Abstract
A cylindrical spiral gear, part of a locomotive axle assembly, cracked ten days after it had been press-fit onto a shaft, after which it sat in place as other repairs were made. Workers at the locomotive shop reported hearing a sound, and upon inspecting the gear, found a crack extending radially from the bore to the surface of one of the tooth flanks. The crack runs the entire width of the bore, passing through an oil hole in the hub, across the spoke plate and out to the tip of one of the teeth. Design requirements call for the gear teeth to be carburized, while the remaining surfaces, protected by an anti-carburizing coating, stay unchanged. Based on extensive testing, including metallographic examination, microstructural analysis, microhardness testing, and spectroscopy, the oil hole was not protected as required, evidenced by the presence of a case layer. This oversight combined with the observation of intergranular fracture surfaces and the presence of secondary microcracks in the case layer point to hydrogen embrittlement as the primary cause of failure. It is likely that hydrogen absorption occurred during the gas carburizing process.

Abstract
During a routine shear-pin check, the end lug on the barrel of the forward canopy actuator on a naval aircraft was found to have fractured. The lug was forged from aluminum alloy 2014-T6. Investigation (visual inspection, 2x views, and 140X micrographs etched with Keller's reagent) supported the conclusion that the cause of failure was SCC resulting from exposure to a marine environment. The fracture occurred in normal operation at a point where damage from pitting and intergranular corrosion acted as a stress raiser, not because of overload. The pitting and intergranular attack on the lug were evidence that the surface protection of the part had been inadequate as manufactured or had been damaged in service and not properly repaired in routine maintenance. Recommendations included anodizing the lug and barrel in sulfuric acid and giving them a dichromate sealing treatment, followed by application of a coat of paint primer. During routine maintenance checks, a careful examination was suggested to look for damage to the protective coating, and any necessary repairs should be made by cleaning, priming, and painting. Severely corroded parts should be removed from service.

Abstract
Aluminum alloy BS.1476-HE.15 by virtue of its high strength and low density finds application in the form of bars or sections for cranes, bridges, and other such structures where a reduction in dead weight load and inertia stresses is advantageous. Bars and sections in H.15 alloy are mostly produced by extrusion. Some material processed this way has been prone to exfoliation corrosion. Extended aging for 24 h at a temperature of 185 deg C (365 deg F) virtually suppresses the tendency for exfoliation corrosion to develop. Also, the use of a sprayed coating, either of aluminum or Al-1Zn alloy, was effective in halting and preventing this form of attack. While alarming, the appearance of exfoliation corrosion provides a valuable warning to the engineer or inspector before a severe weakening of the particular sections has occurred.

Abstract
Gas turbines and other types of combustion turbomachinery are susceptible to hot corrosion at elevated temperatures. Two such cases resulting in the failure of a gas turbine component were investigated to learn more about the hot corrosion process and the underlying failure mechanisms. Each component was analyzed using optical and scanning electron microscopy, energy dispersive spectroscopy, mechanical testing, and nondestructive techniques. The results of the investigation provide insights on the influence of temperature, composition, and microstructure and the contributing effects of high-temperature oxidation on the hot corrosion process. Preventative measures are also discussed.

Abstract
The outlet-piping system of a steam-reformer unit failed by extensive cracking at four weld locations. The welded system consisted of Incoloy 800 (Fe-32Ni-21Cr-0.05C) pipe and fittings. The exterior surfaces of the system were insulated with rock wool that did not contain weatherproofing. On-site visual examination and magnetic testing indicated severe external corrosion of most of the piping. The system showed extensive cracking in weld HAZ. One specimen indicated that corrosion extended to a depth of 3.2 mm and cracks were seen at the edge of the cover bead and in the HAZ of the weld. Metallographic examination showed that cracking was intergranular and that adjacent grain boundaries had undergone deep intergranular attack. Examination at higher magnification revealed heavy carbide precipitation, primarily at grain boundaries, indicating that the alloy had been sensitized, which resulted from heating during welding. Electron probe x-ray microanalysis showed the outside surface of the tube did not have the protective chromium oxide scale normally found on Incoloy 800. The inside surface of the tube had a thin chromium oxide protective scale. This evidence supported the conclusions that the deep oxidation greatly decreased the strength of the weld HAZ and cracking followed.

Abstract
This paper describes an investigation on the failure of a large leaded bronze bearing that supports a nine-ton roller of a plastic calendering machine. At the end of the normal service life of a good bearing, which lasted for seven years, a new bearing was installed. However the new one failed catastrophically within a few days, generating a huge amount of metallic wear debris and causing pitting on the surface of the cast iron roller. Following the failure, samples were collected from both good and failed bearings. The samples were analyzed chemically and their microstructures examined. Both samples were subjected to accelerated wear tests in a laboratory type pin-on-disk apparatus. During the tests, the bearing materials acted as pins, which were pressed against a rotating cast iron disk. The wear behaviors of both bearing materials were studied using weight loss measurement. The worn surfaces of samples and the wear debris were examined by light optical microscope, SEM, and energy-dispersive x-ray microanalyzer. It was found that the laboratory pin-on-disk wear data correlated well with the plant experience. It is suggested that the higher lead content ~18%) of the good bearing compared with 7% lead of the failed bearing helped to establish a protective transfer layer on the worn surface. This transfer layer reduced metal-to-metal contact between the bearing and the roller and resulted in a lower wear rate. The lower lead content of the failed bearing does not allow the establishment of a well-protected transfer layer and leads to rapid wear.

Abstract
During disassembly of an engine that was to be modified, a fractured turbine blade was found. When the fracture was examined at low magnification, it was observed that a fatigue fracture had originated on the concave side of the leading edge and had progressed slightly more than halfway from the leading edge to the trailing edge on the concave surface before ultimate failure occurred in dynamic tension. Analysis (including visual inspection, SEM, and 250x/500x micrographic examination) supported the conclusions that the blades failed due to thermal fatigue. Recommendations included application of a protective coating to the blades, provided the coating was sufficiently ductile to avoid cracking during operation to prevent surface oxidation. Such a coating would also alleviate thermal differentials, provided the thermal conductivity of the coating exceeded that of the base metal. It was also determined that directionally solidified blades could minimize thermal fatigue cracking by eliminating intersection of grain boundaries with the surface. However, this improvement would be more costly than applying a protective coating.

Abstract
A failed H-11 tool steel pylon attachment stud was found during a routine walk-around inspection. The stud exhibited gross localized corrosion pitting at several different areas on its surface. Light general rust was also evident. Severe pitting occurred near the fracture location. The fracture face contained evidence of intergranular SCC as well as ductile dimples. The protective coating was found to be an inorganic water-base aluminide coating having a coating thickness of 7.5 to 13 micron (0.3 to 0.5 mil). The coating was of a nonuniform mottled nature. It was concluded that the failure of the pylon attachment stud was caused by general corrosion followed by SCC. The stud was not adequately protected against corrosion by the coating. It was recommended that the coating be applied to a thickness of 38 to 75 micron (1.5 to 3 mil) to provide long-time corrosion resistance. The coating must be either burnished or cured at 540 deg C (1000 deg F) to provide cathodic protection to the steel. Other coatings, such as cadmium or aluminum, were also recommended if a thinner coating is needed.

Abstract
The 4340 steel main rotor yoke of a helicopter failed during a hovering exercise. Visual examination of the yoke revealed no evidence of gross external damage. Visual fracture surface examination, macrofractography, scanning electron micrography, and metallography of a section cut from the yoke in the region of the cracking indicated that the failure was caused by fatigue-crack initiation and growth from severe corrosion damage to a pillow-block bolt hole. Corrosion occurred because of failure of the protection scheme. An upgraded corrosion protection scheme for the bolt holes was recommended, along with nondestructive inspection of the region at intervals determined by fractographic analysis of the fatigue crack growth.

Abstract
A cast iron cylinder liner from a diesel engine suffered localized damage on the cooling water side leading to serration of the edges and heavy pitting. This heavy damage was cavitation damage, frequently observed in diesel motor cylinders. To combat such damage the following measures are recommended in the specialist literature: reduction in piston play; reduction in the amplitude by thicker-walled linings; hard chromizing of the cooling water side; and, addition of a protective oil to the cooling water. The effect of the protective oil is presumably based on a film of oil which forms on the cylinder surface and which is not so easily scoured off during vibration. The effect of the imploding vacuum bubbles is reduced by the oil film which can renew itself from the emulsion.

Abstract
On 9 March 2000, a gasoline pipeline failed near Greenville, TX releasing approximately 12,000 barrels of fuel. After the on-scene portion of the investigation was completed, an 8.5 ft. (2.6 m) section of the 28 in. (71 cm) diam pipe was sent to the materials laboratory for examination. Examination included optical and scanning electron microscopy of the fracture surfaces and metallographic examination of cross sections through the fracture surface. From the outer to inner edge of the fracture surface, three different areas were observed. Fracture features in area 1 were obliterated by corrosion. The fracture features in region 2 were relatively smooth, and striations were observed, typical of fatigue. In region 3, dimple features were observed, typical of ductile overstress. Also, corrosion pits were observed on the outer surface of the pipe section in locations where the protective black tar-like coating was cracked.

Abstract
A 321 stainless steel radar coolant-system assembly fabricated by torch brazing with AWS type 3A flux, failed at the brazed joint when subjected to mild handling before installation, after being stored for about two years. It was revealed by visual examination of the failed braze that the filler metal had not covered all mating surfaces. Lack of a metallurgical bond between the brazing alloy and stainless steel and instead mechanical bonding of the filler metal to an oxide layer on the stainless steel surface was revealed by examination of the broken joint at the cup. It was indicated by the thickness of the oxide layer that the steel surface was not protected from oxidation by the flux during torch heating. It was concluded that the failure was caused by lack of a metallurgical bond between the brazing alloy and the stainless steel. Components made of 347 stainless steel (better brazeability) brazed with a larger torch tip (wider heat distribution) and AWS type 3B flux (better filler-metal flow) were recommended for radar coolant-system assembly.

Abstract
A corrosion resistant chromium nickel steel (X 2 Cr-Ni-Mo 18 10) worm drive used in a chemical plant at 80 deg C and 100 to 200 atm pressure to transport media containing chloride failed during normal operation. Visual inspections showed that the entire surface of the gear was covered with fine branching cracks and was flaking off. Microscopic examination showed that the unetched polished material had disintegrated to an average depth of 1 mm below the surface. A micrograph of the etched surface revealed numerous deformation lines and transgranular cracking. The failure was thus due to stress-corrosion cracking and additional corrosion due to ventilation elements. Because austenitic chromium nickel steels are prone to stress-corrosion cracking, particularly in the presence of chlorine compounds at high temperatures, and because austenitic rust- and acid-resistant steels are prone to smearing and work hardening during machining, it was recommended that these types of steels be machined only with sharp, short tools mounted in rigid structures. In addition, residual stresses should be eliminated by post-process annealing in a protective atmosphere.

Abstract
A forged aluminum alloy 2014-T6 catapult-hook attachment fitting (anodized by the chromic acid process to protect it from corrosion) from a naval aircraft broke in service. Spectrographic analysis, visual examination, microscopic examination, and tensile analysis showed minute cracks on the inside surface of a bearing hole, and small areas of pitting corrosion were visible on the exterior surface of the fitting. The analysis also revealed a small number of rosettes, suggestive of eutectic melting, in an otherwise normal structure. These examinations and analyses support the conclusion that the presence of chromic acid stain on the fracture surface proved that the forging had cracked before anodizing. This suggest that the crack initiated during straightening, either after machining or after heat treatment. The structure and composition of the alloy appear to have been acceptable. Ductility was acceptable so rosettes found in the microstructure are believed to have been nondamaging. Had they contributed to the failure, the ductility would have been very low. The recommendations included inspection for cracks and revising the manufacturing process to include a fluorescent liquid-penetrant inspection before anodizing, because chromic acid destroys the penetrant. This inspection would reduce the possibility of cracked parts being used in service.

Abstract
Piping and structural components used in space launch facilities such as NASA's Kennedy Space Center and the Air Force's Cape Canaveral Air Station face extreme operating conditions. Launch effluent and residue from solid rocket boosters react with moisture to form hydrochloric acid that settles on exposed surfaces as they are being subjected to severe mechanical loads imparted during lift-off. Failure analyses were performed on 304 stainless steel tubing that ruptured under such conditions, while carrying various gases, including nitrogen, oxygen, and breathing air. Hydrostatic testing indicated a burst strength of 13,500 psi for the intact sections of tubing. Scanning electron microscopy and metallographic examination revealed that the tubing failed due to corrosion pitting exacerbated by stress-corrosion cracking (SCC). The pitting originated on the outer surface of the tube and ranged from superficial to severe, with some pits extending through 75% of the tube's wall thickness. The SCC emanated from the pits and further reduced the service strength of the component until it could no longer sustain the operating pressure and final catastrophic fracture occurred. Corrosion-resistant coatings added after the investigation have proven effective in preventing subsequent such failures.

Abstract
An aluminum brass seawater surface condenser failed due to pitting after less than one year of service. Large pits filled with a green deposit were evidenced under the nonuniform black scale present over the entire inside surface of the tube. The black deposit was identified as primarily copper sulfide, with zinc and aluminum sulfides while the green deposit was revealed to be copper chloride. The combination of sulfide and chloride attack on the tubes was concluded to have resulted in the failure. Injection of ferrous sulfate upstream of the condenser which could aid the formation of protective oxide films was recommended.

Abstract
Post-service destructive evaluation was performed on two commercially pure zirconium pump impellers. One impeller failed after short service in an aqueous hydrochloric acid environment. Its exposed surfaces are bright and shiny, covered with pockmarks, and peppered with pitting. Uniform corrosion is evident and two deep linear defects are present on impeller blade tips. In contrast, the undamaged impeller surfaces are covered with a dark oxide film. This and many other impellers in seemingly identical service conditions survive long lives with little or no apparent damage. No material or manufacturing defects were found to explain the different service performance of the two impellers. Microstructure, microhardness and material chemistry are consistent with the specified material. Examination reveals the damage mechanism to be corrosion-enhanced cavitation erosion, the most severe form of erosion corrosion. Cavitation damage to the protective oxide film caused the zirconium to lose its normally outstanding corrosion resistance. The root cause of the impeller failure is most likely the introduction of excessive air into the pump due to low liquid level, a bad seal or inadequate head. Corrosion pitting, crevice corrosion, and solidification cracks (casting defect) also contributed to the failure.

Rough operation of the roller bearing mounted in an electric motor/gearbox assembly was observed. The bearing components made of low-alloy steel (4620 or 8620) and the cup, cone and rollers were carburized, hardened and tempered. The contact surfaces of these components (cup, cone and roller) were revealed to be uniformly electrolytically etched by visual examination. The action similar to anodic etching was believed to have occurred as a result of stray currents in the electric motor (not properly grounded) and the presence of an electrolyte (moisture) between the cup and roller surfaces of the bearing. As a remedial action, the bearing was insulated for protection from stray currents by grounding of the motor and the moisture was kept out by sealing both bearings in the assembly.

Abstract
High-chromium steel pipes 42.25 x 3.25 mm from a blast furnace gas fired recuperator for the preheating of air were heavily oxidized and perforated in places. It was found that the blast furnace gas had a high sulfur content. Both the carburization and the formation of sulfide proved that in addition, from time to time at least, combustion was incomplete and the operation was carried out in a reducing atmosphere, with the result that oxygen deficiency prevented the formation or maintenance of a protective surface layer on the external surface of the pipes. The sulfur would probably not have damaged the nickel-free steel used here at the given temperatures if it had been present as sulfur dioxide in an oxidizing atmosphere. The damage was therefore caused primarily by an incorrectly conducted combustion process.
