Abstract
AM350 stainless steel bellows used in the control rod drive mechanism of a fast breeder reactor failed after 1000 h of service in sodium at 550 deg C (1020 deg F). Helium leak testing indicated that leaks had occurred at various regions of the welded joints between the convolutes in the bellows. The weld failure was attributed to poor quality assurance during fabrication, which resulted in cracklike openings at the fusion zone. The openings extended during tensile loading. Use of proper welding procedures and quality control measures were recommended to prevent future failures.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

Abstract
Failure analysis was performed on a fractured impeller from a boiler feed pump of a fossil fuel power plant. The impeller was a 12% Cr martensitic stainless steel casting. The failure occurred near the outside diameter of the shroud in the vicinity of a section change at the shroud/vane junction. Sections cut from the impeller were examined visually and by SEM fractography. Microstructural, chemical, and surface analyses and surface hardness tests were conducted on the impeller segments. The results indicated that the impeller failed in fatigue with casting defects increasing stress and initiating fracture. In addition, the composition and hardness of the impeller did not meet specifications. Revision of the casting process and institution of quality assurance methods were recommended.

Abstract
Cold cracking of structural steel weldments is a well-documented failure mechanism, and extensive work has been done to recognize welding and materials selection parameters associated with it. These efforts, however, have not fully eliminated the occurrence of such failures. This article examines a case of cold cracking failure in the construction industry. Fortunately, the failure was identified prior to final erection of the structural members and the weld was successfully reworked. The article explains how various welding parameters, such as electrode/wire selection, joint design, and pre/postheating, played a role in the failure. Human factors and fabrication practices that contributed to the problem are covered as well.

Abstract
An HK-40 alloy tubing weld in a reformer furnace of a petrochemical plant failed by leaking after a shorter time than that predicted by design specifications. Leaking occurred because of cracks that passed through the thickness of the weldment. Analysis of the cracked tubing indicated that the sulfur and phosphorus contents of the weld metal were higher than specified, the thickness was narrower at the weld, and the mechanical resistance of the weld metal was lower than specified. Cracking initiated at the weld root by coalescence of creep cavities. Propagation and expansion was aided by internal carburization. Quality control of welding procedures and filler metal was recommended.

Abstract
A heat transport pump in a heavy water reactor failed (exhibiting excessive vibration) during a restart following a brief interruption in coolant flow due to a faulty valve. The pump had developed a large crack across the entire length of a bearing journal. An investigation to establish the root cause of the failure included chemical and metallurgical analysis, scanning electron fractography, mechanical property testing, finite element analysis of the shrink fitted journal, and a design review of the assembly fits. The journal failure was attributed to corrosion fatigue. Corrective actions to make the journals less susceptible to future failures were implemented and the process by which they were developed is described.

Abstract
Personnel responsible for laboratory protection at some plants are required to participate in exercises simulating a breach of security at the site. This document reports a metallurgical investigation of blank firing adapters (BFA), one of which exploded during such a training exercise. Determination of the cause of the explosion was the primary objective of the examination. Metallographic studies included the examination of BFAs fabricated from two different types of alloys that were tested for shock reaction. Optical microscopy supported by electron microscopy and analytical methods were used. Our investigation supports the supposition that a live round of ammunition was inadvertently fired.

Abstract
This article provides an outline of the issues to consider in performing a probabilistic life assessment. It begins with an historical background and introduces the most common methods. The article then describes those methods covering subjects such as the required random variable definitions, how uncertainty is quantified, and input for the associated random variables, as well as the characterization of the response uncertainty. Next, it focuses on specific and generic uncertainty propagation techniques: first- and second-order reliability methods, the response surface method, and the most frequently used simulation methods, standard Monte Carlo sampling, Latin hypercube sampling, and discrete probability distribution sampling. Further, the article discusses methods developed to analyze the results of probabilistic methods and covers the use of epistemic and aleatory sampling as well as several statistical techniques. Finally, it illustrates some of the techniques with application problems for which probabilistic analysis is an essential element.

Abstract

It reviews four fundamental categories of physical root causes, namely, design deficiencies, material defects, manufacturing/installation defects, and service life anomalies, with examples. Abstract
In the EMD-2 Joint Directed Attack Munition (JDAM), the A357 aluminum alloy housing had been redesigned and cast via permanent mold casting, but did not meet the design strength requirements of the previous design. Mechanical tests on thick and thin sections of the forward housing assembly revealed tensile properties well below the allowable design values. Radiology and CT evaluations revealed no casting defects. Optical microscopy revealed porosity uniformly distributed throughout the casting on the order of 0.1 mm pore diam. Scanning electron microscopy revealed elongated pores, which indicated turbulent filling of the mold. Spherical pores would have indicated the melt had been improperly degassed. Based on these findings, it was recommended that the manufacturer analyze and redesign the gating system to eliminate the turbulent flow problem during the permanent mold casting process.

Abstract
Bearing in mind the three-legged stool approach of device design/manufacturing, patient factors, and surgical technique, this article aims to inform the failure analyst of the metallurgical and materials engineering aspects of a medical device failure investigation. It focuses on the device "failures" that include fracture, wear, and corrosion. The article first discusses failure modes of long-term orthopedic and cardiovascular implants. The article then focuses on short-term implants, typically bone screws and plates. Lastly, failure modes of surgical tools are discussed. Abstract
Nondestructive metallographic examination of materials frequently must be performed on-site when the component in question cannot be moved or destructively examined. Often, it is imperative that specific microstructural information (i.e., material type, heat treatment condition, homogeneity, etc.) be obtained either before initial use of a component, or before the use of a component can be safely resumed. In this paper, the use of standard metallurgical laboratory equipment, and the procedures required to conduct nondestructive on-site metallographic analyses of engineering materials, is presented. As an example, the materials and metallographic techniques employed in an actual on-site investigation of a gas tungsten-arc weldment joining two large diameter Ti-6Al-4V alloy cylinders are discussed in depth to illustrate what can be accomplished.

Abstract
This article reviews the most common reasons for failures and the purpose of a failure investigation. It discusses the nine steps for the organization of a good failure investigation. The three basic tools that are helpful in any failure investigation, namely, a fault tree, a failure mode assessment chart, and a technical plan for resolution chart, are reviewed. Abstract
Nuclear power plants typically experience two or three high-cycle fatigue failures of stainless steel socket-welded connections in small bore piping during each plant-year of operation. This paper discusses fatigue-induced failure in socket-welded joints and the strategy Texas Utilities Electric Company (TU Electric) has implemented in response to these failures. High-cycle fatigue is invisible to proven commercial nondestructive evaluation (NDE) methods during crack initiation and the initial phases of crack growth. Under a constant applied stress, cracks grow at accelerating rates, which means cracks extend from a detectable size to a through-wall crack in a relatively short time. When fatigue cracks grow large enough to be visible to NDE, it is likely that the component is near the end of its useful life. TU Electric has determined that an inspection program designed to detect a crack prior to the component leaking would involve frequent inspections at a given location and that the cost of the inspection program would far exceed the benefits of avoiding a leak. Instead, TU Electric locates these cracks by visually monitoring for leaks. Field experience with fatigue-induced cracks in socket-welded joints has confirmed that visual monitoring does detect cracks in a timely manner, that these cracks do not result in catastrophic failures, and that the plant can be safely shut down in spite of a leaking socket-welded joint in a small bore pipe. Historical data from TU Electric and Southwest Research Institute are presented regarding the frequency of failures, failure locations, and the potential causes. The topics addressed include 1) metallurgical and fractographic features of fatigue cracks at the weld toe and weld root; 2) factors that are associated with fatigue, such as mechanical vibration, internal pulsation, joint design, and welding workmanship; and 3) implications of a leaking crack on plant safety. TU Electric has implemented the use of modified welding techniques for the fabrication of socket-welded joints that are expected to improve their ability to tolerate fatigue.

Abstract
The structural collapse of an iron-ore bucket-wheel stacker reclaimer at the beginning of operation was investigated by means of mechanical tests, microstructural characterization, and computational structural analysis. The mechanical failure was a consequence of a brittle fracture by cleavage. The crack followed the heat-affected zone of a welded joint connecting a rectangular hollow section member and a plate flange. The main factors contributing to failure were related with a combination of design-in and manufacturing-in factors like high load-strength ratio at the point of failure, local stress concentration as a result of geometry restrictions, and weld defects. This particular section was responsible for the load transfer between the front tie member and the boom extremity, and its failure was the main cause of the catastrophic failure of the equipment.

Abstract
This article describes the methodology for performing a failure modes and effects analysis (FMEA). It explains the methodology with the help of a hot water heater and provides a discussion on the role of FMEA in the design process. The article presents the analysis procedures and shows how proper planning, along with functional, interface, and detailed fault analyses, makes FMEA a process that facilitates the design throughout the product development cycle. It also discusses the use of fault equivalence to reduce the amount of labor required by the analysis. The article shows how fault trees are used to unify the analysis of failure modes caused by design errors, manufacturing and maintenance processes, materials, and so on, and to assess the probability of failure mode occurrence.
