Abstract
After a quick-release fitting of an ejection seat broke, an investigation was performed to determine the manner and cause of crack propagation. Most fractography-based investigations aim to characterize only qualitative characteristics, such as the fracture orientation and origin position, topology, and details of interactions with microstructural features. The aim of this investigation was to use quantitative fractography as a tool to extract information, including striation spacing and size of the stretched zone, in order to make a direct correlation with fracture mechanic concepts. As the crack propagated, striations were created on the fracture surface as a result of service-induced load changes. The size of the striations were measured to estimate crack propagation rate. Remaining lifetime estimates were also made. The dimensions of plastically stretched zones found at the tips of the cracks were evaluated using electron micrograph stereo image pairs to characterize local fracture toughness. To complete the failure analysis, nondestructive evaluation, metallographic examination, and chemical investigations were carried out. No secondary cracks could be found. Most of the broken parts showed that the microstructure, the hardness, and the chemical composition of the Al-alloy were within the specification, but some of the cracked parts were manufactured using a different material than that specified.

The quantitative characterization of fracture surface geometry, that is, quantitative fractography, can provide useful information regarding the microstructural features and failure mechanisms that govern material fracture. This article is devoted to the fractographic techniques that are based on fracture profilometry. This is followed by a section describing the methods based on scanning electron microscope fractography. The article also addresses procedures for three-dimensional fracture surface reconstruction. In each case, sufficient methodological details, governing equations, and practical examples are provided.

Abstract
A fracture mechanics based failure analysis and life prediction of a large centrifugal fan made from low-carbon, medium-strength steel was undertaken following shortcomings in attempts to explain its fatigue life from start stop cycles alone. Measurements of the fracture toughness and flaw size at failure, coupled with quantitative SEM fractography using striation spacing methods, revealed that the cyclic stress amplitudes just prior to failure were much larger than expected, in this particular case. Subsequent improvements in fan design and fabrication have effectively alleviated the problem of slow, high cycle fatigue crack growth, at normal operating stresses in similar fans.

Abstract
Several compressor disks in military fighter and trainer aircraft gas turbine engines cracked prematurely in the bolt hole regions. The disks were made of precipitation-hardened AM355 martensitic stainless steel. Experimental and analytical work was performed on specimens from the fifth-stage compressor disk (judged to be the most crack-prone disk in the compressor) to determine the cause of the failures. Failure was attributed to high-strain low-cycle fatigue during service. It was also determined that the cyclic engine usage assumed in the original life calculations had been under estimated, which led to low-cycle fatigue cracking earlier than expected. Fracture mechanics analysis of the disks was carried out to assess their damage tolerance and to predict safe inspection intervals.

Abstract
A crack was detected in one arm of the right-hand horizontal brace of the nose landing gear shock strut from a large military aircraft. The shock strut was manufactured from a 7049 aluminum alloy forging in the shape of a delta. A laboratory investigation was conducted to determine the cause of failure. It was concluded that the arm failed because of the presence of an initial defect that led to the initiation of fatigue cracking. The fatigue cracking grew in service until the part failed by overload. The initial defect was probably caused during manufacture. Fleet-wide inspection of the struts was recommended.

Abstract
Fractography is the means and methods for characterizing a fractured specimen or component. This includes the examination of fracture-exposed surfaces and the interpretation of the fracture markings as well as the examination and interpretation of crack patterns. This article describes the former of these two parts of fractography. It presents the techniques of fractography and explains fracture markings using glass and ceramic examples. The article also discusses the fracture modes in ceramics and provides examples of fracture origins.

It reviews the basic types of fracture processes, namely, ductile, brittle, fatigue, and creep, principally in terms of fracture appearances, such as microstructure. Abstract
A type 430Ti stainless steel flue gas expansion joint cracked because of caustic-induced stress-corrosion cracking. Energy-dispersive X-ray spectroscope analysis of the fracture surface deposits revealed the presence of sodium and potassium—caustics in hydroxide form. Primary fracture surfaces were all similar in appearance, and a primary crack origin could not be identified. A secondary crack brought to fracture in the laboratory showed brittle, cleavage features rather than classic, tensile overload features. This suggested that the material was embrittled.

Abstract
Cracking was discovered in an in-service, second-stage turbine impeller during a downtime inspection. The fabricated 4300 series low-alloy steel impeller was used in a compressor in an industrial petrochemical plant. It was also reported that a process upset had allowed a 10% NaOH solution to be ingested by the unit. Routine magnetic particle inspection revealed numerous cracks in the hub area and vane tips of the second-stage impeller Additionally, the outside surface of the backing plate showed a cyclic pattern of cracks. An overview of a conventional, systematic metallurgical approach to failure analysis to confirm that the cracking was caused by a caustic stress-corrosion cracking mechanism is presented.

Abstract
An investigation was conducted to determine the cause of numerous cracks and other defects on the surface of a cast ASTM A743 grade CA-15 stainless steel main boiler feed pump impeller. The surface was examined using a stereomicroscope, and macrofractography was conducted on several cross sections removed from the impeller body. Areas that appeared to have the most severe surface damage were sectioned, fractured open, and examined using SEM. The chemistry of the impeller and an apparent repair weld were also analyzed. The examination indicated that the cracks were shrinkage voids from the original casting process. Surface repair welds had been used to fill in or cover over larger shrinkage cavities. It was recommended that more stringent visual and nondestructive examination criteria be established for the castings.

Abstract
Radial cracking occurred adjacent to 11 vanes in a 19-vane impeller operating in a chemical plant environment. The impeller vanes were fillet welded to both the disk and the cover Cracks were next to the fillet welds and near the cover outer diameter They generally did not extend to the outer diameter. The entire impeller surface was tested by the dry magnetic particle method. Visual and microstructural examinations revealed intergranular cracking. Energy-dispersive spectroscopy of corrosion products contained in the cracks disclosed the presence of chlorine and sulfur The failure was attributed to stress-corrosion cracking caused by a corrosive atmosphere.

Abstract
A compression hip screw is a device designed to hold fractures in the area of the femur in alignment and under compression. A side plate, which is an integral part of the device, is attached by screws to the femur, and it holds the compression screw in position. The device analyzed had broken across the eighth hole (of nine holes) from the end of the plate. The detailed metallurgical failure analysis of the device, including metallography and fractography, is reported here. It was found that the device had adequate metallurgical integrity for the application for which it was intended. It is believed that failure was caused by the lack of a screw in the ninth hole. Evidence is also presented which indicates that the device was bent prior to insertion, and the local plastic deformation may have caused structural changes leading to premature crack initiation.

Engineering component and structure failures manifest through many mechanisms but are most often associated with fracture in one or more forms. This article introduces the subject of fractography and aspects of how it is used in failure analysis. The basic types of fracture processes (ductile, brittle, fatigue, and creep) are described briefly, principally in terms of fracture appearances. A description of the surface, structure, and behavior of each fracture process is also included. The article provides a framework from which a prospective analyst can begin to study the fracture of a component of interest in a failure investigation. Details on the mechanisms of deformation, brittle transgranular fracture, intergranular fracture, fatigue fracture, and environmentally affected fracture are also provided.

Abstract
An API type 2 steel clamp located on the riser of a semisubmersible drilling rig between the lower ball joint and riser blowout preventer (BOP) conductor failed after 7 years of service. Failure analysis revealed the cause of failure to be the low toughness of the clamp material. Contributing factors included the presence of a hard, brittle, heat-affected zone and weld defects at the handling pad eye. It was recommended that the replacement clamp be made from a material with good toughness and that any installation of attachments by welding be done according to qualified procedures.

Abstract
A ring-type joint in a reactor pipeline for a hydrocracker unit had failed. Cracks were observed on the flange and the associated ring gasket during an inspection following a periodic shutdown of the unit. The components were manufactured from stabilized grades of austenitic stainless steel; the flange from type 321, and the ring gasket from 347. Examination revealed that the failure occurred by transgranular stress-corrosion cracking, initiated by the presence of polythionic acid. Detailed metallurgical investigation was subsequently conducted to identify what may have caused the formation of polythionic acid in the process gas.

Abstract
The first-stage nozzles of a high-pressure turbine section of an industrial gas turbine exhibited leading and trailing-edge deterioration. The nozzles were made of X-40, a cobalt-base alloy, and were aluminide coated. Failure analysis determined that the deterioration was the result of hot corrosion caused by a combination of contaminants, cooling-hole blockage, and coating loss.

Abstract
Three 1006 carbon steels team/water separators failed in a boiler in installation after several years of service. Annual inspection had revealed no evidence of deterioration until the last inspection, when they were removed from service. Metallurgical investigation determined that the separators had deteriorated because of erosion corrosion. Further analysis of the boiler operation revealed that operational changes made in the last year of service caused an increase in velocity of the water/steam mixture. It was recommended that the operating parameters for the boiler be reevaluated and prior levels of operation be reinstituted.

Abstract
Cracks were discovered in the cast 17-4 PH stainless steel outboard leading edge flap support of an aircraft wing during overhaul inspection. Failure analysis focused on an apparently intergranular area of fracture surface. It was determined that the original mode of crack growth was cleavage, probably caused by cast-in hydrogen. The intergranular appearance resulted from heat treatment of the already cracked part, which caused the formation of grain-boundary “growth figures” on the exposed crack surfaces. It was recommended that the castings be more closely inspected for defects before further processing and that foundry practices be reviewed to correct deficiencies leading to excessive hydrogen absorption during melting and casting.

Abstract
An AMS 4126 (7075-T6) aluminum alloy impeller from a radial inflow turbine fractured during commissioning. Initial examination showed that two adjacent vanes had fractured through airfoils in the vicinity of the vane leading edges, and one vane fractured through an airfoil near the hub in the vicinity of the vane trailing edge. Some remaining vanes exhibited radial and transverse cracks in similar locations. Binocular and scanning electron microscope examinations showed that the cracks had been caused by high-cycle fatigue and had progressed from multiple origins on the vane surface. Structural analysis indicated that the fatigue loading probably had been caused by forced excitation, resulting in the impeller vibrating at its resonant frequency. It was recommended that the impeller design, control systems, and material of construction be changed.
