Abstract
Two clamps that support overhead power lines in an electrified rail system fractured within six months of being installed. The clamps are made of CuNiSi alloy, a type of precipitation-strengthening nickel-silicon bronze. To identify the root cause of failure, the rail operator led an investigation that included fractographic and microstructural analysis, hardness testing, inductively coupled plasma spectroscopy, and finite-element analysis. The fracture was shown to be brittle in nature and covered with oxide flakes, but no other flaws relevant to the failure were observed. The investigation results suggest that the root cause of failure was a forging lap that occurred during manufacturing. Precracks induced by the forging defect and the influence of preload stress (due to bolt torque) caused the premature failure.

Abstract
Materials selection is an important engineering function in both the design and failure analysis of components. This article briefly reviews the general aspects of materials selection as a concern in proactive failure prevention during design and as a possible root cause of failed parts. It discusses the overall concept of design and describes the role of the materials engineer in the design and materials selection process. The article highlights the significance of materials selection in both the prevention and analysis of failures.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. The article also describes the role of the materials engineer in the design and materials selection process. It provides information on the significance of materials selection in both the prevention and analysis of failures.

The various methods of furnace, torch, induction, resistance, dip, and laser brazing are used to produce a wide range of highly reliable brazed assemblies. However, imperfections that can lead to braze failure may result if proper attention is not paid to the physical properties of the material, joint design, prebraze cleaning, brazing procedures, postbraze cleaning, and quality control. Factors that must be considered include brazeability of the base metals; joint design and fit-up; filler-metal selection; prebraze cleaning; brazing temperature, time, atmosphere, or flux; conditions of the faying surfaces; postbraze cleaning; and service conditions. This article focuses on the advantages, limitations, sources of failure, and anomalies resulting from the brazing process. It discusses the processes involved in the testing and inspection required of the braze joint or assembly.

Abstract
Several type 316L stainless steel wires in an electrostatic precipitator at a paper plant fractured in an unexpectedly short time. Failed wires were examined using optical and scanning electron microscope, and hardness tests were conducted. Fractography clearly established that fracture was caused by fatigue originating at corrosion pits on the surface of the wire. It was recommended that higher-molybdenum steel in the annealed condition be used to combat pitting corrosion.

Abstract
Mechanical testing is an evaluative tool used by the failure analyst to collect data regarding the macro- and micromechanical properties of the materials being examined. This article provides information on a few important considerations regarding mechanical testing that the failure analyst must keep in mind. These considerations include the test location and orientation, the use of raw material certifications, the certifications potentially not representing the hardware, and the determination of valid test results. The article introduces the concepts of various mechanical testing techniques and discusses the advantages and limitations of each technique when used in failure analysis. The focus is on various types of static load testing, hardness testing, and impact testing. The testing types covered include uniaxial tension testing, uniaxial compression testing, bend testing, hardness testing, macroindentation hardness, microindentation hardness, and the impact toughness test.

Abstract
This article briefly introduces the concepts of failure analysis and root cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It reviews four fundamental categories of physical root causes, namely, design deficiencies, material defects, manufacturing/installation defects, and service life anomalies, with examples. The article describes several common charting methods that may be useful in performing an RCA. It also discusses other failure analysis tools, including review of all sources of input and information, people interviews, laboratory investigations, stress analysis, and fracture mechanics analysis. The article concludes with information on the categories of failure and failure prevention.

Abstract
In 1979, during a routine bridge inspection, a fatigue crack was discovered in the top flange plate of one tie girder in a tied arch bridge crossing the Mississippi River. Metallographic analysis indicated a banding or segregation problem in the middle of the plate, where the carbon content was twice what it should have been. Based on this and results of ultrasonic testing, which revealed that the banding occurred in 24-ft lengths, it was decided to close the bridge and replace the defective steel. The steel used in the construction of this bridge was specified as ASTM A441, commonly used in structural applications. Testing showed an increase in hardness and weight percent carbon and manganese in the banded region. Further testing revealed that the area containing the segregation and coarse grain structure had a lower than expected toughness and a transition temperature 90 deg F higher than specified by the ASTM standards. The fatigue crack growth rate through this area was much faster than expected. All of these property changes resulted from increased carbon levels, higher yield strength, and larger than normal grain size.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

Abstract
A failure analysis was conducted on a flow-sensing device that had cracked while in service. The polysulfone sensor body cracked radially, adjacent to a molded-in steel insert. This article describes the investigative methods used to conduct the failure analysis. The techniques utilized included scanning electron microscopy, Fourier transform infrared spectroscopy, differential scanning calorimetry, thermomechanical analysis, and melt flow rate determination. It was the conclusion of the investigation that the part failed via brittle fracture, with evidence also indicating low cycle fatigue associated with cyclic temperature changes from normal service. The design of the part and the material selection were significant contributing factors because of stresses induced during molding, physical aging of the amorphous polysulfone resin, and the substantial differential in coefficients of thermal expansion between the polysulfone and the mating steel insert.

Abstract
A failed spiral gear and pinion set made from 4320H Ni-Cr-Mo alloy steel operating in a high-speed electric traction motor gear unit driving a rapid transit train were submitted for analysis. The pinion was intact, but the gear had broken into two sections that resulted when two fractured areas went through the body of the gear. Wheel mileage of the assembly was 34,000 miles at the time of failure. All physical and metallurgical characteristics were well within specified standards, and both parts should have withstood normal loading conditions. The primary mode of failure was tooth bending fatigue of the gear from the reverse direction near the toe end. The cause of failure was a crossed-over tooth bearing condition that placed loads at the heel end when going forward and at the toe end when going in reverse. The condition was too consistent to be a deflection under load; therefore, it most likely was permanent misalignment within the assembly.

Abstract
Two 20 MW turbines suffered damage to second-stage blades prematurely. The alloy was determined to be a precipitation-hardening nickel-base superalloy comparable to Udimet 500, Udimet 710, or Rene 77. Typical protective coatings were not found. Test results further showed that the fuel used was not adequate to guarantee the operating life of the blades due to excess sulfur trioxide, carbon, and sodium in the combustion gases, which caused pitting. A molten salt environmental cracking mechanism was also a factor and was enhanced by the working stresses and by the presence of silicon, vanadium, lead, and zinc. A change of fuel was recommended.

Molded plastic couplings used in an industrial application exhibited abnormally brittle properties, as compared to previously produced components. The couplings were specified to be molded from a custom-compounded glass-filled nylon 6/12 resin. An inspection of the molding resin used to produce the discrepant parts revealed that the pellets were of two general types, neither of which matched the pellets from a retained resin lot. Investigation included visual inspection, micro-FTIR in the ATR mode, and analysis using DSC. The thermograms supported the conclusion that the brittle couplings contained a significant level of contamination, polypropylene and nylon 6/6. The source of the polypropylene was likely the purging compound used to clean the compounding extruder. The origin of the nylon 6/6 resin was unknown but may represent a previously compounded resin.

Abstract
An AISI 4320 H transfer gear shaft that was part of a transmission sustained severe surface damage after 12 h of dynamometer testing at various gearing and torque loads. The damage was characterized by generalized wear and spalling. Examination of a cross section of the shaft that intersected undamaged, burnished, and surface-spalled zones revealed no anomalies in the chemistry, microstructure, or hardness that could have caused the damage. The physical evidence suggested that the operable mechanism was contact fatigue caused by misalignment of the shaft in the assembly.

Abstract
This article reviews the analytical techniques most commonly used in plastic component failure analysis. These include the Fourier transform infrared spectroscopy, differential scanning calorimetry, thermogravimetric analysis, thermomechanical analysis, and dynamic mechanical analysis. The descriptions of the analytical techniques are supplemented by a series of case studies that include pertinent visual examination results and the corresponding images that aid in the characterization of the failures. The article describes the methods used for determining the molecular weight of a plastic resin. It explains the use of mechanical testing in failure analysis and also describes the considerations in the selection and use of test methods.

Abstract
Rolling-contact fatigue (RCF) is a surface damage process due to the repeated application of stresses when the surfaces of two bodies roll on each other. This article briefly describes the various surface cracks caused by manufacturing processing faults or blunt impact loads on ceramic balls surfaces. It discusses the propagation of fatigue cracks involved in rolling contacts. The characteristics of various types of RCF test machines are summarized. The article concludes with a discussion on the various failure modes of silicon nitride in rolling contact. These include the spalling fatigue failure, the delamination failure, and the rolling-contact wear.

Abstract
The Rocky Point Viaduct, located near Port Orford, OR, was replaced after only 40 years of service. A beam from the original viaduct was studied in detail to determine the mechanisms contributing to severe corrosion damage to the structure. Results are presented from the delamination survey, potential and corrosion mapping, concrete chemistry, and concrete physical properties. The major cause of corrosion damage appears to have been the presence of both pre-existing and environmentally-delivered chlorides in the concrete.

Abstract
Silver solid-state bonded components containing uranium failed under zero or low applied load several years after manufacture. The final operation in their manufacture was a proof loading that applied a sustained tensile stress to the bond, which all components passed. The components comprised circular cylinders fabricated by plating a thin layer of silver on each of the contact surfaces (uranium and stainless steel) and pressing the parts together at elevated temperature to solid-state bond the two silver surfaces. The manufacturing process produced a high level of residual stress at the bond. The failures appeared to be predominantly located between the silver layer and the uranium substrate. Normal fracture location of specimens taken from similar components was at the silver/silver bond interface. Laboratory testing revealed that the uranium/silver joint was susceptible to premature failure by stress-corrosion cracking under sustained loading if the atmosphere was saturated with water vapor.
