Abstract
A forging die in a 250-ton press producing brass valves began to show signs of fatigue after a few thousand hits. By the time it reached 30,000 hits, the die was badly damaged and was submitted for analysis along with one of the last forgings produced. The investigation included visual and macroscopic inspection, metallographic and chemical analysis, SEM imaging, optical profilometry, mechanical property testing, and EDX analysis. The die was made of chromium hot-work tool steel and the forgings were made of CuZn39Pb3 heated to an initial working temperature 700 deg C. The entire surface of the die was covered with fatigue cracks and many fillets had been plastically deformed. Several other types of damage were also observed, including areas of oxidation, corrosion pits, voids, abrasive wear, die adhesion, and thermal fatigue. Fatigue cracking was the primary cause of failure with significant contributions from the other damage mechanisms.

Abstract
An investigation was conducted to determine why 16 out of 139 pipe bends cracked during hot induction bending. The pipe conformed to API 5L X65 PSL2 line pipe standards and measured 1016 mm (40 in.) in diam with a wall thickness of 18.5 mm. A metallurgical cross section was removed along a crack on the extrados to document the crack morphology using optical microscopy. In addition to cracking, golden-yellow streaks were visible at the extrados, and the composition was examined using scanning electron microscopy with energy dispersive spectroscopy. Based on the results, investigators concluded the pipe was contaminated with copper at the mill were it was produced.

Abstract
Stainless steel pipe (273-mm OD x 8-mm wall thickness) used in the fabrication of large manifolds developed crack-like decohesions during a routine inductive bending procedure. The imperfections, which were found near the outside diameter, were around 3 mm in length oriented in the circumferential direction and penetrated nearly 2 mm into the pipe wall. The pipes were made of titanium-stabilized austenitic stainless steel X6CrNiMoTi17-12-2. Six hypotheses were considered during the investigation, which ultimately concluded that the failure was caused by liquation cracking due to overheating.

Abstract
Spalled fragments from the work rolls of a steel bar straightening machine were received for failure analysis. Visual inspection coupled with optical and scanning electron microscopy were used as the principal analytical techniques for the investigation. Fractographic analysis revealed the presence of a characteristic fatigue crack propagation pattern (beach marks) and radial chevron marks indicating the occurrence of final overload through a brittle intergranular fracture. The collected evidence suggests that surface-initiated cracks propagated by fatigue led to spalling, resulting in severe work roll damage as well as machine downtime and increased maintenance costs.

Abstract
Radiant tubes that failed prematurely in an ethylene cracking furnace were analyzed to determine the cause of their early demise. The tubes were made from austenitic heat-resistant steel and cracked along their longitudinal axis. New and used tubes were compared using scanning electron microscopy, energy dispersive x-ray spectrometry, and mechanical property testing. This provided critical information and revealed that improper coking and decoking had removed the protective oxide layer (Cr 2 O 3 ) that normally prevents coke deposits from forming on exposed surfaces. Without this layer, coke readily accumulates on the surface of the tubes, fueling carbon diffusion into the metal and a corresponding degradation in microstructure and loss of ductility at high temperatures.

Abstract
Failure analysis was performed on threaded Ti-6Al-4V fasteners that had fractured in the threads during installation. Scanning electron microscopy (SEM) and optical metallography revealed that the fractures initiated in circumferential shear bands present at the thread roots. The fractures propagated by microvoid coalescence typical of that observed in notched tensile specimen fractures of the same material. For comparison, Ti-6Al-4V fasteners from various commercial sources were tested to failure in uniaxial tension and examined in the SEM. In all cases, the fracture appearances were similar to that exhibited by the fasteners that failed during installation. In addition, results of optical microscopy indicated that the geometry and extent of the shear bands appeared to depend on the fabrication process employed by the individual manufacturers. Causes of shear band formation are discussed along with potential methods to eliminate these microstructural in homogeneities.

Abstract
This article describes the general root causes of failure associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be the common sources of failure-inducing defects in bulk working of wrought products. The article discusses the types of imperfections that can be traced to the original ingot product. These include chemical segregation; ingot pipe, porosity, and centerline shrinkage; high hydrogen content; nonmetallic inclusions; unmelted electrodes and shelf; and cracks, laminations, seams, pits, blisters, and scabs. The article provides a discussion on the imperfections found in steel forgings. The problems encountered in sheet metal forming are also discussed. The article concludes with information on the causes of failure in cold formed parts.

Abstract
This article provides a description of the microscale models and mechanisms for deformation and fracture. Macroscale and microscale appearances of ductile and brittle fracture are discussed for various specimen geometries and loading conditions. The article reviews the general geometric factors and materials aspects that influence the stress-strain behavior and fracture of ductile metals. Abstract
This article focuses on characterizing the fracture-surface appearance at the microscale and contains some discussion on both crack nucleation and propagation mechanisms that cause the fracture appearance. It begins with a discussion on microscale models and mechanisms for deformation and fracture. Next, the mechanisms of void nucleation and void coalescence are briefly described. Macroscale and microscale appearances of ductile and brittle fracture are then discussed for various specimen geometries (smooth cylindrical and prismatic) and loading conditions (e.g., tension compression, bending, torsion). Finally, the factors influencing the appearance of a fracture surface and various imperfections or stress raisers are described, followed by a root-cause failure analysis case history to illustrate some of these fractography concepts.

Abstract
This article briefly reviews the analysis methods for spalling of striking tools with emphasis on field tests conducted by A.H. Burn and on the laboratory tests of H.O. McIntire and G.K. Manning and of J.W. Lodge. It focuses on the metallography and fractography of spalling. The macrostructure and microstructure of spall cavities are described, along with some aspects of the numerous specifications for striking/struck tools. The article also describes the availability of spall-resistant metals and the safety aspects of striking/struck tools in railway applications.
