Abstract
Galvanized A36 steel unsleeved shear-type anchor bolts failed during installation. The galvanized steel bolts were approximately 18 mm (0.7 in.) in diameter with a 90 deg bend between the long and short legs. As-fractured, sawcut, and unfractured specimens were examined. Failure analysis revealed dark thumbnail regions at the fracture origins and a very narrow and uniform shear lip. The thumbnail region exhibited zinc deposits with no apparent fracture detail, indicating preexisting cracks that had occurred before galvanizing. The balance of the fracture exhibited a transgranular mode with cleavage and ductile, dimpled shear. Hardness values as high as 35 HRC were measured in the bend area. The as-galvanized bolts fractured in a brittle manner. Failure was attributed to improper bending of the bolts, which provided a severely cold-worked bend area susceptible to strain-age embrittlement.

Abstract
The dished ends of a heavy water/helium storage tank manufactured from 8 mm (0.3 in.) thick type 304 stainless plate leaked during hydrotesting. Repeated attempts at repair welding did not alleviate the problem. Examination of samples from one dished end revealed that the cracking was confined to the heat affected zone (HAZ) surrounding circumferential welds and, to a lesser extent, radial welds that were part of the original construction. Most of the cracks initiated and propagated from the inside surface of the dished ends. Microstructures of the base metal, HAZ, and weld metal indicated severe sensitization in the HAZ due to high heat input during welding. An intergranular corrosion test confirmed the observations. The severe sensitization was coupled with residual stresses and exposure of the assembly to a coastal atmosphere during storage prior to installation. This combination of factors resulted in failure by stress-corrosion cracking. Implementation of a new repair procedure was recommended. Repairs were successfully made using the new procedure, and all cracks in the weld repair zones were eliminated.

Abstract
A weld that attached the head to the shell of a preheater containing steam at 1.4 MPa (200 psi) and was used in the manufacture of paper cracked in service. The original joint contained a 6.4 by 50 mm backing ring that had been tack welded to the inside surface of the shell in a position to project beyond the fully beveled top edge of the shell. The projecting edge of the ring fitted against a deep undercut on the inner corner of the rim of the head. The internal 90-deg angle in this undercut was sharp, with almost no fillet. A bevel from the lower edge of the undercut to the outside of the head completed the groove for the circumferential attachment weld. Investigation (visual inspection and actual size views etched in hot 50% hydrochloric acid) supported the conclusion that cracking occurred in the HAZ in the head of the original design, originating in the sharp corner of the undercut, which was an inherent stress raiser. Recommendations included revised joint design to ensure full root penetration.

Abstract
This article provides assistance to a failure analyst in broadening the initial scope of the investigation of a physical engineering failure in order to identify the root cause of a problem. Abstract
A steam heated exchanger was designed for concentrating sulfuric acid. Tantalum was selected for the tubing and the tube sheet liner because of its outstanding corrosion resistance. However, although the exchanger passed a searching shop inspection, it leaked during site testing. Considerable argument ensued about whether the cracking observed was due to poor welding during fabrication, or through abuse during handling on site. An SEM examination of the fractures revealed high cycle, low amplitude fatigue, and the problem was traced to vibration during road transport. Further failures were avoided by improved design and packing. This paper illustrated the value of SEM fractography when a rapid investigation is needed under the pressures of a fast moving project.

Abstract
Corrosion in potable and nonpotable water systems has been well documented in the past, and new research discusses innovations in water treatment and materials that are designed to enhance the quality of a water system, whether commercial or residential. This paper is a collection of five case histories on the failure of copper and steels as used in potable and non-potable water systems. The case histories cover a range of applications in which copper and steel products have been used. Copper and steel pipes are the two most commonly used materials in residential, commercial and industrial applications. The projects that are discussed cover these three important applications. The purpose of presenting this information is to allow the reader to gain an understanding of real life corrosion issues that affect plumbing materials, how they should have been addressed during the design of the water system, and how a water system should be maintained during service. We share this information in the hope that the reader will gain some limited knowledge of the problems that exist, and apply that knowledge in designing or using water systems in day-to day life.

Abstract
Microstructure, corrosion, and fracture morphologies of prestressed steel wires that failed in service on concrete siphons at the Central Arizona Project (CAP) are discussed. The CAP conveys water for municipal, industrial, and agricultural use through a system of canals, tunnels, and siphons from Lake Havasu to just south of Tucson, AZ. Six siphons were made from prestressed concrete pipe units 6.4 m (21 ft) in diam and 7.7 m long, making them the largest circular precast structures ever built. The pipe was manufactured on site and consisted of a 495-mm thick concrete core, wrapped with ASTM A648 steel prestressing wire. All of the CAP failures evaluated were attributed to corrosion. Longitudinal splits reduced the service life of the pipe significantly by facilitating corrosion and introducing sharp cracks into the microstructure of the wire. A few failures were attributed to general corrosion, where the cross section of the wire is reduced until the strength of the wire is exceeded. Most of the failures evaluated were attributed to stress-corrosion cracking.

Abstract
Two steam-methane reformer furnaces were subjected to short-time heat excursions because of a power outage, which resulted in creep bulging in the Incoloy 800 outlet pigtails, requiring complete replacement. Each furnace had three cells, consisting of 112 vertical tubes per cell, each filled with a nickel catalyst. The tubes were centrifugally cast from ASTM A297, grade HK-40 (Fe-25Cr-20Ni-0.40C), heat-resistant alloy. The tube was concluded after metallurgical inspection to have failed from creep rupture (i.e., stress rupture). A project for detecting midwall creep fissuring was instigated as a result of the failure. It was concluded after laboratory radiography and macroexamination that if the fissure were large enough to show on a radiograph, either with or without the catalyst, the tube could be expected to fail within one year. The set up for in-service radiograph examination was described. The tubes of the furnace were radiographed during shut down and twenty-four tubes in the first furnace and 53 in the second furnace showed significant fissuring. Although, radiography was concluded to be a practical technique to provide advance information, it was limited to detecting fissures caused by third-stage creep in tubes because of the cost involved in removing the catalysts.

Abstract
Although field corrosion tests had indicated that type 316L stainless steel would be a suitable material for neutralization tanks, the vessels suffered severe corrosion when placed in service. Welded coupons of type 316L had been tested along with similar Alloy 20Cb® (UNS NO8020) specimens in a lead-lined tank equipped with copper coils that had served in this function prior to construction of the new tanks. Both materials exhibited virtually no corrosion and no preferential weld attack. Type 316L was selected for the project. The subsequent corrosion was the result of the borderline passivity of type 316L in hot dilute sulfuric acid (about 0.1%). Inaccuracy of the testing was attributed to the presence of cupric ions in the lead-lined vessel fluids, which had been released by corrosion of the copper coils. Careful control of both temperature and pH was recommended to reduce the corrosion to an acceptable limit.

Abstract
A type 431 stainless steel mushroom-head closure fractured in service at a hydrogen pressure of 3000 atm. Fracture occurred at room temperature after miscellaneous chemical service that included exposures to hydrogen at temperatures from ambient to 350 deg C (662 deg F). Investigation (visual inspection and 2400x/6600x TEM analysis) supported the conclusion that failure was caused by hydrogen embrittlement, not SCC as might have been suspected. No recommendations were made.
