Abstract
An experimental high-temperature rotary valve was found stuck due to growth and distortion after approximately 100 h. Gas temperatures were suspected to have been high due to overfueled conditions. Both the rotor and housing in which it was stuck were annealed ferritic ductile iron similar to ASTM A395. Visual examination of the rotor revealed unusually heavy oxidation and thermal fatigue cracking along the edge of the gas passage. Material properties, including microstructure, composition, and hardness, of both the rotor and housing were evaluated to determine the cause of failure. The microstructure of the rotor was examined in three regions. The shaft material, the heavy section next to the gas passage and the thin edge of the rotor adjacent to the gas passage. The excessive gas temperatures were responsible for the expansion and distortion that prevented rotation of the rotor. Actual operating temperatures exceeded those intended for this application. The presence of transformation products in the brake-rotor edge indicated that the lower critical temperature had been exceeded during operation.

Abstract
A tube in a radiant superheater, the boiler of which is coal fired, failed by creep after 17 years of service. The failed tube was specified to be made of ASME SA-213, grade T-22. Measurable swelling of the tube diameter by about 2.4 mm and tube wastage caused by corrosion or erosion were observed. Log stress versus Larson-Miller Parameter (LMP) plots were produced to assess the remaining life of the superheater. It was revealed that the estimated operating temperature of 1060 deg F was higher than the estimated design temperature of 1000 deg F and that the tube wastage had increased the actual operating stress. Tube wastage and high operating temperatures hastened the failure. A better understanding of the material condition of this superheater was recommended to verify all the suspect hot tubes.

A brazing-furnace muffle 34 cm (13 in.) wide, 26 cm (10 in.) high, and 198 cm (78 in.) long, was fabricated from nickel-base high-temperature alloy sheet and installed in a gas-fired furnace used for copper brazing of various assemblies. The operating temperature of the muffle was reported to have been closely controlled at the normal temperature of 1175 deg C (2150 deg F); a hydrogen atmosphere was used during brazing. After about five months of continuous operation, four or five holes developed on the floor of the muffle, and the muffle was removed from service. Analysis (visual inspection, x-ray spectrometry, and metallographic examination) supported the conclusion that the muffle failed by localized overheating in some areas to temperatures exceeding 1260 deg C (2300 deg F). The copper found near the holes had dripped to the floor from assemblies during brazing. The copper diffused into the nickel-base alloy and formed a grain-boundary phase that was molten at the operating temperature. The presence of this phase caused localized liquefaction and weakened the alloy sufficiently to allow formation of the holes. No recommendations were made.

Abstract
A T-bolt was part of the coupling for a bleed air duct of a jet engine on a transport plane. Specifications required that the 4.8 mm diam component be made of AISI type 431 stainless steel and heat treated to 44 HRC. The operating temperature of the duct is 425 to 540 deg C (800 to 1000 deg F), but that of the bolt is lower. The T-bolt broke after three years of service. The expected service life was equal to that of the aircraft. It was found that the bolt broke as a result of SCC. Thermal stresses were induced into the bolt by intermittent operation of the jet engine. Mechanical stresses were induced by tightening of the clamp around the duct, which in effect acted to straighten the bolt. The action of these stresses on the carbides that precipitated in the grain boundaries resulted in fracture of the bolt. Due to the operating temperatures of the duct near the bolt, the material was changed to A-286, which is less susceptible to carbide precipitation. The bolt is strengthened by shot peening and rolling the threads after heat treatment. Avoiding temperatures in the sensitizing range is desirable, but difficult to ensure because of the application.

Abstract
Turbine buckets in a 37.5-MW gas turbine made of Udimet 500 superalloy failed in service. The power plant was located 1 km (0.6 miles) from the Pacific Ocean and operated on No. 2 diesel fuel, which was supplied by tanker ship. Turbine bucket failures occurred on three units after 2500 to 6400 h of operation. Investigation (visual inspection, metallographic examination, and stress analysis) supported the conclusion that the differing microstructure of the airfoil resulted in changes in mechanical properties. Because normal operation includes cycling of loads and temperatures, the shroud tip fractured due to thermomechanical fatigue in its degraded state. Recommendations included special chromium or silicon-rich coating to minimize corrosion in gas turbines operating in a marine environment with operating temperatures in the range of type 2 corrosion (650 to 750 deg C, or 1200 to 1380 deg F). Additionally, it was suggested that fuel delivery, handling, and treatment be high quality, to maintain fuel contamination within design limits, and inlet air filtration must be designed for the coastal site. Also, changing the bucket tip by increasing its thickness and changing the casting technique would reduce the stress and make the design more tolerant of corrosion.

Abstract
The top tube of a horizontal superheater bank in the reheat furnace of a steam generator ruptured after seven years in service. The rupture was found to have occurred in the ferritic steel tubing (2.25Cr-1Mo steel (ASME SA-213, grade T-22)) near the joint where it was welded to austenitic stainless steel tubing (type 321 stainless steel (ASME SA-213, grade TP321H)). The surface temperature of the tube was found to be higher than operating temperature in use earlier. The ferritic steel portion of the tube was found to be longitudinally split and heavily corroded in the region of the rupture. A red and white deposit was found on the sides and bottom of the tube in the rupture area. The deposit was produced by attack of the steel by the alkali acid sulfate and had thinned the tube wall. It was concluded that rupture of the tube had occurred due to thinning of the wall by coal-ash corrosion. The thinned tubes were reinforced by pad welding. Type 304 stainless steel shields were welded to the stainless steel portions of the top reheater tubes and were held in place about the chromium-molybdenum steel portions of the tubes by steel bands.

Abstract
The engine of an automobile lost power and compression and emitted an uneven exhaust sound after several thousand miles of operation. When the engine was dismantled, it was found that the outer spring on one of the exhaust valves was too short to function properly. The short steel spring and an outer spring (both of patented and drawn high-carbon steel wire) taken from another cylinder in the same engine were examined in the laboratory to determine why one had distorted and the other had not. Investigation (visual inspection, microstructure examination, and hardness testing) supported the conclusion that the engine malfunctioned because one of the exhaust-valve springs had taken a 25% set in service. Relaxation in the spring material occurred because of the combined effect of improper microstructure (proeutectoid ferrite) plus a relatively high operating temperature. Recommendations included using quenched-and-tempered steel instead of patented and cold-drawn steel or using a more expensive chromium-vanadium alloy steel instead of plain carbon steel; the chromium-vanadium steel would also need to be quenched and tempered.

Abstract
Tube 3 from a utility boiler in service for 13 years under operating conditions of 540 deg C (1005 deg F), 13.7 MPa (1990 psi) and 1,189,320 kg/h (2,662,000 lb/h) incurred a longitudinal rupture near its 90 deg bend while Tube 4 from the same boiler exhibited deformation near its bend. Metallographic examination revealed creep voids near the rupture in addition to graphite nodules. Exposure of the SA209 Grade T1A steel tubing to a calculated mean operating temperature of 530 deg C (983 deg F) for the 13 years resulted in graphitization and subsequent creep failure in Tube 3. The deformation in Tube 4 was likely the result of steam washing from the Tube 3 failure. Graphitization observed remote from the rupture in Tube 3 and in Tube 4 indicated that adjacent tubing also was susceptible to creep failure. In-situ metallography identified other graphitized tubes to be replaced during a scheduled outage.

Abstract
A shrunk-fit 18 Mn-5Cr steel retaining ring failed without warning during normal unit operation of a 380 MW electrical generator. The cause of the ring failure was determined to be intergranular stress-corrosion cracking (IGSCC) because of the high strength of the ring material and the presence of moist hydrogen used to cool the ring. Factors which promoted the failure were higher than normal strength levels in the ring material, lower than normal ring operating temperatures, possible moisture in the lubrication oil system, periodic poor performance of the hydrogen dryers, and a ring design which allowed water to become trapped in a relief groove. These factors caused pitting in the ring in an estimated 100 hours of operation. The ring had been inspected previously 18 months prior to the failure and no defects or pitting were found. Calculations showed that a 0.127-cm (0.050-in.) deep pit could grow to a critical size in 3000 to 4000 hours of operation. To prevent further failures, it was recommended that the ring be replaced with an 18 Mn-18Cr alloy with superior resistance to IGSCC. A program of periodic inspection and replacement of other retaining rings in the system was also recommended.

Abstract
A gas-fired, ASTM A-106 Grade B carbon steel vaporizer failed on three different occasions during attempts to bring the vaporizer on line. Dye penetrant examination indicated the presence of multiple packets of ductile cracks on the inside of the coil radius at the bottom of the horizontal axis coils. Visual examination of the inside of the tubing indicated the presence of a carbonaceous deposit resulting from decomposition of the heat-exchanging fluid. Subsequent metallographic examination and microhardness testing indicated that the steel was heated to a temperature above the allowable operating temperature for the fluid. The probable cause for failure is thermal fatigue due to the localized overheating. Flow conditions inside the tubing should be reexamined to ensure suitable conditions for annular fluid flow.

Abstract
A detailed failure analysis was conducted on an ammonia refrigerant condenser tube component that failed catastrophically during its initial hours of operation. Evidence collected clearly demonstrated that the weld between a pipe and a dished end contained a sharp unfused region at its root (lack of penetration). Component failure had started from this weld defect. The hydrogen absorbed during welding facilitated crack initiation from this weld defect during storage of the component after welding. Poor weld toughness at the low operating temperature facilitated crack growth during startup, culminating in catastrophic failure as soon as the crack exceeded critical length.

Abstract
A power plant using two steam generators (vertical U-tube and shell heat exchangers, approximately 21 m (68 ft) high with a steam drum diameter of 6 m (20 ft)) experienced a steam generator tube rupture. Each steam generator contained 11,012 Inconel alloy 600 (nickel-base alloy) tubes measuring 19 mm OD, nominal wall thickness of 1.0 mm (0.042 in.), and average length of 18 m (57.75 ft). The original operating temperature of the reactor coolant was 328 deg C (621 deg F). A tube removal effort was conducted following the tube rupture event. Investigation (visual inspection, SEM fractographs, and micrographs) showed evidence of IGSCC initiating at the OD and IGA under ridgelike deposits that were analyzed and found to be slightly alkaline to very alkaline (caustic) in nature. Crack oxide analysis indicated sulfate levels in excess of expected values. The analysis supported the conclusion that that the deposits formed at locations that experienced steam blanketing or dryout at the higher levels of the steam generators. Recommendations included steam generator water-chemistry controls, chemical cleaning, and reduction of the primary reactor coolant system temperature.

Abstract
The exhaust valve of a truck engine failed after 488 h of a 1000 h laboratory endurance test. The valve was made of 21-2 valve steel in the solution treated and aged condition and was faced with Stellite 12 alloy. The failure occurred by fracture of the underhead portion of the valve. Analysis (visual inspection, electron probe x-ray microanalysis, hardness testing, 4.5x fractograph) supported the conclusions that failure of the valve stem occurred by fatigue as a result of a combination of a nonuniform bending load, which caused a mild stress-concentration condition, and a high operating temperature in a corrosive environment. When the microstructure near the stem surface was examined, it was apparent that carbide spheroidization had occurred. Also, there was a coarsening of the carbide network within the austenite grains. The microstructure indicated that the underhead region of the valve was heated to about 930 deg C (1700 deg F) during operation. The cause of fatigue fracture, therefore, was a combination of non-uniform bending loads and overheating. No recommendations were made.

Abstract
A portion of the wall of a reactor vessel used in burning impurities from carbon particles failed by localized melting. The vessel was made of Hastelloy X (Ni-22Cr-9Mo-18Fe). Considering the service environment, melting could have been caused either by excessive carburization (which would have lowered the melting point of the alloy markedly) or by overheating. A small specimen containing melted and unmelted metal was removed from the vessel wall and examined metallographically. It was observed that the interface between the melted zone and the unaffected base metal was composed of large grains and enlarged grain boundaries. An area a short distance away from the melted zone was fine grained and relatively free of massive carbides. This evidence supported the conclusion that the vessel failed by melting that resulted from heating to about 1230 to 1260 deg C (2250 to 2300 deg F), which exceeded normal operating temperatures, and carburization was not the principal cause of failure. No recommendations were made.

Abstract
A clamp used for securing the hot-air ducting system on fighter aircraft fractured in an area adjacent to a slot near the end of the strap after two or three years of service. The strap was 0.8 mm (0.032 in.) thick, and the V-section was 1.3 mm (0.050 in.) thick; both were made of 19-9 DL heat-resisting alloy. The operating temperature of the duct surrounded by the clamp was 425 to 540 deg C (800 to 1000 deg F). The life of the clamp was expected to equal that of the aircraft. Investigation (visual inspection, chemical analysis, hardness testing, and 540x/2700x images etched with oxalic acid) supported the conclusion that the clamp fractured by SCC because the work metal was sensitized. Sensitization occurred during long-term exposure to the service temperature; the effects of sensitization were intensified as a result of cold forming. Recommendations included using a work metal that is less susceptible to intergranular carbide precipitation.

Heating elements, consisting of strips, 40 mm x 2 mm, of the widely used 80Ni-20Cr resistance heating alloy, and designed to withstand a temperature of 1175 deg C, were rendered unusable by scaling after a few months service in a through-type annealing furnace, Although the temperature supposedly did not exceed 1050 deg C. Structural observations indicated a special case of internal oxidation. The required conditions for this were apparently provided by the moist hydrogen atmosphere of the annealing furnace, in which the chromium was oxidized, while the oxides of iron and nickel were reduced. Even the carbon suffered incomplete combustion and was enriched in the core. Thus, no protective layer could form or be maintained. The intergranular advancement of the oxidation may have been favored by the precipitation of chromium-rich carbides on the austenite grain boundaries. This form of internal oxidation is, in the case of Ni-Cr alloys, known as green rot. Alloys containing iron should be more resistant. As a preventive measure it was recommended to reduce the operating temperature of the strip sufficiently to allow the use of Fe-Ni-Cr alloys.

Abstract
One of the coils in the radiant section of a primary reformer furnace used in an ammonia plant was found leaking. The bottom of one of seven outlet headers (made of ASME SA-452, grade TP316H, stainless steel) was revealed during examination to be ruptured. It was revealed by metallurgical examination that it had failed as a result of intergranular fissuring and oxidation (creep rupture). The ruptured area revealed that the header had failed by conventional long-time creep rupture as a result of exposure to operating temperatures probably between 900 and 955 deg C. Three samples from different sections (ruptured area, slightly bulged but nonruptured area and visually sound metal) were inspected. The presence of pinhead-size intergranular fissures throughout the cross sections of the latter two samples was observed. An ultrasonic attenuation method was employed to investigate the remaining headers. All headers were revealed by ultrasonic readings to be in an advanced stage of creep rupture and no areas were found to be fissured to a degree that they needed immediate replacement. As a conclusion, the furnace was deemed serviceable and it was established that in the absence of local hot spots, the headers would survive for a reasonable period of time.
