Abstract
The causes of cracking of an as-drawn 90-10 cupronickel tube during mechanical working were investigated to determine the source of embrittlement. Embrittlement was sporadic, but when present was typically noted after the first process anneal. Microstructural and chemical analyses were performed on an embrittled section and on a section from a different lot that did not crack during forming. The failed section showed an intergranular fracture path. Examination of the fracture surfaces revealed the presence of tellurium at the grain boundaries. The source of the tellurium was thought to be contamination occurring in the casting process that became concentrated in the recycled skimmings. It was recommended that future material specifications for skimmings and for externally obtained scrap copper include a trace analysis for tellurium.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. The article also describes the role of the materials engineer in the design and materials selection process. It provides information on the significance of materials selection in both the prevention and analysis of failures.

Six cases of failure attributed to microbiologically influenced corrosion (MIC) were analyzed to determine if any of the failures could have been avoided or at least predicted. The failures represent a diversity of applications involving typical materials, primarily stainless steel and copper alloys, in contact with a variety of liquids, chemistries, and substances. Analytical techniques employed include stereoscopic examination, energy dispersive x-ray spectroscopy (EDS), temperature and pH testing, and metallographic analysis. The findings indicate that MIC is frequently the result of poor operations or improper materials selection, and thus often preventable.

Abstract
This article provides an overview of metal additive manufacturing (AM) processes and describes sources of failures in metal AM parts. It focuses on metal AM product failures and potential solutions related to design considerations, metallurgical characteristics, production considerations, and quality assurance. The emphasis is on the design and metallurgical aspects for the two main types of metal AM processes: powder-bed fusion (PBF) and directed-energy deposition (DED). The article also describes the processes involved in binder jet sintering, provides information on the design and fabrication sources of failure, addresses the key factors in production and quality control, and explains failure analysis of AM parts.

Abstract
The repeated failure of rubber-covered rotors and volute liners in a flue gas desulfurization system after conversion from lime slurry reagent to limestone slurry reagent was investigated. The pump was a horizontal 50 x 65 mm (2 x 2.5 in.) Galiger pump with a split cast iron case and open rotor (impeller). Both the case and the ductile iron rotor core were covered by natural rubber. Analyses conducted included surface examination of wear patterns, chemical analysis of materials, measurement of mechanical properties, and in-place flow tests. It was determined that the proximate cause of failure was cavitation and vortexing between the rotor and the lining. The root cause of the failure was the conversion from lime to limestone slurry without appropriate modification of the pump. Conversion to the limestone slurry resulted in fluid dynamics outside the operational limits of the pump. The recommended remedial action was replacement with a pump appropriately sized for the desired pressures and flow rates for limestone slurry.

Abstract
The operator of an electric transit system purchased a large number of tin-plated copper connectors, putting some in service and others in reserve. Later, when some of the reserve connectors were inspected, the metal surfaces were covered with spots consisting of an ash-like powder and the plating material had separated from the substrate in many areas. Several connectors, including some that had been in service, were examined to determine what caused the change. The order stated that the connectors were to be coated with a layer of tin-bismuth (2% Bi) to guard against tin pest, a type of degradation that occurs at low temperatures. Based on the results of the investigation, which included SEM/EDS analysis, inductively coupled plasma spectroscopy, and x-ray diffraction, the metal surfaces contained less than 0.1% Bi and thus were not adequately protected against tin pest, which was confirmed as the failure mechanism in the investigation.

Abstract
An air blower in an electric power plant failed unexpectedly when a roller bearing in the drive motor fractured along its outer ring. Both rings, as well as the 18 rolling elements, were made from GCr15 bearing steel. The bearing also included a machined brass (MA/C3) cage and was packed with molybdenum disulfide (MoS 2 ) lithium grease. Metallurgical structures and chemical compositions of the bearing’s matrix materials were inspected using a microscope and photoelectric direct reading spectrometer. SEM/EDS was used to examine the local morphology and composition of fracture and contact surfaces. Chemical and thermal properties of the bearing grease were also examined. The investigation revealed that the failure was caused by wear due to dry friction and impact, both of which worsened as a result of high-temperature degradation of the bearing grease. Fatigue cracks initiated in the corners of the outer ring and grew large enough for a fracture to occur.

Abstract
This paper describes the superplastic characteristics of shipbuilding steel deformed at 800 °C and a strain rate less than 0.001/s. After the superplastic deformation, the steel presents mixed fractures: by decohesion of the hard (pearlite and carbides) and ductile (ferrite) phases and by intergranular sliding of ferrite/ferrite and ferrite/pearlite, just as it occurs in stage III creep behavior. The behavior is confirmed through the Ashby-Verrall model, according to which the dislocation creep (power-law creep) and diffusion creep (linear-viscous creep) occur simultaneously.

Abstract
A catastrophic brittle fracture occurred in a welded steel (ASTM A517 grade H) trapezoidal cross-section box girder while the concrete deck of a large bridge was being poured. The failure occurred across the full width of a 57 mm (2 1 4 in.) thick, 760 mm (30 in.) wide flange and arrested 100 mm (4 in.) down the slant web. Failure analysis revealed a major deficiency in fracture toughness. The failure occurred as a brittle fracture after the formation of a welding hot crack and approximately 40 mm (1 1 2 in.) of slow crack growth. It was recommended that bridges fabricated from this grade of steel undergo frequent inspection and that stringent test requirements be imposed as a condition of use in non-redundant main load-carrying components.

Abstract
During testing of compressors under start/stop conditions, several helical suspension springs failed. The ensuing failure investigation showed that the springs failed due to fatigue. The analysis showed that during start/stop testing the springs would undergo both a lateral and axial deflection, greatly increasing the torsional stresses on the spring. To understand the fatigue limits under these test conditions, a bench test was used to establish the fatigue strength of the springs. The bench tests showed that the failed springs had an unacceptable surface texture that reduced the fatigue life. Based on an understanding of the compressor motion, a Monte Carlo model was developed based on a linear damage theory to predict the fatigue life of the springs during start/stop conditions. The results of this model were compared to actual test data. The model showed that the design was marginal even for springs with acceptable surface texture. The model was then used to predict the fatigue life requirements on the bench test such that the reliability goals for the start/stop testing would be met, thus reducing the risk in qualifying the compressor.

It describes normal impact wear and compound impact wear, as well as the features of impact wear testing apparatus such as ballistic impact wear apparatus and pivotal hammer impact wear apparatus. Most mechanical components continue to be functional beyond the zero wear limit, and their usefulness is normally connected with the loss of a specific depth of material. The article reviews the zero impact wear model and some measurable impact wear models. It presents a case study illustrating the impact of wear failure on automotive engine inlet valves and seat inserts.

Abstract
This article provides assistance to a failure analyst in broadening the initial scope of the investigation of a physical engineering failure in order to identify the root cause of a problem. The engineering design process, including task clarification, conceptual design, embodiment design, and detail design, is reviewed. The article discusses the design process at the personal and project levels but takes into consideration the effects of some higher level influences and interfaces often found to contribute to engineering failures.

Abstract
Impact or percussive wear is defined as the wear of a solid surface that is due to percussion, which is a repetitive exposure to dynamic contact by another body. Impact wear, however, has many analogies to the field of erosive wear. The main difference is that, in impact wear situations, the bodies tend to be large and contact in a well-defined location in a controlled way, unlike erosion where the eroding particles are small and interact randomly with the target surface. This article describes some generic features and modes of impact wear of metals, ceramics, and polymers. It discusses the processes involved in testing and modeling of impact wear, and includes two case studies.

It begins by explaining the necessity of conducting a failure analysis, followed by the objectives of a failure analysis. Then, the article discusses the processes involved in failure analysis, including codes and standards. Next, fabrication flaws that can develop into failures of in-service pressure vessels and piping are covered. This is followed by sections discussing in-service mechanical and metallurgical failures, environment-assisted cracking failures, and other damage mechanisms that induce cracking failures. This article focuses on the mechanisms of microbially induced or influenced corrosion (MIC) of metallic materials as an introduction to the recognition, management, and prevention of microbiological corrosion failures in piping, tanks, heat exchangers, and cooling towers. It discusses the degradation of various protective systems, such as corrosion inhibitors and lubricants. The article describes the failure analysis of steel, iron, copper, aluminum, and their alloys. It also discusses the probes available to monitor conditions relevant to MIC in industrial systems and the sampling and analysis of conditions usually achieved by the installation of removable coupons in the target system. The article also explains the prevention and control strategies of MIC in industrial systems.
