Abstract
A stainless steel screw securing an orthopedic implant fractured and was analyzed to determine the cause. Investigators used optical and scanning electron microscopy to examine the fracture surfaces and the microstructure of the austenitic stainless steel from which the screw was made. The results of the study indicated that the screw failed due to fatigue fracture stemming from surface cracks generated by stress concentration likely caused by grooves left by improper machining.

Abstract
Explosive cladding is a viable method for cladding different materials together, but the complicated behavior of materials under ballistic impacts raises the probability of interfacial shear failure. To better understand the relationship between impact energy and interfacial shear, investigators conducted an extensive study on the shear strength of explosively cladded Inconel 625 and plain carbon steel samples. They found that by increasing impact energy, the adhesion strength of the resulting cladding can be improved. Beyond a certain point, however, additional impact energy reduces shear strength significantly, causing the cladding process to fail. The findings reveal the decisive role of plastic strain localization and the associated development of microcracks in cladding failures. An attempt is thus made to determine the optimum cladding parameters for the materials of interest.

Abstract
The failure of a boiler operating at 540 °C and 9.4 MPa was investigated by examining material samples from the near-failure region and by thermodynamic analysis. A scanning Auger microprobe, SEM, and commercial thermodynamic software codes were used in the investigation. Results indicated that the boiler failure was caused by grain-boundary segregation of phosphorous, tin, and nitrogen and the in-service formation of carbide films and granules on the grain boundaries.

Abstract
A forging die in a 250-ton press producing brass valves began to show signs of fatigue after a few thousand hits. By the time it reached 30,000 hits, the die was badly damaged and was submitted for analysis along with one of the last forgings produced. The investigation included visual and macroscopic inspection, metallographic and chemical analysis, SEM imaging, optical profilometry, mechanical property testing, and EDX analysis. The die was made of chromium hot-work tool steel and the forgings were made of CuZn39Pb3 heated to an initial working temperature 700 deg C. The entire surface of the die was covered with fatigue cracks and many fillets had been plastically deformed. Several other types of damage were also observed, including areas of oxidation, corrosion pits, voids, abrasive wear, die adhesion, and thermal fatigue. Fatigue cracking was the primary cause of failure with significant contributions from the other damage mechanisms.

Abstract
Rolling-contact fatigue (RCF) is a common failure mode in components subjected to rolling or rolling-sliding contact. This article provides a basic understanding of RCF and a broad overview of materials and manufacturing techniques commonly used in industry to improve component life. A brief discussion on coatings to improve surface-initiated fatigue and wear is included, due to the similarity to RCF and the increasing criticality of this failure mode. The article presents a working knowledge of Hertzian contact theory, describes the life prediction of rolling-element bearings, and provides information on physics and testing of rolling-contact fatigue. Processes commonly used to produce bearings for demanding applications are also covered.

Abstract
This paper describes the superplastic characteristics of shipbuilding steel deformed at 800 °C and a strain rate less than 0.001/s. After the superplastic deformation, the steel presents mixed fractures: by decohesion of the hard (pearlite and carbides) and ductile (ferrite) phases and by intergranular sliding of ferrite/ferrite and ferrite/pearlite, just as it occurs in stage III creep behavior. The behavior is confirmed through the Ashby-Verrall model, according to which the dislocation creep (power-law creep) and diffusion creep (linear-viscous creep) occur simultaneously.

Abstract
In this article, we report the outcome of an investigation made to uncover the premature fracture of crusher jaws produced in a local foundry. A crusher jaw that had failed while in service was studied through metallographic techniques to determine the cause of the failure. Our investigation revealed that the reason for the fracture was the presence of large carbides at the grain boundaries and in the grain matrix. This led to the formation of microcracks that propagated along the grain boundaries under in-service working forces. It is also believed that the precipitation of carbides at the grain boundaries may have occurred because of improper heat treatment, but not because of a deficiency in composition.

Abstract
The shafts on two centrifugal pumps failed during use in a petroleum refinery. Light optical microscopy and scanning electron microscopy were used to analyze the damaged materials to determine the cause of failure. The results showed that one shaft, made of duplex stainless steel, failed by fatigue fracture, and the other, made of 316 austenitic stainless steel, experienced a similar fracture, which was promoted by the presence of nonmetallic inclusion particles.

Abstract
This paper describes an investigation on the failure of a large leaded bronze bearing that supports a nine-ton roller of a plastic calendering machine. At the end of the normal service life of a good bearing, which lasted for seven years, a new bearing was installed. However the new one failed catastrophically within a few days, generating a huge amount of metallic wear debris and causing pitting on the surface of the cast iron roller. Following the failure, samples were collected from both good and failed bearings. The samples were analyzed chemically and their microstructures examined. Both samples were subjected to accelerated wear tests in a laboratory type pin-on-disk apparatus. During the tests, the bearing materials acted as pins, which were pressed against a rotating cast iron disk. The wear behaviors of both bearing materials were studied using weight loss measurement. The worn surfaces of samples and the wear debris were examined by light optical microscope, SEM, and energy-dispersive x-ray microanalyzer. It was found that the laboratory pin-on-disk wear data correlated well with the plant experience. It is suggested that the higher lead content ~18%) of the good bearing compared with 7% lead of the failed bearing helped to establish a protective transfer layer on the worn surface. This transfer layer reduced metal-to-metal contact between the bearing and the roller and resulted in a lower wear rate. The lower lead content of the failed bearing does not allow the establishment of a well-protected transfer layer and leads to rapid wear.

Abstract
A group of control valves that regulate production in a field of sour gas wellheads performed satisfactorily for three years before pits and cracks were detected during an inspection. One of the valves was examined using chemical and microstructural analysis to determine the cause of failure and provide preventive measures. The valve body was made of A216-WCC cast carbon steel. Its inner surface was covered with cracks stemming from surface pits. Investigators concluded that the failure was caused by a combination of hydrogen-induced corrosion cracking and sulfide stress-corrosion cracking. Based on test data and cost, A217-WC9 cast Cr–Mo steel would be a better alloy for the application.

Abstract
High-temperature corrosion can occur in numerous environments and is affected by various parameters such as temperature, alloy and protective coating compositions, stress, time, and gas composition. This article discusses the primary mechanisms of high-temperature corrosion, namely oxidation, carburization, metal dusting, nitridation, carbonitridation, sulfidation, and chloridation. Several other potential degradation processes, namely hot corrosion, hydrogen interactions, molten salts, aging, molten sand, erosion-corrosion, and environmental cracking, are discussed under boiler tube failures, molten salts for energy storage, and degradation and failures in gas turbines. The article describes the effects of environment on aero gas turbine engines and provides an overview of aging, diffusion, and interdiffusion phenomena. It also discusses the processes involved in high-temperature coatings that improve performance of superalloy.

Abstract
Metal-induced embrittlement is a phenomenon in which the ductility or the fracture stress of a solid metal is reduced by surface contact with another metal in either the liquid or solid form. This article summarizes some of the characteristics of liquid-metal- and solid-metal-induced embrittlement. This phenomenon shares many of these characteristics with other modes of environmentally induced cracking, such as hydrogen embrittlement and stress-corrosion cracking. The discussion covers the occurrence, failure analysis, and service failures of the embrittlement. The article also briefly reviews some commercial alloy systems in which liquid-metal-induced embrittlement or solid-metal-induced embrittlement has been documented and describes some examples of cracking due to these phenomena, either in manufacturing or in service.

Abstract
Both rods in a Harrington rod cervical stent failed after a short time in service. Metallurgical analysis revealed a significant number of notches as well as enlarged grain size in one of the two rods, rough shallow-cracked surfaces along the bend profiles, possible signs of corrosion, and fractures (on both rods) near indentations imparted by retaining clamps. The observations suggest that surface roughness and bending defects initiated cracking that led to the fatigue failure of the compromised rod, followed some time later by the overload fracture of the second rod.

Abstract
Metal-induced embrittlement is a phenomenon in which the ductility or fracture stress of a solid metal is reduced by surface contact with another metal in either liquid or solid form. This article summarizes the characteristics of solid metal induced embrittlement (SMIE) and liquid metal induced embrittlement (LMIE). It describes the unique features that assist in arriving at a clear conclusion whether SMIE or LMIE is the most probable cause of the problem. The article briefly reviews some commercial alloy systems where LMIE or SMIE has been documented. It also provides some examples of cracking due to these phenomena, either in manufacturing or in service.

Abstract
A double-walled, hemispherical metal beam exit window made of alloy 718 developed a crack during service, leading to coolant leakage. The window had been exposed to radiation damage from 800 MeV protons and a cyclic stress from 600 MPa tensile to near zero induced by numerous temperature cycles calculated to be from 400 to 30 deg C (752 to 86 deg F). The window was activated to >200 Sv/h. It was determined through analysis using remote handling techniques and hot cells that the crack initiated near a spot weld used to affix thermocouples to the window surface. In addition to analysis of the crack, some of the irradiated material from the window was used to measure mechanical properties. Hot cell techniques for preparation of samples and testing were developed to determine true operating conditions of radiation, strain, and temperature.

Corrosive wear is defined as surface damage caused by wear in a corrosive environment, involving combined attacks from wear and corrosion. This article begins with a discussion on several typical forms of corrosive wear encountered in industry, followed by a discussion on mechanisms for corrosive wear. Next, the article explains testing methods and characterization of corrosive wear. Various factors that influence corrosive wear are then covered. The article concludes with general guidelines for material selection against corrosive wear.

Abstract
Erosion occurs as the result of a number of different mechanisms, depending on the composition, size, and shape of the eroding particles; their velocity and angle of impact; and the composition of the surface being eroded. This article describes the erosion of ductile and brittle materials with the aid of models and equations. It presents three examples of erosive wear failures, namely, abrasive erosion, erosion-corrosion, and cavitation erosion.

Abstract
Metallurgical SEM analysis provides many insights into the failure of biomedical materials and devices. The results of several such investigations are reported here, including findings and conclusions from the examination a total hip prosthesis, stainless steel and titanium compression plates, and hollow spinal rods. Some of the failure mechanisms that were identified include corrosive attack, corrosion plus erosion-corrosion, inclusions and stress gaps, production impurities, design flaws, and manufacturing defects. Failure prevention and mitigation strategies are also discussed.
