Abstract
A system of carbon steel headers, handling superheated water of 188 deg C (370 deg F) at 2 MPa (300 psi) for automobile-tire curing presses, developed a number of leaks within about four months after two to three years of leak-free service. All the leaks were in shielded metal arc butt welds joining 200 mm (8 in.) diam 90 deg elbows and pipe to 200 mm (8 in.) diam welding-neck flanges. A flange-elbow-flange assembly and a flange-pipe assembly that had leaked were removed for examination. Investigation (visual inspection, hardness testing, chemical analysis, magnetic-particle testing, radiographic inspection, and 2% nital etched 1.7x views) showed varying IDs on the assemblies and supported the conclusions that the failures of the butt welds were the result of fatigue cracks caused by cyclic thermal stresses that initiated at stress-concentrating notches at the toes of the interior fillet welds on the surfaces of the flanges. Recommendations included using ultrasonic testing to identify the appropriate joints and then replacing them. Special attention to accuracy of fit-up in the replacement joints was also recommended to achieve smooth, notch-free contours on the interior surfaces.

Abstract
The connecting end of two forged medium-carbon steel rods used in an application in which they were subjected to severe low-frequency loading failed in service. The fractures extended completely through the connecting end. The surface hardness of the rods was found to be lower than specifications. The fractures were revealed to be in areas of the transition regions that had been rough ground to remove flash along the parting line. The presence of beach marks, indicating fatigue failure, was revealed by examination. The fracture origin was confirmed by the location and curvature of beach marks to be the rough ground surface. An incipient crack 9.5 mm along with several other cracks on one of the fractured rods was revealed by liquid penetration examination. Metallographic examination of the fractured rods indicated a banded structure consisting of zones of ferrite and pearlite. It was established that the incipient cracks found in liquid-penetrant inspection had originated at the surface in the banded region, in areas of ferrite where this constituent had been visibly deformed by grinding. Closer control on the microstructure, hardness of the forgings and smooth finish in critical area was recommended.

Abstract
Plunger shafts machined from 4150 steel bar stock were involved in a series of fatigue failures. The fractures consistently occurred at two locations on the shafts: the shaft fillet and either side of a machined notch. The material specification for the shafts required 41xx series steel with a carbon content of 0.38 to 0.53%, a hardness of 35 to 40 HRC for the shaft, and a hardness of 50 to 55 HRC for the notch (which was case hardened). Analysis (visual inspection, chemical analysis, hardness testing, and magnetic particle inspection) supported the conclusions that all the fractures were fatigue-induced failures due to sharp radii in the fillets. The stress-concentrating effects of the fillets caused fatigue cracks to initiate and grow under cyclic loading until the crack depth was critical, causing the shaft to fail and rendering the assembly inoperative. Recommendations included increasing the radii of the notch and shaft fillets. If fatigue cracking had continued to be a problem with this component, shot peening of the subject radii would be appropriate. This process produces residual compressive stresses in the surface of the part, thereby retarding initiation of fatigue cracks.

Abstract
The repeated failure of a welded ASTM A283 grade D pipe that was part of a 6 km (4 mi) line drawing and conducting river water to a water treatment plant was investigated. Failure analysis was conducted on sections of pipe from the third failure. Visual, macrofractographic, SEM fractographic, metallographic, chemical, and mechanical property (tension and impact toughness) analyses were conducted. On the basis of the tests and observations, it was concluded that the failure was the combined result of poor notch toughness (impact) properties of the steel, high stresses in the joint area, a possible stress raiser at the intersection of the spiral weld and girth weld, and sudden impact loading, probably due to water hammer. Use of a semi- or fully killed steel with a minimum Charpy V-notch impact value of 20 J (15 ft·lbf) at 0 deg C (32 deg F) was recommended for future water lines. Certified test results from the steel mill, procedure qualification tests of the welding, and design changes to reduce water hammer were also recommended.

Abstract
A reversible four-way carbon steel flap valve in a thermal power station failed after 7 years of service. The flap had been fabricated by welding two carbon steel plates to both sides of a carbon steel forging. The valve was used for reversing the flow direction of seawater in the cooling system of a condenser. Visual examination of the flap showed crystalline fracture, indicating a brittle failure. Metallographic examination, chemical analyses, and tensile and impact testing indicated that the failure was caused by the notch sensitivity of the forging material, which resulted in low toughness. It was recommended that fully killed carbon steel with a fine-grain microstructure be used. Redesign of the flap to remove the step in the forging that acted as a notch was also recommended.

Abstract
Airfoil-shape impingement cooling tubes were fabricated of 0.25 mm (0.010 in.) thick Hastelloy X sheet stock, then pulse-laser-beam butt welded to cast Hastelloy X base plugs. Each weldment was then inserted through the base of a hollow cast turbine blade for a jet engine. The weldments were finally secured to the bases of the turbine blades by a brazing operation. One of the laser beam attachment welds broke after a 28-h engine test run. Exposure of the fracture surface for study under the electron microscope revealed the joint had broken in stress rupture. Failure was caused by tensile overload from stress concentration at the root of the laser beam weld, which was caused by the sharp notch created by the lack of full weld penetration. Radiographic inspection of all cooling-tube weldments was made mandatory, with rejection stipulated for joints containing subsurface weld-root notches. In addition, all turbine blades containing cooling-tube weldments were reprocessed by back-brazing. Back brazed turbine blades were reinstalled in the engine and withstood the full 150-h model test run without incident.

Abstract
An articulated rod (made from 4337 steel (AMS 6412) forging, quenched and tempered to 36 to 40 HRC) used in an overhauled aircraft engine was fractured after being in operation for 138 h. Visual examination revealed that the rod was broken into two pieces 6.4 cm from the center of the piston-pin-bushing bore. The fracture was nucleated at an electroetched numeral 5 on one of the flange surfaces. A notch, caused by arc erosion during electroetching, was revealed by metallographic examination of a polished-and-etched section through the fracture origin. A remelted zone and a layer of untempered martensite constituted the microstructure of the metal at the origin. Small cracks, caused by the high temperatures developed during electro-etching, were observed in the remelted area. It was concluded that fatigue fracture of the rod was caused by the notch resulting from electroetching and thus electroetched marking of the articulated rods was discontinued as a corrective measure.

Abstract
An aircraft fuel-nozzle-support assembly exhibited cracks along the periphery of a fusion weld that attached a support arm to a fairing in a joint that approximated a T-shape in cross section. The base metal was type 321 stainless steel. Examination showed a good-quality weld penetrating to the support arm beneath, but revealed notch configurations at the inner mating surfaces at each edge of the fairing, the result of welding a poor fit-up of the support arm to the fairing. Fractures that originated at the cracks were examined by stereomicroscope and were found to contain fatigue marks that indicated crack propagation from multiple origins at the inner surface of the weld edge. Fatigue cracking was initiated at stress concentrations created by the notches at the inner surfaces between the support arm and the fairing, enhanced by poor fit-up in preparation for welding.

Abstract
Three bearing bosses from the cover of scrap shears were sent in for examination. They had torn off the base plate to which they had been welded by fillet welds all around. Two of these were examined. They showed entirely the same symptoms. The bosses had broken away on three sides along the welds. The cleaved fractures in the burned notches propagated partially above and partially below several incipient cracks which may have been fatigue fractures. Metallographic sections showed that the fractures had occurred either at the burned notches near the transition from the weld to the sheet, or else they ran in the sheet material next to the weld. The quality of the welds could not be judged because the opposite fracture pieces to which they adhered had not been sent in. It was concluded that the breakaway of these bosses was at least favored by overheating and hardening.

Abstract
During construction of a revolving sky-tower observatory, a 2.4 m (8 ft) diam cylindrical column developed serious circumferential cracks overnight at the 14 m (46 ft) level where two 12 m (40 ft) sections were joined by a girth weld. The temperatures ranged from 12 deg C (53 deg F) to 7 deg C (45 deg F) that night. The column was shop fabricated in 12 m (40 ft) long sections of 19 mm (3/4 in.) thick steel plate of ASTM A36 steel. Crack initiation was caused by high residual stress during girth welding, and the presence of notches formed by the termination of the incomplete welds. Continuation of the cracks was attributed to the brittle condition of the steel when cooled by the night air. A steel with a much lower ductile-to-brittle transition temperature is essential for this type of structure. Other necessary steps include better control of the girth-welding, choice of a more favorable electrode to avoid porosity, careful termination of all welds to avoid formation of notches, and completion of all welds before other sections of the column are erected.

Abstract
A section from a stop-block guide fell to the floor on a crane runway after it failed. A brittle crystalline-type break was disclosed by examination of the fracture surface. The point of initiation was in a hardened heat-affected layer that had developed during flame cutting and welding. The metal was identified to be 1020 steel. It was indicated by the coarse as-rolled structure (grain size of ASTM 00 to 4) of the base metal that the weldment (stop block and guide) had not been normalized. The brittle failure was evaluated to have been initiated at a metallurgical and mechanical notch produced by flame cutting and welding. As corrective measures, fully silicon-killed 1020 steel with a maximum grain size of ASTM 5 were used to make new stop-block weldments. The weldments were normalized at 900 deg C after flame cutting and welding to improve microstructure and impact strength. All flame-cut surfaces were ground to remove notches.

After several years' use, an eyebolt suffered brittle fracture in the first turn of the thread. The fracture started at the notch at the root of the thread. Neither localized material defect nor an old crack were present. The investigation showed that instead of the specified steel quality St 37-2 N, a steel with about 0.5% C had been used. The microstructure with the coarse ferrite network indicated that the forged eye bolt had been normalized either at too high a temperature or not at all. In any case the anneal at 900 deg C produced a considerably more finely grained structure. In addition, the nature of the fracture and the results of the notched bar impact tests showed that in spite of the high C-content, the eye bolt had become brittle as a result of aging.

Abstract
Several thousands of new 16 mm diam alloy steel sling chains used for handling billets failed by chain-link fractures. No failures were found to have occurred before delivery of the new chains. It was observed that the links had broken at the weld. It was found that all failures had occurred in links having hardness values in the range of 375 to 444 HRB. It was revealed by the supplier that the previous hardness level of 302 to 375 HRB was increased to minimize wear which made the links were made notch sensitive and resulted in fractures that initiated at the butt-weld flash on the inside surfaces of the links. A further reduction in ductility was believed to have been caused by lower temperatures during winter months. Thus, the failure was concluded to have been caused in a brittle manner caused by the notch sensitivity of the high hardness material at lower temperatures. The chains were retempered to a hardness of 302 to 375 HRB as a corrective measure and subsequently ordered chains had this hardness as a requirement.
