Abstract
The US Army Research Laboratory performed a failure investigation on a broken main landing gear mount from an AH-64 Apache attack helicopter. A component had failed in flight, and initially prevented the helicopter from safely landing. In order to avoid a catastrophe, the pilot had to perform a low hover maneuver to the maintenance facility, where ground crews assembled concrete blocks at the appropriate height to allow the aircraft to safely touch down. The failed part was fabricated from maraging 300 grade steel (2,068 MPa [300 ksi] ultimate tensile strength), and was subjected to visual inspection/light optical microscopy, metallography, electron microscopy, energy dispersive spectroscopy, chemical analysis, and mechanical testing. It was observed that the vacuum cadmium coating adjacent to the fracture plane had worn off and corroded in service, thus allowing pitting corrosion to occur. The failure was hydrogen-assisted and was attributed to stress corrosion cracking (SCC) and/or corrosion fatigue (CF). Contributing to the failure was the fact that the material grain size was approximately double the required size, most likely caused from higher than nominal temperatures during thermal treatment. These large grains offered less resistance to fatigue and SCC. In addition, evidence of titanium-carbo-nitrides was detected at the grain boundaries of this material that was prohibited according to the governing specification. This phase is formed at higher thermal treatment temperatures (consistent with the large grains) and tends to embrittle the alloy. It is possible that this phase may have contributed to the intergranular attack. Recommendations were offered with respect to the use of a dry film lubricant over the cadmium coated region, and the possibility of choosing an alternative material with a lower notch sensitivity. In addition, the temperature at which this alloy is treated must be monitored to prevent coarse grain growth. As a result of this investigation and in an effort to eliminate future failures, ARL assisted in developing a cadmium brush plating procedure, and qualified two Army maintenance facilities for field repair of these components.

The shafts on two centrifugal pumps failed during use in a petroleum refinery. Light optical microscopy and scanning electron microscopy were used to analyze the damaged materials to determine the cause of failure. The results showed that one shaft, made of duplex stainless steel, failed by fatigue fracture, and the other, made of 316 austenitic stainless steel, experienced a similar fracture, which was promoted by the presence of nonmetallic inclusion particles.

A 1984 Chevrolet Blazer was being pushed by three youths after it ran out of gas when it was hit from behind by a 1979 Mercury Cougar. One of the youths was crushed between the two vehicles and killed. Optical microscopy was used to examine the tungsten filaments from the headlamps of the Cougar and from the four signal/emergency/parking lights of the Blazer to determine whether the vehicles lights were in use at the time of the accident. Based on degree of distension and brittle fragmentation, it was determined that the low-beam headlights of the Cougar and the parking lights of the Blazer were on at the time of the collision.

Abstract
A failed laser mirror and another complete mirror of the same construction were analyzed. The laser mirror consisted of three layers of material brazed together to form channels through which the cooling water flows. Samples were analyzed with light optical and scanning electron microscopy. The corrosion product contained molybdenum and copper with a trace of gold. The base material was analyzed as molybdenum with negligible alloying additions. The primary mode of corrosion attack on the base material appeared to be intergranular, although uniform corrosion was evident also. It was concluded that corrosion attack sufficiently weakened the base material and the brazed joints, allowing catastrophic failure of the mirror due to the pressure of the cooling water. It was recommended that the mirrors be cleaned of all corrosion products present as a result of past service conditions and proof tested. It was recommended that the water system consisting of deionized water and formaldehyde be replaced with water having a low oxygen content and a cathodic inhibitor (oxygen scavenger).

Abstract
This article reviews photographic principles, namely, visual examination, field photographic documentation, and laboratory photographic documentation, as applied to failure analysis and the specific techniques employed in both the field and laboratory. It provides information on the photographic equipment used in failure analysis and on film and digital photography. The article describes the basics of photography and the uses of different types of lighting in photography of a fractured surface. The article also addresses the techniques involved in macrophotography and microscopic photography as well as other special techniques.

Abstract
Failure analysis is an investigative process that uses visual observations of features present on a failed component fracture surface combined with component and environmental conditions to determine the root cause of a failure. The primary means of recording the conditions and features observed during a failure analysis investigation is photography. Failure analysis photographic imaging is a combination of both science and art; experience and proper imaging techniques are required to produce an accurate and meaningful fracture surface photograph. This article reviews photographic principles and techniques as applied to failure analysis, both in the field and in the laboratory. The discussion covers the processes involved in field and laboratory photographic documentations, provides a description of professional digital cameras, and gives information on photographic lighting and microscopic photography. Special techniques can be employed to deal with highly reflective conditions and are also described in this article.

Abstract
Fractography is the means and methods for characterizing a fractured specimen or component. This includes the examination of fracture-exposed surfaces and the interpretation of the fracture markings as well as the examination and interpretation of crack patterns. This article describes the former of these two parts of fractography. It presents the techniques of fractography and explains fracture markings using glass and ceramic examples. The article also discusses the fracture modes in ceramics and provides examples of fracture origins.

Abstract
On 9 March 2000, a gasoline pipeline failed near Greenville, TX releasing approximately 12,000 barrels of fuel. After the on-scene portion of the investigation was completed, an 8.5 ft. (2.6 m) section of the 28 in. (71 cm) diam pipe was sent to the materials laboratory for examination. Examination included optical and scanning electron microscopy of the fracture surfaces and metallographic examination of cross sections through the fracture surface. From the outer to inner edge of the fracture surface, three different areas were observed. Fracture features in area 1 were obliterated by corrosion. The fracture features in region 2 were relatively smooth, and striations were observed, typical of fatigue. In region 3, dimple features were observed, typical of ductile overstress. Also, corrosion pits were observed on the outer surface of the pipe section in locations where the protective black tar-like coating was cracked.

Abstract
Failure analysis is an investigative process in which the visual observations of features present on a failed component and the surrounding environment are essential in determining the root cause of a failure. This article reviews the basic photographic principles and techniques that are applied to failure analysis, both in the field and in the laboratory. It discusses the processes involved in visual examination, field photographic documentation, and laboratory photographic documentation of failed components. The article describes the operating principles of each part of a professional digital camera. It covers basic photographic principles and manipulation of settings that assist in producing high-quality images. The need for accurate photographic documentation in failure analysis is also presented.

Abstract
Several stainless steel coils cracked during a routine unwinding procedure, prompting an investigation to determine the cause. The analysis included optical and scanning electron microscopy, energy-dispersive x-ray spectrometry, and tensile testing. An examination of the fracture surfaces revealed a brittle intercrystalline mode of fracture with typical manifestations of clear grain facets. Branched and discrete stepwise microcracks were also found along with unusually high levels of residual hydrogen. Mechanical tests revealed a marked loss of tensile ductility in the defective steel with elongations barely approaching 8%, compared to 50% at the time of delivery weeks earlier. Based on the timing interval and the fact that failure occurred at operating stresses well below the yield point of the material, the failure is being attributed to hydrogen-induced damage. Potential sources of hydrogen are considered as are remedial measures for controlling hydrogen content in steels.

Abstract
The purpose of this investigation was to determine the root cause of the differences noted in the fatigue test data of main rotor spindle assembly retaining rods fabricated from three different vendors, as part of a Second Source evaluation process. ARL performed dimensional verification, accessed overall workmanship, and measured the respective surface roughness of the rods in an effort to identify any discrepancies. Next, mechanical testing was performed, followed by optical and electron microscopy, and chemical analysis. Finally, ARL performed laboratory heat treatments at the required aging temperature and follow-up mechanical testing.

Four cadmium-plated ASTM A193 grade B studs from a steam line connector associated with a power turbine failed unexpectedly in a nil-ductility manner. Fracture surfaces were covered with a light-colored, lustrous deposit. Optical microscope, SEM, and EDS analyses were conducted on sections from one of the studs and revealed that the coating on the fracture surface was cadmium. The fracture had multiple origins, and secondary cracks also contained cadmium. The fracture topography was intergranular. The failures were attributed to liquid metal embrittlement caused by the presence of a cadmium plating and operating temperatures at approximately the melting point of cadmium. It was recommended that components exposed to the cadmium be replaced.

A nickel alloy cylinder plated with chromium along its inner liner, installed in a commercial ice cream freezer, showed gray discoloration along its OD surface. The discolored parts exhibited significantly reduced cooling efficiency as compared with new cylinders. During operation, the OD of the cylinder was exposed to liquid ammonia refrigerant containing lubricant from the compressor. The lubricant (mineral oil) was intended to separate from the ammonia and be recirculated through the compressor. Nondestructive portable optical microscopy, XRF, EDS, and XPS analyses showed that the discoloration on the cylinder was associated with metal oxidation products coated with a thin oil film. One of the recommendations was to plate the OD of the cylinder with hard chromium to increase its resistance to erosion. Another recommendation was to reduce the amounts of water contamination in the refrigerant.

Abstract
Six transformer brackets failed in service, sending a group of three pole-mounted transformers to the ground below. The brackets were made from acrylonitrile-butadiene-styrene (ABS) resin and had been in service for more than 30 years. Remnants of the fractured brackets were analyzed using optical and scanning electron microscopy, Fourier transform infrared spectroscopy (FTIR), and thermogravimetric analysis (TGA). The exterior surfaces of all six brackets were alike and shared similar features, including witness marks, discoloration, mechanical deformation, and secondary cracking, along with crack networks. Both FTIR and TGA analyses indicated that the surface material was in a highly degraded state, likely due to weathering and thermal and ultraviolet exposure. This, in turn, led to the formation of cracks that propagated under the cyclic forces of vibration and wind. As the cracks grew larger, the weight of the transformer eventually overloaded the brackets, resulting in failure.

Abstract
To determine the effect of severe service on cast 357 aluminum pistons, a metallurgical evaluation was made of four pistons removed from the engine of the Hawk-Offenhauser car which had been driven by Rich Muther in the first Ontario, California 500 race. The pistons were studied by visual inspection, hardness traverses, radiography, dye penetrant inspection, chemical analysis, macrometallography, optical microscopy, and electron microscopy. The crown of one piston had a rough, crumbly deposit, which was detachable with a knife. Two pistons had remains of carbonaceous deposits. The fourth was severely hammered. It was concluded that the high temperatures developed in this engine created an environment too severe for 357 aluminum. Surfaces were so hot that the low-melting constituent melted. Then, the alloy oxidized rapidly to form Al2O3, an abrasive which further aggravated problems. The temperature in much of the piston was high enough to cause softening by overaging, lowering strength.

Abstract
A crack was detected in one arm of the right-hand horizontal brace of the nose landing gear shock strut from a large military aircraft. The shock strut was manufactured from a 7049 aluminum alloy forging in the shape of a delta. A laboratory investigation was conducted to determine the cause of failure. It was concluded that the arm failed because of the presence of an initial defect that led to the initiation of fatigue cracking. The fatigue cracking grew in service until the part failed by overload. The initial defect was probably caused during manufacture. Fleet-wide inspection of the struts was recommended.

Abstract
Metallographic examination is one of the most important procedures used by metallurgists in failure analysis. Typically, the light microscope (LM) is used to assess the nature of the material microstructure and its influence on the failure mechanism. Microstructural examination can be performed with the scanning electron microscope (SEM) over the same magnification range as the LM, but examination with the latter is more efficient. This article describes the major operations in the preparation of metallographic specimens, namely sectioning, mounting, grinding, polishing, and etching. The influence of microstructures on the failure of a material is discussed and examples of such work are given to illustrate the value of light microscopy. In addition, information on heat-treatment-related failures, fabrication-/machining-related failures, and service failures is provided, with examples created using light microscopy.

Abstract
The electroplated tappet adjusting screws used in diesel engines failed during initial bend testing. The analysis of the failure showed that the fracture was nucleated from the subsurface of the screw. The fracture surface was intergranular at the ID and OD region and microvoid coalescence in the center. The improper baking after electroplating of the screw led to H2-induced blistering/cracking. The high strength of the threaded region of the adjusting screw increased the failure propensity.
