Abstract
The cause of fracture of two piston rods of hammers of a drop forge was determined. The first rod of 180 mm diam consisted of an unalloyed steel with 0.37% C and 0.67% Mn and had a strength of 56 kp/sq mm at 26% elongation. Fatigue fractures propagated from several points which could be recognized as flaky cracks already in the fracture, and which later were united. No material defects could be detected in the cross section parallel to the fracture plane except for these very short cracks. These comparatively insignificant defects were sufficient to cause the fracture during high impact fatigue stresses in the drop forge. The second piston rod of 120 mm diam consisted of a steel with 0.25% C and 1.00% Mn. It allegedly had 57 kp/sq mm tensile strength and 26% elongation. The basic structure of the 120 mm piston rod was ferritic-pearlitic and hardness of 155 Brinell was accordingly low, corresponding to approximately 53 kp/sq mm tensile strength. The incipient fractures had no connection with the material defects in this shaft and therefore the fracture could not have been caused by them. Probably the low strength of the piston rod was insufficient for the high stresses.

Abstract
A 2.3 mm diam steel wire broke during cable twisting. The fracture occurred obliquely to the longitudinal axis of the wire and showed a constriction at the end. Therefore it was a ductile fracture. File mark type work defects were noticeable on the wire surface at both sides of the fracture, but they had no effect on the breakage of the wire. Away from the fracture area, the wire had a normal structure of hyperfine lamellar pearlite (sorbite) of a “patented” and cold drawn steel wire. In the vicinity of the fracture, the cementite of the pearlite was partially spheroidized, while at the fracture itself it was completely spheroidized. Therefore the wire was locally annealed at this point. It was likely that the wire cracked at this point during the last drawing and then broke during twisting due to its lower strength in the weakened cross section after prior deformation.

Abstract
The failure of a high speed steel twist drill which caused injury to the user was investigated thoroughly to settle a legal suit. The drill was being used to remove a stud that broke in the vertical wall of a metalworking machine (upsetter) after drilling a pilot hole. The drill had shattered suddenly with a bang which caused a chip to be dislodged and cause the injury. A large nonmetallic inclusion parallel to the axis near the center of the drill was revealed in an unetched longitudinal section. Carbide bands in a martensitic matrix were indicated in an etched sample. It was concluded by the plaintiff's metallurgist that the failed drill was defective as the steel contained nonmetallic inclusions and carbide segregation which made it brittle. It was revealed by the defendant that the twist drill met all specifications of M1 high-speed steel and investigated several other drills without failure to prove that the failure was caused by use in excessive conditions. It was revealed by examination that the point of the broken drill was not the original point put on at manufacture but came from regrinding. Both technical and legal details have been discussed.

Abstract
A valve spring made of 4.1 mm diam wire, designed to withstand 10,000,000 stress cycles, fractured after only 2,000,000 cycles. The surface displayed impressions which indicated it had been treated by shot blasting. The spring has broken in two places. Fracture 1 was a torsional fatigue fracture which has started from a lobe-like surface defect and not, as is usual, from a point on the most highly stressed inner surface. Fracture 2, on the other hand, was a bending fatigue fracture with a starting point on both the inner and the outer surface of the spiral. The objective of the shot blasting, to put the surface into a state of even compressive internal stress, which must first be overcome during subsequent bending and torsional loading before the boundary zone comes under tensile stress, was therefore not realized in this case. On the contrary, the shot blasting led to a state of internal stress which favored fracture of the spring.

Abstract
The structural collapse of an iron-ore bucket-wheel stacker reclaimer at the beginning of operation was investigated by means of mechanical tests, microstructural characterization, and computational structural analysis. The mechanical failure was a consequence of a brittle fracture by cleavage. The crack followed the heat-affected zone of a welded joint connecting a rectangular hollow section member and a plate flange. The main factors contributing to failure were related with a combination of design-in and manufacturing-in factors like high load-strength ratio at the point of failure, local stress concentration as a result of geometry restrictions, and weld defects. This particular section was responsible for the load transfer between the front tie member and the boom extremity, and its failure was the main cause of the catastrophic failure of the equipment.

Abstract
An aftercooler was of conventional design and fitted with brass tubes through which cooling-water circulated. Air at 100 psi pressure was passed over the outsides of the tubes, entering the vessel near to the upper tubeplate on one side and leaving it by a branch adjacent to the lower tubeplate on the opposite side. After a mishap, the paint had been burned off the upper half of the shell. Internally, most of the tubes were found to be twisted or bent. The casing of the pump used to circulate the cooling water was also found to be cracked after the mishap. All the evidence pointed to the probability that a fire had occurred within the vessel. Some months before the failure, one of the tubes situated towards the center of the nest developed a leak. Owing to the difficulty of inserting a replacement tube, the defective one was scaled by means of a length of screwed rod fitted with nuts and washers at each end. This assembly became loose, thereby allowing air under pressure to enter the waterside of the cooler and expel the water, leading to overheating and ultimately to the damage described.

Abstract
An automatic press for making burlap bags had been used for several years. The press failed after being shipped by truck for a distance of about 400 mi. The objective was to determine whether failure occurred during or before shipment. The large piece which broke off the press included a section of the ways and a heavy adjustable mechanism which normally rides on these ways. The weight of the broken section was estimated at several hundred pounds. There was no support for the broken piece beyond the point of breakage. The material was a commercial cast iron, and the largest proportion of the fracture area was fresh and bright. It was concluded that this was a fresh fracture which occurred during shipment, and the crack itself was not present prior to shipment. The fact that a material defect of some sort was present and probably determined the location of the crack was apparently not significant as far as its usage was concerned. The failure could have been avoided by providing support underneath the overhanging member.

In a shipyard one of the two posts of a loading gear fractured under a comparatively small load at the point where it was welded into the ship’s deck. The post consisted of several pipe lengths that were produced by longitudinal seam welding of 27 mm thick sheets. The sheet metal was a construction steel of 60 to 75 kp/sq mm strength. Thick-walled parts of steels of such high strength must be preheated to approximately 200 deg C along the edges prior to welding to minimize the strong heat losses by the cold mass of the part. In the case under investigation this either was not done at all or the preheating was not high enough or sufficiently uniform. This damage was therefore caused by a welding defect.

Hypotheses as to the cause of the leak included a defect in the pot, overuse, overheating, and poor foundry practices. Scanning electron microscopy, transmission electron microscopy, optical microscopy, and x-ray microanalysis in conjunction with dimensional analysis, phase diagrams and thermodynamics considerations were employed to evaluate the various hypotheses. All evidence pointed to an oxide mass in the area where the hole developed, likely introduced during the steelmaking process.

Abstract
Two slitting saw blades were delivered for the purpose of determining the cause of damage. One had cracked while the other one came from a prior sheet delivery, that had less tendency to crack formation according to the manufacturer. The blades were supposed to have been stamped out of a sheet made from a 55 kp/sq mm strength steel. The saw blades were used for separating steel profiles at high rotational speeds. The cracks in question were located at the base of the teeth, i.e. at the point of highest operating stress. Metallographic examination showed that all cracks were non-decarburized and were free of chromium deposits. Therefore they could not have existed before heat treatment and chrome plating. It was concluded that the damage was due neither to poor quality of the sheet nor to defective stamping or heat treatment, but had occurred later either during surface treatment or during operation.

Abstract
Several stainless steel coils cracked during a routine unwinding procedure, prompting an investigation to determine the cause. The analysis included optical and scanning electron microscopy, energy-dispersive x-ray spectrometry, and tensile testing. An examination of the fracture surfaces revealed a brittle intercrystalline mode of fracture with typical manifestations of clear grain facets. Branched and discrete stepwise microcracks were also found along with unusually high levels of residual hydrogen. Mechanical tests revealed a marked loss of tensile ductility in the defective steel with elongations barely approaching 8%, compared to 50% at the time of delivery weeks earlier. Based on the timing interval and the fact that failure occurred at operating stresses well below the yield point of the material, the failure is being attributed to hydrogen-induced damage. Potential sources of hydrogen are considered as are remedial measures for controlling hydrogen content in steels.

Abstract
Initial investigation showed that a landing gear failure was the result of a hard landing with no evidence of contributory factors. The objective of reexamination was to determine whether there was any evidence of metallurgical failure. The landing gear was primarily an AISI type 6150 Cr-V steel flat spring attached at the top end to the fuselage and at the bottom end to the axle. Failure occurred at the clamping point near the top end of this spring. The failure showed evidence of severe brinelling at one corner in the clamping area. The fracture surfaces were clean, fresh, and indicative of a shock type of failure pattern. Closer examination, however, showed a fatigue crack at one corner. At this point, there was definite evidence of progression and oxidation. It was concluded that the corner in question was subjected to repeated brinelling resulting from normal landing loads, probably accentuated by looseness in the clamping device. The resulting residual tensile stress lowered the effective fatigue strength at that point against drag and side loads.

Abstract
The crankshaft of a six cylinder, 225-hp diesel engine driving a small locomotive was examined. About nine months after installation a fall in oil pressure was traced to damage to No. 5 crank pin bearing. A small lip present on one side of the discontinuity apparently served to scrape the bearing material. The defect was stoned smooth, a new bearing fitted, and the engine returned to service. The engine performed satisfactorily for a further twelve months until fracture of the crankshaft through the No. 5 crank pin supervened. The fracture revealed a complex torsional fatigue failure. Microscopic examination revealed that the pin had been hard chromium plated and that the plating followed the curved edge of the outer extremity of the defect. This crank pin contained an inherent defect in the form of a slag inclusion or crack situated at the surface. That the crack only showed itself after a period of service suggests that initially it may have been slightly below the surface of the machined pin and some slight extension outwards took place in service.

Abstract
A mild steel hook that was part of the auxiliary hoist of an electric overhead crane used in a foundry was of the shank type and the rated safe working load was 15 tons. Failure took place in a wholly brittle manner, and occurred transversely through the back of the hook. From the direction in which the fracture developed, as indicated by the radial lines on its surface, it was evident that a preexisting defect served to initiate the brittle fracture. Material adjacent to the fracture was decarburized and contained numerous globules of oxide and slag. It was evident, therefore that a fissure was formed during the manufacture of the hook and had not developed in service. The failure was associated with a surface defect, and it was recommended that the other similar hooks at the establishment be crack detected and any similar discontinuities eliminated.

Abstract
To forged AISI 4140 steel trailer kingpins fractured after 4 to 6 months of service. Fractographic and metallographic examination revealed that cracks were present in the spool-flange shoulder region of the defective kingpins prior to installation on the trailers. The cracks grew and coalesced during service. Consideration of the manufacturing process suggested that the cracks were the result of overheating of the kingpin blanks prior to forging, which was exacerbated during forging by deformation heating in the highly-strained region. This view was supported by results of two types of tensile tests conducted near the incipient melting temperature at the grain boundaries. All kingpins made by the supplier of the fractured ones were ultrasonically inspected and six more anticipated to fail were found. It was recommended that the heating of forging blanks be more carefully controlled, especially with respect to the accuracy of the optical pyrometer temperature readout. Also, procedures must be developed such that forging blanks that trigger the over-temperature alarm are reliably and permanently removed from the production line.

Abstract
A controllable pitch propeller (CPP) on a dynamic positioning ship failed after eight months of operation. The CPP design consists of a hollow propeller shaft and a concentrically located pipe that operates inside. The pitch of the propeller blades is controlled hydraulically through the longitudinal displacement of the inner (concentric) pipe. Fractography, microstructural, microhardness, and chemical analyses revealed that the concentric pipe failed due to fatigue. Fatigue cracks initiated along longitudinal welds where wire spacers attach to the external surface of the pipe. The effect of crack-like defects, stress concentration at the weld toe, residual tensile stress, and lack of penetration contributed to a shorter fatigue crack initiation phase and premature failure.
