Abstract
Ball joints made from carburized En 353 (BS970:815A16) steel failed after several hours of being fitted into vehicles. The parts were forged, machined, and thread rolled. The threads were copper plated to prevent carburization. The heat treatment consisted of carburizing in a cyanide bath for 12 hours at 930 deg C. After tempering for 2 h at 170 to 175 deg C, the copper plate was removed by immersing in an acid bath for 45 min. The investigations found the microstructure, hardness, and chemistry all met the specification. The case depth was approximately 0.75 mm to 1.0 mm. The SEM studies showed that it was a brittle fracture and completely intergranular to a depth of about 2.5 mm. It was concluded that the failure was due to hydrogen embrittlement for the following reasons: (i) failure did not occur immediately after loading, (ii) the fracture was intergranular to a depth of two to three times the case depth, (iii) secondary cracks were observed at the surface. The hydrogen was introduced during copper plate removal by acid dipping. If the tempering operation was performed after the acid dip operation, the hydrogen would have been driven out.

Abstract
The piston rod of a steering damper on a single decker bus fractured after 100,000 miles of service in the fully-extended left full-lock position. The steering damper, which is similar in shape and operation to a telescopic shock absorber, was secured by ball joints locked with slotted nuts. The steel piston rod fractured at the axle end leaving approximately 5 mm of rod welded to a securing ferrule. The failure was caused by a fatigue mechanism. Small surface cracks formed during welding in the heat-affected zone close to an unradiused shoulder in the piston. Under alternating stresses in normal service these cracks propagated through the piston rod made less tough by the extended weld heat-affected zone.

Abstract
Visual examination, optical and scanning electron microscopy were used to determine the cause of failure in the connector groove of a marine riser coupling. The specified steel was AISI 4142 (0.40 to 0.45% C; 0.75 to 1.00% Mn; 0.20 to 0.35% Si; 0.80 to 1.10% Cr; 0.15 to 0.25% Mo) normalized from 9000C. Microscopic examination revealed the crack's initiation point and subsequent propagation. SEM examination of chemically stripped corrosion showed that corrosion fatigue and stress corrosion might have contributed to the initial slow crack growth. Impact tests revealed a fracture transition temperature in excess of 1000C. The sequence of events leading to failure was detailed. The main recommendation was to quench and temper existing couplings and to use a lower carbon quenched and tempered steel for new couplings.

Abstract
Three samples from a ruptured 316 stainless steel tube were examined. The tube, 114 mm OD, wall thickness 8.00 mm, with 13 mm thick 321 stainless steel fins welded to the outer surface of the tube, was part of a heater through which sour gas, containing methane plus H2S and CO, passed at 1150 psig. The sour gas was heated to 600 deg F by burners playing on the outside of the tube burning “sweet” gas plus air. The inner and outer surfaces of all samples showed evidence of corrosive attack. Electron probe microanalysis showed the corrosion products contained sulfur with iron, together with nickel to a lesser extent. Local thinning, cavitation, and ductile deformation markings associated with the unmatched sample taken from the center of the fire showed the tube ruptured as a result of overheating. Overheating while the temperature recorder was off the chart caused severe loss of tube strength, resulting in ductile rupture. The minimum overheating temperature could be deduced at around 1200 deg F due to the presence of a eutectic observed metallographically within the surface corrosion products.

Abstract
Plate perforation occurred in the cylindrical section and walls of the inlet foot (2.38 mm thick Incoloy 825 plate welded using INCO welding rod 135) of an inert gas fire prevention system in an oil tanker. Cross-sectional microprobe analysis showed the corrosion product to contain sulfur, mainly from the flue gas, and calcium and chlorine, mainly from the sea water. The gray corrosion product was interspersed with rust and a black carbonaceous deposit. Corrosion pitting and poor weld penetration, with carbide precipitation and heavy etching at grain boundaries, indicated sensitization and susceptibility to aqueous intergranular corrosion. Chemical analysis showed the predominant acid radical to be sulfate (6.20% in the carbonaceous deposit and 0.60% in the corrosion product), suggesting that oxidation of SO2 in the flue gas caused the corrosion. Moisture condensation, the carbon acting as a cathode, and alloy susceptibility to intergranular corrosion contributed to the corrosion.

Abstract
A failure analysis was conducted in late 1996 on two rolls that had been used in the production of iron and steel powder. The rolls had elongated over their length such that the roll trunnions had impacted with the furnace wall refractory. The result was distortion and bowing of the roll bodies which necessitated their removal from service. The initial analysis found large quantities of nitrogen had been absorbed by the roll shell. Further research indicated nitrogen pickup accounted for 3% volumetric growth for every 1% by weight nitrogen absorption. This expansion was sufficient to account for the dimensional change observed in the failed rolls. This paper details the failure analysis and resulting research it inspired. It also provides recommendations for cast material choice in highly nitriding atmospheres.

Abstract
Nuclear power plants typically experience two or three high-cycle fatigue failures of stainless steel socket-welded connections in small bore piping during each plant-year of operation. This paper discusses fatigue-induced failure in socket-welded joints and the strategy Texas Utilities Electric Company (TU Electric) has implemented in response to these failures. High-cycle fatigue is invisible to proven commercial nondestructive evaluation (NDE) methods during crack initiation and the initial phases of crack growth. Under a constant applied stress, cracks grow at accelerating rates, which means cracks extend from a detectable size to a through-wall crack in a relatively short time. When fatigue cracks grow large enough to be visible to NDE, it is likely that the component is near the end of its useful life. TU Electric has determined that an inspection program designed to detect a crack prior to the component leaking would involve frequent inspections at a given location and that the cost of the inspection program would far exceed the benefits of avoiding a leak. Instead, TU Electric locates these cracks by visually monitoring for leaks. Field experience with fatigue-induced cracks in socket-welded joints has confirmed that visual monitoring does detect cracks in a timely manner, that these cracks do not result in catastrophic failures, and that the plant can be safely shut down in spite of a leaking socket-welded joint in a small bore pipe. Historical data from TU Electric and Southwest Research Institute are presented regarding the frequency of failures, failure locations, and the potential causes. The topics addressed include 1) metallurgical and fractographic features of fatigue cracks at the weld toe and weld root; 2) factors that are associated with fatigue, such as mechanical vibration, internal pulsation, joint design, and welding workmanship; and 3) implications of a leaking crack on plant safety. TU Electric has implemented the use of modified welding techniques for the fabrication of socket-welded joints that are expected to improve their ability to tolerate fatigue.

Corrosion in potable and nonpotable water systems has been well documented in the past, and new research discusses innovations in water treatment and materials that are designed to enhance the quality of a water system, whether commercial or residential. This paper is a collection of five case histories on the failure of copper and steels as used in potable and non-potable water systems. The case histories cover a range of applications in which copper and steel products have been used. Copper and steel pipes are the two most commonly used materials in residential, commercial and industrial applications. The projects that are discussed cover these three important applications. The purpose of presenting this information is to allow the reader to gain an understanding of real life corrosion issues that affect plumbing materials, how they should have been addressed during the design of the water system, and how a water system should be maintained during service. We share this information in the hope that the reader will gain some limited knowledge of the problems that exist, and apply that knowledge in designing or using water systems in day-to day life.

Abstract
The US Army Research Laboratory performed a failure investigation on a broken main landing gear mount from an AH-64 Apache attack helicopter. A component had failed in flight, and initially prevented the helicopter from safely landing. In order to avoid a catastrophe, the pilot had to perform a low hover maneuver to the maintenance facility, where ground crews assembled concrete blocks at the appropriate height to allow the aircraft to safely touch down. The failed part was fabricated from maraging 300 grade steel (2,068 MPa [300 ksi] ultimate tensile strength), and was subjected to visual inspection/light optical microscopy, metallography, electron microscopy, energy dispersive spectroscopy, chemical analysis, and mechanical testing. It was observed that the vacuum cadmium coating adjacent to the fracture plane had worn off and corroded in service, thus allowing pitting corrosion to occur. The failure was hydrogen-assisted and was attributed to stress corrosion cracking (SCC) and/or corrosion fatigue (CF). Contributing to the failure was the fact that the material grain size was approximately double the required size, most likely caused from higher than nominal temperatures during thermal treatment. These large grains offered less resistance to fatigue and SCC. In addition, evidence of titanium-carbo-nitrides was detected at the grain boundaries of this material that was prohibited according to the governing specification. This phase is formed at higher thermal treatment temperatures (consistent with the large grains) and tends to embrittle the alloy. It is possible that this phase may have contributed to the intergranular attack. Recommendations were offered with respect to the use of a dry film lubricant over the cadmium coated region, and the possibility of choosing an alternative material with a lower notch sensitivity. In addition, the temperature at which this alloy is treated must be monitored to prevent coarse grain growth. As a result of this investigation and in an effort to eliminate future failures, ARL assisted in developing a cadmium brush plating procedure, and qualified two Army maintenance facilities for field repair of these components.

The fatigue failure of a wire rope used on a skip hoist in an underground mine has been studied as part of the ongoing research by the Bureau of Mines into haulage and materials handling hazards in mines. Macroscopic correlation of individual wire failures with wear patterns, fractography, and microhardness testing were used to gain an understanding of the failure mechanism. Wire failures occurred predominantly at characteristic wear sites between strands. These wear sites are identifiable by a large reduction in diameter; however, reduction in area was not responsible for the location of failure. Fractography revealed multiple crack initiation sites to be located at other less noticeable wear sites or opposite the characteristic wear site. Microhardness testing revealed hardening, and some softening, at wear sites.

Abstract
Three separate corrosion mechanisms were involved in the failure of an AISI type 304 stainless steel pipe elbow. The major cracks, including the one that penetrated the wall, tend to be wide-mouthed, tapering to a blunt tip, with corrosion products filling much of the crack space. This was characteristic of corrosion fatigue. The second type of cracking originated at some of the major cracks. These cracks were branched and transgranular, which is characteristic of stress-corrosion caused by chlorides. The third crack mode, an intergranular network, was most probably the result of hydrogen sulphide attack. The 13-year service life of the elbow made it difficult, if not impossible, to determine the order of the corrosion mechanisms or the length of time it took to reach the present state of degradation after the initiation of corrosion. Based on the long service life the present material has given, it was recommended that it be used again.

Abstract
AISI type 321 stainless steel heat exchanger tubes failed after only three months of service. Macroscopic examination revealed that the leaks were the result of localized pitting attack originating at the water side surfaces of the tubes. Metallographic sections were prepared from both sets of tubes. Microscopic examination revealed that the pits had a small mouth with a large subsurface cavity which is typical of chloride pitting of austenitic stainless steel. However, no pitting was found in other areas of the system, where the chloride content of the process water was higher. This was attributed to the fact that they were downstream from a deaeration unit. It was concluded that the pitting was caused by a synergistic effect of chlorine and oxygen in the make-up water. Because it was not possible to install a deaeration unit upstream of the heat exchangers, it was recommended that a molybdenum-bearing stainless steel such as 316L or 317L be used instead of 321.

Abstract
This investigation involved two AISI 304L acid storage tanks and one AISI 304L spent solvent tank from a sewage treatment facility. After installation, these tanks were hydrostatically tested using sewage effluent. No leaks were found and after a year or two, the tanks were drained and filled with nitric acid in preparation for service. Three weeks later the two acid tanks were found to be leaking from the bottom. Samples from the spent solvent tank revealed that pitting was located in a depressed area near a suction hole, beneath a black residue. It was concluded that the acid tanks failed by chloride-induced pitting initiated by microbial activity. Further, the spent solvent tank failed by a similar, but anaerobic mechanism. The use of the effluent for the hydrostatic test and the failure to remove it and clean and dry the tanks was the primary cause of failure. Localized carbide segregation in the original plate served as preferential corrosion sites. Had the tanks been hydrostatically tested in a proper manner, the pitting may not have occurred.

Abstract
During a planned shut-down in 1990 it appeared that the bottom manifold parts made of wrought Incoloy 800H had undergone diametrical expansion of up to 2% due to creep. Further, cracking at the outer diam was found. It was decided to replace these parts. Microscopical investigations showed that the cracking could not be caused by creep. It was found that the cracking was confined to a 4-mm deep coarse-grained zone (ASTM 0-1) at the outer diameter. The cracking appeared to be caused by strain-induced intergranular oxidation. When the cracks reached the fine-grained material, the oxidation-cracks stopped. To determine the residual creep life of the sound (non-cracked) bottom manifold material, iso-stress creep tests were performed. It was found that tertiary creep started at 7% strain. The time-to-rupture was greater than 100,000 h. It was concluded that the bottom manifold (and thus the furnace) could be used safely during the foreseen production period.

Abstract
Several compressor disks in military fighter and trainer aircraft gas turbine engines cracked prematurely in the bolt hole regions. The disks were made of precipitation-hardened AM355 martensitic stainless steel. Experimental and analytical work was performed on specimens from the fifth-stage compressor disk (judged to be the most crack-prone disk in the compressor) to determine the cause of the failures. Failure was attributed to high-strain low-cycle fatigue during service. It was also determined that the cyclic engine usage assumed in the original life calculations had been under estimated, which led to low-cycle fatigue cracking earlier than expected. Fracture mechanics analysis of the disks was carried out to assess their damage tolerance and to predict safe inspection intervals.

Abstract
Some of the admiralty brass tubes were failing in a heat exchanger. The heat exchanger cooled air by passing river water through the inside of the tubes. The wall thickness of all tubes ranged between 1.19 to 1.27 mm (0.047 to 0.050 in.). General intergranular corrosion occurred at the inside surfaces of the tubes. Transgranular stress-corrosion cracking, probably the result of sulphates under basic conditions, and dezincification occurred also as the result of galvanic corrosion under the deposits in the tubes. Recommendations were to use a closed-loop water system to eliminate sulphates, ammonia, etc., and to run trials on one unit with tubes of other alloys such as 80-20 Cu-Ni or 70-30 Cu-Ni to evaluate their performance prior to any large scale retubing operations.

Abstract
Cluster bomb tailcone assemblies each containing two aluminum die-cast components were rejected because of the poor surface condition of the die castings. Numerous heat checks were found on the surfaces of the tailcones and radiographic inspection revealed inclusions, gas holes, and shrinkage defects in the castings. Most of the components failed to meet required mechanical properties because of these casting defects.

Abstract
Welded low-carbon steel bomb fins were rejected because of poor weld practice. Visual and metallographic examination revealed that the resistance plug welds that attach the outer skin to the inner spar displayed inadequate weld penetration. Recommended changes to the resistance welding parameters resulted in acceptable welds.

Abstract
Several air heat exchangers failed in service in a pulp and paper operation. The tubes were made from AISI 316 stainless steel with an extruded aluminum fin mechanically bonded to the outside. Originally, the failures were blamed on poor tube to header welds. The units were sent back to the manufacturer for repair. Some of the units failed the hydrostatic test after they were repaired. Microscopic examination revealed the presence of branched transgranular cracks characteristic of stress-corrosion cracking. Only some of the tubes failed and these did so by stress-corrosion cracking. The most probable primary cause of the stress-corrosion cracking was local high residual stresses indicated by the areas of high hardness in the tubes. Low halogens in the water and airborne corrodents found normally in a pulp and paper mill were all that were required in the presence of high residual stresses in the tubes to initiate stress-corrosion cracking. Use of a low-carbon grade of stainless steel such as 316L was recommended to facilitate formation of the tube without producing excessive residual stresses. It was recommended also that failed units be segregated until it can be determined if the failure was related to operating pressure or some other unique cause.

Abstract
After six years of service, three water shut-off valves on a copper water line in a residential building were found to be inoperative. Macroscopic examination of the valves after disassembly revealed that all three failed at the key that holds the spindle in the gate. In addition, the color near the key changed from yellow to red-brown. The gate was made from leaded red brass (85-5-5-5) while the spindle was made from silicon brass. It was concluded that the valves failed by dezincification resulting from bimetallic galvanic corrosion. It is common in the valve industry to use components made of different alloys in the same valve, but this is not the best approach for all applications.
