Abstract
Waterwall tube failure samples removed from a coal- and oil-fired boiler in service for 12 years exhibited localized underdeposit corrosion and hydrogen damage. EDS and XRD revealed that bulk internal deposits collected from the tubes contained metallic copper which can accelerate corrosion through galvanic effects and can promote hydrogen damage. Ultrasonic testing was recommended to locate tubes with severe gouging and corrosion, which are suspect locations for hydrogen damage. The source of the copper should be identified and future chemical cleaning of the boiler should address its presence in the waterwall tubes.

Abstract
Severe pitting was found on the internal surfaces of SA-210 Grade C waterwall tubing of a coal-fired boiler at a cogeneration facility. Metallographic examination showed the pits to be elliptical, having an undercut morphology with supersurface extensions,. a type of pitting characteristic of acidic attack. Energy-dispersive X-ray spectroscope revealed the presence of chlorine in the pit deposits, indicating that the pitting was promoted by underdeposit chloride attack. The presence of copper in deposits on the internal surface of the tubing may have acted as a secondary factor. Acidic conditions may have formed during a low-pH excursion that reportedly occurred several years prior. To prevent future failures, severely damaged tubing must be replaced. Internal deposit buildup must be removed by chemical cleaning to prevent further pitting. Water quality needs continued monitoring and maintenance to ensure that another low-pH excursion does not occur.

Abstract
Two identical “D” tube package boilers, installed at separate plants, experienced a number of tube ruptures after relatively short operating times. The tubes, which are joined by membranes, experienced localized bulging and circumferential cracking along the fireside crown as a result of overheating and thermal fatigue. It was recommended that recent alterations to the steam-drum baffling be remodified to improve circulation in the boiler and prevent further overheating. Several thermocouples were attached to tubes in problem areas of the boiler to monitor the effects of the steam-drum modifications on tube wall temperatures.

Abstract
Plate perforation occurred in the cylindrical section and walls of the inlet foot (2.38 mm thick Incoloy 825 plate welded using INCO welding rod 135) of an inert gas fire prevention system in an oil tanker. Cross-sectional microprobe analysis showed the corrosion product to contain sulfur, mainly from the flue gas, and calcium and chlorine, mainly from the sea water. The gray corrosion product was interspersed with rust and a black carbonaceous deposit. Corrosion pitting and poor weld penetration, with carbide precipitation and heavy etching at grain boundaries, indicated sensitization and susceptibility to aqueous intergranular corrosion. Chemical analysis showed the predominant acid radical to be sulfate (6.20% in the carbonaceous deposit and 0.60% in the corrosion product), suggesting that oxidation of SO2 in the flue gas caused the corrosion. Moisture condensation, the carbon acting as a cathode, and alloy susceptibility to intergranular corrosion contributed to the corrosion.

Abstract
Three 1006 carbon steels team/water separators failed in a boiler in installation after several years of service. Annual inspection had revealed no evidence of deterioration until the last inspection, when they were removed from service. Metallurgical investigation determined that the separators had deteriorated because of erosion corrosion. Further analysis of the boiler operation revealed that operational changes made in the last year of service caused an increase in velocity of the water/steam mixture. It was recommended that the operating parameters for the boiler be reevaluated and prior levels of operation be reinstituted.

Abstract
Failures in boilers and other equipment taking place in power plants that use steam as the working fluid are discussed in this article. The discussion is mainly concerned with failures in Rankine cycle systems that use fossil fuels as the primary heat source. The general procedure and techniques followed in failure investigation of boilers and related equipment are discussed. The article is framed with an objective to provide systematic information on various damage mechanisms leading to the failure of boiler tubes, headers, and drums, supplemented by representative case studies for a greater understanding of the respective damage mechanism.

Abstract
The phenomenon of on-load corrosion is directly associated with the production of magnetite on the water-side surface of boiler tubes. On-load corrosion may first be manifested by the sudden, violent rupture of a boiler tube, such failures being found to occur predominantly on the fire-side surface of tubes situated in zones exposed to radiant heat where high rates of heat transfer pertain. In most instances, a large number of adjacent tubes are found to have suffered, the affected zone frequently extending in a horizontal band across the boiler. In some instances, pronounced local attack has taken place at butt welds in water-wall tubes, particularly those situated in zones of high heat flux. To prevent on-load corrosion an adequate flow of water must occur within the tubes in the susceptible regions of a boiler. Corrosion products and suspended matter from the pre-boiler equipment should be prevented from entering the boiler itself. Also, it is good practice to reduce as far as possible the intrusion of weld flash and other impedances to smooth flow within the boiler tubes.

Abstract
This article explains the main types and characteristic causes of failures in boilers and other equipment in stationary and marine power plants that use steam as the working fluid with examples. It focuses on the distinctive features of each type that enable the failure analyst to determine the cause and suggest corrective action. The causes of failures include tube rupture, corrosion or scaling, fatigue, erosion, and stress-corrosion cracking. The article also describes the procedures for conducting a failure analysis.

The boiler was more than 100 years old, but still performed satisfactorily when fired up at regular intervals. Chemical analysis showed the steel to be a low-carbon EN2 type. Microstructural examination indicated brittle fracture caused by strain aging. Replacement of all studs on the man hole cover and examination of the boiler steel in other places were recommended.

Abstract
A back wall riser tube in a high pressure boiler failed, interrupting operations in a cogeneration plant. The failure occurred in a tube facing the furnace, causing eight ruptured openings over a 1.8 m section. The investigation consisted of an on-site visual inspection, nondestructive testing, energy dispersive x-ray analysis, and inductively coupled plasma mass spectrometry. The tube was made from SA 210A1 carbon steel that had been compromised by wall thinning and the accumulation of fire and water-side scale deposits. Investigators determined that the tube failed due to prolonged caustic attack that led to ruptures in areas of high stress. The escaping steam eroded the outer surface of the tube causing heavy loss of metal around the rupture points.

Following leakage which developed within the furnace of a horizontal multi-tubular type boiler, examination revealed a series of cracks adjacent to the stiffening rings in the first plain furnace ring. The fire-side surface of the sample was coated with a layer of oxide scale. Microscopical examination of sections through the cracks showed them to be filled with oxide and to be of the multi-branched type, having blunt terminations. The general nature of the cracks was characteristic of cracking from thermal or corrosion fatigue, as results from the operation of varying stresses in an oxidizing or corrosive environment. The cracking in this particular case was due principally to the inordinately large gap between the components. Additionally, several of the sealing welds of the tubes to the back tube plate were cracked in a radial manner, and it would appear that in addition, abnormal thermal conditions may well have been experienced intermittently in service.

Within the first few months of operation of an 8 km (5 mile) long 455 mm (18 in.) diam high-pressure steam line between a coal-fired electricity-generating plant and a paper mill, several of the Inconel 600 bellows failed. The steam line operated at 6030 kPa (875 psi) and 420 deg C (790 deg F). Metallographic sections, energy-dispersive x-ray spectra, chemical analyses, tensile tests, and Auger microscope analyses showed the failed bellows met the specifications for the material. However, investigation also showed entire oxide thickness was contaminated with relatively large amounts of sodium, calcium, potassium, aluminum, and sulfur, alkali, alkali earth, and other contaminants that completely permeated even the thin oxides on the fracture surfaces. Additional investigation of the purity of the steam itself as reported by the power plant showed that corrosion and cracks were ultimately caused by the steam. While under normal operation, the steam's purity posed no problem to the material, during boiler cleaning operations, the generating plant had allowed contamination to get into the steam line.

A tube in a radiant superheater, the boiler of which is coal fired, failed by creep after 17 years of service. The failed tube was specified to be made of ASME SA-213, grade T-22. Measurable swelling of the tube diameter by about 2.4 mm and tube wastage caused by corrosion or erosion were observed. Log stress versus Larson-Miller Parameter (LMP) plots were produced to assess the remaining life of the superheater. It was revealed that the estimated operating temperature of 1060 deg F was higher than the estimated design temperature of 1000 deg F and that the tube wastage had increased the actual operating stress. Tube wastage and high operating temperatures hastened the failure. A better understanding of the material condition of this superheater was recommended to verify all the suspect hot tubes.

A 680,000 kg (750 ton) per day ammonia unit was shut down following a fire near the outlet of the waste heat exchanger. The fire had resulted from leakage of ammonia from the type 316 stainless steel outlet piping. The outlet piping immediately downstream from the waste heat exchanger consisted of a flange made from a casting, and a reducing cone, a short length of pipe, and a 90 deg elbow, all made of 13 mm thick plate. A liner wrapped with insulation was welded to the smaller end of the reducing cone. All of the piping up to the flange was wrapped with insulation. Investigation (visual inspection, 10x unetched images, liquid-penetrant inspection, and chemical analysis of the insulation) supported the conclusion that the failure occurred in the area of the flange-to-cone weld by SCC as the result of aqueous chlorides leached from the insulation around the liner by condensate. Recommendations included eliminating the chlorides from the system, maintaining the temperature of the outlet stream above the dewpoint at all times, or that replacing the type 316 stainless steel with an alloy such as Incoloy 800 that is more resistant to chloride attack.

The tubes of a stationary industrial boiler, 64 mm in diam and made of 1.25Cr-0.5Mo steel (ASME SA-213, grade T-11) failed by two different types of rupture. Noticeable swelling of the tubes in the area of rupture was revealed by visual examination. The tubes with slight longitudinal splits were interpreted to have failed by stress rupture resulting from prolonged overheating at 540 to 650 deg C as the microstructure exhibited extensive spheroidization and coalescence of carbides. The larger ruptures were tensile failures that resulted from rapid overheating to 815 to 870 deg C as a completely martensitic structure was revealed at the edges of the ruptures in these tubes because of rapid quenching by escaping fluid. The prolonged-overheating failures were concluded to have been the primary ruptures and that local loss of circulation had caused rapid overheating in adjacent tubes. Poor boiler circulation and high furnace temperatures were believed to have caused the prolonged overheating.

A root cause failure analysis was performed on a vaporizer coil removed from a horizontal forced circulation vaporizer. The carbon steel coil was wound in a right-hand helix with a coil centerline diameter of about 2 m. The vaporizer was gas fired and used Dowtherm A as the heat transfer fluid. Design conditions are based on annular fluid flow to cool the coil wall. NDE, metallographic and fractographic examinations were performed. Numerous, circumferentially oriented, OD initiating cracks were found near the crown for two coils near the non-fired end of the vaporizer. The cracking was confined to the inner diameter of the vaporizer coil at positions from 4:00 to 7:00. The cracking was characterized as transgranular and the fracture surface had beach marks. The failure mechanism was thermal fatigue. The heat transfer calculation predicted that dryout of the coil would occur for coils at the non-fired end of the vaporizer during low flow transients. Dryout results in rapid increase in the tube wall temperature. Thermal cycling of the coil is completed by liquid quenching resulting from resumption of normal flow rates and the return to annular flow. The probable root cause of failure was low flow transient operation.

Abstract
The failure of T12 reheater tubes that had been in service for only 3000 h was investigated. The thickness of the tubes was visibly reduced by heavy oxidation corrosion on the inner and outer walls. The original pearlite substrate completely decomposed. Uniform oxide scale observed on the inner wall showed obvious vapor oxidation corrosion characteristics. Corrosion originated in the grain boundary, and selective oxidation occurred due to ion diffusion in the substrate. The layered oxide scale on the inner wall is related to the different diffusion rates for different cations. Exposure to high temperature corrosive flux accelerated the corrosion on the outer wall. Microstructure degradation and the corrosion characteristics observed indicate that the tubes failed primarily because of overheating, which is confirmed by calculations.

Failure occurred in a steel superheater tube in a power plant. The tube was specified as ASTM A 213 grade T 22, and the reported operating conditions were 13 MPa (1900 psi) at 482 deg C (900 deg F). The tube carried superheated steam and was coal fired. Investigation (visual inspection, 2% nital etched 297x images, chemical analysis, and SEM fractographs) supported the conclusion that the superheater tube failed as a result of long-term overheating. Substantial creep damage reduced the strength of the tube to the point that overload failure occurred. No recommendations were made.

This article addresses the effects of damage to equipment and structures due to explosions (blast), fire, and heat as well as the methodologies that are used by investigating teams to assess the damage and remaining life of the equipment. It discusses the steps involved in preliminary data collection and preparation. Before discussing the identification, evaluation, and use of explosion damage indicators, the article describes some of the more common events that are considered in incident investigations. The range of scenarios that can occur during explosions and the characteristics of each are also covered. In addition, the article primarily discusses level 1 and level 2 of fire and heat damage assessment and provides information on level 3 assessment.

Abstract
The intermediate pressure (IP) turbine of a thermal generating station is driven by steam from the boiler's reheater. On one particular IP turbine, a thick deposit was found on the insides of the rotor blade shrouds in two instances two years apart. The source of the deposits was not known; bulk chemical analysis had simply shown that iron was a major component. Optical microscopy and electron microprobe analysis were used to identify the deposits. In the first instance, the deposit was found to be debris that was left in the reheater tubes during boiler modification and swept to the turbine by the steam. There were still some of these debris particles present when the incident two years later was investigated but generally the second deposit was found to be of two layer oxide particles which were shown to have spalled from 2-14% chromium reheater tube surfaces.
