Abstract
A railway tank car developed a fracture in the region of the sill and shell attachment during operation at -34 deg C (-30 deg F). On either side of the sill-support member, cracking initiated at the weld between a 6.4 mm thick frontal cover plate and a 1.6 mm thick side support plate. The crack then propagated in a brittle manner upward through the side plate, through the welds attaching the side plate to a 25 mm (1 in.) thick shell plate (ASTM A212, grade B steel), and continued for several millimeters in the shell plate before terminating. Other plates involved were not positively identified but were generally classified as semi-killed carbon steels. Investigation (visual inspection, hardness testing, chemical analysis, Charpy V-notch testing, and drop-weight testing) supported the conclusions that the fracture was initiated by weld imperfections and propagated in a brittle manner as a result of service stresses acting on the plate having low toughness at the low service temperatures encountered. Recommendations included that the specifications for the steel plates be modified to include a toughness requirement and that improved welding and inspection practices be performed to reduce the incidence of weld imperfections.

This article describes the general root causes of failure associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be the common sources of failure-inducing defects in bulk working of wrought products. The article discusses the types of imperfections that can be traced to the original ingot product. These include chemical segregation; ingot pipe, porosity, and centerline shrinkage; high hydrogen content; nonmetallic inclusions; unmelted electrodes and shelf; and cracks, laminations, seams, pits, blisters, and scabs. The article provides a discussion on the imperfections found in steel forgings. The problems encountered in sheet metal forming are also discussed. The article concludes with information on the causes of failure in cold formed parts.

Abstract
This article briefly reviews the general causes of weldment failures, which may arise from rejection after inspection or failure to pass mechanical testing as well as loss of function in service. It focuses on the general discontinuities observed in welds, and shows how some imperfections may be tolerable and how the other may be root-cause defects in service failures. The article explains the effects of joint design on weldment integrity. It outlines the origins of failure associated with the inherent discontinuity of welds and the imperfections that might be introduced from arc welding processes. The article also describes failure origins in other welding processes, such as electroslag welds, electrogas welds, flash welds, upset butt welds, flash welds, electron and laser beam weld, and high-frequency induction welds.

This article describes the underlying fundamentals, applications, the relevance and necessity of performing proper stress analysis in conducting a failure analysis. It presents an introduction to the stress analysis of bodies containing crack-like imperfections and the topic of fracture mechanics. The fracture mechanics approach is an important part of stress analysis at the tips of sharp cracks or discontinuities. The article reviews fracture mechanics concepts, including linear elastic fracture mechanics, elastic-plastic fracture mechanics, and subcritical fracture mechanics. It also provides information on the applications of fracture mechanics in failure analysis.

Abstract

Abstract
Splined rotor shafts (constructed from 1151 steel) used on small electric motors were found to miss one spline each from several shafts before the motors were put into service. Apparent peeling of splines on the induction-hardened end of each rotor shaft was revealed by visual and stereo-microscopic examination. One tooth on each shaft was found to be broken off. It was revealed by metallographic examination of an unetched section through the fractured tooth that the fracture surface was concave and had an appearance characteristic of a seam. Partial decarburization of the surface was revealed after etching with 1% nital. The presence of a crack, with typical oxides found in seams at its root, was disclosed by an unetched section through the shaft in an area unaffected by induction heating. The etched samples revealed similar decarburization as was noted on the fracture surface of the tooth. It was concluded that the seam had been present before the shaft was heat treated and these seams acted as stress raisers during induction hardening to cause the shaft failure. It was recommended that the specifications should specify that the shaft material should be free of seams and other surface imperfections.

Abstract
A sand-cast gray iron flanged nut was used to adjust the upper roll on a 3.05 m (10 ft) pyramid-type plate-bending machine. The flange broke away from the body of the nut during service. Analysis (visual inspection and 150x micrographs of sections etched with nital) supported the conclusions that brittle fracture of the flange from the body was the result of overload caused by misalignment between the flange and the roll holder. The microstructure contained graphite flakes of excessive size and inclusions in critical areas; however, these metallurgical imperfections did not appear to have had significant effects on the fracture. Recommendations included carefully and properly aligning the flange surface with the roll holder to achieve uniform distribution of the load. Also, a more ductile metal, such as steel or ductile iron, would be more suitable for this application and would require less exact alignment.

Abstract
A new supplier for aluminum die castings was being evaluated, and the castings failed to meet the durability test requirements. Specifically, the fatigue life of the castings was low. Initial inspection of the fatigue fracture surfaces revealed large-scale porosity visible to the naked eye. New castings with reduced porosity also failed the durability tests. The fatigue fracture surfaces of additional casting fragments were very rough and contained multiple ratchet marks along the inner fillet. These observations indicated the fatigue process was heavily influenced by the presence of surface imperfections. Improving the surface finish or choosing a stronger alloy, were more likely to improve part durability than reducing the porosity.

Abstract
A very large diameter worm gear that had been in service in a dam for more than 60 years exhibited cracks and was removed. It was reported that the high-strength, low-ductility cast bronze gear was only rarely stressed during service, associated with infrequent opening and closing of gates. Due to the age of the gear and the time frame of its manufacture, no original material specifications or strength requirements could be located. Likewise, no maintenance records of possible repairs to the gear were available. Investigation (visual inspection, chemical analysis, tension and hardness testing, 119x SEM images, and potassium dichromate etched 297x metallographic images) supported the conclusion that the bronze gear cracked via mixed-mode overload, rather than by a progressive mechanism such as fatigue or stress-corrosion cracking. The cracking was not associated with regions that would be highly stressed and did not appear to be consistently correlated to casting imperfections, repair welds, or associated heat-affected zones. Cracking across the gear face suggested that bending forces from misalignment were likely responsible for the cracking. Recommendations included further review of the potential root cause.

A stick-shift from an automobile transmission failed in service. Failure occurred by stable fatigue crack propagation at locations R and L. Plausibly, the crack initiated at location R due to maximum tensile stress, which was perhaps further concentrated by a surface imperfection. As the crack progressed, the service load shifted to the section marked L. This overload on location L appears to have caused another fatigue crack that propagated first over the smooth-banded region. Catastrophic failure occurred when the component became unstable under the applied load.

Abstract
Two sand-cast low-alloy steel equalizer beams (ASTM A 148, grade 105-85) designed to distribute the load to the axles of a highway truck broke after an unreported length of service. Normal service life would have been about 805,000 km (500,000 mi) of truck operation. Investigation (visual inspection, chemical analysis, tensile testing, unetched 65x and 1% nital etched 65x magnification) supported the conclusions that the steel was too soft for the application – probably due to improper heat treatment. Fracture of the equalizer beams resulted from growth of mechanical cracks that were formed before the castings were heat treated. Recommendations included the following changes in processing: better gating and risering in the foundry to achieve sounder castings; better shakeout practice to avoid mechanical damage; better inspection to detect imperfections; and normalizing and tempering to achieve better mechanical properties.

Abstract
Cracking in gas turbine blades was found to initiate from a mechanism of low-cycle fatigue (LCF). LCF is induced during thermal loading cycles in gas turbines. However, metallography of two cracked blades revealed a change in microstructure at as-cast surfaces for depths up to 0.41 mm (0.016 in.). Evaluation by SEM confirmed the difference in structure was associated with a lack of formation of coarse gamma prime structure in the matrix. Microhardness and miniature tensile test results indicated lower strength consistent with the absence of the coarse gamma prime constituent. The blade vendor found that the lot of hot isostatically pressed (HIP) blade castings had been exposed to an improper atmosphere during the HIP process, resulting in the weakened structure. Because subsequent failures were found in blades that did not come from the suspect HIP lot, the scope of the problem was considered generic, and the conclusion was that the primary failure mechanism was LCF. Material imperfections were a secondary deficiency that had the effect of causing the blades from the bad HIP lot to crack first.

Abstract
A commercial aircraft wheel half, machined from an aluminum alloy 2014 forging that had been heat treated to the T6 temper, was removed from service because a crack was discovered in the area of the grease-dam radius during a routine inspection. Neither the total number of landings nor the roll mileage was reported, but about 300 days had elapsed between the date of manufacture and the date the wheel was removed from service. The analysis (visual inspection, macrographs, micrographs, electron microprobe) supported the conclusions that the wheel half failed by fatigue. The fatigue crack originated at a material imperfection and progressed in more than one plane because changes in the direction of wheel rotation altered the direction of the applied stresses. Recommendations included rewriting the inspection specifications to require sound forgings.

Abstract
This article focuses on characterizing the fracture-surface appearance at the microscale and contains some discussion on both crack nucleation and propagation mechanisms that cause the fracture appearance. It begins with a discussion on microscale models and mechanisms for deformation and fracture. Next, the mechanisms of void nucleation and void coalescence are briefly described. Macroscale and microscale appearances of ductile and brittle fracture are then discussed for various specimen geometries (smooth cylindrical and prismatic) and loading conditions (e.g., tension compression, bending, torsion). Finally, the factors influencing the appearance of a fracture surface and various imperfections or stress raisers are described, followed by a root-cause failure analysis case history to illustrate some of these fractography concepts.

Stainless steel pipe (273-mm OD x 8-mm wall thickness) used in the fabrication of large manifolds developed crack-like decohesions during a routine inductive bending procedure. The imperfections, which were found near the outside diameter, were around 3 mm in length oriented in the circumferential direction and penetrated nearly 2 mm into the pipe wall. The pipes were made of titanium-stabilized austenitic stainless steel X6CrNiMoTi17-12-2. Abstract
A carbon steel (ASTM A515 grade 70) pressure vessel failed by brittle fracture while being hydro tested in the fabricating shop. The fracture origin was a small crack at a welding arc strike associated with the toe of a nozzle weld. A fracture mechanics calculation indicated that this imperfection, although small, initiated fracture because of the local geometry and stress conditions and the low toughness of the steel. It was recommended that (1) the probability of flaws be reduced by welding over or grinding out arc strikes, (2) the local stresses be lowered by post weld stress relief and improved weld toe geometry, and (3) toughness be improved by specifying fine-grain steel and/ or by normalizing.

However, imperfections that can lead to braze failure may result if proper attention is not paid to the physical properties of the material, joint design, prebraze cleaning, brazing procedures, postbraze cleaning, and quality control. Factors that must be considered include brazeability of the base metals; joint design and fit-up; filler-metal selection; prebraze cleaning; brazing temperature, time, atmosphere, or flux; conditions of the faying surfaces; postbraze cleaning; and service conditions. This article focuses on the advantages, limitations, sources of failure, and anomalies resulting from the brazing process. It discusses the processes involved in the testing and inspection required of the braze joint or assembly.

The primary purpose of this article is to describe general root causes of failure that are associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be common sources of failure-inducing defects in the bulk working of wrought products. The article addresses the types of flaws or defects that can be introduced during the steel forging process itself, including defects originating in the ingot-casting process. Defects found in nonferrous forgings—titanium, aluminum, and copper and copper alloys—also are covered.
