Abstract
Distortion failure occurs when a structure or component is deformed so that it can no longer support the load it was intended to carry. Every structure has a load limit beyond which it is considered unsafe or unreliable. Estimation of load limits is an important aspect of design and is commonly computed by classical design or limit analysis. This article discusses the common aspects of failure by distortion with suitable examples. Analysis of a distortion failure often must be thorough and rigorous to determine the root cause of failure and to specify proper corrective action. The article summarizes the general process of distortion failure analysis. It also discusses three types of distortion failures that provide useful insights into the problems of analyzing unusual mechanisms of distortion. These include elastic distortion, ratcheting, and inelastic cyclic buckling.

Abstract
Distortion often is observed in the analysis of other types of failures, and consideration of the distortion can be an important part of the analysis. This article first considers that true distortion occurs when it was unexpected and in which the distortion is associated with a functional failure. Then, a more general consideration of distortion in failure analysis is introduced. Several common aspects of failure by distortion are discussed and suitable examples of distortion failures are presented for illustration. The article provides information on methods to compute load limits, errors in the specification of the material, and faulty process and their corrective measures to meet specifications. It discusses the general process of material failure analysis and special types of distortion and deformation failure.

Abstract
A hook on a two-leg chain (each 13 mm diam, included angle 60 deg) failed at the junction of the eye and shank while lifting a 4990 kg load. The diam of the hook at this junction was approximately 22 mm. Light intergranular oxidation at the surface on the side of the hook where cracking started was revealed by visual examination of the fracture region. Almost 50% of the fracture surface was found to contain beach marks (indicative of fatigue failure) while the remainder contained cleavage facets. A medium-coarse acicular as-forged structure was revealed by metallographic examination and the metal was showed by chemical analysis to be semikilled 1015 steel. The fatigue fracture was concluded to have initiated in the intergranular oxidation region and the failure of the hook was contributed by the poor fatigue and impact properties of the forged structure. As a corrective measure, the chain-sling hook was replaced with one made of normalized, fully killed, finegrain 1020 steel.

Abstract
A resistance-welded chain link made from 16 mm diam 4615 steel failed while lowering a 9070 kg load of billets into a rail car after being in service for 13 months. Beach marks, typical of fatigue were found to have originated at the inside of the link which broke at the weld. Cracks in the weld zone (up to 1.2 mm deep) were revealed during metallographic examination of a section through the fracture surface. The cracks were filled with scale which indicated that they had formed during resistance welding of the link. The defect was thus attributed to the weld defects which initiated the fatigue failure by acting as stress raisers. The welding method was changed by the manufacturer and all chains were replaced with defect free chains.

Abstract
An 8640 steel shaft installed in a fuel-injection-pump governor that controlled the speed of a diesel engine used in trucks and tractors broke after few days of operation. The mechanism that drove the shaft was designed to include a slip clutch to protect the governor shaft from shock loading. It was revealed by visual examination that the fracture had initiated in the sharp corner at the bottom of a longitudinal hole which was part of a force feed lubricating system. Beach marks were observed on the fracture surfaces. It was revealed by further examination that the slip clutch was removed in an effort to reduce cost and hence the shaft was subjected to increased vibration and shock loading. Insufficient fatigue limit of the shaft was revealed by fatigue testing of the shafts taken from stock in a rotating-beam machine. As a corrective measure, the fatigue limit of shafts was increased to 760 MPA by nitriding for 10 h at 515 deg C.

Abstract
A portion of the roof of a single story building collapsed during a thunder storm. A failure analysis was conducted to determine whether this structural failure was due to improper design, substandard construction materials, faulty erection, or extreme weather conditions. The failure analysis consisted of an onsite inspection, macrofractographic examination of the fractures where the girders were welded to the columns, macrofractographic examination of the fractured trusses, metallographic examination of the girder and truss materials, chemical analysis of the low-carbon steel girder and truss materials, and mechanical testing of the truss material. It was concluded that substandard structural components in combination with faulty construction was responsible for this service failure.

Abstract
High-horsepower electric motors were utilized to drive large compressors (made of 4340 steel shafts and gear-type couplings) required in a manufacturing process. The load was transmitted by two keys 180 deg apart. Six of the eight compressor shafts were found cracked in a keyway and one of them fractured after a few months of operation. Visual examination of fractured shaft revealed that the cracks originated from one of the keyways and propagated circumferentially around the shaft. The shaft and coupling slippage was indicated by the upset keys and this type of fracture. The shaft surface both near and in the keyways indicated fretting which greatly reduced the fatigue limit of the shaft metal and initiated fatigue cracks. Fatigue marks were observed on the fractured key. Repetitive impact loading was responsible for propagation of the cracks. The high cyclic bending stresses were caused by misalignment between the electric motor and compressor and were transmitted to the shaft through the geared coupling. Flexible-disk couplings capable of transmitting the required horsepower were installed on the shafts as a corrective measure.

A 17-4 PH steering actuator rod end body broke during normal take-off. Results of failure analysis revealed that the wall thickness of the race was much below the design limits, thus causing the race to rest on the body's swaged edges rather than on the load carrying centerline of the body. This assembly condition generated abnormal high loads on the swaged edges, ultimately resulting in fatigue failure. To prevent a recurrence of similar failure in the future, the dimensions of the race in the spherical bearing were changed, no further failure occurred.

Abstract
Bending fatigue caused crack propagation and catastrophic failures at several locations near the welds on the low-carbon steel tubular cargo box frame of police three-wheel motorcycles. ANSYS finite element analysis revealed that bending stresses in some of the frame members were aggravated by poor detail design between vertical and horizontal tubes. Stresses observed in the ANSYS analysis were not sufficient to cause the onset of fatigue. However when compounded by stress concentration factors and in-service dynamic loading, the frame could have been regularly subjected to stresses over the fatigue limit of the material. A strain gage static loading test verified FEM results, and finite element techniques were applied in the design of reinforcing members to renovate the frames. Material properties were determined and welding procedures specified for the reinforcing members. Inspection intervals were devised to avoid future problems.

Abstract
Turbine buckets in a 37.5-MW gas turbine made of Udimet 500 superalloy failed in service. The power plant was located 1 km (0.6 miles) from the Pacific Ocean and operated on No. 2 diesel fuel, which was supplied by tanker ship. Turbine bucket failures occurred on three units after 2500 to 6400 h of operation. Investigation (visual inspection, metallographic examination, and stress analysis) supported the conclusion that the differing microstructure of the airfoil resulted in changes in mechanical properties. Because normal operation includes cycling of loads and temperatures, the shroud tip fractured due to thermomechanical fatigue in its degraded state. Recommendations included special chromium or silicon-rich coating to minimize corrosion in gas turbines operating in a marine environment with operating temperatures in the range of type 2 corrosion (650 to 750 deg C, or 1200 to 1380 deg F). Additionally, it was suggested that fuel delivery, handling, and treatment be high quality, to maintain fuel contamination within design limits, and inlet air filtration must be designed for the coastal site. Also, changing the bucket tip by increasing its thickness and changing the casting technique would reduce the stress and make the design more tolerant of corrosion.

Abstract
Two intermediate impeller drive gears (made of AMS 6263 steel, gas carburized, hardened, and tempered) exhibited evidence of pitting and abnormal wear after production tests in test-stand engines. The gears were examined for hardness, case depth, and microstructure of case and core. It was found that gear 1 had a lower hardness than specified while the case hardness of gear 2 was found to be within limits. Both the pitting and the wear pattern were revealed to be more severe on gear 1 than on gear 2. Surface-contact fatigue (pitting) of gear 1 (cause of lower carbon content of the carburized case and hence lower hardness) was found to be the reason for failure. It was recommended that the depth of the carburized case on impeller drive gears be increased from 0.4 to 0.6 mm to 0.6 to 0.9 mm to improve load-carrying potential and wear resistance. A minimum case-hardness requirement was set at 81 HRA.

Abstract
Grease-wiper springs for cams formed from stampings of 0.25-mm thick carbon spring steel (0.65 to 0.80% C) fractured at the 0.025 mm radius on the stamped 135 deg corner at a 90 deg bend after 5,000,000 cycles. Tool marks 2 to 2.3 mm from the center of the stamped bend were disclosed by visual examination. Fatigue striations originating from cracks at the 0.025 mm radius inside corner at the bend were revealed by SEM of the fractured surface. The maximum stress at the bend, in stock of maximum thickness and as a function of the radius of the 135 deg corner, was indicated by stress calculations to be very close to the maximum allowable fluctuating stress for the material. The wiper springs were concluded to be fractured in fatigue and the cyclic loading resulted from cam rotation. The maximum applied stress approached the allowable limit due to high stress-concentration factor in the spring (caused by the very small inside radius). The corner radius was increased to 0.76 mm and the tools were re-polished to avoid tool marks.

Abstract
Stainless steel is frequently used for bone fracture fixation in spite of its sensitivity to pitting and cracking in chloride containing environments (such as organic fluids) and its susceptibility to fatigue and corrosion fatigue. A 316L stainless steel plate implant used for fixation of a femoral fracture failed after only 16 days of service and before bone callus formation had occurred. The steel used for the implant met the requirements of ASTM Standard F138 but did contain a silica-alumina inclusion that served as the initiation point for a fatigue/corrosion fatigue fracture. The fracture originated as a consequence of stress intensification at the edge of a screw hole located just above the bone fracture; several fatigue cracks were also observed on the opposite side of the screw hole edge. The crack propagated in a brittle-like fashion after a limited number of cycles under unilateral bending. The bending loads were presumably a consequence of leg oscillation during assisted perambulation.

Abstract
A new crane failed during the overload test following erection. A test load of 5 tons at the end of the jib (rated capacity 4 tons) was in the process of being slewed at the time of this failure. Inspection revealed that the collapse had resulted from the opening out of one eye of the rimming steel tie-bar of the main jib at the lower splice. This permitted the pin to pass through and allowed the jib to fall. Examination subsequently revealed that brittle fracture of two of the corner angles of the tower head assembly had also occurred. Had the tie-bar material been of satisfactory quality and/or, if the end that failed had been flamecut instead of sheared, then the damage resulting from the excessive overload would have been limited to yielding of the material in the region of the pin-joint. Such yielding on an overload test further indicated that the scantlings of the pin-joints were inadequate. Two other crane failures showed that failure resulted from the use of rimming steel, and embrittlement of the material was evident.

Abstract
Several case-hardened and zinc-plated carbon-manganese steel wheel studs fractured in a brittle manner after very limited service life. The fracture surfaces of both front and rear studs showed no sign of fatigue beach marks or deformation in the form of shear lips that would indicate either a fatigue mechanism or ductile overload failure. SEM analysis revealed that the mode of fracture was intergranular decohesion, which indicates an environmental influence in the fracture mechanism. The primary fracture initiated at a thread root and propagated by environmentally-assisted slow crack growth until final fracture. The natural stress concentration at the thread root, when tightened to the required clamp load concomitant with the presence of cracks in the carburized case, was sufficient to exceed the critical stress intensity for hydrogen-assisted stress cracking (HASC). The zinc plating exacerbated the situation by providing a strong local corrosion cell in the form of a sacrificial anode region adjacent to the cracked thread. The enhanced generation of hydrogen in a corrosive environment subsequently lead to HASC of the wheel studs.

Abstract
Mechanical testing is an evaluative tool used by the failure analyst to collect data regarding the macro- and micromechanical properties of the materials being examined. This article provides information on a few important considerations regarding mechanical testing that the failure analyst must keep in mind. These considerations include the test location and orientation, the use of raw material certifications, the certifications potentially not representing the hardware, and the determination of valid test results. The article introduces the concepts of various mechanical testing techniques and discusses the advantages and limitations of each technique when used in failure analysis. The focus is on various types of static load testing, hardness testing, and impact testing. The testing types covered include uniaxial tension testing, uniaxial compression testing, bend testing, hardness testing, macroindentation hardness, microindentation hardness, and the impact toughness test.

Gears can fail in many different ways, and except for an increase in noise level and vibration, there is often no indication of difficulty until total failure occurs. This article reviews the major types of gears and the basic principles of gear-tooth contact. It discusses the loading conditions and stresses that effect gear strength and durability. The article provides information on different gear materials, the common types and causes of gear failures, and the procedures employed to analyze them. Finally, it presents a chosen few examples to illustrate a systematic approach to the failure examination.
