Abstract
A 20 ton polar crane motor fell during a 3400 kg (7500 lb) lift, narrowly missing personnel working beneath the crane. Witnesses reported that the motor fall was preceded by a falling oil mass, and it was believed that the motor was intact prior to impact. The maintenance history of the crane showed that the motor had been removed, repaired, and reinstalled 2 years prior to the failure. Observations of oil leakage were noted yearly up to the failure. The motor casing was held onto the adapter plate by eight 14-20 UNC x 25 mm (1 in.) long hex socket cap screws. Examination of the motor adapter plate, motor casing shards (aluminum), the gear side of the motor housing, and seven fractured cap screws (ASTM A574) showed that the motor casing was intact at the time of “uncontrolled descent” and that the screws had failed by high nominal stress reverse bending load fatigue, which was probably the result of insufficient torque on the bolts.

Abstract
Some 99.90 pure tin tubes (0.15 mm thick) used for packaging a chemical compound cracked on bending and underwent brittle fracture prior to filling, while others remained ductile and showed no sign of failure. Examination showed that specimens prepared by mechanical methods such as electrolytic and hand polishing and the vibration method resulted in poor edge and crack edge definition due to material thickness. Etching experiments involved a grain surface attack and hence produced a rather strong surface relief from which the grain boundary cracks could again not clearly be differentiated. The sections were therefore examined unetched in polarized light. The microstructure of the cracked tubes was shown to have much smaller grains than the ductile and showed cracks from the surface down along the grain boundaries. Material hardness also differed between the unusable tubes and the ductile, and chemical analysis showed a higher level of aluminum in the brittle specimens. Failure obviously occurred due to the high material aluminum content that increased hardness which then caused embrittlement at the surface which led to cracks or fracture on bending. Since no explanation of how the aluminum entered the tin was available, no recommendations could be made.

Abstract
The results of a failure analysis of a series of Cr-Mo-V steel turbine studs which had experienced a service lifetime of some 50,000 h are described. It was observed that certain studs suffered complete fracture while others showed significant defects located at the first stress bearing thread. Crack extension was the result of marked creep embrittlement and reverse temper embrittlement (RTE). Selected approaches were examined to assess the effects of RTE on the material toughness of selected studs. It was observed that Auger electron microscopy results which indicated the extent of grain boundary phosphorus segregation exhibited a good relationship with ambient temperature Charpy data. The electrochemical polarization kinetic reactivation, EPR, approach, however, proved disappointing in that the overlapping scatter in the minimum current density, Ir, for an embrittled and a non-embrittled material was such that no clear decision of the toughness properties was possible by this approach. The initial results obtained from small punch testing showed good agreement with other reported data and could be related to the FATT. Indeed, this small punch test, combined with a miniature sample sampling method, represents an attractive approach to the toughness assessment of critical power plant components.

Abstract
Military aircraft use a cartridge ignition system for emergency engine starts. Analysis of premature failures of steel (AISI 4340) breech chambers in which the solid propellant cartridges were burned identified corrosion as one problem with an indication that stress-corrosion cracking may have occurred. A study was made for stress-corrosion cracking susceptibility of 4340 steel in a paste made of the residues collected from used breech chambers. The constant extension rate test (CERT) technique was employed and SCC susceptibility was demonstrated. The residues, which contained both combustion products from the cartridges and corrosion products from the chamber, were analyzed using elemental analysis and x-ray diffraction techniques. Electrochemical polarization techniques were also utilized to estimate corrosion rates.

Abstract
This study examines the role of calcium-precipitating bacteria (CPB) in heat exchanger tube failures. Several types of bacteria, including Serratia sp. (FJ973548), Enterobacter sp. (FJ973549, FJ973550), and Enterococcus sp. (FJ973551), were found in scale collected from heat exchanger tubes taken out of service at a gas turbine power station. The corrosive effect of each type of bacteria on mild steel was investigated using electrochemical (polarization and impedance) techniques, and the biogenic calcium scale formations analyzed by XRD. It was shown that the bacteria contribute directly to the formation of calcium carbonate, a critical factor in the buildup of scale and pitting corrosion on heat exchanger tubes.

Abstract
Welded steel storage vessels used to hold mildly alkaline solution were produced in exactly the same manner from deep-drawn aluminum-killed SAE 1006 low-carbon steel sheet. After the cylindrical shell was drawn, a top low-carbon steel closure was welded to the inside diameter. The containers were then filled with the slightly alkaline solution, pressurized, and allowed to stand under ambient conditions. A small number, less than 1%, were returned because they began to leak in service. Inspection revealed general corrosion and pitting on the inner surfaces. However, other tanks that experienced the same service conditions developed no corrosion. Corrosion was linked to forming defects that provided sites for localized corrosion, and to lack of steam drying after cleaning, which increased susceptibility to general corrosion.

Abstract
This article addresses the forms of corrosion that contribute directly to the failure of metal parts or that render them susceptible to failure by some other mechanism. It describes the mechanisms of corrosive attack for specific forms of corrosion such as galvanic corrosion, uniform corrosion, pitting and crevice corrosion, intergranular corrosion, and velocity-affected corrosion. The article contains a table that lists combinations of alloys and environments subjected to selective leaching and the elements removed by leaching.

Abstract
Corrosion is the electrochemical reaction of a material and its environment. This article addresses those forms of corrosion that contribute directly to the failure of metal parts or that render them susceptible to failure by some other mechanism. Various forms of corrosion covered are galvanic corrosion, uniform corrosion, pitting, crevice corrosion, intergranular corrosion, selective leaching, and velocity-affected corrosion. In particular, mechanisms of corrosive attack for specific forms of corrosion, as well as evaluation and factors contributing to these forms, are described. These reviews of corrosion forms and mechanisms are intended to assist the reader in developing an understanding of the underlying principles of corrosion; acquiring such an understanding is the first step in recognizing and analyzing corrosion-related failures and in formulating preventive measures.

Abstract
A group of control valves that regulate production in a field of sour gas wellheads performed satisfactorily for three years before pits and cracks were detected during an inspection. One of the valves was examined using chemical and microstructural analysis to determine the cause of failure and provide preventive measures. The valve body was made of A216-WCC cast carbon steel. Its inner surface was covered with cracks stemming from surface pits. Investigators concluded that the failure was caused by a combination of hydrogen-induced corrosion cracking and sulfide stress-corrosion cracking. Based on test data and cost, A217-WC9 cast Cr–Mo steel would be a better alloy for the application.

The Rocky Point Viaduct, located near Port Orford, OR, was replaced after only 40 years of service. A beam from the original viaduct was studied in detail to determine the mechanisms contributing to severe corrosion damage to the structure. Results are presented from the delamination survey, potential and corrosion mapping, concrete chemistry, and concrete physical properties. The major cause of corrosion damage appears to have been the presence of both pre-existing and environmentally-delivered chlorides in the concrete.

Abstract
An octagonal steel ingot weighing 13 tons made of manganese-molybdenum steel developed gaping cross-cracks on all eight sides in the forging press during initial pressure application. It was reported that the steel had been melted in a basic 12-ton arc furnace, oxygenated, furnished with 42 kg of 75% ferrosilicon and 12 kg aluminum additions, alloyed with 160 kg of 80% ferromanganese, and finally deoxidized in the ladle with 42 kg calcium silicon. For metallographic examination a plate approximately 100 mm thick was cut parallel to one of the eight planes. Platelet-like particles could be discerned on the conchoidal fracture planes with the SEM. The precipitates proved to be thin and partially transparent platelets of a hexagonal crystal lattice whose parameters resemble those of AIN. The precipitates were at least in part still undissolved in spite of the long holding period at high initial forging temperature. Another block melted under the same conditions and immediately after the defective one, was forged into a gear ring without any trouble. This ring was free of grain boundary precipitates, but it contained only 0.012 % AI and 0.0102 % N.

Four tanks made from type 304L stainless steel were removed from storage. Atmospheric corrosion on the outside of the tanks and pitting and crevice corrosion on the inside were visible. Metallographic examination revealed that the internal corrosion had been caused by crevices related to weld spatter and uneven weld deposit and by service water that had not been drained after hydrostatic testing. External corrosion was attributed to improper passivation. It was recommended that the surfaces be properly passivated and that, before storage, the interiors be rinsed with demineralized water and dried.

Abstract
An austenitic stainless steel (type 316/316L stainless steel, schedule 40, 64 mm (2.5 in.) diam and larger) piping network used in the fire-sprinkler system in a large saltwater passenger and car ferry failed by rapid leaking. Operating conditions involved stagnant seawater at ambient temperatures. The pipe was in service for four weeks when three leaks appeared. Investigation (visual inspection and photographic images) supported the conclusion that the failure was caused by attack and corrosion damage of Cl ions in conditions that were ideal for three modes of highly accelerated pitting of austenitic stainless steel: the bottom surface, weld or HAZ pits, and crevices. Recommendations included proper material selection for piping, flanges, and weld rods with greater corrosion resistance. Proper filtering to prevent entrained abrasives and timely breakdown inspections were also advised.

The purpose of this investigation was to determine the cause of the ultrasonic signal attenuation noted during an inspection of a composite aircraft component. Although ultrasonics was able to identify the location of the defective areas, destructive analysis had to be utilized to determine the exact nature of the defect. The investigation describes how cross-sectioning, fractography, and chemical analysis were utilized to determine the type of defect responsible for the signal attenuation.

Abstract
Although field corrosion tests had indicated that type 316L stainless steel would be a suitable material for neutralization tanks, the vessels suffered severe corrosion when placed in service. Welded coupons of type 316L had been tested along with similar Alloy 20Cb® (UNS NO8020) specimens in a lead-lined tank equipped with copper coils that had served in this function prior to construction of the new tanks. Both materials exhibited virtually no corrosion and no preferential weld attack. Type 316L was selected for the project. The subsequent corrosion was the result of the borderline passivity of type 316L in hot dilute sulfuric acid (about 0.1%). Inaccuracy of the testing was attributed to the presence of cupric ions in the lead-lined vessel fluids, which had been released by corrosion of the copper coils. Careful control of both temperature and pH was recommended to reduce the corrosion to an acceptable limit.

Abstract
An evaluation of indications in the main turbine building column horizontal plate welds was conducted by the joint efforts of field metallography and nondestructive examinations. The turbine building main column horizontal plate welds were selected at random and were inspected to find discontinuities, metallurgical evaluation of the discontinuities, analysis of any failure modes, and determination of the best repair techniques. The welds were made with prequalified joints in accordance with AWS D1.1-77 and required only visual inspection. More sensitive inspection methods were applied to the welds in order to better define the indications found with the visual inspections. Cracks were found in 17 field welds and in two test plate welds. The causes of the cracking are related to the weld design and installation procedure. Three field welds were rejected because of the depth of the cracks. The NDT inspections, evaluations, method of field metallography, analysis and conclusions are discussed with recommendations for corrective actions in the following report.

This article focuses on the mechanisms of microbially induced or influenced corrosion (MIC) of metallic materials as an introduction to the recognition, management, and prevention of microbiological corrosion failures in piping, tanks, heat exchangers, and cooling towers. It discusses the degradation of various protective systems, such as corrosion inhibitors and lubricants. The article describes the failure analysis of steel, iron, copper, aluminum, and their alloys. It also discusses the probes available to monitor conditions relevant to MIC in industrial systems and the sampling and analysis of conditions usually achieved by the installation of removable coupons in the target system. The article also explains the prevention and control strategies of MIC in industrial systems.

It describes corrosion failure analysis and discusses corrective and preventive approaches to mitigate corrosion-related failures of metals. These include: change in the environment; change in the alloy or heat treatment; change in design; use of galvanic protection; use of inhibitors; use of nonmetallic coatings and liners; application of metallic coatings; use of surface treatments, thermal spray, or other surface modifications; corrosion monitoring; and preventive maintenance.

A flanged 100 mm (4 in.) diam low-carbon steel spool piece lined with Teflon was removed from a sulfuric acid denitrification system after cracks were observed in the painted coating. Visual and microstructural examination along with SEM fractography revealed scaled iron oxides on all opened crack surfaces. The surfaces had a faceted morphology, indicating intergranular fracture. Cracks originated at the interface between the tube and the Teflon liner Corrosion products were found caked into the intergranular region between the liner and the spool. The portion of the liner that had been exposed to the process stream was discolored. Failure of the spool was attributed to stress-corrosion cracking promoted by the presence of nitrates. Nitric acid contaminant in the sulfuric acid stream had diffused through the liner and accumulated in the annular space. Use of a liner that is more impermeable to the diffusion of ionic species was recommended.

A number of seamless pipe nipples of 70 mm diam and 3.5 mm wall thickness made of steel type 35.8 were oxyacetylene welded to collectors of greater wall thickness with a round bead. X-ray examination showed crack initiation in the interior of the nipples close to the root of the weld seam. The cracks only appeared where the originally deposited bead was remelted in the regions of overlap. Given the construction and welding technique used, it would have been preferable to make the nipples of a steel lower in sulfur content. However, by taking advantage of all the potential in shaping and welding technology, it should be possible to prevent crack formation with steel type 35.8 of normal composition.
