Abstract
Soft-soldered copper pipe joints used in refrigerating plants failed. The solder had not adhered uniformly to the pipe surface. In addition, there were some longitudinal grooves on the pipe surfaces, parts of which were not filled with solder. The unsoldered areas formed cavities within the joints, some of which had been in direct communication with the outsides via the grooves or interconnected cavities. On cooling, moisture condensed on the external surfaces. Some of this was drawn by capillary action into the cavities in open communication with the external surface. On continued cooling to below freezing-point, water that entered the cavities solidified. This was accompanied by a slight increase in volume, which collapsed the pipe walls. In the examples, the pipe ends had not been properly tinned. The solder used was found to be of the tin-antimony type, containing about 5% antimony, which is more difficult to use than the usual tin-lead alloys. The use of this particular type of solder was a contributory factor in the production of unsound joints in the samples examined.

Abstract
Due to the recent requirement of higher integration density, solder joints are getting smaller in electronic product assemblies, which makes the joints more vulnerable to failure. Thus, the root-cause failure analysis for the solder joints becomes important to prevent failure at the assembly level. This article covers the properties of solder alloys and the corresponding intermetallic compounds. It includes the dominant failure modes introduced during the solder joint manufacturing process and in field-use applications. The corresponding failure mechanism and root-cause analysis are also presented. The article introduces several frequently used methods for solder joint failure detection, prevention, and isolation (identification for the failed location).

Abstract
In a copper hot water system, a bent pipe was soldered into a straight pipe with twice the diameter. The neighborhood of the soldered joint was covered with corrosion product predominantly blue-green in color, presumably carbonates. When these corrosion products were scratched off it was seen that the copper beneath this layer had not suffered noticeable attack. The object of the examination was the localized deep cavities located almost symmetrically to both sides of the inserted end of the narrower tube on the internal wall of the wider tube which had in one place been eaten right through. The symmetrical location on each side of the point of insertion of the narrower pipe and the localized sharp delineation of the attack indicated erosion due to the formation of turbulence. By avoiding sharp transitions and abrupt changes in cross section it is possible to design the pipe work so that localized turbulence is obviated. Degassing and cleansing of the water also would reduce the danger of erosion particularly in the case of softened water, which takes up oxygen and carbon dioxide very readily thus becoming particularly aggressive.

Abstract
A sprinkler head unit that was installed in a smoking lounge of a multi story office building in 1975 failed, causing substantial water damage. There was no fire in the building. A set of four sprinkler heads -- three that had been installed in 1975 (the failed unit, an unfailed unit from the same room, and an unfailed unit from another room) and an unused 1991 unit -- were examined. casting revealed no material defects or mechanical damage. Because of several environmental factors, it was suspected that the failed unit was exposed to temperatures much above the normal office environment. On this basis, it was concluded that creep of the solder alloy was the most probable cause of failure.

Abstract
A T-piece from a copper hot water system failed. Microscopic examination of a polished section revealed a main crack and branching transcrystalline cracks running from the outer surface of the pipe into the pipe wall. The crack appearance indicated disintegration by stress-corrosion cracking. Although copper is not susceptible in the pure state, it is prone to stress-corrosion cracking under tensile stress in the presence of other elements in a damp ammoniacal atmosphere. The material was not defective, but a phosphorus-deoxidized copper type. The residual phosphorus combined with oxygen to form phosphorus pentoxide. Hard soldering in turn prevented the formation of cuprous oxide, and hydrogen embrittlement occurred.

Abstract
Several surface-mount chip resistor assemblies failed during monthly thermal shock testing and in the field. The resistor exhibited a failure mode characterized by a rise in resistance out of tolerance for the system. Representative samples from each step in the manufacturing process were selected for analysis, along with additional samples representing the various resistor failures. Visual examination revealed two different types of termination failures: total delamination and partial delamination. Electron probe microanalysis confirmed that the fracture occurred at the end of the termination. Transverse sections from each of the groups were examined metallographically. Consistent interfacial separation was noted. Fourier transform infrared and EDS analyses were also performed. It was concluded that low wraparound termination strength of the resistors had caused unacceptable increases in the resistance values, resulting in circuit nonperformance at inappropriate times. The low termination strength was attributed to deficient chip design for the intended materials and manufacturing process and exacerbated by the presence of polymeric contamination at the termination interface.

Abstract
A portable propane container with a name-plate soldered onto it exploded in service. When the vessel was inspected afterwards, it was found to have developed a crack in the top end plate. A portion of the end plate cut out to include the midlength and one termination of the crack was examined microscopically. This revealed that the crack was associated with intergranular penetration by molten metal. The microstructure in general was indicative of a good-quality mild steel. It was evident from that solder that was responsible for the penetration and that fused brass from the hand wheel had not played any part. Tensile stress was present at the time of the failure sufficiently high to enable solder penetration to take place. The use of soft solder as a medium for attaching name-plates directly on to stressed steel parts is not recommended. It would be preferable to use a welded-on patch plate or to employ one of the high-strength, non-metallic adhesives.

Abstract
The operator of an electric transit system purchased a large number of tin-plated copper connectors, putting some in service and others in reserve. Later, when some of the reserve connectors were inspected, the metal surfaces were covered with spots consisting of an ash-like powder and the plating material had separated from the substrate in many areas. Several connectors, including some that had been in service, were examined to determine what caused the change. The order stated that the connectors were to be coated with a layer of tin-bismuth (2% Bi) to guard against tin pest, a type of degradation that occurs at low temperatures. Based on the results of the investigation, which included SEM/EDS analysis, inductively coupled plasma spectroscopy, and x-ray diffraction, the metal surfaces contained less than 0.1% Bi and thus were not adequately protected against tin pest, which was confirmed as the failure mechanism in the investigation.

Abstract
Nondestructive testing (NDT), also known as nondestructive evaluation (NDE), includes various techniques to characterize materials without damage. This article focuses on the typical NDE techniques that may be considered when conducting a failure investigation. The article begins with discussion about the concept of the probability of detection (POD), on which the statistical reliability of crack detection is based. The coverage includes the various methods of surface inspection, including visual-examination tools, scanning technology in dimensional metrology, and the common methods of detecting surface discontinuities by magnetic-particle inspection, liquid penetrant inspection, and eddy-current testing. The major NDE methods for internal (volumetric) inspection in failure analysis also are described.

Abstract
A welded elbow assembly (AISI type 321 stainless steel, with components joined with ER347 stainless steel filler metal by gas tungsten arc welding) was part of a hydraulic-pump pressure line for a jet aircraft. The other end of the tube was attached to a flexible metal hose, which provided no support and offered no resistance to vibration. The line was leaking hydraulic fluid at the nut end of the elbow. Investigation supported the conclusion that failure was by fatigue cracking initiated from a notch at the root of the weld and was propagated by cyclic loading of the tubing as the result of vibration and inadequate support of the hose assembly. Recommendations included changing the joint design from a cylindrical lap joint to a square-groove butt joint. Also, an additional support was recommended for the hose assembly to minimize vibration at the elbow.

Abstract
While undergoing vibration testing, a type 347 stainless steel inlet header for a fuel-to-air heat exchanger cracked in the header tube adjacent to the weld bead between the tube and header duct. Investigation (visual inspection and liquid penetrant inspection) supported the conclusion that the crack in the header tube was the result of a stress concentration at the toe of the weld joining a doubler collar to the tube. The stress concentration was caused by undercutting from poor welding technique and an unfavorable joint design that did not permit a good fit-up. Recommendations included manufacturing the doubler collar so that it could be placed in intimate contact with the header duct, and a revised weld procedure was recommended to result in a smaller, controlled, homogeneous weld joint with less distortion.

A weld that attached the head to the shell of a preheater containing steam at 1.4 MPa (200 psi) and was used in the manufacture of paper cracked in service. The original joint contained a 6.4 by 50 mm backing ring that had been tack welded to the inside surface of the shell in a position to project beyond the fully beveled top edge of the shell. The projecting edge of the ring fitted against a deep undercut on the inner corner of the rim of the head. The internal 90-deg angle in this undercut was sharp, with almost no fillet. A bevel from the lower edge of the undercut to the outside of the head completed the groove for the circumferential attachment weld. Investigation (visual inspection and actual size views etched in hot 50% hydrochloric acid) supported the conclusion that cracking occurred in the HAZ in the head of the original design, originating in the sharp corner of the undercut, which was an inherent stress raiser. Recommendations included revised joint design to ensure full root penetration.

Abstract
Stainless steel liners (AISI type 321) used in bellows-type expansion joints in a duct assembly installed in a low-pressure nitrogen gas system failed in service. The duct assembly consisted of two expansion joints connected by a 32 cm (12 in.) OD pipe of ASTM A106 grade B steel. Elbows made of ASTM A234 grade B steel were attached to each end of the assembly, 180 deg apart. A 1.3 mm (0.050 in.) thick liner with an OD of 29 cm (11 in.) was welded inside each joint. The upstream ends were stable, but the downstream ends of the liners remained free, allowing the components to move with the expansion and contraction of the bellows. Investigation (visual inspection, hardness testing, and 30x fractographs) supported the conclusion that the liners failed in fatigue initiated at the intersection of the longitudinal weld forming the liner and the circumferential weld by which it attached to the bellows assembly. Recommendations included increasing the thickness of the liners from 1.3 to 1.9 mm (0.050 to 0.075 in.) in order to damp some of the stress-producing vibrations.
