Abstract
This article describes the underlying fundamentals, applications, the relevance and necessity of performing proper stress analysis in conducting a failure analysis. It presents an introduction to the stress analysis of bodies containing crack-like imperfections and the topic of fracture mechanics. The fracture mechanics approach is an important part of stress analysis at the tips of sharp cracks or discontinuities. The article reviews fracture mechanics concepts, including linear elastic fracture mechanics, elastic-plastic fracture mechanics, and subcritical fracture mechanics. It also provides information on the applications of fracture mechanics in failure analysis.

Abstract
Engineered components fail predominantly in four major ways: fracture, corrosion, wear, and undesirable deformation (i.e., distortion). Typical fracture mechanisms feature rapid crack growth by ductile or brittle cracking; more progressive (subcritical) forms involve crack growth by fatigue, creep, or environmentally-assisted cracking. Corrosion and wear are another form of progressive material alteration or removal that can lead to failure or obsolescence. This article primarily covers the topic of abrasive wear failures, covering the general classification of wear. It also discusses methods that may apply to any form of wear mechanism, because it is important to identify all mechanisms or combinations of wear mechanisms during failure analysis. The article concludes by presenting several examples of abrasive wear.

Abstract
Several pressurized air containers (i.e., diving tanks) made of non-heat-treatable Al-5Mg aluminum alloy failed catastrophically. Catastrophic failure occurred when a subcritical stress corrosion crack reached a critical size. Critical crack size for unstable propagation was reached prior to wall penetration, which could have led to subsequent loss of pressure, resulting in explosion of the cylinder. It was recommended that more stress corrosion resistant alloys be used for sea diving applications. Furthermore, cylinders should have a reduced wall thickness that can be determined employing the “leak-before-break” design philosophy, developed using fracture mechanics, to eliminate the possibility of catastrophic ruptures.

Abstract
An air system on a marine platform unexpectedly shut down due to the failure of a union nut, which led to an investigation to quantify the material limitations of bronze alloys in corrosive marine environments. The study focused on two alloys: Al-Si bronze, as used in the failed component, and Ni-Al bronze, which has a history of success in naval applications. Material samples were examined using chemical analysis, SEM imaging, and corrosion testing. Investigators also analyzed precracked tension specimens, exposing them to different conditions to quantify stress intensity thresholds for environmentally assisted cracking. Al-Si bronze was found to be susceptible to subcritical intergranular cracking in air and seawater, whereas Ni-Al bronze was unaffected. Both materials, however, are susceptible to cracking in the presence of ammonia, although the subcritical crack growth rate is two to three times higher in Ni-Al bronze. Based on the results of this work, the likelihood of subcritical cracking under various conditions can be reasonably estimated, which, in the case at hand, proved to be quite high.

Abstract
Cracks were discovered in the cast 17-4 PH stainless steel outboard leading edge flap support of an aircraft wing during overhaul inspection. Failure analysis focused on an apparently intergranular area of fracture surface. It was determined that the original mode of crack growth was cleavage, probably caused by cast-in hydrogen. The intergranular appearance resulted from heat treatment of the already cracked part, which caused the formation of grain-boundary “growth figures” on the exposed crack surfaces. It was recommended that the castings be more closely inspected for defects before further processing and that foundry practices be reviewed to correct deficiencies leading to excessive hydrogen absorption during melting and casting.

Abstract
Metal-induced embrittlement is a phenomenon in which the ductility or fracture stress of a solid metal is reduced by surface contact with another metal in either liquid or solid form. This article summarizes the characteristics of solid metal induced embrittlement (SMIE) and liquid metal induced embrittlement (LMIE). It describes the unique features that assist in arriving at a clear conclusion whether SMIE or LMIE is the most probable cause of the problem. The article briefly reviews some commercial alloy systems where LMIE or SMIE has been documented. It also provides some examples of cracking due to these phenomena, either in manufacturing or in service.

Abstract
This article describes concepts and tools that can be used by the failure analyst to understand and address deformation, cracking, or fracture after a stress-related failure has occurred. Issues related to the determination and use of stress are detailed. Stress is defined, and a procedure to deal with stress by determining maximum values through stress transformation is described. The article provides the stress analysis equations of typical component geometries and discusses some of the implications of the stress analysis relative to failure in components. It focuses on linear elastic fracture mechanics analysis, with some mention of elastic-plastic fracture mechanics analysis. The article describes the probabilistic aspects of fatigue and fracture. Information on crack-growth simulation of the material is also provided.

Abstract
Metal-induced embrittlement is a phenomenon in which the ductility or the fracture stress of a solid metal is reduced by surface contact with another metal in either the liquid or solid form. This article summarizes some of the characteristics of liquid-metal- and solid-metal-induced embrittlement. This phenomenon shares many of these characteristics with other modes of environmentally induced cracking, such as hydrogen embrittlement and stress-corrosion cracking. The discussion covers the occurrence, failure analysis, and service failures of the embrittlement. Abstract
A bracket which formed part of the carrier of a chain conveyor system used to transport components through a continuous oven fractured. A brittle crack originated on the inside of the right-angled bend, the surface having oxidized subsequently. The remaining portion of the fracture resulted from fatigue. Shallow oxidized regions adjacent to the inside surface of the bend indicated pre-existing cracks. A sulphur print on the edge of the bracket showed the material was rolled from a rimming steel ingot. The general appearance of the fracture, and the fact failure took place where embrittlement had developed following plastic straining and service at a temperature of 260 deg C (500 deg F) suggested that failure resulted from strain-age embrittlement.

Abstract
A ductile iron T-hook hook was reported to have fractured in service. It was further reported that the hook had been subjected to a load that did not exceed 5900 kg (13,000 lb) at the time of fracture. No information was provided regarding the type of metal used to manufacture the hook. A failure analysis was requested to determine the cause of fracture. Two hooks were submitted for examination. Analysis (visual inspection, 2.7x light fractography, chemical analysis, 110x SEM fractography, 27x/110x/215x nital-etched micrographs) supported the conclusions that this component fractured in service as a consequence of ductile tensile overload. Evidence indicates that the fractured region was subjected to a load exceeding the capacity of the material. Because the information available from the service application indicated that the component had not been subjected to a stress that exceeded 5900 kg (13,000 lb), the observations made in this investigation suggested that either the load was underestimated or that the indicated load was applied at a more rapid rate (perhaps with a jerk), which would tend to increase the effective force of the load.

This article reviews some mechanistic aspects of ductile and brittle crack propagation, including a discussion on mixed-mode cracking, which may also occur when an overload failure is caused by a combination of ductile and brittle cracking mechanisms. It describes the general aspects of fracture modes and mechanisms. The article discusses some of the material, mechanical, and environmental factors that may be involved in determining the root cause of an overload failure. It also presents examples of thermally and environmentally induced embrittlement effects that can alter the overload fracture behavior of metals.

Abstract
A piece of wheel flange separated from the main landing gear wheel of a C130 aircraft as it taxied on a runway. The wheel was a 2014-T61 aluminum alloy forging and had been in service nearly 20 years. Fractographic evidence indicated that the initial crack growth was caused by high-cycle fatigue. The crack grew to approximately 8 in. in length before final catastrophic fracture. Fatigue analyses accurately predicted the cyclic life demonstrated by the failed wheel since its last inspection, assuming an initial crack length of 13 to 25 mm (0.5 to 1.0 in.). It was recommended that the inspection interval be reduced to one-third of its original duration for the current level of inspection reliability, or that inspection procedures be improved in order that cracks substantially smaller than 13 mm (0.5 in.) can be reliably detected.

Abstract
Following an accident in which a light pickup truck left the road and overturned, one of the rear axles, made of approximately 0.30C steel, was found to be fractured adjacent to the bearing lock nut. A keyway was present in the failed area, as were threads for the lock nut. Fracture surfaces of the failed axle and exemplar fractures obtained from simulation tests were studied using scanning electron microscope. The examination showed that the outer perimeter fracture in the axle was very flat and composed of cleavage and that the interior portion was composed of both cleavage and dimples. No evidence of prior cracking was found. The exemplar specimens from the simulation impact testing failed in a manner consistent with that observed in the axle. The examination confirmed that the failure was a one-time impact overload fracture and not the result of any prior crack in the material, indicating that the axle failure did not initiate the accident.

Life assessment of structural components is used to avoid catastrophic failures and to maintain safe and reliable functioning of equipment. The failure investigator's input is essential for the meaningful life assessment of structural components. This article provides an overview of the structural design process, the failure analysis process, the failure investigator's role, and how failure analysis of structural components integrates into the determination of remaining life, fitness-for-service, and other life assessment concerns. The topics discussed include industry perspectives on failure and life assessment of components, structural design philosophies, the role of the failure analyst in life assessment, and the role of nondestructive inspection. Abstract
Failure of structural polymeric materials under cyclic application of stress or strain is a subject of industrial importance. The understanding of fatigue mechanisms (damage) and the development of constitutive equations for damage evolution, leading to crack initiation and propagation as a function of loading or displacement history, represent a fundamental problem for scientists and engineers. This article describes the approaches to predict fatigue life and discusses the difference between thermal and mechanical fatigue failure of polymers.

This article aims to identify and illustrate the types of overload failures, which are categorized as failures due to insufficient material strength and underdesign, failures due to stress concentration and material defects, and failures due to material alteration. It describes the general aspects of fracture modes and mechanisms. The article briefly reviews some mechanistic aspects of ductile and brittle crack propagation, including discussion on mixed-mode cracking. Factors associated with overload failures are discussed, and, where appropriate, preventive steps for reducing the likelihood of overload fractures are included. The article focuses primarily on the contribution of embrittlement to overload failure. The embrittling phenomena are described and differentiated by their causes, effects, and remedial methods, so that failure characteristics can be directly compared during practical failure investigation. The article describes the effects of mechanical loading on a part in service and provides information on laboratory fracture examination.

Engineering component and structure failures manifest through many mechanisms but are most often associated with fracture in one or more forms. This article introduces the subject of fractography and aspects of how it is used in failure analysis. The basic types of fracture processes (ductile, brittle, fatigue, and creep) are described briefly, principally in terms of fracture appearances. A description of the surface, structure, and behavior of each fracture process is also included. The article provides a framework from which a prospective analyst can begin to study the fracture of a component of interest in a failure investigation. Details on the mechanisms of deformation, brittle transgranular fracture, intergranular fracture, fatigue fracture, and environmentally affected fracture are also provided.

Abstract
Failures in boilers and other equipment taking place in power plants that use steam as the working fluid are discussed in this article. The discussion is mainly concerned with failures in Rankine cycle systems that use fossil fuels as the primary heat source. The general procedure and techniques followed in failure investigation of boilers and related equipment are discussed. The article is framed with an objective to provide systematic information on various damage mechanisms leading to the failure of boiler tubes, headers, and drums, supplemented by representative case studies for a greater understanding of the respective damage mechanism.
