Abstract
The fatigue failure of a wire rope used on a skip hoist in an underground mine has been studied as part of the ongoing research by the Bureau of Mines into haulage and materials handling hazards in mines. Macroscopic correlation of individual wire failures with wear patterns, fractography, and microhardness testing were used to gain an understanding of the failure mechanism. Wire failures occurred predominantly at characteristic wear sites between strands. These wear sites are identifiable by a large reduction in diameter; however, reduction in area was not responsible for the location of failure. Fractography revealed multiple crack initiation sites to be located at other less noticeable wear sites or opposite the characteristic wear site. Microhardness testing revealed hardening, and some softening, at wear sites.

It describes manufacturing and design defects of various products. The article explains a design that is analyzed from the human factors viewpoint and details the preventive measures of the defects, with examples. This article provides assistance to a failure analyst in broadening the initial scope of the investigation of a physical engineering failure in order to identify the root cause of a problem. The engineering design process, including task clarification, conceptual design, embodiment design, and detail design, is reviewed. Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. Abstract
A detailed fracture mechanics evaluation is the most accurate and reliable prediction of process equipment susceptibility to brittle fracture. This article provides an overview and discussion on brittle fracture. The discussion covers the reasons to evaluate brittle fracture, provides a brief summary of historical failures that were found to be a result of brittle fracture, and describes key components that drive susceptibility to a brittle fracture failure, namely stress, material toughness, and cracklike defect. It also presents industry codes and standards that assess susceptibility to brittle fracture. Additionally, a series of case study examples are presented that demonstrate assessment procedures used to mitigate the risk of brittle fracture in process equipment.

Abstract
On 21 April 1995, the contents of a large blender (6 cu m) reacted and caused an explosion that killed and injured a number of workers at a plant in Lodi, NJ. A mixture of sodium hydrosulfite and aluminum powder was being mixed at the time of the accident. This report focuses on evaluations of the blender to determine if material or mechanical failures were the cause of the accident. The results indicate that the mixing vessel was metallurgically sound and did not contribute to the initiation of the failure. However, the vessel was not designed for mixing chemicals that must be isolated from water and excessive heat. Water leaking into the vessel through a graphite seal may have initiated the reactions that caused the accident.

Abstract
A truck-mounted hydraulic crane had a horizontal thrust bearing with one race attached to the truck and the other to the rotating crane. The outside race of the bearing was driven by a pinion gear, and it is through this mechanism that the crane body rotated about a vertical axis. The manufacturer welded the inner race to the carrier in a single pass. After several years of service, the attachment weld between the bearing inner race and the turntable failed in the area adjacent to the heat-affected zone. The fracture zone where there was the greatest tension was heavily oxidized. In the zone where the bearing was in compression, there was a clean surface indicating recent fracture. Finally, there were areas where the weld did not meet AWS specifications for convexity or concavity. These areas were weak enough to allow fatigue cracks to initiate. Recommendations to prevent reoccurrence of the failure include the use of bolts in lieu of welding, a welding schedule that reduces the propensity of lamellar tearing, and the use of an alloy that precludes lamellar tearing. However, if abuse of the crane was the primary cause of failure, none of these recommendations would have prevented deterioration of the machine to an extent that would have rendered the failure improbable.

Abstract
A fire in a storage yard engulfed several propane delivery trucks, causing one of them to explode. A series of elevated-temperature stress-rupture tears developed along the top of the truck-mounted tank as it was heated by the fire. Unstable fracture then occurred suddenly along the length of the tank and around both end caps, following the girth welds that connect them to the center portion of the tank. The remaining contents of the tank were suddenly released, aerosolized, and combusted, creating a powerful boiling liquid expanding vapor explosion (BLEVE). Based on the metallography of the tank pieces, the approximate tank temperature at the onset of explosion was determined. Metallurgical analysis provided additional insights as well as a framework for making tanks less susceptible to this destructive failure mechanism.

This article discusses the failure analysis of several steel transmission pipeline failures, describes the causes and characteristics of specific pipeline failure modes, and introduces pipeline failure prevention and integrity management practices and methodologies. Abstract
A paper drier head manufactured from gray cast iron was removed from service as a result of NDE detection of crack-like surface discontinuities. This component was subjected to internal steam pressure to provide heat energy for drying. Investigation (visual inspection, chemical analysis, mechanical testing, as-polished 54x magnification, etched with nital 33x/54x/215x/230x magnification) supported the conclusions that the NDE indications were the consequence of a cold-shut condition in the casting. The cold shut served as a stress-concentration site and was therefore a potential source of crack initiation. The combination of low material strength and a casting defect was a potential source of unexpected fracture during service, because the component was under pressure from steam. Recommendations included removing other dryer heads exhibiting similar discontinuities and/or material quality from service.

Abstract
A hi-rail device is a vehicle designed to travel both on roads and on rails. In this case, a truck was modified to accept the wheels for rail locomotion. The rear wheel/axle set was attached to the truck frame. Both the front and rear wheel/axle sets were raised by means of a hydraulic cylinder driven off the PTO of the truck. The wheel/axle set was rigidly fixed into an up or down position by the use of locking pins. It was assumed by the manufacturer that there would be no load on the cylinder once the wheel/axle set was in its locked position. However, as the cylinder pivoted about its mounting trunnion and extended during its motion, it interfered with a frame member. This caused both a bending load and a rotational movement. These effects caused a combination of fretting, galling, and fatigue to the internal thread structure of the clevis. As a result of these deleterious effects, failure of the thread structure of the clevis occurred. The failure occurred where the cylinder rod screws into the clevis. The rod was manufactured from 1045 steel.

This article focuses on the general methods and approaches from the perspective of a reconstruction analyst and includes discussions relevant to materials failure analysts at the incident scene. The elements of accident reconstruction are described. These have conceptual similarity with the principles for failure analysis of material incidents that are less complex than a large-scale accident. The article provides a brief review of some general concepts on the use of modeling which can be a very powerful tool for information pertaining to the reconstruction of an accident where the model can be a physical, mathematical, or logical representation of a physical system or process.

Two titanium alloy wing attachment bolts from a commercial jetliner failed during the course of a routine service operation. Failure of the bolts occurred during the re-torque process as the wing was being reattached. Metallurgical failure analysis indicated that the fracture mechanism was ductile overload and that the mechanical properties of the bolts were consistent with exemplar bolts that had been supplied. After eliminating other sources of excessive load application, the most probable cause of failure was ascribed to variances between the frictional characteristics of the bolt at the time of re-torque and at the time of initial torque application several years earlier.

Abstract
This article briefly introduces the concepts of failure analysis and root cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It reviews four fundamental categories of physical root causes, namely, design deficiencies, material defects, manufacturing/installation defects, and service life anomalies, with examples. The article describes several common charting methods that may be useful in performing an RCA. It also discusses other failure analysis tools, including review of all sources of input and information, people interviews, laboratory investigations, stress analysis, and fracture mechanics analysis. The article concludes with information on the categories of failure and failure prevention.
