Abstract
The locking collar on a machine failed suddenly when the shaft it restrained was inadvertently subjected to an axial load slightly higher than the allowable working load. The locking collar fractured abruptly, producing four large fragments. This allowed the shaft to be propelled forcefully in the direction of the load, causing substantial damage to other machinery components in the vicinity. The failed component, which was 43 cm (17 in.) in diameter, was machined from 4140 plate and heat treated to 34 to 36 HRC. Analysis (visual inspection, composite micrographs, scanning electron microscopy, and mechanical-property analysis) supported the conclusions that the alloy steel plate used in this application contained significant brittle microstructural fibering or banding. This condition produced considerable anisotropy in ductility and toughness as revealed by mechanical testing. Unfortunately, the potential effects of anisotropy were apparently neglected when this component was designed and manufactured from the plate stock, because the loading was applied in a direction that stressed the weakest planes in the material, that is, a direction normal to the fibering. No recommendations were made.

Abstract
Two types of chromium-plated hydraulic cylinders failed by cracking on their outer surfaces. In one case, the parts had a history of cracking in the nominally unstressed, as-fabricated condition. In another, cracks were detected after the cylinders were subjected to a pressure impulse test. Both part types were made of 15-5 PH (UNS S15500) precipitation hardening stainless steel. Hydrogen embrittlement cracking was the likely cause of failure for both part types. Cracking of the as-fabricated parts was ultimately prevented by changing the manufacturing procedure to allow for a reheat treatment. For parts that cracked after pressure testing, excessive dimensional changes precluded the inclusion of a reheat treatment as a manufacturing step, and further failure was averted by carefully employing proper machining practices, avoiding abusive machining.

Abstract
Grease-wiper springs for cams formed from stampings of 0.25-mm thick carbon spring steel (0.65 to 0.80% C) fractured at the 0.025 mm radius on the stamped 135 deg corner at a 90 deg bend after 5,000,000 cycles. Tool marks 2 to 2.3 mm from the center of the stamped bend were disclosed by visual examination. Fatigue striations originating from cracks at the 0.025 mm radius inside corner at the bend were revealed by SEM of the fractured surface. The maximum stress at the bend, in stock of maximum thickness and as a function of the radius of the 135 deg corner, was indicated by stress calculations to be very close to the maximum allowable fluctuating stress for the material. The wiper springs were concluded to be fractured in fatigue and the cyclic loading resulted from cam rotation. The maximum applied stress approached the allowable limit due to high stress-concentration factor in the spring (caused by the very small inside radius). The corner radius was increased to 0.76 mm and the tools were re-polished to avoid tool marks.

Abstract
A trunnion bolt that was part of a coupling in a metropolitan railway system failed in service, causing cars to separate. The bolt had been in service for more than ten years prior to failure. Visual examination showed that the failure resulted from complete fracture at the grease port and surface groove located at midspan. Drillings machined from the bolt underwent chemical analysis, which confirmed that the material was AISI 1045 carbon steel, in accordance with specifications. Two sections cut from the bolt were subjected to metallographic examination and hardness testing. The fracture origin was typical of fatigue. The ultimate tensile strength of the bolt was in excess of requirements. Wear patterns indicated that the bolt had been frozen in position for a protracted period and subjected to repeated bending stresses, which resulted in fatigue cracking and final complete fracture. It was recommended that proper lubrication procedures be maintained to allow free rotation of the bolts while in service.

Abstract
Routine magnetic-particle inspection revealed crack indications in a number of shafts produced from hot-rolled 4130 steel bar. A pronounced indication of this size is cause for rejection if the defect is not eliminated during subsequent machining. A microstructural analysis of the shaft cross section revealed that the crack was approximately 0.5 mm (0.020 in.) deep and oriented in a radial direction. Furthermore, no stringer-type nonmetallic inclusions were observed in the vicinity of the flaw, which did not display the intergranular characteristics of a quench crack. The defect did, however, contain substantial amounts of oxide, which evidently resulted from the hot-working operation. This evidence supports the conclusion that the appearance of this discontinuity, with the long axis parallel to the working direction and radial orientation with regard to depth, strongly suggests a seam produced during rolling. Use of components with surface-defect indications as small as 0.5 mm (0.02 in.) can be risky in certain circumstances. Depending on the orientation of the flaw with respect to applied loads, the nature of the applied forces (for example, cyclic), and the operating environment, such a surface flaw can become the initiating site for a fatigue crack or a corrosion-related failure.

Plunger shafts machined from 4150 steel bar stock were involved in a series of fatigue failures. The fractures consistently occurred at two locations on the shafts: the shaft fillet and either side of a machined notch. The material specification for the shafts required 41xx series steel with a carbon content of 0.38 to 0.53%, a hardness of 35 to 40 HRC for the shaft, and a hardness of 50 to 55 HRC for the notch (which was case hardened). Analysis (visual inspection, chemical analysis, hardness testing, and magnetic particle inspection) supported the conclusions that all the fractures were fatigue-induced failures due to sharp radii in the fillets. The stress-concentrating effects of the fillets caused fatigue cracks to initiate and grow under cyclic loading until the crack depth was critical, causing the shaft to fail and rendering the assembly inoperative. Recommendations included increasing the radii of the notch and shaft fillets. If fatigue cracking had continued to be a problem with this component, shot peening of the subject radii would be appropriate. This process produces residual compressive stresses in the surface of the part, thereby retarding initiation of fatigue cracks.

Abstract
Two 38 mm (1.5 in.) diam threaded stud bolts that were part of a steel mold die assembly from a plastics molding operation were examined to determine their serviceability. Chemical analysis showed the material to be a plain carbon steel that approximated 1045. Visual examination revealed evidence of severe hammer blows to the clevis and boss areas and a gap between the die and the underside of the boss. Magnetic particle inspection showed cracks at the thread roots that, when examined metallographically, were found to contain MnS stringers. The cracking of the threads was attributed to a poor stud bolt design, which allowed a high stress concentration to occur at the base of the threads upon application of a lateral load. It was recommended that bolts of a new design that incorporated a stress-relieving groove be used. Threading of the bolt to eliminate the gap between the lower face of the boss and the die and an improved method of inserting or removing the bolt to avoid hammering (use of a wrench on a square or hexagonal boss) were also recommended.

Abstract
Proper stress analysis during component design is imperative for accurate life and performance prediction. The total stress on a part is comprised of the applied design stress and any residual stress that may exist due to forming or machining operations. Stress-corrosion cracking may be defined as the spontaneous failure of a metal resulting from the combined effects of a corrosive environment and the effective component of tensile stress acting on the structure. However, because of the orientation dependence in aluminum, it is the residual stress occurring in the most susceptible direction that must be considered of primary importance in material selection for design configuration. A Navy UH-1N helicopter main rotor blade grip manufactured from a 2014-T6 aluminum alloy forging failed because of a design flaw that left a high residual tensile stress along the short transverse plane; this in turn provided the necessary condition for stress corrosion to initiate. A complete failure investigation to ascertain the exact cause of the failure was conducted utilizing stereomicroscopic examination, scanning electron microscopy, metallographic inspection and interpretation, energy-dispersive chemical analysis, physical and mechanical evaluation. Stereomicroscopic examination of the opened crack fracture surface revealed one large fan-shaped region that had propagated radially through the thickness of the material from two distinct origin areas on the internal diam of the grip. Higher magnification inspection near the origin area revealed a flat, wood-like appearance. Scanning electron microscopy divulged the presence of substantial mud cracking and intergranular separation on the fracture surface. Metallographic examination revealed intergranular cracking and substantial leaf separation along the elongated grains parallel to the fracture surface. Chemical composition and hardness requirements were found to be as specified. The blade grip failed due to a stress corrosion crack which initiated on the inner diam and propagated in the short transverse direction through the thickness of the component. The high residual tensile stress in the part resulting from the forging and exposed after machining of the inner diam, combined with the presence of moisture, provided the necessary conditions to facilitate crack initiation and propagation.

A portion of two large spur tooth bull gears made from 4147H Cr-Mo alloy steel that had spalling teeth was submitted for evaluation. The gears were taken from a final drive wheel reduction unit of a very large open-pit mining truck. The parts had met the material and initial heat treat hardening specifications. The mode of failure was tooth profile spalling. By definition, spalling originates at a case/core interface or at the juncture of a hardened/nonhardened area. The cause of this failure was either insufficient or no induction-hardened case along the active profile. Abstract
A helicopter rapidly lost altitude and struck a tree, causing a fire and severe damage. The hose clamp which was the subject of this investigation was one of two used on a short length of hose between the turbocharger and the carburetion system. The purpose of this examination was to determine whether the hose failed during or before the accident. Fracture in the failed clamp was accompanied by obvious permanent deformation and evidence of local shearing at the ends of the perforation where fracture occurred, and in the adjacent perforation. The first test involved tightening the clamps to failure with a torque wrench. In no case did the band material fracture. In a second attempt to duplicate the failure, a tensile testing machine was used to pull the two fittings apart while the hose was clamped in place. When the testing machine was operated at maximum head travel (approximately 20 in. per min.), one of the hose clamps broke in the same manner as the clamp in question. The manner of failure during the tension test indicated this clamp failed at the time of the crash because of a sudden separation between the turbocharger and the remainder of the aircraft.

Abstract
The failure of an ATAR engine accessory angle drive gear assembly caused an engine flame-out in a Mirage III aircraft of the Royal Australian Air Force (RAAF) during a landing. Stripping of the engine revealed that the bevel gear locating splines (16 NCD 13) had failed. Visual and low-power microscope examination of the spline of the shaft showed evidence of fretting wear debris; similar wear was observed on the splines of the mating bevel gear. It was concluded that the splines had failed by severe fretting wear. Fretting damage was also observed on the shaft face adjacent to the splines and on the bevel gear abutment shoulder. Additional tests included a metrological inspection of the shaft, bevel gear and support ring; metallographic examination of a section from the shaft; chemical analysis of the shaft material (16 NCD 13); and hardness testing of a sample of the yoke material. The wear had been caused by incorrect machining of the shaft splines, which prevented the bevel gear nut from locating correctly against the gear.

Abstract
A forged, cadmium-plated electroslag remelt (ESR) 4340 steel mixer pivot support of the rotor support assembly located on an Army attack helicopter was found to be broken in two pieces during an inspection. Visual inspection of the failed part revealed significant wear on surfaces that contacted the bushing and areas at the machined radius where the cadmium coating had been damaged, which allowed corrosion pitting to occur. Optical microscopy showed that the crack origin was located at the machined radius within a region that was severely pitted. Electron microscopy revealed that most of the fracture surface failed in an intergranular fashion. Energy dispersive spectroscopy determined that deposits of sand, corrosion and salts were found within the pits. The failure started by hydrogen charging as a result of corrosion, and was aggravated by the stress concentration effects of pitting at the radius and the high notch sensitivity of the material. The failure mechanism was hydrogen-assisted and was most likely a combination of stress-corrosion cracking and corrosion fatigue. Recommendations were to improve the inspection criteria of the component in service and the material used in fabrication.

Abstract
A truck-mounted hydraulic crane had a horizontal thrust bearing with one race attached to the truck and the other to the rotating crane. The outside race of the bearing was driven by a pinion gear, and it is through this mechanism that the crane body rotated about a vertical axis. The manufacturer welded the inner race to the carrier in a single pass. After several years of service, the attachment weld between the bearing inner race and the turntable failed in the area adjacent to the heat-affected zone. The fracture zone where there was the greatest tension was heavily oxidized. In the zone where the bearing was in compression, there was a clean surface indicating recent fracture. Finally, there were areas where the weld did not meet AWS specifications for convexity or concavity. These areas were weak enough to allow fatigue cracks to initiate. Recommendations to prevent reoccurrence of the failure include the use of bolts in lieu of welding, a welding schedule that reduces the propensity of lamellar tearing, and the use of an alloy that precludes lamellar tearing. However, if abuse of the crane was the primary cause of failure, none of these recommendations would have prevented deterioration of the machine to an extent that would have rendered the failure improbable.

Nondestructive testing (NDT), also known as nondestructive evaluation (NDE), includes various techniques to characterize materials without damage. This article focuses on the typical NDE techniques that may be considered when conducting a failure investigation. The article begins with discussion about the concept of the probability of detection (POD), on which the statistical reliability of crack detection is based. The coverage includes the various methods of surface inspection, including visual-examination tools, scanning technology in dimensional metrology, and the common methods of detecting surface discontinuities by magnetic-particle inspection, liquid penetrant inspection, and eddy-current testing. Abstract
When complex designs, transient loadings, and nonlinear material behavior must be evaluated, computer-based techniques are used. This is where the finite-element analysis (FEA) is most applicable and provides considerable assistance in design analysis as well as failure analysis. This article provides a general view on the applicability of finite-element modeling in conducting analyses of failed components. It highlights the uses of finite-element modeling in the area of failure analysis and design, with emphasis on structural analysis. The discussion covers the general development and both general- and special-purpose applications of FEA. The special-purpose applications of FEA covered are piping and pressure vessel analysis, impact analysis, and microelectronic and microelectromechanical systems analysis. The article provides case histories that involved the use of FEA in failure analysis.

A multi-million dollar, four-color printing press used to produce a major weekly magazine was breaking pinions (shouldered shafts) on rolls. The cause of fracture was cyclic fatigue. Steel quality and heat treatment met expected standards. The pinion fracture showed multiple origins indicating rotational vibration fatigue. Keeping bolts tight solved this problem. In another case, grinding machines were unable to produce surfaces of uniform quality and smoothness on steel bearing products. Measurements showed that self-excited vibrations were created when particular steels were ground. It was found that the natural frequency of the wheel truing device was the culprit. Abstract
A type 410 stainless steel circulating water pump shaft used in a fossil power steam generation plant failed after more than 7 years of service. Visual examination showed the fracture surface to be coated with a thick, spalling, rust-colored scale, along with evidence of pitting. Samples for SEM fractography, EDS analysis, and metallography were taken at the crack initiation site. Hardness testing produced a value of approximately 27 HRC. The examinations clearly established that the shaft failed by fatigue. The fatigue crack originated at a localized region on the outside surface where pitting and intergranular cracking had occurred. The localized nature of the initial damage indicated that a corrosive medium had concentrated on the surface, probably due to a leaky seal. Reduction of hardness to 22 HRC or lower and inspection of seals were recommended to prevent future failures.
