Abstract
The paper discusses the analysis of a coating defect on a high phosphorus electroless nickel (Ni-11 wt. % P) deposit plated on an aluminum alloy substrate. Preliminary investigations had indicated that the elongated defects were possibly caused by the entrapment of long fibers or particles during the plating. The possible sources of fibers were identified. The SEM/EDS analysis of fibers collected from the air duct filters correlated very well with the defect shape and the EDS profile collected from under the defect site. It appears that the fibers from air duct filters directly above the plating line were blown into the plating tank and getting co-deposited. The paper describes the step-by-step analysis of the defect that led to successful identification of the root cause of the defect.

Abstract
When a large LPG low-carbon steel storage tank was put into service for the first time and filled beyond the proof testing level, a brittle fracture crack initiated at a fillet weld between a stiffener ring and the wall. The crack propagated to a length of 5.5 m and arrested. Analysis showed that the plates satisfied the criteria of BS 4741. It was concluded that the cause of crack initiation was the lack of a mouse hole at the junction between the stiffening ring and the wall of the tank. The tank was repaired and put back in service. When it was filled beyond the proof test level, again a brittle crack was initiated at a horizontal weld defect and propagated vertically, destroying the tank and the liquefaction plant. The initiation site was a thumbnail elliptical crack in a horizontal weld, having a depth of 1.5 mm, and a length of 4.5 mm. This showed that as late the mid-1970s, misunderstanding of brittle fracture led to the wrong design and construction of an LPG storage tank. The best design specification is to use a correlation between LAST, the Lowest Anticipated Service Temperature, and the DBTT measured by either Charpy tests or DTT.

Abstract
The impeller of a 4 ft. diam extraction fan driven by a 120 hp motor at 1,480 rpm. disrupted suddenly. The majority of the vanes had become detached where they were welded to the plates. At other locations, separation of the vanes was accompanied by tearing of the adjacent plate, failure being initiated at the weld fillets of the inner end of the vanes. An unusual feature was that the blades disclosed regions having a pronounced striated and stepped appearance. The etched microstructure was typical of a low carbon rolled plate having the usual banded appearance. A cross section through the fillet welds and zone showed lamellar tearing, which confirmed that failure had occurred in weld metal adjacent to the fusion face of the fillet to the vane. Results of the investigation indicated that the primary cause of failure of the impeller was the development of fatigue cracks from the unwelded roots of the fillet welds, by which the vanes were attached to the supporting plates. The impeller would have shown increased resistance to fatigue crack initiation if the T joint between the vanes and plates had been of the full penetration type.

Abstract
During a hydraulic test on one of the boilers in a range, leakage occurred from the lower surface of a horizontal S bend in the main steam pipe between the drum connector box and the junction valve. The pipe in question was 15 in. bore and had been in service for about 50 years. Specimens were prepared for microscopical examination to include the defective zone and a section through a circumferential crack. The defective zone was found to contain numerous inclusions of slag and oxides of globular form. Regions surrounding the inclusions were decarburized, the indications being that this region of the plate had been heated to an excessively high temperature. A corrosion-fatigue fissure was at one location, this having originated at the internal surface of the pipe and run into an inclusion in the defective zone. The failure resulted from the development of corrosion-fatigue fissures which originated at a zone containing defects introduced at the time of manufacture. These may have had their origin in the ingot from which the plate was rolled or, alternatively, be indicative of a zone which suffered overheating and local burning at the time the forge weld was made.

Abstract
Two cracks were discovered in a deck plate of an aircraft during overhaul and repair after 659 h of service. The cracks were on opposite sides of the deck plate in the flange joggles. The plate had been formed from 7178-T6 aluminum alloy sheet. Analysis (visual inspection, 0.2x/2x/2.3x electron microscope fractographs, hardness testing, and electrical conductivity testing) supported the conclusions that the failure was caused by fatigue cracks originating on the inside curved surface of the flanges. The cracks had initiated in surface defects caused by either corrosion pitting or forming notches, acting in combination with lateral forces evidenced by the moderate distortion of the fastener holes. Recommendations included eliminating the surface defects by revised cleaning and/or forming procedures. Revised design and installation should also alleviate the lateral forces.

Abstract
After less than 30 days in service, several cadmium-plated retaining rings, made of 4140 steel tubing and heat treated to 36 to 40 HRC, broke during operation that included holding components of a segmented fitting in place under a constant load. Photographic and 100x nital-etched micrographic examination showed a microstructure of tempered martensite with low inclusion content as well as a pit or burned spot on the outer area of the ring. The defect was approximately 0.18 mm (0.007 in.) deep and 0.5 mm (0.020 in.) in diam and had a hardness of 58 to 60 HRC. The base metal adjacent to the defect had a hardness of 36 to 40 HRC. Small cracks or fissures were also evident within the defect. Thus, the rings failed in brittle fracture as the result of an arc strike (or burn) on the surface of the ring. At the site of the arc strike, a small region of hard, brittle untempered martensite was formed as the result of an arc strike during the cadmium-plating operation. Fracture occurred readily when the ring was stressed. No recommendations were made.

Abstract
The crankshaft of a six cylinder, 225-hp diesel engine driving a small locomotive was examined. About nine months after installation a fall in oil pressure was traced to damage to No. 5 crank pin bearing. A small lip present on one side of the discontinuity apparently served to scrape the bearing material. The defect was stoned smooth, a new bearing fitted, and the engine returned to service. The engine performed satisfactorily for a further twelve months until fracture of the crankshaft through the No. 5 crank pin supervened. The fracture revealed a complex torsional fatigue failure. Microscopic examination revealed that the pin had been hard chromium plated and that the plating followed the curved edge of the outer extremity of the defect. This crank pin contained an inherent defect in the form of a slag inclusion or crack situated at the surface. That the crack only showed itself after a period of service suggests that initially it may have been slightly below the surface of the machined pin and some slight extension outwards took place in service.

Abstract
An open electrical circuit was found between plated through-holes in a six-layer printed circuit board after thermal cycling. The copper plating was very thin in the failure area but did make an electrical contact during initial testing. During thermal cycling, differential z-expansion between the epoxy board and copper caused the thin plating to crack. During electrical testing of a four-layer circuit board, an open electrical circuit was found between the plated through-holes. Plating discontinuity was caused by poor drilling using a dull drill with improper speed (rpm) and/or feed rate as was observed by nonuniform plating and nodule formation in the plated layer. In a third example, an open electrical circuit was found in a six-layer board between two adjacent plated through-holes. A plating void was on one side of the conductor joining the two holes. Continuity was found when tested from one side of the board but lost when tested from the other. In a fourth case, an open circuit found between a plated through-hole and contact pad on a six-layer printed circuit board was caused by an etching defect.

Abstract
The plate used to treat a pseudarthrosis in the proximal femur was investigated for reasons of non-progress of healing. Fatigue cracks were revealed on the top surface of the small section of the plate at the fifth screw hole. The plate was found to be heavily loaded by comparison of intensity of these structures, compared to results of systematic crack-initiation experiments. It was revealed by fatigue bending tests that the fatigue life of plates with asymmetrically arranged holes is at least as long as for plates with holes situated in the center. Fatigue began at the large section only after a fatigue crack begins to propagate into the small plate section. A large secondary crack which had developed parallel to the main crack in the center of the surface was revealed. The fifth hole was situated at the transition between the supporting bone and the defect and hence stress concentration was revealed to be high.

Abstract
A helicopter tail rotor blade spar failed in fatigue, allowing the outer section of the blade to separate in flight. The 7075-T7351 aluminum alloy blade had fiberglass pockets. The blade spar was a hollow “D” shape, and corrosion pits were present on the inner surface of the hollow spar A single corrosion pit, 0.38 mm (0.015 in.) deep, led to a fatigue failure of the spar The failure initiated on the pylon side of the blade. Dimensional analysis of the spar near the failure revealed measurements within engineering drawing tolerances. Though corrosion pitting was present, there was an absence of significant amounts of corrosion product and all of the pits were filled with corrosion-preventative primer. This indicated that the pitting occurred during spar manufacture, prior to the application of the primer The pitting resulted from multiple nickel plating and defective plating removal by acid etching. Post-plating baking operations subsequently reduced the fatigue strength of the spar.

Abstract
Evidence of destructive pitting on the gear teeth (AMS 6263 steel) in the area of the pitchline was exhibited by an idler gear for the generator drive of an aircraft engine following test-stand engine testing. The case hardness was investigated to be lower than specified and it was suggested that it had resulted from surface defects. A decarburized surface layer and subsurface oxidation in the vicinity of pitting were revealed by metallographic examination of the 2% nital etched gear tooth sample. It was concluded that pitting had resulted as a combination of both the defects. The causes for the defects were reported based on previous investigation of heat treatment facilities. Oxide layer was caused by inadequate purging of air before carburization while decarburization was attributed to defects in the copper plating applied to the gear for its protection during austenitizing in an exothermic atmosphere. It was recommended that steps be taken during heat treatment to ensure neither of the two occurred.

Abstract
The operator of an electric transit system purchased a large number of tin-plated copper connectors, putting some in service and others in reserve. Later, when some of the reserve connectors were inspected, the metal surfaces were covered with spots consisting of an ash-like powder and the plating material had separated from the substrate in many areas. Several connectors, including some that had been in service, were examined to determine what caused the change. The order stated that the connectors were to be coated with a layer of tin-bismuth (2% Bi) to guard against tin pest, a type of degradation that occurs at low temperatures. Based on the results of the investigation, which included SEM/EDS analysis, inductively coupled plasma spectroscopy, and x-ray diffraction, the metal surfaces contained less than 0.1% Bi and thus were not adequately protected against tin pest, which was confirmed as the failure mechanism in the investigation.

Abstract
Lakes in zinc die castings are areas encompassed by irregular lines or waves on flat or slightly contoured surfaces which are intended to look uniform. The laked areas have to be removed by polishing before the castings can be plated. This adds considerably to the overall cost of production. Castings examined were of an automobile name-plate holder with two flat sides of approximately 113 sq cm. All castings produced during a trial showed laking defects, the number and position varying from casting to casting. It was found that formation of metal waves and lakes depended primarily on the design of the gate and runner system and operating conditions. High flow efficiencies, with adequate feeding to all sections of the die, and short cavity fill times are desirable.

Abstract
In 1979, during a routine bridge inspection, a fatigue crack was discovered in the top flange plate of one tie girder in a tied arch bridge crossing the Mississippi River. Metallographic analysis indicated a banding or segregation problem in the middle of the plate, where the carbon content was twice what it should have been. Based on this and results of ultrasonic testing, which revealed that the banding occurred in 24-ft lengths, it was decided to close the bridge and replace the defective steel. The steel used in the construction of this bridge was specified as ASTM A441, commonly used in structural applications. Testing showed an increase in hardness and weight percent carbon and manganese in the banded region. Further testing revealed that the area containing the segregation and coarse grain structure had a lower than expected toughness and a transition temperature 90 deg F higher than specified by the ASTM standards. The fatigue crack growth rate through this area was much faster than expected. All of these property changes resulted from increased carbon levels, higher yield strength, and larger than normal grain size.

Abstract
A large four-engine aircraft was on a cargo flight at night when a loud bang was heard, accompanied by a loss of power from both engines on the left side. After an emergency landing, it was discovered that the propellers from both left side engines were missing. The initial investigation determined that the four-bladed propeller from the left inboard engine had separated in flight, subsequently impacting the left outboard engine, causing its propeller to separate also. Three years later, the left inboard propeller hub was recovered. All four blades had separated through the shank area adjacent to the hub. Detailed SEM examination confirmed a fatigue mode of failure in this area with a primary single origin on the inside surface of the shank. The main fatigue origin site was coincident with one of the defects on the inner surface of the blade shank. The most probable source for creating the defects on the ID bore of the shank was the blade tip chrome plating process, which was carried out during the last overhaul prior to the failure.
