Abstract
Metallographic examination is one of the most important procedures used by metallurgists in failure analysis. Typically, the light microscope (LM) is used to assess the nature of the material microstructure and its influence on the failure mechanism. Microstructural examination can be performed with the scanning electron microscope (SEM) over the same magnification range as the LM, but examination with the latter is more efficient. This article describes the major operations in the preparation of metallographic specimens, namely sectioning, mounting, grinding, polishing, and etching. The influence of microstructures on the failure of a material is discussed and examples of such work are given to illustrate the value of light microscopy. In addition, information on heat-treatment-related failures, fabrication-/machining-related failures, and service failures is provided, with examples created using light microscopy.

It discusses various metallographic specimen preparation techniques, namely, sectioning, mounting, grinding, polishing, and electrolytic polishing. The article also describes the microstructure examination of various materials, with emphasis on failure analysis, and concludes with information on the examination of replicas with light microscopy.

Abstract
Textile-machine crankshafts forged from 4140 steel fractured transversely on one cheek during one to three years of service. The cause of failure for two forgings (one complete fractured forging and second a section that contained the shorter shaft fracture cheek) was determined. Indication of fatigue failure was revealed by visual examination of the fracture surfaces. Rough grooves from hot trimming of the flash were visible on the surface of the cheeks. The outer face of one cheek of the throw on the forging contained shallow surface folds. Slightly decarburized forged surface was identified around one of the folds and a fatigue crack initiated in the fold and propagated across the cheek. Properties representative of 4140 steel, quenched and tempered to a hardness of 20 to 22 HRC, were observed. Tempered bainite was revealed in the general microstructure. As a corrective measure, the forgings were normalized, hardened and tempered to 28 to 32 HRC before being machined to increase fatigue strength and extremely rough surfaces were removed by careful grinding.

Abstract
The pawl spring which was part of a selector switch used in telephone equipment failed. The springs were blanked from 0.4 mm (0.014 in.) thick tempered 1095 steel and then nickel plated. Numerous pits around the rivet holes were revealed by microscopic examination of longitudinal specimens. Delaminations that were formed at inclusion sites during punching of the rivet holes and that were filled with nickel during the plating operation were revealed by microscopic examination of the rivet hole. These delaminations were interpreted to have acted as stress raisers and initiated the fracture. Long, narrow sulfide stringers which were the probably the cause of delamination in this spring material were revealed in the raw material used to make the springs. It was concluded that fracture of the springs was caused by fatigue that had originated at delaminations around the rivet holes.

Abstract
Cracks on the outer surface near a hanger lug were revealed by visual inspection of a type 316 stainless steel main steam line of a major utility boiler system. Cracking was found to have initiated at the outside of the pipe wall or immediately beneath the surface. The microstructure of the failed pipe was found to consist of a matrix precipitate array (M23C6) and large s-phase particles in the grain boundaries. A portable grinding tool was used to prepare the surface and followed by swab etching. All material upstream of the boiler stop valve was revealed to have oriented the cracking normally or nearly so to the main hoop stress direction. Residual-stress measurements were made using a hole-drilling technique and strain gage rosettes. Large tensile axial residual stresses were measured at nearly every location investigated with a large residual hoop stress was found for locations before the stop valve. It was concluded using thermal stress analysis done using numerical methods and software identified as CREPLACYL that one or more severe thermal downshocks might cause the damage pattern that was found. The root cause of the failure was identified to be thermal fatigue, with associated creep relaxation.

Fretting and fretting corrosion at the contact area between the screw hole of a type 316LR stainless steel bone plate and the corresponding screw head was studied. The attack on the 316LR stainless steel was only shallow. Mechanical grinding and polishing structures were exhibited by a large portion of the contact area. Fine corrosion pits in the periphery were observed and intense mechanical material transfer that can take place during fretting was revealed. Smearing of material layers over each other during wear was observed and attack by pitting corrosion was interpreted to be possible.

Abstract
Liquid penetrant inspection of an ASTM A296 grade CA-15 residual heat removal pump impeller from a nuclear plant revealed a crack like indication that approximated the outer contour of the wear ring. Examination of a section containing the crack and three sections from near the main crack indication revealed that the failure was caused by hot cracking related to original weld repairs performed on the impeller casting.

Abstract
Identification of alloys using quantitative chemical analysis is an essential step during a metallurgical failure analysis process. There are several methods available for quantitative analysis of metal alloys, and the analyst should carefully approach selection of the method used. The choice of appropriate analytical techniques is determined by the specific chemical information required, the condition of the sample, and any limitations imposed by interested parties. This article discusses some of the commonly used quantitative chemical analysis techniques for metals. The discussion covers the operating principles, applications, advantages, and disadvantages of optical emission spectroscopy (OES), inductively coupled plasma optical emission spectroscopy (ICP-OES), X-ray spectroscopy, and ion chromatography (IC). In addition, information on combustion analysis and inert gas fusion analysis is provided.

Abstract
Type 347 stainless steel moderator circuit branch piping in a pressurized hot water reactor was experiencing frequent leakage. Investigation of the problem involved failure analysis of leaking pipe specimens, analytical stress analysis, and determination of “leak-before-break” conditions using fracture mechanics and thermal fatigue simulation tests. Failure analysis indicated that cracking had been initiated by thermal fatigue. Data from the analysis were used in making the leak-before-break predictions. It was determined that the cracks could grow to two-thirds of the circumferential length of the pipe without catastrophic failure. A thin stainless steel sleeve was inserted in the branch pipe to resolve the problem.

Abstract
The crankshaft of a 37.5-hp, 3-cylinder oil engine was examined. The engine had been dismantled for the purpose of a general overhaul and in the course of this work the crankpins were chromium-plated before regrinding. The engine was returned to service and after running for 290 h the crankshaft broke at the junction of the No. 3 crankpin and the crankweb nearest to the flywheel. A typical fatigue crack had originated at a number of points in the root of the fillet to the web. In its early stages it ran slightly into the web but turned back to the pin when it encountered the oil hole. The shaft had been made from a heat-treated alloy steel. The thickness of the plating was approximately 0.025 in. and numerous cracks were visible in it, several of which had given rise to cracks in the steel below. The primary cause of the crankshaft failure was the plating of the crankpins. The presence of the grooves alone would result in considerable intensification of stress in zones which are normally highly stressed, while the crazy cracking introduced a multiplicity of stress-raisers of a type almost ideal from the point of view of initiating fatigue cracks.

Abstract
Examination of several fighter aircraft main landing gear legs revealed unusual cracking in the hard chromium plating that covered the sliding section of the inner strut. The cracking was associated with cracks in the 35 NCD 16 steel beneath the plating. A detailed investigation revealed that the cracking was caused by the combination of incorrect grinding procedure, the presence of hydrogen, and fatigue. The grinding damage generated tensile stresses in the steel, which caused intergranular cracking during the plating cycle. The intergranular cracks were initiation sites for fatigue crack growth during service. It was recommended that the damaged undercarriage struts be withdrawn from service pending further analysis and development of a repair technique.

Nondestructive metallographic examination of materials frequently must be performed on-site when the component in question cannot be moved or destructively examined. Often, it is imperative that specific microstructural information (i.e., material type, heat treatment condition, homogeneity, etc.) be obtained either before initial use of a component, or before the use of a component can be safely resumed. In this paper, the use of standard metallurgical laboratory equipment, and the procedures required to conduct nondestructive on-site metallographic analyses of engineering materials, is presented. As an example, the materials and metallographic techniques employed in an actual on-site investigation of a gas tungsten-arc weldment joining two large diameter Ti-6Al-4V alloy cylinders are discussed in depth to illustrate what can be accomplished.

Abstract
Field metallography and replication were performed on a type 316 stainless steel column in diglycol amine vacuum service to determine the cause of visible OD pitting on the column in several areas above the insulation support rings. The examination revealed transgranular stress-corrosion cracking beneath the pitted areas on the OD. The likely cause of the cracking was chloride stress corrosion, with chlorides deriving from the marine atmosphere and concentrating under the insulation around the support rings. A complete insulation evaluation, including repair or replacement, was recommended to prevent chloride buildup. Painting of the steel surface with an epoxy-phenolic or epoxy-coal tar was also suggested.

Abstract
Upon arrival at the erection site, an AISI type 316L stainless steel tank intended for storage of fast breeder test reactor coolant (liquid sodium) exhibited cracks on its shell at two of four shell/nozzle fillet-welded joint regions. The tank had been transported from the manufacturer to the erection site by road, a distance of about 800 km (500 mi). During transport, the nozzles were kept at an angle of 45 deg to the vertical because of low clearance heights in road tunnels. The two damaged joints were unsupported at their ends inside the vessel, unlike the two uncracked nozzles. Surface examination showed ratchet marks at the edges of the fracture surface, indicating that loading was of the rotating bending type. Electron fractography using the two-stage replica method revealed striation marks characteristic of fatigue fracture. The striations indicated that the cracks had advanced on many “mini-fronts,” also indicative of nonuniform loading such as rotating bending. It was recommended that a support be added at the inside end of the nozzles to rigidly connect with the shell. In addition to avoiding transport problems, this design modification would reduce fatigue loading that occurs in service due to vibration of the nozzles during filling and draining of the tank.

Abstract
Failure analysis was performed on a fractured impeller from a boiler feed pump of a fossil fuel power plant. The impeller was a 12% Cr martensitic stainless steel casting. The failure occurred near the outside diameter of the shroud in the vicinity of a section change at the shroud/vane junction. Sections cut from the impeller were examined visually and by SEM fractography. Microstructural, chemical, and surface analyses and surface hardness tests were conducted on the impeller segments. The results indicated that the impeller failed in fatigue with casting defects increasing stress and initiating fracture. In addition, the composition and hardness of the impeller did not meet specifications. Revision of the casting process and institution of quality assurance methods were recommended.

Some 99.90 pure tin tubes (0.15 mm thick) used for packaging a chemical compound cracked on bending and underwent brittle fracture prior to filling, while others remained ductile and showed no sign of failure. Examination showed that specimens prepared by mechanical methods such as electrolytic and hand polishing and the vibration method resulted in poor edge and crack edge definition due to material thickness. Etching experiments involved a grain surface attack and hence produced a rather strong surface relief from which the grain boundary cracks could again not clearly be differentiated. The sections were therefore examined unetched in polarized light. The microstructure of the cracked tubes was shown to have much smaller grains than the ductile and showed cracks from the surface down along the grain boundaries. Material hardness also differed between the unusable tubes and the ductile, and chemical analysis showed a higher level of aluminum in the brittle specimens. Failure obviously occurred due to the high material aluminum content that increased hardness which then caused embrittlement at the surface which led to cracks or fracture on bending. Since no explanation of how the aluminum entered the tin was available, no recommendations could be made.

The first part of this article focuses on two major forms of machining-related failures, namely machining workpiece (in-process) failures and machined part (in-service) failures. Discussion centers on machining conditions and metallurgical factors contributing to (in-process) workpiece failures, and undesired surface layers and metallurgical factors contributing to (in-service) machined part failures. The second part of the article discusses the effects of microstructure on machining failures and their preventive measures.
