Abstract
A plant had manufactured and heat treated their product in house for years. As time went on, the special steel that they had been using became more expensive, and a switch was made to a more common and less highly alloyed material. However, no change in hardness specifications were made, because calculations of ideal critical diameter and analysis of available hardenability data indicated that the original hardness specification could be met. There was, however, less room for process variation. The parts ended up containing temper carbides, developed heavy decarburization, and experienced excessive distortion because they were left in the furnace for extended and varying periods with the temperature “turned down a couple hundred degrees.”

Abstract
The crankshaft of a 37.5-hp, 3-cylinder oil engine was examined. The engine had been dismantled for the purpose of a general overhaul and in the course of this work the crankpins were chromium-plated before regrinding. The engine was returned to service and after running for 290 h the crankshaft broke at the junction of the No. 3 crankpin and the crankweb nearest to the flywheel. A typical fatigue crack had originated at a number of points in the root of the fillet to the web. In its early stages it ran slightly into the web but turned back to the pin when it encountered the oil hole. The shaft had been made from a heat-treated alloy steel. The thickness of the plating was approximately 0.025 in. and numerous cracks were visible in it, several of which had given rise to cracks in the steel below. The primary cause of the crankshaft failure was the plating of the crankpins. The presence of the grooves alone would result in considerable intensification of stress in zones which are normally highly stressed, while the crazy cracking introduced a multiplicity of stress-raisers of a type almost ideal from the point of view of initiating fatigue cracks.

Abstract
During a routine inspection on an aircraft assembly line, an airframe attachment bolt was found to be broken. The bolt was one of 12 that attach the lower outboard longeron to the wing carry-through structure. Failure occurred on the right-hand forward bolt in this longeron splice attachment. The bolt was fabricated from PH13-8Mo stainless steel heat treated to have an ultimate tensile strength of 1517 to 1655 MPa (220 to 240 ksi). A water-soluble coolant was used in drilling the bolt hole where this fastener was inserted. Investigation (visual inspection, 265 SEM images, hardness testing, auger emission spectroscopy and secondary imaging spectroscopy, tensile testing, and chemical analysis) supported the conclusion that failure of the attachment bolt was caused by stress corrosion. The source of the corrosive media was the water-soluble coolant used in boring the bolt holes. Recommendations included inspecting for corrosion all the bolts that were installed using the water-soluble coolant at the spliced joint areas, rinsing all machined bolt holes with a noncorrosive agent, and installing new PH13-8Mo stainless steel bolts with a polysulfide wet sealant.

Abstract
The master connecting rod of a reciprocating aircraft engine revealed cracks during routine inspection. The rods were forged from 4337 (AMS 6412) steel and heat treated to a specified hardness of 36 to 40 HRC. H-shaped cracks in the wall between the knuckle-pin flanges were revealed by visual examination. The cracks were originated as circumferential cracks and then propagated transversely into the bearing-bore wall. No inclusions in the master rod were detected by magnetic-particle and x-ray inspection. Three large inclusions lying approximately parallel to the grain direction and fatigue beach marks around two of the inclusions were revealed by macroscopic examination of the fracture surface. Large nonmetallic inclusions that consisted of heavy concentrations of aluminum oxide (Al2O3) were revealed by microscopic examination of a section through the fracture origin. The forging vendors were notified about the excess size of the nonmetallic inclusions in the master connecting rods and a nondestructive-testing procedure for detection of large nonmetallic inclusions was established.

Abstract
In a continuously cast aluminum press stud, two small foreign metal slivers were found that had caused difficulties with the cable sheathing press. Spectroscopic examination revealed the slivers consisted of a chromium-molybdenum-vanadium steel with minor (unintentional) additions of copper, nickel, and cobalt. A steel of similar composition, X38Cr-MoV5 1 (W-No. 2343) was used for hot working tools. The sliver originated from a damaged press tool.

Abstract
Failure analysis results were employed to identify a better alloy. Chipper knives used in the field to chip logs failed frequently. The knives were made of alloys with a composition of Fe-0.48C-0.30Mn-0.90Si-8.50Cr-1.35Mo-1.20W-0.30V. The development of tougher alloy steel with superior properties was initiated. The nominal composition of Fe-0.50C-0.30Mn-0.40Si-5.00Cr-2.00Mo was developed which achieved the goals of edge retention, resistance to softening under frictional heating, wear resistance, ease of heat treatment, dimensional stability in heat treatment, grindability, and low alloy cost. A chip harvester made from this composition was tested in field with older composition knives. It was found that the new knives outperformed the older knives. The key to the development was interpreted to be careful study of a number of failed knives with different problems used in different types of operations.

Abstract
An air compressor was installed at a chemical plant in which nitric acid was produced by burning ammonia with air. It was a 5000 hp, 5-stage centrifugal machine running at 6000 rpm, compressing air to 5 atm. Failure of the first stage impeller occurred due to a segment from the back plate becoming detached. On the remaining portion, cracks were visible running between the holes for rivets by which the vanes were attached. Metallographic examination of selected sections from the backplate revealed the material to be in the hardened and tempered condition, and the cracking to have initiated on the internal surface of the plate at the crevice between the plate and the vane. It was evident that the impeller failed by stress-corrosion cracking, which initiated in the crevice between the vanes and back plate and propagated through the plate along the line of the rivets where working stresses would be greatest. The compressor intake was situated in the vicinity of nitric acid pumps which had a history of leakage troubles, and which had evidently given rise to the nitrates found on the impeller.

Abstract
The bolt in a bolt and thimble assembly used to connect a wire rope to a crane hanger bracket was worn excessively. Two worn bolts, one new bolt, and a new thimble were examined. Specifications required the bolts to be made of 4140 steel heat treated to a hardness of 277 to 321 HRB. Thimbles were to be made of cast 8625 steel, but no heat treatment or hardness were specified. Analysis (visual inspection, hardness testing, and metallographic examination) supported the conclusion that the wear was due to strikingly difference hardness measurements in the bolt and thimble. Recommendations included hardening and tempering the bolts to the hardness range of 375 to 430 HRB. The thimbles should be heat treated to a similar microstructure and the same hardness range as those of the bolt. Molybdenum disulfide lubricant can be liberally applied during the initial installation of the bolts. A maintenance lubrication program was not suggested, but galling could be reduced by periodic application of a solid lubricant.

Two 25 x 40 mm (1 x 1.5 in.) AISI 4150 hot-rolled steel bars that cracked during heat treatment were examined to determine whether the heat treating procedure had contributed to the failure. Metallographic examination of a cross section taken through the fracture revealed an oxide coating on both sides of the fracture surface. The oxide was also found on the top and bottom sides of the sample. Sawcut sides of the bar did not exhibit the oxide layer The presence of the oxide in the fracture, combined with its absence on all exterior surfaces, indicated that the fracture occurred as a result of an oxide seam in the original material rather than from oxide from heat treating. Nondestructive testing prior to machining and heat treatment was recommended.

A heat-treated, cadmium-plated AISI 8740 steel bolt broke through the head-to-shank fillet while being handled during assembly. Fractographic and metallographic examination of the bolt traced the cause of failure to quench cracking, which occurred when the part was water cooled following hot heading and prior to the production run. The process chart for hot heading was changed from water quenching to air cooling following the forming operation.

Abstract
This article introduces some of the general sources of heat treating problems with particular emphasis on problems caused by the actual heat treating process and the significant thermal and transformation stresses within a heat treated part. It addresses the design and material factors that cause a part to fail during heat treatment. The article discusses the problems associated with heating and furnaces, quenching media, quenching stresses, hardenability, tempering, carburizing, carbonitriding, and nitriding as well as potential stainless steel problems and problems associated with nonferrous heat treatments. The processes involved in cold working of certain ferrous and nonferrous alloys are also covered.

Abstract
Field fatigue failures occurred in a hand-operated gear shift lever mechanism made of 1049 medium carbon steel hardened to 269 to 285 HB. The failures occurred in the 3.18 mm (0.127 in.) radius. Redesign increased the shift lever's diameter to 25 mm (1 in.) and the radius to 4.75 mm (0.187 in.). Also, instead of the as-forged surface, it was expedient to machine the radius. The as-forged surface at 360 MPa (52 ksi) maximum working stress would not ensure satisfactory life because the recalculated maximum stress was 390 MPa (57 ksi). However, the machined surface with a maximum working stress of 475 MPa (69 ksi) gives a safe margin above the 390 MPa (57 ksi) requirement for design stress. Interpreting these values, the forged surface should have a life expectancy of 1,000,000 cycles of stress. However, because the load cycle was somewhat uncertain, the machined radius was chosen to obtain a greater margin of safety. Redesigning eliminated the failures.

Abstract
A forged alloy steel arm of a lifting fork with an approximate cross section of 150 x 240 mm (5.92 x 9.45 in.) fractured after only a short service life on a lift truck. The fracture surface had the appearance of a fracture originating from a surface crack. Analysis (visual inspection, 200x micrographs, chemical analysis, and metallographic examination) supported the conclusion that the primary cause of the failure was the brittleness (lack of impact toughness) of the steel. The coarse bainitic microstructure was inadequate for the service application. The microstructure resulted from either improper heat treatment or no heat treatment after the forging operation. The surface cracks in the lifting-fork arm acted as starter notches (stress raisers), assisting in the initiation of fracture. No recommendations were made.

Abstract
A preflight inspection found a broken diaphragm from a side controller fabricated from 17-7 PH stainless steel in the RH 950 heat treatment condition. Failure occurred by cracking of the base of the flange-like diaphragm. The crack traveled 360 deg around the diaphragm. Scanning electron microscopy (SEM) revealed that the failure occurred by a brittle intergranular mechanism and stress-corrosion cracking (SCC), and indicated a failure mode of selective grain-boundary separation. The diaphragms were heat treated in batches of 25. An improper heat treatment could have resulted in the formation of grain boundary precipitates, including chromium carbides. It was concluded that failure of the diaphragm was due to a combination of sensitization caused by improper heat treatment and subsequent SCC. It was recommended that the remaining 24 sensor diaphragms from the affected batch be removed from service. In addition, a sample from each heat treat batch should be submitted to the Strauss test (ASTM A262, practice E) to determine susceptibility to intergranular corrosion. Also, it was recommended that a stress analysis be performed on the system to determine whether a different heat treatment (which would offer lower strength but higher toughness) could be used for this part.

Abstract
One percent Cr-Mo low alloy constructional steel is widely used for high tensile applications, e.g., for manufacture of high tensile fasteners, heat treated shafts and axles, for automobile applications such as track pins for high duty tracked vehicles etc. The steel is fairly through hardening and heat treatment does not present any serious difficulty. Care is still required in processing to avoid decarburization. In an application of track pins for tracked vehicles, bars about 22 mm diam were required in heat treated and centerless-ground condition prior to induction hardening of the surface. Indifferent results were obtained in induction hardening; cracks were noticed, and patchy hardness figures were obtained on the final product in several batches. Metallographic examination of transverse sections through the defective areas showed decarburization to varying degrees, i.e., from partial to total decarburization. Observations suggested the defects originated at the stages of ingot making and rolling. This was apparently the reason for complete decarburization of the area with original surface defect which opened up further in the oxidizing atmosphere of the furnace with low melting clinkers from scale and furnace lining filling up the crevice of the original defect.

Abstract
Spalled fragments from the work rolls of a steel bar straightening machine were received for failure analysis. Visual inspection coupled with optical and scanning electron microscopy were used as the principal analytical techniques for the investigation. Fractographic analysis revealed the presence of a characteristic fatigue crack propagation pattern (beach marks) and radial chevron marks indicating the occurrence of final overload through a brittle intergranular fracture. The collected evidence suggests that surface-initiated cracks propagated by fatigue led to spalling, resulting in severe work roll damage as well as machine downtime and increased maintenance costs.

Abstract
The splined shaft (1040 steel, heat treated to a hardness of 44 to 46 HRC and a tensile strength of approximately 1448 MPa, or 210 ksi) from a front-end loader used in a salt-handling area broke after being in service approximately two weeks while operating at temperatures near -18 deg C (0 deg F). During the summer, similar shafts had a service life of 5 to eight months. Examination of the fracture surface showed brittle fatigue cracks, and visual examination of the splines disclosed heavy chatter marks at the root of the spline, with burrs and tears at the fillet area. Evidence found supports the conclusion that the shaft failed as the result of stress in the sharp fillets and rough surfaces at the root of the splines. Cold weather failure occurred sooner than in hot weather because ductile-to-brittle transition temperature of the 1040 steel shaft was too high. Recommendations include redesign of the fillet radius to a minimum of 1.6 mm (0.06 in.) and a maximum surface finish in the spline area of 0.8 microns. Material for the shafts should be modified to a nickel alloy steel, heat treated to a hardness of 28 to 32 HRC before machining.
