Abstract
The repeated occurrence of random cracks in the fillet radius portion of low-alloy steel (38KhA) end frame forgings following heat treatment was investigated. Microstructural analyses were carried out on both the failed part and disks of the rolled bar from which the part was made. Subsurface cracks were found to be zigzag and discontinuous as well as intergranular in nature. A mixed mode of fracture involving ductile and brittle flat facets was observed. Micropores and rod-shaped manganese sulfide inclusions were also noted. The material had a hydrogen content of 22 ppm, and cracking was attributed to hydrogen embrittlement. Measurement of hydrogen content in the raw material prior to fabrication was recommended. Careful control of acid pickling procedures for descaling of the hot-rolled bars was also deemed necessary.

Abstract
Several stainless steel coils cracked during a routine unwinding procedure, prompting an investigation to determine the cause. The analysis included optical and scanning electron microscopy, energy-dispersive x-ray spectrometry, and tensile testing. An examination of the fracture surfaces revealed a brittle intercrystalline mode of fracture with typical manifestations of clear grain facets. Branched and discrete stepwise microcracks were also found along with unusually high levels of residual hydrogen. Mechanical tests revealed a marked loss of tensile ductility in the defective steel with elongations barely approaching 8%, compared to 50% at the time of delivery weeks earlier. Based on the timing interval and the fact that failure occurred at operating stresses well below the yield point of the material, the failure is being attributed to hydrogen-induced damage. Potential sources of hydrogen are considered as are remedial measures for controlling hydrogen content in steels.

Abstract
A 10-cm (4-in.) chain link used in operating a large dragline bucket failed after several weeks in service. The link was made of cast low-alloy steel (similar to ASTM A487, class 10Q) that had been normalized, hardened, and tempered to give a yield strength of approximately 1034 MPa (150 ksi). A hydrogen flake approximately 5 cm (2 in.) in diam was observed at the center of the fracture surface. Beach marks indicative of fatigue encircled the hydrogen flake and covered nearly all of the remaining fracture surface. The failure of this linkways caused by an excessive hydrogen content. Abstract
This article describes the general root causes of failure associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be the common sources of failure-inducing defects in bulk working of wrought products. The article discusses the types of imperfections that can be traced to the original ingot product. These include chemical segregation; ingot pipe, porosity, and centerline shrinkage; high hydrogen content; nonmetallic inclusions; unmelted electrodes and shelf; and cracks, laminations, seams, pits, blisters, and scabs. The article provides a discussion on the imperfections found in steel forgings. The problems encountered in sheet metal forming are also discussed. The article concludes with information on the causes of failure in cold formed parts.

Abstract
The U-0.8wt%Ti alloy is often used in weapon applications where high strength and fairly good ductility are necessary. Components are immersion quenched in water from the gamma phase to produce a martensitic structure that is amenable to aging. Undesirable conditions occur when a component occasionally cracks during the quenching process, and when tensile specimens fail prematurely during mechanical testing. These two failures prompted an investigative analysis and a series of studies to determine the causes of the cracking and erratic behavior observed in this alloy. Quench-related failures whereby components that cracked either during or immediately after the heat treatment/quenching operation were sectioned for metallographic examination of the microstructure to examine the degree of phase transformation. Examination of premature tensile specimen failures by scanning electron microscopy and X-ray imaging of fracture surfaces revealed pockets of inclusions at the crack origins. In addition, tests were conducted to evaluate the detrimental effects of internal hydrogen on ductility and crack initiation in this alloy.

Abstract
Fracture of a cadmium-plated accumulator ring forged from 4140 steel was discovered during inspection and disassembly of a hydraulic-accumulator system stored at a depot. The ring had broken into five small and two large segments. The small segments of the broken ring displayed very flat fracture surfaces with no apparent yielding, but the two large segments did show evidence of bending (yielding) near the fractures. In addition, some segments contained fine radial cracks. Analysis (visual inspection, optical microscopy on polished-and-etched specimens, hardness testing, and chemical analysis) supported the conclusion that the failure was caused due to brittle fatigue, as evidenced by the intergranular nature of the fracture path. Also, hydrogen penetration occurred during the plating operation and was not relieved subsequently as required.

Abstract
During the construction of a large-diam pipeline, several girth welds had to be cut out as a result of radiographic interpretation. The pipeline was constructed of 910 mm (36 in.) diam x 13 mm (0.5 in.) wall thickness grade X448 (x65) line pipe. The girth welds were fabricated using standard vertical down stove pipe-welding procedures with E7010 cellulosic electrodes. The crack started partially as a result of incomplete fusion on the pipe side wall, which in turn was a result of misalignment of the two pipes. The crack was typical of hydrogen cracking. Girth welds can be made using cellulosic electrodes. For high-risk girth welds, an increase in preheat and/or a reduction in the local stress by controlling lift height or depositing the hot pass locally before lifting may be required.

Abstract
Hydrogen embrittlement is the brittleness affecting copper and copper alloys containing oxygen which develops during heat treatment at temperatures of about 400 deg C (752 deg F) and above in an atmosphere containing hydrogen. The phenomenon of hydrogen embrittlement of copper and its alloys is illustrated by examples from practice and reference is made to data from recent publications on the subject. Embrittlement due to this cause can only be identified by microscopic examination because other modes of failure in copper; e.g., from heat cracking, mechanical overload, the formation of low melting point eutectics or corrosion; show a similar appearance when investigated on a macroscopic scale.

Abstract
The failures of two aircraft components, one from a landing gear and the other from an ejector rack mechanism, were investigated. Both were made from PH 13-8 Mo (UNS S13800) precipitation-hardening stainless steel which had been heat treated to the H1000 and H950 tempers respectively and then chromium plated. The parts were characterized metallographically and mechanically and were found to be compliant. Detailed fractographic examination revealed that the first stage of both failures was similar: subsurface initiation of numerous cracks with a wide range of orientations and cleavage like features. The cracking was followed by fatigue in one case and catastrophic failure in the other. Hydrogen embrittlement was identified as the most likely mechanism of failure.

Abstract
A few Cr-Mo steel piston rods from different production batches were found identically cracked in the eye end near the radius after chrome plating and baking treatment. Two of them cracked in the plating stage itself instantly broke on slight tapping. Cracking initiated from the outer base surface of the forked eye end. The 40 mm diam forged piston rods were subjected to plating after heavy machining on the part without any stress-relieving treatment. Also, time lapses between plating and baking were varied from 3 to 11 h. The brittle cracking along forked eye-end radius portion was attributed to hydrogen embrittlement that occurred during chrome plating.

A cooler of an ammonia synthesis plant was destroyed after three years of service due to the rupture of a distribution manifold. Synthesis gas under high pressure and at about 300 deg C, consisting of 10% NH3 and unconverted gas of 25% N2 and 75% H2 content, was water-cooled externally to room temperature in this unit. The fracture had the typical flat-gray fibrous structure of a material destroyed by hydrogen. Specimens for the metallographic investigation showed that the structure appeared to have been loosened by intergranular separations. DVM notched impact specimens from the affected area yielded low specific impact energy values. These are the significant characteristics of hydrogen attack. The attack penetrated to a depth of 13 to 16 mm. It was recommended that the manifolds be made of hydrogen-resistant steel instead of the unalloyed steel used.

Abstract
This article provides an overview of the classification of hydrogen damage. Some specific types of the damage are hydrogen embrittlement, hydrogen-induced blistering, cracking from precipitation of internal hydrogen, hydrogen attack, and cracking from hydride formation. The article focuses on the types of hydrogen embrittlement that occur in all the major commercial metal and alloy systems, including stainless steels, nickel-base alloys, aluminum and aluminum alloys, titanium and titanium alloys, copper and copper alloys, and transition and refractory metals. The specific types of hydrogen embrittlement discussed include internal reversible hydrogen embrittlement, hydrogen environment embrittlement, and hydrogen reaction embrittlement. The article describes preservice and early-service fractures of commodity-grade steel components suspected of hydrogen embrittlement. Some prevention strategies for design and manufacturing problem-induced hydrogen embrittlement are also reviewed.

Abstract
Hydrogen damage is a term used to designate a number of processes in metals by which the load-carrying capacity of the metal is reduced due to the presence of hydrogen. This article introduces the general forms of hydrogen damage and provides an overview of the different types of hydrogen damage in all the major commercial alloy systems. It covers the broader topic of hydrogen damage, which can be quite complex and technical in nature. The article focuses on failure analysis where hydrogen embrittlement of a steel component is suspected. It provides practical advice for the failure analysis practitioner or for someone who is contemplating procurement of a cost-effective failure analysis of commodity-grade components suspected of hydrogen embrittlement. Some prevention strategies for design and manufacturing problem-induced hydrogen embrittlement are also provided.

Abstract
Corrosion failure occurred in a titanium clad tubesheet because of a corrosive tube-side gas-liquid mixture leaking through fatigue cracks in the seal welds at tube-to-tubesheet joints. The tubesheet was a carbon steel plate clad with titanium on the tube side face. The seal weld cracks were initiated by cyclic stress imposed by exchanger tubes. The gas-liquid mixture passed through cracks under tube-side pressure, resulting in severe corrosion of the steel backing plate. The failure started with the loosening of the expanded tube-to-tubesheet joints. Loose joints allowed the exchanger tubes to impose load on seal welds and the shell side cooling water entered the crevice between the tubesheet and the tubes. The cooling water in the crevice caused galvanic reaction and embrittlement of seal welds. Brittle crack opening and crack propagation in seal welds occurred due to the cyclic stress imposed by the tubes. The cyclic stress arised from the thermal cycling of the heat exchanger. The possible effects of material properties on the failure of the tubesheet are discussed.

After four months at a temperature of 400 to 5000 C, pipes at a gas generating plant were so heavily eroded they had to be replaced. Three sections of pipe, from different locations, were analyzed to determine whether mechanical wear or corrosion caused the damage. Samples of corrosion product from each pipe section were analyzed for carbon, sulfur, and iron and were found to consist mainly of iron sulfide mixed with soot and rust. The damage resulted from a high content of hydrogen sulfide in the gas (6% CO2, 20% CO, 8 to 12% H2, 0.5 to 1.5% CH4, remainder N2). To process the coal in question, the pipes material should be a heat-resistant steel that contains more chromium and has greater resistance to hydrogen sulfide.

Abstract
During natural gas drilling in the EMS region in 1956, considerable numbers of longitudinal cracks and transverse fractures occurred in the connecting pieces of the bore rods. The connectors were screwed onto the rods by means of a fine thread and tightly joined with it by shrinkage at 530 deg C. The connectors were made of SAE 4140 Cr-Mo steel. The material for the rod pipes was Fe-0.4C-1Mn steel. Structural stresses played a role in the cracking. Iron sulfide formed on the fracture planes and flake-like stress cracks occurred in the steel. The hydrogen sulfide content of the gas was the cause of damage. Hydrogen liberated by reaction with the iron caused the formation of iron sulfide after penetration of the steel, which had an explosive effect during molecular separation under high pressure. This in turn caused the crack formation in conjunction with the external and residual stresses.

Abstract
A riveted 0.25% carbon steel oil-storage tank in Oklahoma was dismantled and reassembled in Minnesota by welding to form a storage tank for soybean oil. An opening was cut in the side of the tank to admit a front-end loader. A frame of heavy angle iron was welded to the tank and drilled for bolting on a heavy steel plate. The tank was filled to a record height. In mid-Jan the temperature dropped to -31 deg C (-23 deg F), with high winds. The tank split open and collapsed. The welding used the shielded metal arc process with E6010 electrodes, which could lead to weld porosity, hydrogen embrittlement, or both. At subzero temperatures, the steel was below its ductile-to-brittle transition temperature. These circumstances suggest a brittle condition. Steps to avoid this type of failure: For cold conditions, the steel plate should have a low carbon content and a high manganese-to-sulfur ratio and be in a normalized condition, low-hydrogen electrodes and welding practices should be used, all corners should be generously radiused, the welds should be inspected and ground or dressed to minimize stress concentrations, postweld heating is advisable, and radiographic and penetrant inspection tests should be performed.

Diode leads that were to be made from OFHC copper were instead made from copper with high oxygen content. The leads had a nickel underplating, a gold final plating, and were brazed to the diode package in a hydrogen atmosphere. After brazing, the leads became embrittled. SEM examination of the fractured leads revealed voids and some oxidized areas surrounded by ductile fracture areas. High pressure steam pockets observed as voids in the microstructure caused hydrogen embrittlement of the leads. The obvious corrective action was to ensure that the lead material was OFHC copper.

Abstract
Severely reduced wall thickness was encountered at the liquid line of a lead-bath pan that was used in a continuous strip or wire oil-tempering unit. Replacement of the pan was necessary after six months of service. The pan, 6.9 m (22.5 ft) long, 0.6 m (2 ft) wide, and 38 cm (15 in.) deep with a 2.5-cm (1-in.) wall thickness, was a type 309 stainless steel weldment. Operating temperatures of the lead bath in the pan ranged from 805 deg C (1480 deg F) at the entry end to 845 deg C (1550 deg F) at the exit end. Analysis (visual inspection. metallographic analysis, moisture testing, and etched micrographs using Murakami's reagent) supported the conclusions that thinning of the pan walls at the surface of the molten lead resulted from using coke of high moisture content and from the low fluctuating coke level. Recommendations included reducing the supply of oxygen attacking the grain boundaries and the hydrogen that readily promoted decarburization with the use of dry (2 to 3% moisture content) coke. Maintaining a thick layer of coke over the entire surface of molten lead in the pan would exclude atmospheric oxygen from the grain boundaries.
