Abstract
Milling machine arbors were inserted with satellite spindles having a maximum speed of 1500 rpm, and broke out between the groove and the flange. The appearance of the fracture surface was the same on both arbors. The pronounced scan lines characterized the fractures as fatigue fractures. The appearance of the fracture in the arbors indicated ductile fatigue fracture which had its origin in the radii between groove and flange. These radii of 0.15 and 0.2 mm were too small for the load on the milling machine. In addition there were grooves at the base of the radii which had an unfavorable effect on the life of the component by acting as notches with their resulting stress concentration. Considering the great hardness of the case, the small radii would have been critical even without grooves. Measures were taken so that the critical radius of the milling machine was increased and the surface roughness measured more precisely.

Abstract
A 4340 steel shaft, the driving member of a large rotor subject to cyclic loading and frequent overloads, broke after three weeks of operation. The driving shaft contained a shear groove at which the shaft should break if a sudden high overload occurred, thus preventing damage to an expensive gear mechanism. The rotor was subjected to severe chatter, which was an abnormal condition resulting from a series of continuous small overloads occurring at a frequency of around three per second. Investigation (visual inspection, hardness testing, and hot acid etch images) supported the conclusion that the basic failure mechanism was fracture by torsional fatigue, which started at numerous surface shear cracks, both longitudinal and transverse, that developed in the periphery of the root of the shear groove. These shear cracks resulted from high peak loads caused by chatter. The shear groove in the shaft had performed its function, but at a lower overload level than intended. Recommendations included increasing the fatigue strength of the shaft by shot peening the shear groove to minimize chatter.

Abstract
Cast iron bearing caps in tractor engines fractured repeatedly after only short operating periods. The fracture originated in a cast-in groove and ran approximately radially to the shaft axis. The smallest cross section was at the point of fracture. The core structure of the caps consisted of graphite in pearlitic-ferritic matrix. Casting stresses did not play a decisive role because of the simple shape of the pieces that were without substantial cross sectional variations. Two factors exerted an unfavorable effect in addition to comparatively low strength. First, the operating stress was raised locally by the sharp-edged groove, and second, the fracture resistance of the cast iron was lowered at this critical point by the existence of a ferritic bright border. To avoid such damage in the future it was recommended to observe one or more of the following precautions: 1) Eliminate the grooves; 2) Remove the ferritic bright border; 3) Avoid undercooling in the mold and therefore the formation of granular graphite; 4) Inoculate with finely powdered ferrosilicon into the melt for the same purpose; and, 5) Anneal at lower temperature or eliminate subsequent treatment in consideration of the uncomplicated shape of the castings.

Abstract
A 150 cm ID boiler drum made form ASTM A515, grade 70, steel failed during final hydrotesting at a pressure of approximately 26 MPa. Brittle fractures were revealed in between two SA-106C nozzles and remainder was found to involve tearing. Short, flat segments of fracture area, indicative of pre-existing cracks, were revealed by examination of the fracture surface at the drain grooves arc gouged at the nozzle sites. A thin layer of material with a dendritic structure was observed at the groove surface. The dendritic layer was revealed by qualitative microprobe analysis to contain over 1% C, higher than the carbon content of the base metal. The cracks in the drain groove surface could have occurred after arc gouging, during subsequent stress-relieving, or during the hydrostatic test. Flame cutting is not recommended for the type of steel used in the boiler drum because it can lead to local embrittlement and stress raisers, potentially initiating major failures.

Abstract
An 8 in. diam stainless steel black liquor feed pipe to a carbon steel digester had failed within one year of service. The material was type 316 molybdenum-containing austenitic stainless steel. The service environment was alkaline black liquor at 175 deg C (350 deg F). The pipe had developed cracks on the inside surface coincident with an external support gusset. The cracks initiated at wide corrosion grooves. The early stages were corrosion-assisted fatigue cracks. The cracks initiated at the corrosion grooves and propagated as transgranular SCC with characteristic branching. Evaluation indicated the cracks were localized in an area of high cyclic stresses as a consequence of geometrical constraints on the piping and unsupported cantilever loads. No cracks were found elsewhere in the pipe. In the absence of highly localized service stresses (exceeding yield strength of the material), the corrosion grooving and subsequent SCC would not have occurred in this service environment. The pipe support system was modified with additional gussets to reduce the magnitude of cyclic stresses at the critical areas. The modification was apparently successful.

Abstract
The main shaft in a locomotive turbocharger fractured along with an associated bearing sleeve. Visual and fractographic examination revealed that the shaft fractured at a sharp-edged groove between two journals of different cross-sectional area. The dominant failure mechanism was low-cycle rotation-bending fatigue. The bearing sleeve failed as a result of abrasive and adhesive wear. Detailed metallurgical analysis indicated that the sleeve and its respective journal had been subjected to abnormally high temperatures, increasing the amount of friction between the sleeve, bearing bush, and journal surface. The excessive heat also softened the induction-hardened case on the journal surface, decreasing its fatigue strength. Fatigue crack initiation occurred at the root fillet of the groove because of stress concentration.

Abstract
Two hot water reheat coil valves from a heating/ventilating/air-conditioning system failed in service. The values, a 353 copper alloy 19 mm (3/4 in.) valve and a 360 copper alloy 13 mm (1/2 in.) valve, had been failing at an increasing rate. The failures were confined to the stems and seats. Visual examination revealed severe localized metal loss in the form of deep grooves with smooth and wavy surfaces. Metallographic analysis of the grooved areas revealed uniform metal loss. No evidence of intergranular or selective attack indicating erosion-corrosion was observed, Recommendations included use of a higher-copper brass, cupronickel, or Monel for the valve seats and stems and operation of the valves in either the fully opened or closed position.

Abstract
The engine on a jet aircraft was shut down immediately as it produced excessive vibration. Complete failure of the cage in one of the two main-shaft ball bearings (placed side by side in the engine) was revealed in the dismantled engine. The ball bearings (made of vacuum-melted 52100 steel) were both of the single-row deep-groove type with split inner rings and were designed to operate at a maximum temperature of 175 deg C. Overtempering of the rings was indicated by the reduced hardness in comparison to unfailed rings. Severe damage to approximately 20% of the load-bearing surface, with more damage on one of the shoulders of the groove, was revealed during examination of the outer raceway of the bearing which indicated misalignment of the bearing. No damage other than spalling cavities in the inner-ring raceway, caused by the elongated subsurface inclusion revealed by metallographic examination of circumferential section of the largest cavity, was exhibited by the second bearing. It was concluded that the fracture of the cage was caused by overheating and misalignment caused excessive stressing of the bearing on the main shaft.

Abstract
A 3 in. diam shaft was found to have suffered excessive wear on one of the journals and was built up by welding. While it was in the lathe prior to turning down the built-up region, a crack was discovered in the root of the oil-seal groove and subsequently the end of the shaft was broken off with hammer blows. The fracture surface was duplex in nature, there being an annular region surrounding a central zone, which suggests that the fracture developed in two stages. Microscopic examination confirmed that the fracture was of the brittle type. The shaft material showed a microstructure typical of a medium-carbon steel (carbon approximately 0.4%) in the normalized condition, a material not weldable by ordinary methods. It was concluded that the post-welding crack arose primarily from the thermal contraction which developed in the weld metal on cooling. It is probable that if the built-up zone had extended beyond the oil seal groove, failure in the manner would not have occurred. Experience indicated however, that failure from fatigue cracking would still have been likely to occur.

Abstract
Soft-soldered copper pipe joints used in refrigerating plants failed. The solder had not adhered uniformly to the pipe surface. In addition, there were some longitudinal grooves on the pipe surfaces, parts of which were not filled with solder. The unsoldered areas formed cavities within the joints, some of which had been in direct communication with the outsides via the grooves or interconnected cavities. On cooling, moisture condensed on the external surfaces. Some of this was drawn by capillary action into the cavities in open communication with the external surface. On continued cooling to below freezing-point, water that entered the cavities solidified. This was accompanied by a slight increase in volume, which collapsed the pipe walls. In the examples, the pipe ends had not been properly tinned. The solder used was found to be of the tin-antimony type, containing about 5% antimony, which is more difficult to use than the usual tin-lead alloys. The use of this particular type of solder was a contributory factor in the production of unsound joints in the samples examined.
