Abstract
A support arm on a front-end loader failed in a brittle manner while lifting a load. The arm had a cross section of 50 x 200 mm (2 x 8 in.). Material used for the arm was hot-rolled ASTM A572, grade 42 (type 1), steel, which exhibited poor impact properties in the as-rolled condition and had a ductile-to-brittle transition temperature exceeding 93 deg C (200 deg F). This transition temperature was much too high for the application. It was recommended that a modified ASTM A572, grade 42 (0.15% C max), type 1 or 2, steel be used (type 1, which contains niobium, may be needed to meet strength requirements). The steel should be specified to be killed, fine-grained, and normalized, with Charpy V-notch impact-energy values of 20 J (15 ft·lbf) at -46 deg C (-50 deg F) in the longitudinal direction and 20 J (15 ft·lbf) at -29 deg C (-20 deg F) in the transverse direction.

Abstract
A case of continual product refinement stimulated by product failures was described. Brittle fracture of gas transmission line pipe steels occurred demonstrating a poor combination of materials, environment, manufacturing and installation problems, and loads. Initial efforts were concentrated towards decreasing the Charpy ductile-to-brittle transition temperature to avoid brittle fracture. It was subsequently revealed that the absorbed energy on the upper shelf of the Charpy energy-temperature curve was critical for arresting a moving crack. Both fracture initiation and fracture propagation were needed be controlled. It was concluded that improved steel processing procedures, chiefly hot-working temperature and deformation control, were also required to optimize microstructure and properties.

Abstract
A 75 cm OD x 33 mm thick pipe in a horizontal section of a hot steam reheat line ruptured after 15 years in service. The failed section was manufactured from rolled plate of material specification SA387, grade C. The longitudinal seam weld was a double butt-weld that was V-welded from both sides and failure was found to propagate along the longitudinal seam and its HAZ. The fracture surface near the inner wall of the pipe was found to have a bluish gray appearance, while the fracture surface near the outer wall was rust colored (oxides). The transverse-to-the-weld specimen from the longitudinal seam weld was revealed to have lower elongation and a shear type failure rather than the cup-cone failures. It was concluded that the welded longitudinal seam exhibited embrittlement. A low-ductility intergranular fracture that progressed through the weld metal was revealed by scanning electron microscopy. The cracks were revealed to be in existence for some time before the final failure which was indicated by the extent and amount of corrosion products. It was concluded that low ductility was responsible for the original initiation of cracks in the pipe.

Abstract
The splined shaft (1040 steel, heat treated to a hardness of 44 to 46 HRC and a tensile strength of approximately 1448 MPa, or 210 ksi) from a front-end loader used in a salt-handling area broke after being in service approximately two weeks while operating at temperatures near -18 deg C (0 deg F). During the summer, similar shafts had a service life of 5 to eight months. Examination of the fracture surface showed brittle fatigue cracks, and visual examination of the splines disclosed heavy chatter marks at the root of the spline, with burrs and tears at the fillet area. Evidence found supports the conclusion that the shaft failed as the result of stress in the sharp fillets and rough surfaces at the root of the splines. Cold weather failure occurred sooner than in hot weather because ductile-to-brittle transition temperature of the 1040 steel shaft was too high. Recommendations include redesign of the fillet radius to a minimum of 1.6 mm (0.06 in.) and a maximum surface finish in the spline area of 0.8 microns. Material for the shafts should be modified to a nickel alloy steel, heat treated to a hardness of 28 to 32 HRC before machining.

Abstract
Pinhole defects were found in a main combustion chamber made from NARloy-Z after an unexpectedly short time in service. Analysis indicated that the throat section of the liner had been exposed to very severe environmental conditions of high temperature and high oxygen content, which caused ductility loss and grain-boundary separation. The excessive oxygen content in the liner was attributed to diffusion from an oxygen-rich environment that had resulted from nonuniform mixing of propellants. The internal oxygen embrittled the alloy and reduced its thermal conductivity, which resulted in a higher hot-gas wall temperature and associated degradation of mechanical properties.

Abstract
The repeated occurrence of random cracks in the fillet radius portion of low-alloy steel (38KhA) end frame forgings following heat treatment was investigated. Microstructural analyses were carried out on both the failed part and disks of the rolled bar from which the part was made. Subsurface cracks were found to be zigzag and discontinuous as well as intergranular in nature. A mixed mode of fracture involving ductile and brittle flat facets was observed. Micropores and rod-shaped manganese sulfide inclusions were also noted. The material had a hydrogen content of 22 ppm, and cracking was attributed to hydrogen embrittlement. Measurement of hydrogen content in the raw material prior to fabrication was recommended. Careful control of acid pickling procedures for descaling of the hot-rolled bars was also deemed necessary.

Abstract
Recurring, premature failures occurred in TiN-coated M2 gear hobs used to produce carbon steel ring gears. Fractographic and metallographic examination, microhardness testing, and chemical analysis by means of EDS revealed that the primary cause of failure was a coarse cellular carbide network, which created a brittle path for fracture to occur longitudinally. As the cellular carbide network must be dispersed and refined during hot working of the original bar of material, the hobs were not salvageable. Minor factors contributing to the hob failures were premature wear resulting from lower matrix hardness and high sulfur content of the material, which contributed to lower ductility through increased nucleation sites. It was recommended that the hob manufacturer specify a minimum amount of required reduction for the original bar of tool steel material, to provide for sufficient homogenization of the carbides in the resultant hob, and lower sulfur content.

Abstract
A pipe in the lateral wall of a boiler powering an aircraft carrier flat-top boat failed during a test at sea. The pipe was made from ASTM 192 steel, an adequate material for the application. Microstructural analysis along with equipment operating records provided valuable insight into what caused the pipe to rupture. Although the pipe had been replaced just 50 h before the accident, the analysis revealed incrustations and corrosion pits on the inner walls and oxidation on the outer walls. Microstructural changes were also observed, indicating that the steel was exposed to high temperatures. The combined effect of pitting, incrustations, and phase transformations caused the pipe to rupture.

The information provided in this article is intended for those individuals who want to determine why a casting component failed to perform its intended purpose. It is also intended to provide insights for potential casting applications so that the likelihood of failure to perform the intended function is decreased. The article addresses factors that may cause failures in castings for each metal type, starting with gray iron and progressing to ductile iron, steel, aluminum, and copper-base alloys. It describes the general root causes of failure attributed to the casting material, production method, and/or design. The article also addresses conditions related to the casting process but not specific to any metal group, including misruns, pour shorts, broken cores, and foundry expertise. The discussion in each casting metal group includes factors concerning defects that can occur specific to the metal group and progress from melting to solidification, casting processing, and finally how the removal of the mold material can affect performance.

This article focuses on the general root causes of failure attributed to the casting process, casting material, and design with examples. The casting processes discussed include gravity die casting, pressure die casting, semisolid casting, squeeze casting, and centrifugal casting. Cast iron, gray cast iron, malleable irons, ductile iron, low-alloy steel castings, austenitic steels, corrosion-resistant castings, and cast aluminum alloys are the materials discussed. This article aims to identify and illustrate the types of overload failures, which are categorized as failures due to insufficient material strength and underdesign, failures due to stress concentration and material defects, and failures due to material alteration. It describes the general aspects of fracture modes and mechanisms. The article briefly reviews some mechanistic aspects of ductile and brittle crack propagation, including discussion on mixed-mode cracking. Factors associated with overload failures are discussed, and, where appropriate, preventive steps for reducing the likelihood of overload fractures are included. The article focuses primarily on the contribution of embrittlement to overload failure. The embrittling phenomena are described and differentiated by their causes, effects, and remedial methods, so that failure characteristics can be directly compared during practical failure investigation. The article describes the effects of mechanical loading on a part in service and provides information on laboratory fracture examination.

Abstract
A number of failures involving carbon and alloy steels were analyzed to assess the effects of inclusions and their influence on mechanical properties. Inclusions, including brittle oxides and more ductile manganese sulfides (MnS), affect fatigue endurance limit, fatigue crack propagation rates, fracture toughness, notch toughness, and transverse tensile properties, and do so in an anisotropic manner with respect to rolling direction. Significant property anisotropy has been documented in the failures investigated, providing evidence that designers failed to account for it. Typical fracture morphologies observed in such cases and metallographic appearances of MnS-containing materials are illustrated.

Abstract
Stainless steel pipe (273-mm OD x 8-mm wall thickness) used in the fabrication of large manifolds developed crack-like decohesions during a routine inductive bending procedure. The imperfections, which were found near the outside diameter, were around 3 mm in length oriented in the circumferential direction and penetrated nearly 2 mm into the pipe wall. The pipes were made of titanium-stabilized austenitic stainless steel X6CrNiMoTi17-12-2. Six hypotheses were considered during the investigation, which ultimately concluded that the failure was caused by liquation cracking due to overheating.

Abstract
A spherical carbon steel fixed-catalyst bed reactor, fabricated from French steel A42C-3S, approximately equivalent to ASTM A201 grade B, failed after 20 years of service while in a standby condition. The unit was found to contain primarily hydrogen at the time of failure. The vessel had a type 304 stainless steel shroud around the catalyst bed as protection against the overheating that was possible if the gas bypassed the bed through the refractory material. The failure was observed to have begun at the toe of the shroud-support ring weld. The ring was found to have a number of small cracks at the root of the weld. The cleavage mode of fracture was confirmed by SEM. The presence of extensive secondary cracking and twinning (Neumann bands) where the fracture followed the line of the shroud-support ring was revealed by metallography. It was revealed by refinery maintenance records that the ring had been removed for hydrotest and welded without any postweld heat treatment. The final cause of failure was concluded to be cracking that developed during the installation of the new shroud ring. Stress-relief heat treatments were recommended to be performed to reduce residual-stress levels after welding.

Abstract
A ring clamp (8740 (AMS 6322), steel forged and cadmium plated) used for attaching ducts to an aircraft engine became loose after three hours of service. When the clamp was removed from the engine, the hinge tabs on one clamp half were found to be broken. Analysis (visual inspection and microscopic and metallographic examination) supported the conclusion that both hinge tabs on the clamp half fractured in a brittle manner as the result of gross overheating, or burning, during forging. The mechanical properties of the metal, especially toughness and ductility, were greatly reduced by burning. Evidence that burning was confined to the hinge end of the clamp indicated that the metal was overheated before or during the upset forging operation. Recommendations included notifying the supplier of the burned condition on the end of the clamp. The clamps should be macroetched before cadmium plating to detect overheating. The clamps in stock should be inspected to ensure that the metal had not been weakened by overheating during the upset forging operation.

A heat-treated, cadmium-plated AISI 8740 steel bolt broke through the head-to-shank fillet while being handled during assembly. Fractographic and metallographic examination of the bolt traced the cause of failure to quench cracking, which occurred when the part was water cooled following hot heading and prior to the production run. Abstract
Galvanized A36 steel unsleeved shear-type anchor bolts failed during installation. The galvanized steel bolts were approximately 18 mm (0.7 in.) in diameter with a 90 deg bend between the long and short legs. As-fractured, sawcut, and unfractured specimens were examined. Failure analysis revealed dark thumbnail regions at the fracture origins and a very narrow and uniform shear lip. The thumbnail region exhibited zinc deposits with no apparent fracture detail, indicating preexisting cracks that had occurred before galvanizing. The balance of the fracture exhibited a transgranular mode with cleavage and ductile, dimpled shear. Hardness values as high as 35 HRC were measured in the bend area. The as-galvanized bolts fractured in a brittle manner. Failure was attributed to improper bending of the bolts, which provided a severely cold-worked bend area susceptible to strain-age embrittlement.

This article describes the general root causes of failure associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be the common sources of failure-inducing defects in bulk working of wrought products. The article discusses the types of imperfections that can be traced to the original ingot product. These include chemical segregation; ingot pipe, porosity, and centerline shrinkage; high hydrogen content; nonmetallic inclusions; unmelted electrodes and shelf; and cracks, laminations, seams, pits, blisters, and scabs. The article provides a discussion on the imperfections found in steel forgings. The problems encountered in sheet metal forming are also discussed. The article concludes with information on the causes of failure in cold formed parts.

Abstract
This article briefly reviews the factors that influence the occurrence of intergranular (IG) fractures. Because the appearance of IG fractures is often very similar, the principal focus is placed on the various metallurgical or environmental factors that cause grain boundaries to become the preferred path of crack growth. The article describes in more detail some typical mechanisms that cause IG fracture. It discusses the causes and effects of IG brittle cracking, dimpled IG fracture, IG fatigue, hydrogen embrittlement, and IG stress-corrosion cracking. The article presents a case history on IG fracture of steam generator tubes, where a lowering of the operating temperature was proposed to reduce failures.

It discusses the mechanisms of IG fractures, including the dimpled IG fracture, the IG brittle fracture, and the IG fatigue fracture.
