Abstract
On 21 April 1995, the contents of a large blender (6 cu m) reacted and caused an explosion that killed and injured a number of workers at a plant in Lodi, NJ. A mixture of sodium hydrosulfite and aluminum powder was being mixed at the time of the accident. This report focuses on evaluations of the blender to determine if material or mechanical failures were the cause of the accident. The results indicate that the mixing vessel was metallurgically sound and did not contribute to the initiation of the failure. However, the vessel was not designed for mixing chemicals that must be isolated from water and excessive heat. Water leaking into the vessel through a graphite seal may have initiated the reactions that caused the accident.

Abstract
Microstructure, corrosion, and fracture morphologies of prestressed steel wires that failed in service on concrete siphons at the Central Arizona Project (CAP) are discussed. The CAP conveys water for municipal, industrial, and agricultural use through a system of canals, tunnels, and siphons from Lake Havasu to just south of Tucson, AZ. Six siphons were made from prestressed concrete pipe units 6.4 m (21 ft) in diam and 7.7 m long, making them the largest circular precast structures ever built. The pipe was manufactured on site and consisted of a 495-mm thick concrete core, wrapped with ASTM A648 steel prestressing wire. All of the CAP failures evaluated were attributed to corrosion. Longitudinal splits reduced the service life of the pipe significantly by facilitating corrosion and introducing sharp cracks into the microstructure of the wire. A few failures were attributed to general corrosion, where the cross section of the wire is reduced until the strength of the wire is exceeded. Most of the failures evaluated were attributed to stress-corrosion cracking.

This article considers two mechanisms of cavitation failure: those for ductile materials and those for brittle materials. It examines the different stages of cavitation erosion. The article explains various cavitation failures including cavitation in bearings, centrifugal pumps, and gearboxes. It provides information on the cavitation resistance of materials and other prevention parameters. The article describes two American Society for Testing and Materials (ASTM) standards for the evaluation of erosion and cavitation, namely, ASTM Standard G 32 and ASTM Standard G 73. Abstract
Friction and wear are important when considering the operation and efficiency of components and mechanical systems. Among the different types and mechanisms of wear, adhesive wear is very serious. Adhesion results in a high coefficient of friction as well as in serious damage to the contacting surfaces. In extreme cases, it may lead to complete prevention of sliding; as such, adhesive wear represents one of the fundamental causes of failure for most metal sliding contacts, accounting for approximately 70% of typical component failures. This article discusses the mechanism and failure modes of adhesive wear including scoring, scuffing, seizure, and galling, and describes the processes involved in classic laboratory-type and standardized tests for the evaluation of adhesive wear. It includes information on standardized galling tests, twist compression, slider-on-flat-surface, load-scanning, and scratch tests. After a discussion on gear scuffing, information on the material-dependent adhesive wear and factors preventing adhesive wear is provided.

Abstract
This paper summarizes several cases of metallurgical failure analysis of surgical implants conducted at the Laboratory of Failure Analysis of IPT, in Brazil. Investigation revealed that most of the samples were not in accordance with ISO standards and presented evidence of corrosion assisted fracture. Additionally, some components were found to contain fabrication/processing defects that contributed to premature failure. The implant of nonbiocompatible materials results in immeasurable damage to patients as well as losses for the public investment. It is proposed that local sanitary regulation agencies create mechanisms to avoid commercialization of surgical implants that are not in accordance with standards and adopt the practice of retrieval analysis of failed implants. This would protect the public health by identifying and preventing the main causes of failure in surgical implants.

It describes the complications related to implants and provides a list of major standards for orthopedic implant materials. The article illustrates the body environment and its interactions with implants. The considerations for designing internal fixation devices are also described. The article analyzes failed internal fixation devices by explaining the failures of implants and prosthetic devices due to implant deficiencies, mechanical or biomechanical conditions, and degradation. Abstract
Chain link, a part of a mechanism for transferring hot or cold steel blooms into and out of a reheating furnace, broke after approximately four months of service. The link was cast from 2% Cr austenitic manganese steel and was subjected to repeated heating to temperatures of 455 to 595 deg C (850 to 1100 deg F). Examination included visual inspection, macrograph of a nital-etched specimen from an as-received chain link 1.85x, micrographs of a nital-etched specimen from an as-received chain link 100x/600x, normal microstructure of as-cast standard austenitic manganese steel 100x, micrograph of a nital-etched specimen that had been austenitized 20 min at 1095 deg C (2000 deg F) and air cooled 315x, and micrograph of the same specimen after annealing 68 h at 480 deg C (900 deg F) 1000x). Investigation supported the conclusions that the chain link failed in a brittle manner, because the austenitic manganese steel from which it was cast became embrittled after being reheated in the temperature range of 455 to 595 deg C (850 to 1100 deg F) for prolonged periods of time. The alloy was not suitable for this application, because of its metallurgical instability under service conditions.

Abstract
A special eyebolt was used to lift prefabricated concrete panels weighing approximately 16 cwt. Two eyebolts were used with a spreader bar to give a vertical lift on each eyebolt. Following failure of one eyebolt, which resulted in dropping of the load and subsequent failure of the other one, a complete eyebolt was submitted for assessment. Microscopic examination indicated a medium carbon-manganese steel had been used for the lower screwed portion of the eyebolt. Failure may have been due to brittle fracture or to fatigue, both of which could have been initiated at cracks in the hardened material in the region of the weld securing the screwed portion to the intermediate collar and which may have formed at the time of manufacture. Out-of-squareness of the thread with the collar, as was seen in the example submitted, gave rise to bending stresses when the bolt was tightened down, and this could have been a further factor which promoted failure. It was suggested that the design and construction could be improved by either making the component in one piece or, if it was desired, to adapt a standard eyebolt.

Abstract
Tube sheets (found to be copper alloy C46400, or naval brass, and 5 cm (2 in.) thick) of an air compressor aftercooler were found to be cracked and leaking approximately 12 to 14 months after they had been retubed. Most of the tube sheets had been retubed several times previously because of unrelated tube failures. Sanitary (chlorinated) well water was generally used in the system, although filtered process make-up water (river water) containing ammonia was occasionally used. Investigation (visual inspection, chemical analysis, mercurous nitrate testing, unetched 5X micrographs, and 250X micrographs etched in 10% ammonium persulfate solution) supported the conclusion that the tube sheets failed by SCC as a result of the combined action of internal stresses and a corrosive environment. The internal stresses had been induced by retubing operations, and the environment had become corrosive when ammonia was introduced into the system by the occasional use of process make-up water. Recommendations included making a standard procedure to stress relieve tube sheets before each retubing operation. The stress relieving should be done by heating at 275 deg C (525 deg F) for 30 min and slowly cooling for 3 h to room temperature.

Abstract
The failure of a reformer tube furnace manifold has been examined using metallography. It has been shown that the cause of failure was thermal fatigue; the damage was characterized by the presence of voids produced by creep mechanisms operating during the high temperature cycle under high local stress. The study indicates that standard metallographic procedures can be used to identify failure modes in high temperature petrochemical plants.

A detailed fracture mechanics evaluation is the most accurate and reliable prediction of process equipment susceptibility to brittle fracture. This article provides an overview and discussion on brittle fracture. The discussion covers the reasons to evaluate brittle fracture, provides a brief summary of historical failures that were found to be a result of brittle fracture, and describes key components that drive susceptibility to a brittle fracture failure, namely stress, material toughness, and cracklike defect. It also presents industry codes and standards that assess susceptibility to brittle fracture. Additionally, a series of case study examples are presented that demonstrate assessment procedures used to mitigate the risk of brittle fracture in process equipment.

Abstract
Reliability-centered maintenance (RCM) is a systematic methodology for preventing failures. This article begins by discussing the history of RCM and uses Society of Automotive Engineers (SAE) all-industry standard JA1011 as its model to describe the key characteristics of an RCM process. It then expands on questions involved in RCM process, offering definitions when necessary. Next, the article describes the approach of RCM to failure modes and effects analysis (FMEA), the failure management policies available under RCM, and the criteria of RCM for deciding when a specific failure management policy is technically feasible. Then, after discussing the ways that RCM classifies failure effects in terms of consequences, it describes how RCM uses failure consequences to identify the best failure management policy for each failure mode. Next, the building blocks of RCM are put together to create a failure management program. The article ends with a discussion on some practical issues pertaining to RCM that lie outside the scope of SAE JA1011.

Abstract
This article provides an outline of the issues to consider in performing a probabilistic life assessment. It begins with an historical background and introduces the most common methods. The article then describes those methods covering subjects such as the required random variable definitions, how uncertainty is quantified, and input for the associated random variables, as well as the characterization of the response uncertainty. Next, it focuses on specific and generic uncertainty propagation techniques: first- and second-order reliability methods, the response surface method, and the most frequently used simulation methods, standard Monte Carlo sampling, Latin hypercube sampling, and discrete probability distribution sampling. Further, the article discusses methods developed to analyze the results of probabilistic methods and covers the use of epistemic and aleatory sampling as well as several statistical techniques. Finally, it illustrates some of the techniques with application problems for which probabilistic analysis is an essential element.
