics, a cold-water quench below 25 deg C (75 deg F) would appear to be far superior to an elevated-temperature quench to minimize exfoliation for 7075 forgings in the T6 temper.

Abstract
The US Army Research Laboratory performed a failure investigation on a broken main landing gear mount from an AH-64 Apache attack helicopter. A component had failed in flight, and initially prevented the helicopter from safely landing. In order to avoid a catastrophe, the pilot had to perform a low hover maneuver to the maintenance facility, where ground crews assembled concrete blocks at the appropriate height to allow the aircraft to safely touch down. The failed part was fabricated from maraging 300 grade steel (2,068 MPa [300 ksi] ultimate tensile strength), and was subjected to visual inspection/light optical microscopy, metallography, electron microscopy, energy dispersive spectroscopy, chemical analysis, and mechanical testing. It was observed that the vacuum cadmium coating adjacent to the fracture plane had worn off and corroded in service, thus allowing pitting corrosion to occur. The failure was hydrogen-assisted and was attributed to stress corrosion cracking (SCC) and/or corrosion fatigue (CF). Contributing to the failure was the fact that the material grain size was approximately double the required size, most likely caused from higher than nominal temperatures during thermal treatment. These large grains offered less resistance to fatigue and SCC. In addition, evidence of titanium-carbo-nitrides was detected at the grain boundaries of this material that was prohibited according to the governing specification. This phase is formed at higher thermal treatment temperatures (consistent with the large grains) and tends to embrittle the alloy. It is possible that this phase may have contributed to the intergranular attack. Recommendations were offered with respect to the use of a dry film lubricant over the cadmium coated region, and the possibility of choosing an alternative material with a lower notch sensitivity. In addition, the temperature at which this alloy is treated must be monitored to prevent coarse grain growth. As a result of this investigation and in an effort to eliminate future failures, ARL assisted in developing a cadmium brush plating procedure, and qualified two Army maintenance facilities for field repair of these components.

Abstract
The accident at Three Mile Island Unit No. 2 on 28 March 1979 was the worst nuclear accident in US history. By Jan 1990, it was possible to electrochemically machine coupons from the lower head using a specially designed tool. The specimens contained the ER308L stainless steel cladding and the A533 Grade B plate material to a depth of about mid-wall. The microstructures of these specimens were compared to that of specimens cut from the Midland, Michigan reactor vessel, made from the same grade and thickness but never placed in service. These specimens were subjected to known thermal treatments between 800 and 1100 deg C for periods of 1 to 100 min. Microstructural parameters in the control specimens and in those from TMI-2 were quantified. Selective etchants were used to better discriminate desired microstructural features, particularly in the cladding. This report is a progress report on the quantification of changes in both the degree of carbide precipitation and delta ferrite content and shape in the cladding as a function of temperature and time to refine the estimates of the maximum temperatures experienced.

Abstract
Hardenability evaluation is typically applied to heat treatment process control, but can also augment standard metallurgical failure analysis techniques for steel components. A comprehensive understanding of steel hardenability is an essential complement to the skills of the metallurgical failure analyst. The empirical information supplied by hardenability analysis can provide additional processing and service insight to the investigator. The intent of this paper is to describe some applications of steel thermal response concepts in failure analysis, and several case studies are included to illustrate these applications.

Abstract
A deformed steel tube was received for failure analysis after buckling during a heat-treat operation. The tube was subjected to various metallurgical tests as well as nondestructive testing to confirm the presence of residual stresses. The microstructure of the tube was found to be homogenous and had no banded structure. However, x-ray diffraction analysis confirmed the presence of up to 6% retained austenite which likely caused the tube to buckle during the 910 °C heat treating procedure.

Abstract
A short fracture section of a forged and normalized Ck 35 (DIN 17200) steel slide showed three distinct zones: a dark colored crystalline area, an incipient crack propagating into a far advanced, rubbed fracture surface, and a fine crystalline final break. Metallographic examination showed the dark incipient crack was present before the last heat treatment and was oxidized and decarburized prior to the conclusion of the annealing process. The crack ran perpendicular to the fiber, so it was not formed before or during forging. It was a thermal stress crack produced during flame cutting of the middle section of the slide. The initial crack acted as a sharp notch favoring the formation of the fatigue fracture which lead to the failure of the slide.

Abstract
A T-bolt was part of the coupling for a bleed air duct of a jet engine on a transport plane. Specifications required that the 4.8 mm diam component be made of AISI type 431 stainless steel and heat treated to 44 HRC. The operating temperature of the duct is 425 to 540 deg C (800 to 1000 deg F), but that of the bolt is lower. The T-bolt broke after three years of service. The expected service life was equal to that of the aircraft. It was found that the bolt broke as a result of SCC. Thermal stresses were induced into the bolt by intermittent operation of the jet engine. Mechanical stresses were induced by tightening of the clamp around the duct, which in effect acted to straighten the bolt. The action of these stresses on the carbides that precipitated in the grain boundaries resulted in fracture of the bolt. Due to the operating temperatures of the duct near the bolt, the material was changed to A-286, which is less susceptible to carbide precipitation. The bolt is strengthened by shot peening and rolling the threads after heat treatment. Avoiding temperatures in the sensitizing range is desirable, but difficult to ensure because of the application.

Abstract
Argonne National Laboratory has conducted analyses of failed components from nuclear power-generating stations since 1974. The considerations involved in working with and analyzing radioactive components are reviewed here, and the decontamination of these components is discussed. Analyses of four failed components from nuclear plants are then described to illustrate the kinds of failures seen in service. The failures discussed are (1) intergranular stress-corrosion cracking of core spray injection piping in a boiling water reactor, (2) failure of canopy seal welds in adapter tube assemblies in the control rod drive head of a pressurized water reactor, (3) thermal fatigue of a recirculation pump shaft in a boiling water reactor, and (4) failure of pump seal wear rings by nickel leaching in a boiling water reactor.

Abstract
A pair of steam generators operating at a pressurized water reactor site were found to be leaking near a closure weld. The generators were the vertical U-tube type, constructed from ASTM A302 grade B steel. The shell material exhibited high hardness values prior to confirmatory heat treatment, indicating high residual stresses in the area of the weld. All cracks were transgranular and were associated with pits on the inside surfaces of the vessels. It was concluded that the cracking was caused by a low-cycle corrosion fatigue phenomenon, with cracks initiating at areas of localized corrosion and propagating by fatigue. An investigation was conducted to determine the factors responsible for the occasional formation of cracks on the parting lines of medium plain carbon and low-alloy medium-carbon steel forgings. The cracks were present on as-forged parts and grew during heat treatment. Examination revealed that areas near the parting line exhibited a large grain structure not present in the forged stock. High-temperature scale was also found in the cracks. It was concluded that the cracks were caused by material being folded over the parting line. The folding occurred because of a mismatch in the forgings and from material flow during trimming and/or material flow during forging.

Abstract
The spontaneous breakage of tempered glass spandrel panels used to cover concrete wall panels on building facades was investigated. Between January 1988 and August 1990, 19 panel failures were recorded. The tinted panels were coated on their exterior surfaces with a reflective metal oxide and covered on the back surfaces with an adherent black polyethylene plastic. Macro fractography, SEM fractography, EDX analysis, and photo elasticimetry were conducted on four of the shattered panels. Small nickel sulfide inclusions were found at the failure origins. Failure of the panels was attributed to growth of the inclusions, coupled with high residual stresses. Fracture mechanics analysis showed that the residual stresses alone were high enough to cause fracture of the glass, with a flaw of the size observed.

Abstract
The defects observed along weldings of stainless steel pipelines employed in marine environments were evidenced by metallographic and electrochemical examination. A compilation of cases on the effect of defective weldings, in addition to improper choice of stainless steel for water pipelines, lead to the conclusion that intercrystalline corrosion in steels involved precipitation of a surplus phase at grain boundaries. Intercrystalline corrosion in austenitic stainless steels due to precipitation of chromium carbides during conditions generated due to welding and ways to avoid the precipitation (including reduction of carbon content, appropriate heat treatment, cold work of steel, reduction of austenitic grain size and stabilizing elements) were described. The presence of microcracks due to highly localized heat concentrations with consequent thermal expansion and considerable shrinkages during cooling was investigated. Abstract
This article describes the characteristics of tools and dies and the causes of their failures. It discusses the failure mechanisms in tool and die materials that are important to nearly all manufacturing processes, but is primarily devoted to failures of tool steels used in cold-working and hot-working applications. It reviews problems introduced during mechanical design, materials selection, machining, heat treating, finish grinding, and tool and die operation. The brittle fracture of rehardened high-speed steels is also considered. Finally, failures due to seams or laps, unconsolidated interiors, and carbide segregation and poor carbide morphology are reviewed with illustrations.

Abstract
High temperature corrosion may occur in numerous environments and is affected by factors such as temperature, alloy or protective coating composition, time, and gas composition. This article explains a number of potential degradation processes, namely, oxidation, carburization and metal dusting, sulfidation, hot corrosion, chloridation, hydrogen interactions, molten metals, molten salts, and aging reactions including sensitization, stress-corrosion cracking, and corrosion fatigue. It concludes with a discussion on various protective coatings, such as aluminide coatings, overlay coatings, thermal barrier coatings, and ceramic coatings.

Abstract
A pipe in the lateral wall of a boiler powering an aircraft carrier flat-top boat failed during a test at sea. The pipe was made from ASTM 192 steel, an adequate material for the application. Microstructural analysis along with equipment operating records provided valuable insight into what caused the pipe to rupture. Although the pipe had been replaced just 50 h before the accident, the analysis revealed incrustations and corrosion pits on the inner walls and oxidation on the outer walls. Microstructural changes were also observed, indicating that the steel was exposed to high temperatures. The combined effect of pitting, incrustations, and phase transformations caused the pipe to rupture.

Abstract
Several fractures occurred in flange studs used for remote handling of radioactive equipment. The studs, of quenched-and-tempered type 414 stainless steel, fractured in the HAZs produced in the studs during the circumferential welding that joined the studs to the flanges. The weld deposits were of type 347 stainless steel, and the flanges were type 304 stainless steel. Metallographic examination of the failed studs revealed that the HAZs contained regions of martensite and that intergranular cracks, which initiated at the stud surfaces during welding, propagated to complete separation under subsequent loading. The studs fractured under service loads as a result of intergranular crack propagation in the HAZ. Rapid heating and cooling during attachment welding produced a martensitic structure in the HAZ of the stud, which cracked circumferentially from the combination of thermal-gradient and phase-change stresses. Joining the studs to the flanges by welding should be discontinued. They should be attached by screw threads, using a key and keyway to prevent turning in service.

Abstract
During 5.7 years of service, dye penetrant inspection of Inconel 800H pigtail connections regularly showed cracks at weld toes. Weld repairs were not able to prevent reoccurrence but often aggravated the condition. Samples containing small, but detectable, reducer-to-pigtail cracks showed intergranular cracks originating at weld toes and filled with oxidation product, which precluded determination of the cracking mechanism. All weldments exhibited high degrees of secondary precipitates, with original fabrication welds exhibiting higher apparent levels than repair welds. SEM/EDS analysis showed base metal grain boundary precipitates to be primarily chromium carbides, but some titanium carbides were also observed. Failure was believed to result from the synergism of thermally driven tube distortion, which resulted in over-stress, and from the intergranular oxidation products and intergranular carbides which contributed to cracking. It was recommended that stresses be reduced and /or that materials and components be changed. Refinements in welding procedures and implementation of preweld/postweld heat treatments were recommended also.
