Abstract
A 230 mm (9 in.) diameter disk attrition mill was scheduled to grind 6.35 mm (0.25 in.) diameter quartz particles to a 0.075 mm (0.003 in.) diameter powder. Due to severe wear on the grinding plates, however, the unit was unable to complete the task of grinding the rock. The mill consisted of a heavy gray cast iron frame, a gravity feeder port, a runner, and a heavy-duty motor. The frame and gravity feeder weighed over 200 kg (440 lb) and, in some areas, was over 25 mm (1 in.) thick. To obtain the operating speed of 200 rpm, a gear system was used to transmit the torque from the 2-hp motor. The runner consisted of a 50 mm (2 in.) diameter shaft and two gray cast iron grinding plates. Investigation (visual inspection, historical review, photographs, model testing of new plates, chemical analysis, hardness testing, optical macrographs, and optical micrographs) supported the conclusion that the primary feed material was harder than the grinding plates, causing wear and eventual failure. Recommendations included reducing the clearance between the flutes and possible material changes.

Abstract
The 8620 steel latch tip, carburized and then induction hardened to a minimum surface hardness of 62 HRC, on the main-clutch stop arm on a business machine fractured during normal operation when the latch tip was subjected to intermittent impact loading. Fractographic examination 9x showed a brittle appearance at the fractures. Micrograph examination of an etched section disclosed several small cracks. Fracture of the parts may have occurred through similar cracks. Also observed was a burned layer approximately 0.075 mm (0.003 in.) deep on the latch surface, and hardness at a depth of 0.025 mm (0.001 in.) in this layer was 52 HRC (a minimum of 55 HRC was specified). Thus, the failure was caused by brittle fracture in the hardness-transition zone as the result of excessive impact loading. The burned layer indicated that the cracks had been caused by improper grinding after hardening. Abstract
The spindle of a helicopter-rotor blade fractured after 7383 h of flight service. At every overhaul (the spindle that failed was overhauled six times and reworked twice), any spindle that showed wear was reworked by grinding the shank to 0.1 mm (0.004 in.) under the finished diam. The spindle was then shot peened with S170 shot to an Almen intensity of 0.010 to 0.012 A. Following shot peening, the shank was nickel sulfamate plated to 0.05 mm (0.002 in.) over the finished diam, ground to finished size, and cadmium plated. Visual and stereomicroscopic exam showed faint grinding marks and circumferential grooves on the surface near the fillet at the junction of the shank and fork, which should have been peened over and covered with peening dimples. Evidence found supports the conclusions that the spindle failed in fatigue that originated near the junction of the shank and fork. The nonuniformity of the shot-peened effect on the shank and fillet portions of the spindle resulted from incomplete peeing. The fracture was of the low-stress high-cycle type, initiated by stresses well below the gross yield strength and propagated by thousands of load cycles. No recommendations were made.

Abstract
Examination of several fighter aircraft main landing gear legs revealed unusual cracking in the hard chromium plating that covered the sliding section of the inner strut. The cracking was associated with cracks in the 35 NCD 16 steel beneath the plating. A detailed investigation revealed that the cracking was caused by the combination of incorrect grinding procedure, the presence of hydrogen, and fatigue. The grinding damage generated tensile stresses in the steel, which caused intergranular cracking during the plating cycle. The intergranular cracks were initiation sites for fatigue crack growth during service. It was recommended that the damaged undercarriage struts be withdrawn from service pending further analysis and development of a repair technique.

Abstract
The cause of low fatigue life measurements obtained during routine fatigue testing of IMI 550 titanium alloy compressor blades used in the first stage of the high-pressure compressor of an aeroengine was investigated. The origin of the fatigue cracks was associated with a spherical bead of metal sticking to the blade surface in each case. Scanning electron microscope revealed that the cracks initiated at the point of contact of the bead with the blade surface. Energy-dispersive X-ray analysis indicated that the bead composition was the same as that of the blade. Detailed investigation revealed that fused material from the blade had been thrown onto the cold blade surface during a grinding operation to remove the targeting bosses from the forgings, thereby causing local embrittlement. It was recommended that extreme care be taken during grinding operations to prevent the hot, fused particles from striking the blade surface.

Abstract
The connecting end of two forged medium-carbon steel rods used in an application in which they were subjected to severe low-frequency loading failed in service. The fractures extended completely through the connecting end. The surface hardness of the rods was found to be lower than specifications. The fractures were revealed to be in areas of the transition regions that had been rough ground to remove flash along the parting line. The presence of beach marks, indicating fatigue failure, was revealed by examination. The fracture origin was confirmed by the location and curvature of beach marks to be the rough ground surface. An incipient crack 9.5 mm along with several other cracks on one of the fractured rods was revealed by liquid penetration examination. Metallographic examination of the fractured rods indicated a banded structure consisting of zones of ferrite and pearlite. It was established that the incipient cracks found in liquid-penetrant inspection had originated at the surface in the banded region, in areas of ferrite where this constituent had been visibly deformed by grinding. Closer control on the microstructure, hardness of the forgings and smooth finish in critical area was recommended.

Abstract
A carbon steel (ASTM A515 grade 70) pressure vessel failed by brittle fracture while being hydro tested in the fabricating shop. The fracture origin was a small crack at a welding arc strike associated with the toe of a nozzle weld. A fracture mechanics calculation indicated that this imperfection, although small, initiated fracture because of the local geometry and stress conditions and the low toughness of the steel. It was recommended that (1) the probability of flaws be reduced by welding over or grinding out arc strikes, (2) the local stresses be lowered by post weld stress relief and improved weld toe geometry, and (3) toughness be improved by specifying fine-grain steel and/ or by normalizing.

This article describes the six fundamental factors that decide a tool's performance. These are mechanical design, grade of tool steel, machining procedure, heat treatment, grinding, and handling. A deficiency in any one of the factors can lead to a tool and die failure. The article presents a seven-step procedure to be followed when looking for the reason for a failure. A review of the results of the seven-point investigation may lead directly to the source of failure or narrow the field of investigation to permit the use of special tests.

Abstract
Presence of transverse marks which were remnant of grinding was indicated in a failed valve spring made from ground rod. The shot-peening pattern was light at this location. A transverse crack was found to grow from one such mark under the influence of local stress fields until it was reoriented to the plane normal to the major tensile axis by sufficient loading. The shot-peening procedure was altered to create adequate surface compression at all stressed points on the springs.

Abstract
Metallographic examination is one of the most important procedures used by metallurgists in failure analysis. Typically, the light microscope (LM) is used to assess the nature of the material microstructure and its influence on the failure mechanism. Microstructural examination can be performed with the scanning electron microscope (SEM) over the same magnification range as the LM, but examination with the latter is more efficient. This article describes the major operations in the preparation of metallographic specimens, namely sectioning, mounting, grinding, polishing, and etching. The influence of microstructures on the failure of a material is discussed and examples of such work are given to illustrate the value of light microscopy. In addition, information on heat-treatment-related failures, fabrication-/machining-related failures, and service failures is provided, with examples created using light microscopy.

Abstract
Failure analysis was performed on a fractured impeller from a boiler feed pump of a fossil fuel power plant. The impeller was a 12% Cr martensitic stainless steel casting. The failure occurred near the outside diameter of the shroud in the vicinity of a section change at the shroud/vane junction. Sections cut from the impeller were examined visually and by SEM fractography. Microstructural, chemical, and surface analyses and surface hardness tests were conducted on the impeller segments. The results indicated that the impeller failed in fatigue with casting defects increasing stress and initiating fracture. In addition, the composition and hardness of the impeller did not meet specifications. Revision of the casting process and institution of quality assurance methods were recommended.

Abstract
Field metallography and replication were performed on a type 316 stainless steel column in diglycol amine vacuum service to determine the cause of visible OD pitting on the column in several areas above the insulation support rings. The examination revealed transgranular stress-corrosion cracking beneath the pitted areas on the OD. The likely cause of the cracking was chloride stress corrosion, with chlorides deriving from the marine atmosphere and concentrating under the insulation around the support rings. A complete insulation evaluation, including repair or replacement, was recommended to prevent chloride buildup. Painting of the steel surface with an epoxy-phenolic or epoxy-coal tar was also suggested.

Abstract
Upon arrival at the erection site, an AISI type 316L stainless steel tank intended for storage of fast breeder test reactor coolant (liquid sodium) exhibited cracks on its shell at two of four shell/nozzle fillet-welded joint regions. The tank had been transported from the manufacturer to the erection site by road, a distance of about 800 km (500 mi). During transport, the nozzles were kept at an angle of 45 deg to the vertical because of low clearance heights in road tunnels. The two damaged joints were unsupported at their ends inside the vessel, unlike the two uncracked nozzles. Surface examination showed ratchet marks at the edges of the fracture surface, indicating that loading was of the rotating bending type. Electron fractography using the two-stage replica method revealed striation marks characteristic of fatigue fracture. The striations indicated that the cracks had advanced on many “mini-fronts,” also indicative of nonuniform loading such as rotating bending. It was recommended that a support be added at the inside end of the nozzles to rigidly connect with the shell. In addition to avoiding transport problems, this design modification would reduce fatigue loading that occurs in service due to vibration of the nozzles during filling and draining of the tank.

Abstract
A femoral knee implant was returned to the casting vendor for analysis after exhibiting poor bond strength between the cast substrate and a sintered porous coating. Both the coating and the substrate were manufactured from a cobalt-chromium-molybdenum alloy. Metallographic analysis indicated that a decarburized layer existed on all surfaces of the casting, which prevented bonding during the sintering thermal cycle. Bead-to-bead bonding within the coating appeared sufficient, and no decarburized layer was present on the bead surfaces. It was concluded that the decarburization did not occur during the sintering thermal cycle. It was recommended that the prosthetic manufacturer investigate atmosphere controls for all thermal cycles prior to coating.

This article describes the characteristics of tools and dies and the causes of their failures. It discusses the failure mechanisms in tool and die materials that are important to nearly all manufacturing processes, but is primarily devoted to failures of tool steels used in cold-working and hot-working applications. It reviews problems introduced during mechanical design, materials selection, machining, heat treating, finish grinding, and tool and die operation. The brittle fracture of rehardened high-speed steels is also considered. Finally, failures due to seams or laps, unconsolidated interiors, and carbide segregation and poor carbide morphology are reviewed with illustrations.

This article discusses failure mechanisms in tool and die materials that are very important to nearly all manufacturing processes. It is primarily devoted to failures of tool steels used in cold working and hot working applications. The processes involved in the analysis of tool and die failures are also covered. In addition, the article focuses on a number of factors that are responsible for tool and die failures, including mechanical design, grade selection, steel quality, machining processes, heat treatment operation, and tool and die setup.

Abstract
The dished ends of a heavy water/helium storage tank manufactured from 8 mm (0.3 in.) thick type 304 stainless plate leaked during hydrotesting. Repeated attempts at repair welding did not alleviate the problem. Examination of samples from one dished end revealed that the cracking was confined to the heat affected zone (HAZ) surrounding circumferential welds and, to a lesser extent, radial welds that were part of the original construction. Most of the cracks initiated and propagated from the inside surface of the dished ends. Microstructures of the base metal, HAZ, and weld metal indicated severe sensitization in the HAZ due to high heat input during welding. An intergranular corrosion test confirmed the observations. The severe sensitization was coupled with residual stresses and exposure of the assembly to a coastal atmosphere during storage prior to installation. This combination of factors resulted in failure by stress-corrosion cracking. Implementation of a new repair procedure was recommended. Repairs were successfully made using the new procedure, and all cracks in the weld repair zones were eliminated.

This article provides an overview of the effects of various material- and process-related parameters on residual stress, distortion control, cracking, and microstructure/property relationships as they relate to various types of failure. It discusses phase transformations that occur during heat treating and describes the metallurgical sources of stress and distortion during heating and cooling. The article summarizes the effect of materials and the quench-process design on distortion and cracking and details the effect of cooling characteristics on residual stress and distortion. It also provides information on the methods of minimizing distortion and tempering. The article concludes with a discussion on the effect of heat treatment processes on microstructure/property-related failures.
