Abstract
The A2 tool steel mandrel, part of a rolling tool used for mechanically joining two tubes was fractured after making five rolled joints. A 6.4 mm diam hole was drilled by EDM through the square end of the hardened mandrel due to difficulty was experienced in withdrawing the tool. The fracture progressed into the threaded section and formed a pyramid-shape fragment after it was initiated at approximately 45 deg through the hole in the square end. An irregular zone of untempered martensite with cracks radiating from the surface of the hole (result of melting around hole) was revealed by metallographic examination. A microstructure of fine tempered martensite containing some carbide particles was exhibited by the core material away from the hole. Brittle fracture characteristics with beach marks were exhibited by the fracture surfaces which is characteristic of a torsional fatigue fracture. Discussion centers on machining conditions and metallurgical factors contributing to (in-process) workpiece failures, and undesired surface layers and metallurgical factors contributing to (in-service) machined part failures. The second part of the article discusses the effects of microstructure on machining failures and their preventive measures.

Abstract
A needle bearing from a filling and seating machine for milk cartons became unusable due to corrosion and fracture of a ring after only four weeks of operation of the machine in a Finnish milk packing plant. These bearings were subject to corrosion by water condensates in this type of environment because of constant temperature changes, and they normally are replaced after eight months. The bearings were lubricated by a molybdenum sulfide paste. Judging by their structure the needles probably consisted of ball bearing steel. They showed corroded initial cracks of the pitting type, i.e., shear-fatigue fractures due to excessive surface pressure. The needles too were overstressed by compression. It seemed that the higher pressure necessary for the pressing of thicker paper accelerated the corrosion, which lead to the crack initiations of the parts and possibly also to impaired lubrication. The machine manufacturer therefore switched to bearings with shells of a complex bronze.

Abstract
Two suction rolls at the first press section of a 25 ft. wide paper machine developed cracks within two years of service. The rolls were austenitic stainless steel castings made of ASTM A 351 Grade CF8M alloy containing molybdenum. The rolls were exposed to slightly acidic white water (pH approximately 4.7) containing chlorides (45 ppm). Visual and liquid penetrant inspections of the rolls revealed extensive cracking at the roll inside surface. The cracks penetrated more than 30 percent of the wall thickness and a few cracks were several inches long. The cracks were preferentially oriented along the roll length and primarily at the roll inside surface. Field metallographic examination showed significant grain boundary chromium-carbide precipitation and intergranular corrosion. The roll failures were attributed to chromium depletion along the grain boundaries (sensitization) resulting from slow cooling of the casting to avoid large residual stresses. The roll manufacturer recommended a proprietary ferritic/austenitic stainless steel as the replacement material for the rolls.

Abstract
A felt guide roll fractured in-service on a paper manufacturing machine, damaging the belt as well as multiple dryer rolls, nearby felt guide rolls, and the frame of the machine. The investigation included visual and stereoscopic examination, chemical and microstructural analysis, microhardness and tensile testing, stress calculations, and vibration measurements. Based on the results, the roll fracture was attributed to high-cycle fatigue associated with a plug weld over one of the five threaded fasteners added to secure a balance weight inside the roll. The balance weight was installed to compensate for variations in wall thickness (i.e., weight distribution) of the pipe product used to make the roll. According to the investigation, resonance and vibration, which were initially considered, did not cause the failure.

Abstract
Several torsion bars had failed in a projectile weaving machine and were analyzed to determine the cause. Specimens prepared from the damaged components were subjected to visual inspection, hardness testing, chemical analysis, and metallurgical evaluations. The failed torsion bars had been fabricated from spring steel which, according to stress calculations, did not have sufficient torsional strength. Examination of the damaged parts confirmed the finding, revealing that all fractures started at a shoulder radius in an area of high stress concentration. Based on the investigation, the shoulder radius should be increased to alleviate stress and the working torsion angle of the bar should be decreased to improve safety factors.

A gear belonging to a pressurized heavy-water reactor refueling machine failed after 10 years in service. The material specified for the gear was a type C90700 bronze. Macroscopic examination focused on three gear teeth that had fractured completely at the roots, and fracture zones were examined using SEM microscopy. Failure of the gears was attributed to heavy wear resulting from misalignment. A lack of adequate lubrication was also noted. Periodic alignment adjustment and lubrication were recommended.

A number of machined end frame steel forgings made of Cr-Si-Mn alloy showed tiny cracks during magnetic particle inspection after heat treatment. The cracks were mostly confined to base edges and fillet radius. No significant abnormality was observed in chemical composition and microstructure. SEM, optical microscopy, and gas analysis revealed that the subsurface discontinuous cracks at the bore edges and in the fillet radius of the heat-treated end frame component had occurred due to hydrogen embrittlement, and not because of faulty heat treatment. This conclusion was supported by the presence of cracklike indications in machined bore surface of the annealed part.

Abstract
Several cases of failures in gray cast iron paper machine dryer rolls were evaluated. The rolls were found have ground outer cylindrical surfaces on which the paper web is dried. They were found to rotate about their longitudinal axes at speeds from 50 to 250 rpm while containing saturated steam from 35 to 380 kPa. Failures were found to occur in the shell body, in a head near a hand hole or a manhole opening, or in a head near the journal-to-head interface. A cleavage fracture was revealed by scanning electron microscopy regardless of the driving stress for failure. Fracture surface were found to exhibit chevron marks typical of fatigue or raised points or tears pointing in the direction of the probable origin of failure. The characteristics of the thinwall cast iron structures like the variation in composition due to pouring from multiple ladles, variation in solidification rates, and variation in tensile strength to be noted during inspection were described.

Milling machine arbors were inserted with satellite spindles having a maximum speed of 1500 rpm, and broke out between the groove and the flange. The appearance of the fracture surface was the same on both arbors. The pronounced scan lines characterized the fractures as fatigue fractures. The appearance of the fracture in the arbors indicated ductile fatigue fracture which had its origin in the radii between groove and flange. These radii of 0.15 and 0.2 mm were too small for the load on the milling machine. In addition there were grooves at the base of the radii which had an unfavorable effect on the life of the component by acting as notches with their resulting stress concentration. Considering the great hardness of the case, the small radii would have been critical even without grooves. Measures were taken so that the critical radius of the milling machine was increased and the surface roughness measured more precisely.

After about two years in service, a 303 stainless steel valve in contact with a carbonated soft drink in a vending machine occasionally dispensed a discolored drink with a sulfide odor. According to the laboratory at the bottling plant, the soft drink in question was strongly acidic, containing citric and phosphoric acids and having a pH of 2.4 to 2.5. Investigation (visual inspection, chemical analysis, immersion testing in the soft drink, and 100x unetched micrographs) supported the conclusion that the failure was caused by the size and distribution of sulfide stringers in the alloy used in the valve. Manganese sulfide stringers in the valve were exposed at end-grain surfaces in contact with the beverage. The stringers, which were anodic to the surrounding metal, were subject to corrosion, producing a hydrogen sulfide concentration in the immediately adjacent liquid. Recommendations included changing the valve material to type 304 stainless steel.

Abstract
A steam-pressurized Yankee dryer shell ruptured during normal operation. Cracking had occurred around much of the circumference at the drive end of the shell, which measured 3.7 m (12 ft) in diameter by 3.4 m (11 ft) long with a head bolted to each end. The crack initiated at a 90 deg corner in contact with the edge of the head. The material was a hardened gray cast iron containing 2.8% Ni and 1.2% Mo. Based on the results of visual, nondestructive, metallographic, and chemical analyses, it was concluded that failure occurred after corrosion fatigue cracking had weakened the shell. An ultrasonic examination of all Yankee dryers of the same type was recommended to look for cracking at the edge of the shell. Modification of the head-to-shell joint was recommended as well.
