Abstract
Microstructural examinations on transverse cross sections of a steam reformer tube, showed the presence of large macrovoids elongated in the radial direction and emanating from the internal surface of the tube. The macrovoids were located at the interdendritic regions, and were partially filled by a Mn-Fe bearing chromium oxide film. The areas adjacent to the oxide film were chemically depleted in C, Cr and Mn and rich in Fe and Ni. Associated with this depletion were a large concentration of microvoids. It was suggested that the dissolution of carbides in areas surrounding the macrovoids and the concentration of stresses at their tips, caused extensive localized plastic deformation which led to the formation of microvoids and subsequently to the spalling of the oxide film. The non-protective character of the film induced a progressive deterioration of the grain boundaries properties. Grain boundary sliding and dislocation motion were enhanced, causing a local increase in the steady state strain rate and the premature failure of the tube.

Abstract
Post-service destructive evaluation was performed on two commercially pure zirconium pump impellers. One impeller failed after short service in an aqueous hydrochloric acid environment. Its exposed surfaces are bright and shiny, covered with pockmarks, and peppered with pitting. Uniform corrosion is evident and two deep linear defects are present on impeller blade tips. In contrast, the undamaged impeller surfaces are covered with a dark oxide film. This and many other impellers in seemingly identical service conditions survive long lives with little or no apparent damage. No material or manufacturing defects were found to explain the different service performance of the two impellers. Microstructure, microhardness and material chemistry are consistent with the specified material. Examination reveals the damage mechanism to be corrosion-enhanced cavitation erosion, the most severe form of erosion corrosion. Cavitation damage to the protective oxide film caused the zirconium to lose its normally outstanding corrosion resistance. The root cause of the impeller failure is most likely the introduction of excessive air into the pump due to low liquid level, a bad seal or inadequate head. Corrosion pitting, crevice corrosion, and solidification cracks (casting defect) also contributed to the failure.

Abstract
During the preproduction stages of forging, an initial batch of 50 mm (2 in.) diam Al-4Cu alloy (L77) extruded bar stock material was found to be cracking randomly. Failure analysis was conducted to determine the metallurgical factors underlying the phenomenon. Microexamination of sections across the defects revealed intergranular cracks tracing a path of round, segregated particles and oxide film discontinuities. The segregated particles were rich in copper It was concluded that the cracking was the result of segregations occurring in poor-quality raw material. The source of segregation was suspected to be the use of improperly made master alloys. Use of improved melting techniques and proper master alloys was recommended.

Abstract
During an acceptance test of the Apollo spacecraft 101 service module prior to delivery, an SPS fuel pressure vessel (SN054) (titanium Ti-6Al-4V, approximately 1.2 m (4 ft) in diam and 3 m (10 ft) long) containing methanol developed cracks adjacent to the welds. The test was stopped. This acceptance test had been run 38 times on similar pressure vessels without problems. The methanol was a safe-fluid replacement for the storable hypergolic fuels (blend of 50% hydrazine and 50% unsymmetrical dimethyl hydrazine). Investigation (visual inspection and 65X images) showed similarities to stress-corrosion resulting from contamination during misprocessing of the vessels. However, another vessel underwent a more severe testing procedure and failed catastrophically. Further investigation supported the conclusion that the failure cause was SCC of titanium in methanol. Attack is promoted by crazing of the protective oxide film. It was learned that minor changes in the testing procedures could inhibit or accelerate the reaction. Recommendations included replacing the methanol with a suitable alternate fluid. Isopropyl alcohol was chosen after considerable testing. This incident further resulted in the imposition of a control specification (MF0004-018) for all fluids that contact titanium for existing and future space designs.

Abstract
An aluminum brass seawater surface condenser failed due to pitting after less than one year of service. Large pits filled with a green deposit were evidenced under the nonuniform black scale present over the entire inside surface of the tube. The black deposit was identified as primarily copper sulfide, with zinc and aluminum sulfides while the green deposit was revealed to be copper chloride. The combination of sulfide and chloride attack on the tubes was concluded to have resulted in the failure. Injection of ferrous sulfate upstream of the condenser which could aid the formation of protective oxide films was recommended.

Abstract
A fuel-nozzle-support assembly showed transverse indications after fluorescent liquid-penetrant inspection of a repair-welded area at a fillet on the front side of the support neck adjacent to the mounting flange. Visual examination disclosed an irregular crack. The crack through the neck was sectioned; examination showed that the crack had extended through the repair weld. The crack had followed an intergranular path. The crack was opened, and binocular-microscope examination of the fracture surface showed that the surface contained dendrites with discolored oxide films that were typical of exposure to air when very hot. Several additional subsurface cracks, typical of hot tears, were observed in and near the weld. There had been too much local heat input in making the repair weld. The result was localized thermal contraction and hot tearing. The cracking of the repair weld was attributed to unfavorable welding practice that accentuated thermal contraction stresses and caused hot tearing. Recommendations involved use of a small-diameter welding electrode, a lower heat input, and deposition in shallow layers that could be effectively peened between passes to minimize internal stress.

Abstract
Cracks, with no other damage, were observed in a niobium alloy (Nb-106) part when it was pulled from several months of protective storage for assembly into a rocket nozzle. SEM views showed the cracks to be intergranular, with contaminant particles on a large number of the grain facets. EDX analysis showed they consisted of niobium and fluorine. Plastic replicas, prepared by standard TEM techniques, were analyzed with selected-area electron diffraction, showing a pattern match for niobium tetrafluoride. Auger analyses showed electron spectra containing peaks representing carbon, oxygen, nitrogen, fluorine, and chlorine. Investigation into the processing history of the part showed the tenacious oxide film formed by the affinity of niobium for oxygen - even when heat treated in a vacuum – was removed with a combination of strong acids: nitric, hydrochloric, hydrofluoric, and lactic, resulting in the contaminants found on the surface. Thus, residues of the cleaning acid on the part had caused SCC during storage, with the tensile stresses necessary to generate SCC assumed to have been residual stresses from the heat treatment. Recommendation was made that more stringent cleaning procedures to remove any trace of the cleaning acids be used.

Abstract
A 321 stainless steel radar coolant-system assembly fabricated by torch brazing with AWS type 3A flux, failed at the brazed joint when subjected to mild handling before installation, after being stored for about two years. It was revealed by visual examination of the failed braze that the filler metal had not covered all mating surfaces. Lack of a metallurgical bond between the brazing alloy and stainless steel and instead mechanical bonding of the filler metal to an oxide layer on the stainless steel surface was revealed by examination of the broken joint at the cup. It was indicated by the thickness of the oxide layer that the steel surface was not protected from oxidation by the flux during torch heating. It was concluded that the failure was caused by lack of a metallurgical bond between the brazing alloy and the stainless steel. Components made of 347 stainless steel (better brazeability) brazed with a larger torch tip (wider heat distribution) and AWS type 3B flux (better filler-metal flow) were recommended for radar coolant-system assembly.

Abstract
Several cases of embrittlement failure are analyzed, including liquid-metal embrittlement (LME) of an aluminum alloy pipe in a natural gas plant, solid metal-induced embrittlement (SMIE) of a brass valve in an aircraft engine oil cooler, LME of a cadmium-plated steel screw from a crashed helicopter, and LME of a steel gear by a copper alloy from an overheated bearing. The case histories illustrate how LME and SMIE failures can be diagnosed and distinguished from other failure modes, and shed light on the underlying causes of failure and how they might be prevented. The application of LME as a failure analysis tool is also discussed.

Abstract
An austenitic stainless steel (type 316/316L stainless steel, schedule 40, 64 mm (2.5 in.) diam and larger) piping network used in the fire-sprinkler system in a large saltwater passenger and car ferry failed by rapid leaking. Operating conditions involved stagnant seawater at ambient temperatures. The pipe was in service for four weeks when three leaks appeared. Investigation (visual inspection and photographic images) supported the conclusion that the failure was caused by attack and corrosion damage of Cl ions in conditions that were ideal for three modes of highly accelerated pitting of austenitic stainless steel: the bottom surface, weld or HAZ pits, and crevices. Recommendations included proper material selection for piping, flanges, and weld rods with greater corrosion resistance. Proper filtering to prevent entrained abrasives and timely breakdown inspections were also advised.

Abstract
Within the first few months of operation of an 8 km (5 mile) long 455 mm (18 in.) diam high-pressure steam line between a coal-fired electricity-generating plant and a paper mill, several of the Inconel 600 bellows failed. The steam line operated at 6030 kPa (875 psi) and 420 deg C (790 deg F). Metallographic sections, energy-dispersive x-ray spectra, chemical analyses, tensile tests, and Auger microscope analyses showed the failed bellows met the specifications for the material. However, investigation also showed entire oxide thickness was contaminated with relatively large amounts of sodium, calcium, potassium, aluminum, and sulfur, alkali, alkali earth, and other contaminants that completely permeated even the thin oxides on the fracture surfaces. Additional investigation of the purity of the steam itself as reported by the power plant showed that corrosion and cracks were ultimately caused by the steam. While under normal operation, the steam's purity posed no problem to the material, during boiler cleaning operations, the generating plant had allowed contamination to get into the steam line.

Abstract
The case and stiffener of an inner-combustion-chamber case assembly failed by completely fracturing circumferentially around the edge of a groove arc weld joining the case and stiffener to the flange. The assembly consisted of a cylindrical stiffener inserted into a cylindrical case that were both welded to a flange. The case, stiffener, flange, and weld deposit were all of nickel-base alloy 718. It was observed that a manual arc weld repair had been made along almost the entire circumference of the original weld. Investigation (visual inspection, 0.5x macrographs, and 10x etched with 2% chromic acid plus HCl views) supported the conclusions that failure was by fatigue from multiple origins caused by welding defects. Ultimate failure was by tensile overload of the sections partly separated by the fatigue cracks. Recommendations included correct fit-up of the case, stiffener, and flange and more skillful welding techniques to avoid undercutting and unfused interfaces.

Next, the article explains testing methods and characterization of corrosive wear. Various factors that influence corrosive wear are then covered. Abstract
Large quantities of aluminum-clad spent nuclear materials have been in interim storage in the fuel storage basins at The Savannah River Site while awaiting processing since 1989. This extended storage as a result of a moratorium on processing resulted in corrosion of the aluminum clad. Examinations of this fuel and other data from a corrosion surveillance program in the water basins have provided basic insight into the corrosion process and have resulted in improvements in the storage facilities and basin operations. Since these improvements were implemented, there has been no new initiation of pitting observed since 1993. This paper describes the corrosion of spent fuel and the metallographic examination of Mark 31A target slugs removed from the K-basin storage pool after 5 years of storage. It discusses the SRS Corrosion Surveillance Program and the improvements made to the storage facilities which have mitigated new corrosion in the basins.

Abstract
A 4340 steel piston engine crankshaft in a transport aircraft failed catastrophically during flight. The fracture occurred in the pin radius zone. Fractographic studies established the mode of failure as fatigue under a complex combination of bending and torsional stresses. SEM examination revealed that the fracture origin was a subsurface defect-a hard refractory (Al2O3) inclusion—in the zone close to the pin radius. Chemical analysis showed the crankshaft material to be of inferior quality. It was recommended that magnetic particle inspection using the dc method be used to cheek for cracks during periodic maintenance overhauls.
