Abstract
A root cause failure analysis was performed on a vaporizer coil removed from a horizontal forced circulation vaporizer. The carbon steel coil was wound in a right-hand helix with a coil centerline diameter of about 2 m. The vaporizer was gas fired and used Dowtherm A as the heat transfer fluid. Design conditions are based on annular fluid flow to cool the coil wall. NDE, metallographic and fractographic examinations were performed. Numerous, circumferentially oriented, OD initiating cracks were found near the crown for two coils near the non-fired end of the vaporizer. The cracking was confined to the inner diameter of the vaporizer coil at positions from 4:00 to 7:00. The cracking was characterized as transgranular and the fracture surface had beach marks. The failure mechanism was thermal fatigue. The heat transfer calculation predicted that dryout of the coil would occur for coils at the non-fired end of the vaporizer during low flow transients. Dryout results in rapid increase in the tube wall temperature. Thermal cycling of the coil is completed by liquid quenching resulting from resumption of normal flow rates and the return to annular flow. The probable root cause of failure was low flow transient operation.

Abstract
Severe pitting corrosion of a carbon steel tube was observed in the air preheater of a power plant, which runs on rice straw firing. Approximately 1450 tubes were removed from Stage 3 of the preheater (air inlet and flue gas outlet) due to corrosion and local bursting. Samples from Stage 2 (where corrosion was low) and Stage 3 (severe corrosion) were taken and subjected to visual inspection, SEM, x-ray diffraction, microhardness measurement, and chemical and microstructural analysis. It was determined that extended non-operation of the plant resulted in the settlement of corrosive species on the tubes in Stage 3. The complete failure of the tube occurred due to diffusion of these elements into the base metal and precipitation of potassium and chlorine compounds along the grain boundaries, with subsequent dislodging of grains. The nonmetallic inclusions acted as nucleating sites for local pitting bursting. Nonuniform heat transfer in Stage 3 operation accelerated the selective corrosion of front-end tubes. The relatively high heat transfer in this stage resulted in condensation of some corrosive gases and consequent corrosion. Continuous operation of the plant with some precautions during assembly of the tubes reduced the corrosion problem.

Abstract
After some 87,000 h of operation, failure took place in the bend of a steam pipe connecting a coil of the third superheater of a steam generator to the outlet steam collector. The unit operated at 538 deg C and 135 kPa, producing 400 t/h of steam. The 2.25Cr-1Mo steel pipe in which failure took place was 50.8 mm in diam with a nominal wall thickness of 8 mm. It connected to the AISI 321 superheater tube by means of a butt weld and was one of 46 such parallel connecting tubes. The Cr-Mo tubing was situated outside the heat transfer zone of the superheater. The overall sequence of failure involved overheating of the Cr-Mo outlet tubes, heavy oxidation, oxide cracking on thermal cycling, thermal fatigue cracking plus oxidation, creep-controlled crack growth, and rapid plastic deformation and rupture. This failure was indicative of excess temperature of the steam coming from the heat transfer zone of the coil. It showed that many damage mechanisms may combine in the transition from fracture initiation to final failure. The presence of grain boundary sliding as an indication of creep damage was useful in the characterization of the stress level as high and showed that the process of creep was not operative throughout the life of the equipment.

Abstract
Heat exchangers are devices used to transfer thermal energy between two or more fluids, between a solid surface and a fluid, or between a solid particulate and a fluid at different temperatures. This article first addresses the causes of failures in heat exchangers. It then provides a description of heat-transfer surface area, discussing the design of the tubular heat exchanger. Next, the article discusses the processes involved in the examination of failed parts. Finally, it describes the most important types of corrosion, including uniform, galvanic, pitting, stress, and erosion corrosion.

Abstract
The phenomenon of on-load corrosion is directly associated with the production of magnetite on the water-side surface of boiler tubes. On-load corrosion may first be manifested by the sudden, violent rupture of a boiler tube, such failures being found to occur predominantly on the fire-side surface of tubes situated in zones exposed to radiant heat where high rates of heat transfer pertain. In most instances, a large number of adjacent tubes are found to have suffered, the affected zone frequently extending in a horizontal band across the boiler. In some instances, pronounced local attack has taken place at butt welds in water-wall tubes, particularly those situated in zones of high heat flux. To prevent on-load corrosion an adequate flow of water must occur within the tubes in the susceptible regions of a boiler. Corrosion products and suspended matter from the pre-boiler equipment should be prevented from entering the boiler itself. Also, it is good practice to reduce as far as possible the intrusion of weld flash and other impedances to smooth flow within the boiler tubes.

Abstract
Failures in boilers and other equipment taking place in power plants that use steam as the working fluid are discussed in this article. The discussion is mainly concerned with failures in Rankine cycle systems that use fossil fuels as the primary heat source. The general procedure and techniques followed in failure investigation of boilers and related equipment are discussed. The article is framed with an objective to provide systematic information on various damage mechanisms leading to the failure of boiler tubes, headers, and drums, supplemented by representative case studies for a greater understanding of the respective damage mechanism.

Abstract
Schedule 80 low-carbon steel pipes used to transfer kraft liquor in a Kamyr continuous pulp digester failed within 18 months after installation. Visual and metallographic examinations established that the cracking initiated on the internal surfaces of the equalizer pipes in the welds and heat-affected zones (HAZs). Fracture/crack morphology was brittle and primarily intergranular and deposits at crack tips were primarily iron oxides with significant amounts of sodium compounds. On these bases, the cracking was characterized as intergranular stress-corrosion cracking (IGSCC). Corrosion-related deterioration was not found, indicating that the material was generally suitable for the intended service. High residual tensile stresses in the welds and HAZS, resulting from field welding under highly constrained conditions using inadequate weld procedures, were the most probable cause of the failures. Minimizing residual stresses through use of welding procedures that include appropriate preweld and interpass temperatures and postweld stress relief heat treatment at 650 deg C (1200 deg F) was recommended to prevent further failures.

Abstract
The structural collapse of an iron-ore bucket-wheel stacker reclaimer at the beginning of operation was investigated by means of mechanical tests, microstructural characterization, and computational structural analysis. The mechanical failure was a consequence of a brittle fracture by cleavage. The crack followed the heat-affected zone of a welded joint connecting a rectangular hollow section member and a plate flange. The main factors contributing to failure were related with a combination of design-in and manufacturing-in factors like high load-strength ratio at the point of failure, local stress concentration as a result of geometry restrictions, and weld defects. This particular section was responsible for the load transfer between the front tie member and the boom extremity, and its failure was the main cause of the catastrophic failure of the equipment.

Chain link, a part of a mechanism for transferring hot or cold steel blooms into and out of a reheating furnace, broke after approximately four months of service. The link was cast from 2% Cr austenitic manganese steel and was subjected to repeated heating to temperatures of 455 to 595 deg C (850 to 1100 deg F). Examination included visual inspection, macrograph of a nital-etched specimen from an as-received chain link 1.85x, micrographs of a nital-etched specimen from an as-received chain link 100x/600x, normal microstructure of as-cast standard austenitic manganese steel 100x, micrograph of a nital-etched specimen that had been austenitized 20 min at 1095 deg C (2000 deg F) and air cooled 315x, and micrograph of the same specimen after annealing 68 h at 480 deg C (900 deg F) 1000x). Investigation supported the conclusions that the chain link failed in a brittle manner, because the austenitic manganese steel from which it was cast became embrittled after being reheated in the temperature range of 455 to 595 deg C (850 to 1100 deg F) for prolonged periods of time. The alloy was not suitable for this application, because of its metallurgical instability under service conditions.

Abstract
A type 316 stainless steel pipe reducer section failed in service of bleached pulp stock transfer within 2 years in a pulp and paper mill. The reducer section fractured in the heat-affected zone of the flange-to-pipe weld on the flange side. The pipe reducer section consisted of 250 and 200 mm (10 and 8 in.) diam flanges welded to a tapered pipe section. The tapered pipe section was 3.3 mm (0.13 in.) thick type 316 stainless steel sheet, and the flanges were 5 mm (0.2 in.) thick CF8M (type 316) stainless steel castings. Visual and metallographic analysis indicated that the fracture was caused by intergranular corrosion/stress-corrosion cracks that initiated from the external surface of the pipe reducer section. Contributory factors were the sensitized condition of the flange and the concentration of corrosive elements from the bleach stock plant environment on the external surface. In the absence of the sensitized condition of the flange, the service of the pipe reducer section was acceptable. A type 316L stainless steel reducer section was recommended to replace the 316 component because of its superior resistance to sensitization.

Abstract
Following leakage which developed within the furnace of a horizontal multi-tubular type boiler, examination revealed a series of cracks adjacent to the stiffening rings in the first plain furnace ring. The fire-side surface of the sample was coated with a layer of oxide scale. Microscopical examination of sections through the cracks showed them to be filled with oxide and to be of the multi-branched type, having blunt terminations. The general nature of the cracks was characteristic of cracking from thermal or corrosion fatigue, as results from the operation of varying stresses in an oxidizing or corrosive environment. The cracking in this particular case was due principally to the inordinately large gap between the components. Additionally, several of the sealing welds of the tubes to the back tube plate were cracked in a radial manner, and it would appear that in addition, abnormal thermal conditions may well have been experienced intermittently in service.

Abstract
Handles welded to the top cover plate of a chemical-plant downcomer broke at the welds when the handles were used to lift the cover. The handles were fabricated of low-carbon steel rod; the cover was of type 502 stainless steel plate. The attachment welds were made with type 347 stainless steel filler metal to form a fillet between the handle and the cover. The structure was found to contain a zone of brittle martensite in the portion of the weld adjacent to the low-carbon steel handle; fracture had occurred in this zone. The brittle martensite layer in the weld was the result of using too large a welding rod and too much heat input, melting of the low-carbon steel handle, which diluted the austenitic stainless steel filler metal and formed martensitic steel in the weld zone. Because it was impractical to preheat and postheat the type 502 stainless steel cover plate, the low-carbon steel handle was welded to low-carbon steel plate, using low-carbon steel electrodes. This plate was then welded to the type 502 stainless steel plate with type 310 stainless steel electrodes. This design produced a large weld section over which the load was distributed.

Abstract
An Incoloy 800H (UNS N08810) transfer line on the outlet of an ethane-cracking furnace failed during decoking of the furnace tubes after nine years in service. A metallographic examination using optical and scanning electron microscopy as well as energy-dispersive x-ray spectroscopy revealed that the failure was due to sulfidation. The source of the sulfur in the furnace effluent was either dimethyl disulfide, injected into the furnace feed to prevent coke formation and carburization of the furnace tubes, or contamination of the feed with sulfur bearing oil.

Abstract
A back wall riser tube in a high pressure boiler failed, interrupting operations in a cogeneration plant. The failure occurred in a tube facing the furnace, causing eight ruptured openings over a 1.8 m section. The investigation consisted of an on-site visual inspection, nondestructive testing, energy dispersive x-ray analysis, and inductively coupled plasma mass spectrometry. The tube was made from SA 210A1 carbon steel that had been compromised by wall thinning and the accumulation of fire and water-side scale deposits. Investigators determined that the tube failed due to prolonged caustic attack that led to ruptures in areas of high stress. The escaping steam eroded the outer surface of the tube causing heavy loss of metal around the rupture points.

Abstract
A gas-fired, ASTM A-106 Grade B carbon steel vaporizer failed on three different occasions during attempts to bring the vaporizer on line. Dye penetrant examination indicated the presence of multiple packets of ductile cracks on the inside of the coil radius at the bottom of the horizontal axis coils. Visual examination of the inside of the tubing indicated the presence of a carbonaceous deposit resulting from decomposition of the heat-exchanging fluid. Subsequent metallographic examination and microhardness testing indicated that the steel was heated to a temperature above the allowable operating temperature for the fluid. The probable cause for failure is thermal fatigue due to the localized overheating. Flow conditions inside the tubing should be reexamined to ensure suitable conditions for annular fluid flow.

Abstract
A 150 mm (6 in.) diam, 1.6 mm (0.065 in.) thick alloy 800 1iner from an internal bypass line in a hydrogen reformer was removed from a waste heat boiler because of severe metal loss. Visual and metallographic examinations of the liner indicated severe metal wastage on the inner surface, along with sooty residue. Patterns similar to those associated with erosion/corrosion damage were observed. Microstructural examination of wasted areas revealed a bulk matrix composed of massive carbides, indicating that gross carburization and metal dusting had occurred. X-ray diffraction analysis showed that the carbides were primarily chromium based (Cr 23 C 7 and Cr 7 C 3 ). The sooty substance was identified as graphite. Wasted areas were ferromagnetic and the degree of ferromagnetism was directly related to the degree of wastage. Three actions were recommended: (1) inspection of the waste heat boiler to determine the extent of metal damage in other areas by measuring the degree of ferromagnetism, (2) replacement of metal determined to be magnetic, and (3) closer monitoring of temperatures in the region of the reformer furnace outlet.

Abstract
The presence of secondary, branching intergranular stress-corrosion cracking in a type 440C stainless bearing caused the analyst to overlook the real culprit, which was a mechanically-initiated, primary transgranular crack that propagated through the steel's hard chromium carbide. Failure was actually caused by overload. Had the original conclusion been accepted, a relatively exotic alloy would have been specified. In another case, brass heat exchanger tube failure was automatically attributed to attack by an acidic cleaner, and a decision was made to stop using the solution. A more thorough analysis showed failure was caused by tube vibration. In a third case, a type 304 stainless steel bellows in a test loop was thought to have failed because of chloride stress corrosion. The report concluded with a recommendation that carbon steel be used as an alternative bellows material. Caustic, not chloride, stress corrosion was the culprit. Had material substitutions been made on the original premise of countering chloride stress corrosion, most of the loop's highly stressed components would have eventually failed.

Abstract
Equipment in which an assembly of in-line cylindrical components rotated in water at 1040 rpm displayed excessive vibration after less than one hour of operation. The malfunction was traced to an aluminum alloy 6061-T6 combustion chamber that was part of the rotating assembly. Analysis (visual inspection, 100x/500x/800x micrographic examination, spectrographic analysis, and hardness testing) supported the conclusions that, as a result of improper heat treatment, the combustion-chamber material was too soft for successful use in this application. Misalignment of the combustion chamber and one or both of the mating parts resulted in eccentric rotation and the excessive vibration that caused malfunction of the assembly. Irregularities in the housing around the combustion chamber and temperature variation relating to the combustion pattern in the chamber were considered to be possible contributing factors to localization of the cavitation erosion. Recommendations included adopting inspection procedures to ensure that the specified properties of aluminum alloy 6061-T6 were obtained and that the combustion chamber and adjacent components were aligned within specified tolerances. In a similar situation, consideration should also be given to raising the pressure in the coolant in order to suppress the formation of cavitation bubbles.

Abstract
Alloy 430 stainless steel tube-to-header welds failed in a heat recovery steam generator (HRSG) within one year of commissioning. The HRSG was in a combined cycle, gas-fired, combustion turbine electric power plant. Alloy 430, a 17% Cr ferritic stainless steel, was selected because of its resistance to chloride and sulfuric acid dewpoint corrosion under conditions potentially present in the HRSG low-pressure feedwater economizer. Intergranular corrosion and cracking were found in the weld metal and heat-affected zones. The hardness in these regions was up to 35 HRC, and the weld had received a postweld heat treatment (PWHT). Metallographic examination revealed that the corroded areas contained undertempered martensite. Fully tempered weld areas with a hardness of 93 HRB were not attacked. No evidence of corrosion fatigue was found. Uneven temperature control during PWHT was the most likely cause of failure.
