Abstract
The century-old Harvard bridge spans the Charles River between Boston and Cambridge. About half of the 23 spans are suspended by wrought iron eyebars. Recent failures of some of these eyebars were examined. The primary cause of failure was the seizure of the joints at the eyebar pin locations as a result of the intrusion of water and salt, and the consequent heavy corrosion of the joint. The seizure of these joints led to high edgewise bending stress in the bars as the bridge underwent thermal movement. The cracking was enhanced by the presence of the corrosive medium so that the cracks were initiated and caused to grow by some combination of corrosion fatigue and stress-corrosion cracking, the former probably being predominant.

Failure occurred in the teeth of a case-hardening Ni-Cr-Mo alloy steel spur gear in the transmission system of heavy duty tracked vehicles. The defects were in the nature of seizure on the involute profile. Scrutiny of the transmission system showed there might be choking in the lubricating oil line. Such would cause seizure of the gears and damage. The incidence of such defects stopped after corrective measures were taken.

Abstract
Friction and wear are important when considering the operation and efficiency of components and mechanical systems. Among the different types and mechanisms of wear, adhesive wear is very serious. Adhesion results in a high coefficient of friction as well as in serious damage to the contacting surfaces. In extreme cases, it may lead to complete prevention of sliding; as such, adhesive wear represents one of the fundamental causes of failure for most metal sliding contacts, accounting for approximately 70% of typical component failures. This article discusses the mechanism and failure modes of adhesive wear including scoring, scuffing, seizure, and galling, and describes the processes involved in classic laboratory-type and standardized tests for the evaluation of adhesive wear. It includes information on standardized galling tests, twist compression, slider-on-flat-surface, load-scanning, and scratch tests. After a discussion on gear scuffing, information on the material-dependent adhesive wear and factors preventing adhesive wear is provided.

Abstract
An accidental overspeed condition during wind tunnel testing resulted in the destruction of a propeller rotor The occurrence was initially attributed to malfunction in the collective pitch control system. All fractured parts in the system were inspected. Highly suspect parts, including the pitch control thrust bearing set, head bolts, hub fork, and actuator rod end, were examined in more detail The thrust bearing set (52100 steel) was identified as the probable source of the uncommanded pitch angle change. A complete failure analysis of the bearing indicated that failure was precipitated by excessive heating, causing cage disintegration, plastic flow of the races and balls, and eventual separation of inner and outer races. It was recommended that the bearing set be resized to accommodate the large thrust as and that a thermocouple be added to monitor the condition of the bearing during testing.

Abstract
The crankshaft of a six cylinder, 225-hp diesel engine driving a small locomotive was examined. About nine months after installation a fall in oil pressure was traced to damage to No. 5 crank pin bearing. A small lip present on one side of the discontinuity apparently served to scrape the bearing material. The defect was stoned smooth, a new bearing fitted, and the engine returned to service. The engine performed satisfactorily for a further twelve months until fracture of the crankshaft through the No. 5 crank pin supervened. The fracture revealed a complex torsional fatigue failure. Microscopic examination revealed that the pin had been hard chromium plated and that the plating followed the curved edge of the outer extremity of the defect. This crank pin contained an inherent defect in the form of a slag inclusion or crack situated at the surface. That the crack only showed itself after a period of service suggests that initially it may have been slightly below the surface of the machined pin and some slight extension outwards took place in service.

Abstract
Fretting is a wear phenomenon that occurs between two mating surfaces; initially, it is adhesive in nature, and vibration or small-amplitude oscillation is an essential causative factor. Fretting generates wear debris, which oxidizes, leading to a corrosion-like morphology. This article focuses on fretting wear related to debris formation and ejection. It reviews the general characteristics of fretting wear, with an emphasis on steel. The review covers fretting wear in mechanical components, various parameters that affect fretting; quantification of wear induced by fretting; and the experimental results, map approach, measurement, mechanism, and prevention of fretting wear. This review is followed by several examples of failures related to fretting wear.

Abstract
Factors which may lead to premature roller bearing failure in service include incorrect fitting, excessive pre-load during installation, insufficient or unsuitable lubrication, over-load, impact load vibration, excessive temperature, contamination by abrasive matter, ingress of harmful liquids, and stray electric currents. Most common modes of failure include flaking or pitting (fatigue), cracks or fractures, creep, smearing, wear, softening, indentation, fluting, and corrosion. The modes of failure are illustrated with examples from practice.

Abstract
This paper describes an investigation on the failure of a large leaded bronze bearing that supports a nine-ton roller of a plastic calendering machine. At the end of the normal service life of a good bearing, which lasted for seven years, a new bearing was installed. However the new one failed catastrophically within a few days, generating a huge amount of metallic wear debris and causing pitting on the surface of the cast iron roller. Following the failure, samples were collected from both good and failed bearings. The samples were analyzed chemically and their microstructures examined. Both samples were subjected to accelerated wear tests in a laboratory type pin-on-disk apparatus. During the tests, the bearing materials acted as pins, which were pressed against a rotating cast iron disk. The wear behaviors of both bearing materials were studied using weight loss measurement. The worn surfaces of samples and the wear debris were examined by light optical microscope, SEM, and energy-dispersive x-ray microanalyzer. It was found that the laboratory pin-on-disk wear data correlated well with the plant experience. It is suggested that the higher lead content ~18%) of the good bearing compared with 7% lead of the failed bearing helped to establish a protective transfer layer on the worn surface. This transfer layer reduced metal-to-metal contact between the bearing and the roller and resulted in a lower wear rate. The lower lead content of the failed bearing does not allow the establishment of a well-protected transfer layer and leads to rapid wear.

Abstract
This case study involves two continuously cast steel crankshaft failures. Three parties performed their own failure analyses: (1) the engine manufacturer responsible for component design, specification, and application; (2) the steel supplier and forging supplier responsible for making the steel, forging the shape, and preliminary heat treatment; and (3) a supplier that provided induction hardening, finish machining, and inspection. An independent engineering firm was subsequently involved, but because each party had its own agenda, there was no agreement on the metallurgical source of the failure and thus no continued analysis to pin down and eliminate the root cause.

Abstract
Gears can fail in many different ways, and except for an increase in noise level and vibration, there is often no indication of difficulty until total failure occurs. This article reviews the major types of gears and the basic principles of gear-tooth contact. It discusses the loading conditions and stresses that effect gear strength and durability. This article provides assistance to a failure analyst in broadening the initial scope of the investigation of a physical engineering failure in order to identify the root cause of a problem. The engineering design process, including task clarification, conceptual design, embodiment design, and detail design, is reviewed. Abstract
This article considers the main characteristics of wear mechanisms and how they can be identified. Some identification examples are reported, with the warning that this task can be difficult because of the presence of disturbing factors such as contaminants or possible additional damage of the worn products after the tribological process. Then, the article describes some examples of wear processes, considering possible transitions and/or interactions of the mechanism of fretting wear, rolling-sliding wear, abrasive wear, and solid-particle erosion wear. The role of tribological parameters on the material response is presented using the wear map concept, which is very useful and informative in several respects. The article concludes with guidelines for the selection of suitable surface treatments to avoid wear failures.

Rolling-element bearings use rolling elements interposed between two raceways, and relative motion is permitted by the rotation of these elements. This article presents an overview of bearing materials, bearing-load ratings, and an examination of failed bearings. Rolling-element bearings are designed on the principle of rolling contact rather than sliding contact; frictional effects, although low, are not negligible, and lubrication is essential. The article lists the typical characteristics and causes of several types of failures. It describes failure by wear, failure by fretting, failure by corrosion, failure by plastic flow, failure by rolling-contact fatigue, and failure by damage. The article discusses the effects of fabrication practices, heat treatment and hardness of bearing components, and lubrication of rolling-element bearings with a few examples.

Reinforced polymers (RPs) are widely used in structural, industrial, automotive, and engineering applications due to their ecofriendly nature and the potential to manipulate their properties. This article addresses the technical synthesis of RPs, referring to their tribological behavior, to provide insights into the contribution and interaction of influential parameters on the wear behavior of polymers. It provides a brief discussion on the effects of significant parameters on RP tribology. The article describes abrasive and adhesive wear and provides a theoretical synthesis of the literature regarding the wear mechanisms of RPs. Abstract
Because of the tough engineering environment of the railroad industry, fatigue is a primary mode of failure. The increased competitiveness in the industry has led to increased loads, reducing the safety factor with respect to fatigue life. Therefore, the existence of corrosion pitting and manufacturing defects has become more important. This article presents case histories that are intended as an overview of the unique types of failures encountered in the freight railroad industry. The discussion covers failures of axle journals, bearings, wheels, couplers, rails and rail welds, and track equipment.
