Abstract
The cause of low fatigue life measurements obtained during routine fatigue testing of IMI 550 titanium alloy compressor blades used in the first stage of the high-pressure compressor of an aeroengine was investigated. The origin of the fatigue cracks was associated with a spherical bead of metal sticking to the blade surface in each case. Scanning electron microscope revealed that the cracks initiated at the point of contact of the bead with the blade surface. Energy-dispersive X-ray analysis indicated that the bead composition was the same as that of the blade. Detailed investigation revealed that fused material from the blade had been thrown onto the cold blade surface during a grinding operation to remove the targeting bosses from the forgings, thereby causing local embrittlement. It was recommended that extreme care be taken during grinding operations to prevent the hot, fused particles from striking the blade surface.

Abstract
Due to the recent requirement of higher integration density, solder joints are getting smaller in electronic product assemblies, which makes the joints more vulnerable to failure. Thus, the root-cause failure analysis for the solder joints becomes important to prevent failure at the assembly level. This article covers the properties of solder alloys and the corresponding intermetallic compounds. It includes the dominant failure modes introduced during the solder joint manufacturing process and in field-use applications. The corresponding failure mechanism and root-cause analysis are also presented. The article introduces several frequently used methods for solder joint failure detection, prevention, and isolation (identification for the failed location).

Abstract
Two titanium alloy wing attachment bolts from a commercial jetliner failed during the course of a routine service operation. Failure of the bolts occurred during the re-torque process as the wing was being reattached. Metallurgical failure analysis indicated that the fracture mechanism was ductile overload and that the mechanical properties of the bolts were consistent with exemplar bolts that had been supplied. After eliminating other sources of excessive load application, the most probable cause of failure was ascribed to variances between the frictional characteristics of the bolt at the time of re-torque and at the time of initial torque application several years earlier.

Abstract
This article covers the three most popular techniques used to characterize the very outermost layers of solid surfaces: Auger electron spectroscopy (AES), X-ray photoelectron spectroscopy (XPS), and time-of-flight secondary ion mass spectrometry (TOF-SIMS). Some of the more important attributes are listed for preliminary insight into the strengths and limitations of these techniques for chemical characterization of surfaces. The article describes the basic theory behind each of the different techniques, the types of data produced from each, and some typical applications. Also discussed are the different types of samples that can be analyzed and the special sample-handling procedures that must be implemented when preparing to do failure analysis using these surface-sensitive techniques. Data obtained from different material defects are presented for each of the techniques. The examples presented highlight the typical data sets and strengths of each technique.

Abstract
X-ray spectroscopy is generally accepted as the most useful ancillary technique that can be added to any scanning electron microscope (SEM), even to the point of being considered a necessity by most operators. While “stand-alone” x-ray detection systems are used less frequently in failure analysis than the more exact instrumentation employed in SEMs, the technology is advancing and is worthy of note due to its capability for nondestructive analysis and application in the field. This article begins with information on the basis of the x-ray signal. This is followed by information on the operating principles and applications of detectors for x-ray spectroscopy, namely energy-dispersive spectrometers, wavelength-dispersive spectrometers, and handheld x-ray fluorescence systems. The processes involved in x-ray analysis in the SEM and handheld x-ray fluorescence analysis are then covered. The article ends with a discussion on the applications of x-ray spectroscopy in failure analysis.

Abstract
This article presents the mechanisms of polymer wear and quantifies wear in terms of wear rate (rate of removal of the material). Interfacial and bulk wear are discussed as well as a discussion on the wear study of "elastomers," "thermosets," "glassy thermoplastics," and "semicrystalline thermoplastics." The article also discusses the effects of environment and lubricant on the wear failures of polymers. It presents a case study on considering nylon as a tribological material and failure examples, explaining wear resistance of polyurethane elastomeric coatings and failure of an acetal gear wheel.

Abstract
The carbon steel feedwater piping at a waste-to-energy plant was suffering from wall thinning and leaking after being in service for approximately six years. Metallographic examination of ring sections removed front the piping revealed a normal microstructure consisting of pearlite and ferrite. However, the internal surface on the thicker regions of the rings exhibited significant deposit buildup, where the thinned regions showed none. No significant corrosion or pitting was observed on either the internal or external surface of the piping. The lack of internal deposits on the affected areas and the evidence of flow patterns indicated that the wall thinning and subsequent failure were caused by internal erosion damage. The exact cause of the erosion could not be determined by the appearance of the piping. Probable causes of the erosion include an excessively high velocity flow through the piping, extremely turbulent flow, and/or intrusions (weld backing rings or weld bead protrusions) on the internal surface of the pipes. Increasing the pipe diameter and decreasing the intrusions on the internal surface would help to eliminate the problem.

Abstract
Plastics or polymers are used in a variety of engineering and nonengineering applications where they are subjected to surface damage and wear. This article discusses the classification of polymer wear mechanisms based on the methodologies of defining the types of wear. The first classification is based on the two-term model that divides wear mechanisms into interfacial and bulk or cohesive. The second is based on the perceived wear mechanism. The third classification is specific to polymers and draws the distinction based on mechanical properties of polymers. In this classification, wear study is separated as elastomers, thermosets, glassy thermoplastics, and semicrystalline thermoplastics. The article describes the effects of environment and lubricant on the wear failures of polymers. It presents a case study on nylon as a tribological material. The article explains the wear failure of an antifriction bearing, a nylon driving gear, and a polyoxymethylene gear wheel.

Abstract
After being in service for ten years, two admiralty brass heat-exchanger tubes from a cooler in a refinery catalytic reforming unit cracked circumferentially in the area of U-bends. A blunt transgranular cracking with minimal branching propagating from the inside surface of the tube was revealed by metallography which was typical of cracking by corrosion fatigue mechanism. Corrosion deposits on both the inside- and outside-diam surfaces were found in the tubes. The presence of copper, zinc, iron, and small amounts of chloride, sulfur, silicon, tin, and manganese was revealed by energy-dispersive analysis of the deposits. It was interpreted by the hardness values (higher than typical for annealed copper tubing) that the tubes may not have been annealed after the U-bends were formed and thus the role of residual stresses in the crack was revealed. It was concluded that the tubes failed by corrosion fatigue initiated by pitting at the inside-diam surface. The tubes were recommended to be annealed after bending to reduce residual stresses from the bending operation to an acceptable level.

Abstract
Proper stress analysis during component design is imperative for accurate life and performance prediction. The total stress on a part is comprised of the applied design stress and any residual stress that may exist due to forming or machining operations. Stress-corrosion cracking may be defined as the spontaneous failure of a metal resulting from the combined effects of a corrosive environment and the effective component of tensile stress acting on the structure. However, because of the orientation dependence in aluminum, it is the residual stress occurring in the most susceptible direction that must be considered of primary importance in material selection for design configuration. A Navy UH-1N helicopter main rotor blade grip manufactured from a 2014-T6 aluminum alloy forging failed because of a design flaw that left a high residual tensile stress along the short transverse plane; this in turn provided the necessary condition for stress corrosion to initiate. A complete failure investigation to ascertain the exact cause of the failure was conducted utilizing stereomicroscopic examination, scanning electron microscopy, metallographic inspection and interpretation, energy-dispersive chemical analysis, physical and mechanical evaluation. Stereomicroscopic examination of the opened crack fracture surface revealed one large fan-shaped region that had propagated radially through the thickness of the material from two distinct origin areas on the internal diam of the grip. Higher magnification inspection near the origin area revealed a flat, wood-like appearance. Scanning electron microscopy divulged the presence of substantial mud cracking and intergranular separation on the fracture surface. Metallographic examination revealed intergranular cracking and substantial leaf separation along the elongated grains parallel to the fracture surface. Chemical composition and hardness requirements were found to be as specified. The blade grip failed due to a stress corrosion crack which initiated on the inner diam and propagated in the short transverse direction through the thickness of the component. The high residual tensile stress in the part resulting from the forging and exposed after machining of the inner diam, combined with the presence of moisture, provided the necessary conditions to facilitate crack initiation and propagation.

Abstract
This paper describes an investigation on the failure of a large leaded bronze bearing that supports a nine-ton roller of a plastic calendering machine. At the end of the normal service life of a good bearing, which lasted for seven years, a new bearing was installed. However the new one failed catastrophically within a few days, generating a huge amount of metallic wear debris and causing pitting on the surface of the cast iron roller. Following the failure, samples were collected from both good and failed bearings. The samples were analyzed chemically and their microstructures examined. Both samples were subjected to accelerated wear tests in a laboratory type pin-on-disk apparatus. During the tests, the bearing materials acted as pins, which were pressed against a rotating cast iron disk. The wear behaviors of both bearing materials were studied using weight loss measurement. The worn surfaces of samples and the wear debris were examined by light optical microscope, SEM, and energy-dispersive x-ray microanalyzer. It was found that the laboratory pin-on-disk wear data correlated well with the plant experience. It is suggested that the higher lead content ~18%) of the good bearing compared with 7% lead of the failed bearing helped to establish a protective transfer layer on the worn surface. This transfer layer reduced metal-to-metal contact between the bearing and the roller and resulted in a lower wear rate. The lower lead content of the failed bearing does not allow the establishment of a well-protected transfer layer and leads to rapid wear.

Abstract
A stamped coin exhibited visible discolored areas, seen as a tan haze on the surface. The discoloration was considered merely cosmetic. The nonstained and stained regions were studied using SEM/EDS. Greater amounts of aluminum and magnesium were found in the stained area as compared with the nonstained region. Some carbon and oxygen were detected in both areas, which may be suggestive of organic substances. Fourier transform infrared spectroscopy (FTIR) revealed traces of hydrocarbons and ether/alcohol materials in the stained area, suggesting that the stain was associated with a cellulose or carbohydrates (sugars). These findings, along with the appearance, suggest that a sugar-containing substance, such as coffee or a soft drink, dried onto the surface of this coin and caused the observed discoloration.

Abstract
Leakage was identified around a coupling welded into a stainless steel holding tank that stored condensate water with low impurity content. The tank and fitting were manufactured from type 304 stainless steel. The coupling joint consisted of an internal groove weld and an external fillet weld. Cracking was found to be apparent on the tank surface, adjacent to the coupling weld. Chlorine, carbon, and oxygen in addition to the base metal elements were revealed by energy-dispersive x-ray spectrometric analysis. A great number of secondary, branching cracks were evident in the weld, heat-affected zone, and base metal. The branching and transgranular cracking was found to emanate primarily from the exterior of the tank. It was concluded that the tank failed as a result of stress-corrosion cracking that initiated at the exterior surface as aqueous chlorides, especially within an acidic environment, have been shown to cause SCC in austenitic stainless steels under tensile stress.

Abstract
A cadmium-plated 4340 Ni-Cr-Mo steel ballast elbow assembly was submitted for failure analysis to determine the element or radical present in an oxidation product found inside the elbow assembly. Energy-dispersive x-ray analysis in the SEM showed that iron was the predominant species, presumably in an oxide form. The inside surface had the appearance of typical corrosion products. Hardness measurements indicated that the 4340 steel was heat treated to a strength of approximately 862 MPa (125 ksi). It was concluded that the oxide detected on the ballast elbow was iron oxide. The possibility that the corrosion products would eventually create a blockage of the affected hole was great considering the small hole diameter (4.2 mm, or 0.165 in.). It was recommended that a quick fix to stop the corrosion would be to apply a corrosion inhibitor inside the hole. This, however, would cause the possibility of inhibitor buildup and the eventual clogging of the hole. A change in the manufacturing process to include a cadmium plating on the hole inside surface was recommended. This was to be accomplished in accordance with MIL specification QQ-P-416, Type II, Class 1. A material change to 300-series stainless steel was also recommended.

Abstract
A forged, cadmium-plated electroslag remelt (ESR) 4340 steel mixer pivot support of the rotor support assembly located on an Army attack helicopter was found to be broken in two pieces during an inspection. Visual inspection of the failed part revealed significant wear on surfaces that contacted the bushing and areas at the machined radius where the cadmium coating had been damaged, which allowed corrosion pitting to occur. Optical microscopy showed that the crack origin was located at the machined radius within a region that was severely pitted. Electron microscopy revealed that most of the fracture surface failed in an intergranular fashion. Energy dispersive spectroscopy determined that deposits of sand, corrosion and salts were found within the pits. The failure started by hydrogen charging as a result of corrosion, and was aggravated by the stress concentration effects of pitting at the radius and the high notch sensitivity of the material. The failure mechanism was hydrogen-assisted and was most likely a combination of stress-corrosion cracking and corrosion fatigue. Recommendations were to improve the inspection criteria of the component in service and the material used in fabrication.
