Abstract
A failure analysis investigation was conducted on a fractured aluminum tailwheel fork which failed moments after the landing of a privately owned, 1955 twin-engine airplane. Nondestructive evaluation via dye-penetrant inspection revealed no discernible surface cracks. The chemical composition of the sand-cast component was identified via optical emission spectroscopy and is comparable to an aluminum sand-cast alloy, AA 712.0. Metallographic evaluation via optical microscopy and scanning electron microscopy revealed a high degree of porosity in the microstructure as well as the presence of deleterious intermetallic compounds within interdendritic regions. Macrohardness testing produced hardness values which are noticeably higher than standard hardness values for 712.0. The primary fracture surfaces indicate evidence of mixed-mode fracture, via intergranular cracking, cleaved intermetallic particles, and dimpled cellular regions in the matrix. The secondary fracture surface demonstrates similar features of intergranular fracture.

The propeller from a small private airplane came off in flight. The head ends of all six attachment bolts remained in the propeller hub when it was found. Two threaded shanks with nuts remained with the engine, while the remaining four shank ends with their nuts were missing. Parts available for examination, in addition to the hub and attachment bolts, were the two propeller blades and the engine crankshaft. The purpose of this examination was to determine the nature and probable cause of failure in the six attachment bolts. Indications of fatigue failure and wear were the major findings in visual and low power microscopic examination. Fracture surfaces indicated failure was initiated in the threads in four bolts and in the shanks in two. The group of four bolts failed primarily due to tensile loads, while the other two bolts failed primarily due to bending loads. It was concluded that failure was due to improper installation torqueing of the bolts.

Abstract
Initial investigation showed that a landing gear failure was the result of a hard landing with no evidence of contributory factors. The objective of reexamination was to determine whether there was any evidence of metallurgical failure. The landing gear was primarily an AISI type 6150 Cr-V steel flat spring attached at the top end to the fuselage and at the bottom end to the axle. Failure occurred at the clamping point near the top end of this spring. The failure showed evidence of severe brinelling at one corner in the clamping area. The fracture surfaces were clean, fresh, and indicative of a shock type of failure pattern. Closer examination, however, showed a fatigue crack at one corner. At this point, there was definite evidence of progression and oxidation. It was concluded that the corner in question was subjected to repeated brinelling resulting from normal landing loads, probably accentuated by looseness in the clamping device. The resulting residual tensile stress lowered the effective fatigue strength at that point against drag and side loads.

Abstract
In this article, we report the outcome of an investigation made to uncover the premature fracture of crusher jaws produced in a local foundry. A crusher jaw that had failed while in service was studied through metallographic techniques to determine the cause of the failure. Our investigation revealed that the reason for the fracture was the presence of large carbides at the grain boundaries and in the grain matrix. This led to the formation of microcracks that propagated along the grain boundaries under in-service working forces. It is also believed that the precipitation of carbides at the grain boundaries may have occurred because of improper heat treatment, but not because of a deficiency in composition.

Abstract
Stainless steel pipe (273-mm OD x 8-mm wall thickness) used in the fabrication of large manifolds developed crack-like decohesions during a routine inductive bending procedure. The imperfections, which were found near the outside diameter, were around 3 mm in length oriented in the circumferential direction and penetrated nearly 2 mm into the pipe wall. The pipes were made of titanium-stabilized austenitic stainless steel X6CrNiMoTi17-12-2. Six hypotheses were considered during the investigation, which ultimately concluded that the failure was caused by liquation cracking due to overheating.

Abstract
The failure of a 90-10 cupronickel heat exchanger tube resulted in flooding of the vessel and subsequently sinking it. The corrosion of the cupronickel alloy was facilitated by the high sulfur content of the seawater in which it operated. The failure modes were anodic dissolution and copper reprecipitation.

Abstract
Piping and structural components used in space launch facilities such as NASA's Kennedy Space Center and the Air Force's Cape Canaveral Air Station face extreme operating conditions. Launch effluent and residue from solid rocket boosters react with moisture to form hydrochloric acid that settles on exposed surfaces as they are being subjected to severe mechanical loads imparted during lift-off. Failure analyses were performed on 304 stainless steel tubing that ruptured under such conditions, while carrying various gases, including nitrogen, oxygen, and breathing air. Hydrostatic testing indicated a burst strength of 13,500 psi for the intact sections of tubing. Scanning electron microscopy and metallographic examination revealed that the tubing failed due to corrosion pitting exacerbated by stress-corrosion cracking (SCC). The pitting originated on the outer surface of the tube and ranged from superficial to severe, with some pits extending through 75% of the tube's wall thickness. The SCC emanated from the pits and further reduced the service strength of the component until it could no longer sustain the operating pressure and final catastrophic fracture occurred. Corrosion-resistant coatings added after the investigation have proven effective in preventing subsequent such failures.

Abstract
Following a fracture mechanics “fitness-for-purpose” analysis of petroleum industry cold service pressure vessels, using the British Standard PD 6493, it was realized that an analogous approach could be used for the failure analysis of a similar pressure vessel dome which had failed in service some years previously. The failed pressure vessel, with a diam of 2.5 m and several meters tall, had been made of 12 mm thick IZETT steel plate of the same type and heat treatment as used in the earlier fitness-for-purpose already measured. Examination of the fracture surfaces suggested, from fatigue striations manifested by SEM, that the vessel was subject to significant fatigue cracking, which was probably corrosion assisted. From COD measurements at the operating temperature of -130 deg C (-202 deg F), and a finite stress analysis, a fracture mechanics evaluation using BS PD6493 yielded realistic critical flaw sizes (in the range 51 to 150 mm). These sizes were consistent with the limited fracture surface observations and such flaws could well have been present in the vessel dome prior to catastrophic failure. For similar pressure vessels, an inspection program based on a leak-before-break philosophy was consequently regarded as acceptable.

Abstract
A precipitation-hardened stainless steel poppet valve assembly used to shut off the flow of hydrazine fuel to an auxiliary power unit was found to leak. SEM and optical micrographs revealed that the final heat treatment designed for the AM-350 bellows material rendered the AM-355 poppet susceptible to intergranular corrosive attack (IGA) from a decontaminant containing hydroxy-acetic acid. This attack provided pathways for which fluid could leak across the sealing surface in the closed condition. It was concluded that the current design is flight worthy if the poppet valve assembly passes a preflight helium pressure test. However a future design should use the same material for the poppet and bellows so that the final heat treatment will produce an assembly not susceptible to IGA.

Abstract
A catastrophic brittle fracture occurred in a welded steel (ASTM A517 grade H) trapezoidal cross-section box girder while the concrete deck of a large bridge was being poured. The failure occurred across the full width of a 57 mm (2 1 4 in.) thick, 760 mm (30 in.) wide flange and arrested 100 mm (4 in.) down the slant web. Failure analysis revealed a major deficiency in fracture toughness. The failure occurred as a brittle fracture after the formation of a welding hot crack and approximately 40 mm (1 1 2 in.) of slow crack growth. It was recommended that bridges fabricated from this grade of steel undergo frequent inspection and that stringent test requirements be imposed as a condition of use in non-redundant main load-carrying components.

Abstract
Lakes in zinc die castings are areas encompassed by irregular lines or waves on flat or slightly contoured surfaces which are intended to look uniform. The laked areas have to be removed by polishing before the castings can be plated. This adds considerably to the overall cost of production. Castings examined were of an automobile name-plate holder with two flat sides of approximately 113 sq cm. All castings produced during a trial showed laking defects, the number and position varying from casting to casting. It was found that formation of metal waves and lakes depended primarily on the design of the gate and runner system and operating conditions. High flow efficiencies, with adequate feeding to all sections of the die, and short cavity fill times are desirable.

Abstract
Life testing of cyclic loaded, miniature extension springs made of 17-7 PH stainless steel wire and AISI 302 Condition B stainless steel wire has shown end hook configuration to be a major source of weakness. To avoid cracking and subsequent fatigue failure, it was found that stress concentration depended on end hook bend sharpness. Also, interference fits are to be avoided in the end hooks of small springs. Additionally, a need for careful consideration of the stress-corrosion properties of candidate materials for spring applications has been demonstrated by stress-corrosion test results for 17-7 PH CH900 and for Custom 455 CH850 stainless steels. Laboratory testing of these two materials in the form of compression springs confirmed the superiority of the 17-7 PH over Custom 455.

Abstract
This article provides information on life assessment strategies and conceptually illustrates the interplay of nondestructive evaluation (NDE) and fracture mechanics in the damage tolerant approach. It presents information on probability of detection (POD) and probability of false alarm (PFA). The article describes the damage tolerance approach to life management of cyclic-limited engine components and lists the commonly used nondestructive evaluation methods. It concludes with an illustration on the role of NDE, as quantified by POD, in fully probabilistic life management.

Abstract
Copper electrical feedthrough pins used in a bolting application in a refrigeration compressor had functioned without failure for years of production and thousands of units. When some of the pins began to fail, an investigation was conducted to determine the cause. Visual examination revealed that the observed fractures were mixed brittle intergranular with ductile microvoid dimples. An extensive analysis of failed samples combined with a process of elimination indicated that the fractures were due to stress-corrosion cracking caused by an unidentified chemical species within the sealed compressor chamber. A unique combination of applied stress, residual stress, stress riser, and grain size helped isolate the failure mechanism to a single production lot of material.

Abstract
This investigation characterizes five surgical stainless steel piercings and one niobium piercing that caused adverse reactions during use, culminating with the removal of the jewelry. Chemical composition shows that none of the materials are in accordance with ISO standards for surgical implant materials. Additionally, none of the stainless steel piercings passed the pitting-resistance criterion of ISO 5832-1, which implies that [%Cr + 3.3(%Mo)] > 26. Under microscopic examination, most of the jewelry revealed the intense presence of linear irregularities on the surface. The lack of resistance to pitting corrosion associated with the poor surface finishing of the stainless steel jewelry may induce localized corrosion, promoting the release of cytotoxic metallic ions (such as Cr, Ni, and Mo) in the local tissue, which can promote several types of adverse effects in the human body, including allergic reactions. The adverse reaction to the niobium jewelry could not be directly associated with the liberation of niobium ions or the residual presence of cytotoxic elements such as Co, Ni, Mo, and Cr. The poor surface finish of the niobium jewelry seems to be the only variable of the material that may promote adverse reactions.

Abstract
A liquid hydrogen main fuel control valve for a rocket engine failed by fracture of the Ti-5Al-2.5Sn body during the last of a series of static engine test firings. Fractographic, metallurgical, and stress analyses determined that a combination of fatigue and unexpected aqueous stress-corrosion cracking initiated and propagated the crack which caused failure. The failure analysis approach and its results are described to illustrate how fractography and fracture mechanics, together with a knowledge of the crack initiation and propagation mechanisms of the valve material under various stress states and environments, helped investigators to trace the cause of failure.

Abstract
As the result of a leak detected in a plate-formed header at PENELEC'S Shawville Unit No. 3, an extensive failure investigation was initiated to determine the origin of cracking visible along the longitudinal weld seam. Fabricated from SA387-D material and designed for a superheater outlet temperature of 566 deg C, the 11.4 cm thick header had operated for approximately 187,000 h at the time of the failure. Discussion focuses on the results of a metallographic examination of boat samples removed from the longitudinal seam weldment in the vicinity of the failure and at other areas of the header where peak temperatures were believed to have been reached. The long-term mechanical properties of the service-exposed base metal and creep-damaged weld metal were determined by creep testing. Based on the utility's decision to replace the header within one to three years, an isostress overtemperature lead specimen approach was taken, whereby failure of a test specimen in the laboratory would precede failures in the plant. These tests revealed approximately a 2:1 difference in life for the base metal as compared to weld metal.

Abstract
The failure mode of through-wall cracking of a butt weld in a 5083-O aluminum alloy piping system in an ethylene plant was identified as mercury liquid metal embrittlement. As a result of this finding, 226 of the more than 400 butt welds in the system were ultrasonically inspected for cracking. One additional weld was found that had been degraded by mercury. A welding team experienced in repairing mercury contaminated piping was recruited to make the repairs. Corrective action included the installation of a sulfur-impregnated charcoal mercury-removal bed and replacement of the aluminum equipment that was in operation prior to the installation of the mercury-removal bed.

Abstract
Alloy 430 stainless steel tube-to-header welds failed in a heat recovery steam generator (HRSG) within one year of commissioning. The HRSG was in a combined cycle, gas-fired, combustion turbine electric power plant. Alloy 430, a 17% Cr ferritic stainless steel, was selected because of its resistance to chloride and sulfuric acid dewpoint corrosion under conditions potentially present in the HRSG low-pressure feedwater economizer. Intergranular corrosion and cracking were found in the weld metal and heat-affected zones. The hardness in these regions was up to 35 HRC, and the weld had received a postweld heat treatment (PWHT). Metallographic examination revealed that the corroded areas contained undertempered martensite. Fully tempered weld areas with a hardness of 93 HRB were not attacked. No evidence of corrosion fatigue was found. Uneven temperature control during PWHT was the most likely cause of failure.

Abstract
A fire in a storage yard engulfed several propane delivery trucks, causing one of them to explode. A series of elevated-temperature stress-rupture tears developed along the top of the truck-mounted tank as it was heated by the fire. Unstable fracture then occurred suddenly along the length of the tank and around both end caps, following the girth welds that connect them to the center portion of the tank. The remaining contents of the tank were suddenly released, aerosolized, and combusted, creating a powerful boiling liquid expanding vapor explosion (BLEVE). Based on the metallography of the tank pieces, the approximate tank temperature at the onset of explosion was determined. Metallurgical analysis provided additional insights as well as a framework for making tanks less susceptible to this destructive failure mechanism.
