Abstract
The 16 mm diam 6 x 37 fiber-core improved plow steel wire rope on a scrapyard crane failed after two weeks of service under normal loading conditions. This type of rope was made of 0.71 to 0.75% carbon steel wires and a tensile strength of 1696 to 1917 MPa. The rope broke when it was attached to a chain for pulling jammed scrap from the baler. The rope was heavily abraded and several of the individual wires were broken. a uniform cold-drawn microstructure, with patches of untempered martensite in regions of severe abrasion and crown wear was revealed by metallographic examination. As a result of abrasion, a hard layer of martensite was formed on the wire. The wire was made susceptible to fatigue cracking, while bending around the sheave, by this brittle surface layer. The carbon content and tensile strength of the wire was found lower than specifications. As a corrective measure, this wire rope was substituted by the more abrasion resistant 6 x 19 rope.

Abstract
A short fracture section of a forged and normalized Ck 35 (DIN 17200) steel slide showed three distinct zones: a dark colored crystalline area, an incipient crack propagating into a far advanced, rubbed fracture surface, and a fine crystalline final break. Metallographic examination showed the dark incipient crack was present before the last heat treatment and was oxidized and decarburized prior to the conclusion of the annealing process. The crack ran perpendicular to the fiber, so it was not formed before or during forging. It was a thermal stress crack produced during flame cutting of the middle section of the slide. The initial crack acted as a sharp notch favoring the formation of the fatigue fracture which lead to the failure of the slide.

Abstract
Failure of a case hardened steel shaft incorporated fuel pump in a turbine-powered aircraft resulted in damage to the aircraft. The disassembled pump was found to be dry and free of any contamination. Damage was exhibited on the pressure side of each spline tooth in the impeller and the relatively smooth cavities and undercutting of the flank on this side indicated that the damage was caused by an erosion or abrasion mechanism. A relatively smooth worn area was formed at the center of each tooth due to an abrasive action and an undulating outline with undercutting was observed on the damaged side. Particles of sand, paint, or plastic, fibers from the cartridge, brass, and steel were viewed in the brown residue on the filter cartridge under a low power microscope and later confirmed by chemical analysis. Large amount of iron was identified by application of a magnet. It was concluded that the combined effect of vibration and abrasive wear by sand and metal particles removed from the splines damaged the shaft. Case hardened spline teeth surface was recommended to increase resistance to wear and abrasion.

Abstract
This article reviews the mechanical behavior and fracture characteristics that discriminate structural polymers from metals. It provides information on deformation, fracture, and crack propagation as well as the fractography involving the examination and interpretation of fracture surfaces, to determine the cause of failure. The fracture modes such as ductile fractures and brittle fractures are reviewed. The article also presents a detailed account of various fracture surface features. It concludes with several cases of field failure in various polymers that illustrate the applicability of available analytical tools in conjunction with an understanding of failure mechanisms.

Abstract
The paper discusses the analysis of a coating defect on a high phosphorus electroless nickel (Ni-11 wt. % P) deposit plated on an aluminum alloy substrate. Preliminary investigations had indicated that the elongated defects were possibly caused by the entrapment of long fibers or particles during the plating. The possible sources of fibers were identified. The SEM/EDS analysis of fibers collected from the air duct filters correlated very well with the defect shape and the EDS profile collected from under the defect site. It appears that the fibers from air duct filters directly above the plating line were blown into the plating tank and getting co-deposited. The paper describes the step-by-step analysis of the defect that led to successful identification of the root cause of the defect.

Abstract
Plastics or polymers are used in a variety of engineering and nonengineering applications where they are subjected to surface damage and wear. This article discusses the classification of polymer wear mechanisms based on the methodologies of defining the types of wear. The first classification is based on the two-term model that divides wear mechanisms into interfacial and bulk or cohesive. The second is based on the perceived wear mechanism. The third classification is specific to polymers and draws the distinction based on mechanical properties of polymers. In this classification, wear study is separated as elastomers, thermosets, glassy thermoplastics, and semicrystalline thermoplastics. The article describes the effects of environment and lubricant on the wear failures of polymers. It presents a case study on nylon as a tribological material. The article explains the wear failure of an antifriction bearing, a nylon driving gear, and a polyoxymethylene gear wheel.

Abstract
The purpose of this investigation was to determine the cause of the ultrasonic signal attenuation noted during an inspection of a composite aircraft component. Although ultrasonics was able to identify the location of the defective areas, destructive analysis had to be utilized to determine the exact nature of the defect. The investigation describes how cross-sectioning, fractography, and chemical analysis were utilized to determine the type of defect responsible for the signal attenuation.

Abstract
A production lot of mechanical hinges used in an automotive application had failed during incoming quality-control routine actuation testing. A change in part supplier had taken place between the approval of the prototype parts that performed acceptably and the receipt of the first lot of production parts. The mechanical hinges were specified to be injection molded from an impact-modified, 13% glass-fiber-reinforced nylon 6/6 resin. Investigation of samples representing the failed components and the original prototype parts included visual inspection, 118x SEM images, micro-FTIR, DSC analysis, and TGA. It supported the conclusion that the hinge assemblies failed through brittle fracture associated with stress overload during the actuation of the parts. The failed part material was found to be degraded, most likely occurring during the compounding of the resin or during the actual molding of the parts. While resins for both failed and non-failed parts produced results characteristic of a 13% glass-fiber-reinforced, impact-modified nylon 6/6, the failed part material, contained a significantly lower level of rubber, which rendered the parts less impact resistant and subsequently lowered the ductility of the molded hinge assemblies. No recommendations were made.

Abstract
This article reviews the abrasive and adhesive wear failure of several types of reinforced polymers, including particulate-reinforced polymers, short-fiber reinforced polymers (SFRP), continuous unidirectional fiber reinforced polymers (FRP), particulate-filled composites, mixed composites (SFRP and particulate-filled), unidirectional FRP composites, and fabric reinforced composites. Friction and wear performance of the composites, correlation of performance with various materials properties, and studies on wear-of failure mechanisms by scanning electron microscopy are discussed for each of these types.

Abstract
A welded natural gas line of 400 mm OD and 9 mm wall thickness made of unalloyed steel with 0.22C had to be removed from service after four months because of a pipe burst. Metallographic examination showed the pipe section located next to the gas entrance was permeated by cracks or blisters almost over its entire perimeter in agreement with the ultrasonic test results. Only the weld seam and a strip on each side of it were crack-free. Based on this investigation, the pipeline was taken out of service and reconstructed. To avoid such failures in the future, two preventative measures may be considered. One is to desulfurize the gas. Based on tests, however, the desulfurization would have to be carried very far to be successful. The second possibility is to dry the gas to such an extent as to prevent condensate, and this corrosion, from forming no matter how low winter temperatures may drop. This measure was ultimately recommended, deemed more effective and cheaper.

Abstract
A helicopter tail rotor blade spar failed in fatigue, allowing the blade to separate during flight. The 2014-T652 aluminum alloy blade had a hollow spar shank filled with lead wool ballast and a thermoset polymeric seal. A corrosion pit was present at the origin of the fatigue zone and numerous trails of corrosion pits were located on the spar cavity's inner surfaces. The corrosion pitting resulted from the failure of the thermoset seal in the spar shank cavity. The seal failure allowed moisture to enter into the cavity. The moisture then served as an electrolyte for galvanic corrosion between the lead wool ballast and the aluminum spar inner surface. The pitting initiated fatigue cracking which led to the spar failure.

Abstract
Locked coil wire ropes, by virtue of their unique design and construction, have specialized applications in aerial ropeways, mine hoist installations, suspension bridge cables, and so forth. In such specialty ropes, the outer layer is constructed of Z-profile wires that provide not only effective interlocking but also a continuous working surface for withstanding in-service wear. The compact construction and fill-factor of locked coil wire ropes make them relatively impervious to the ingress of moisture and render them less vulnerable to corrosion. However, such ropes are comparatively more rigid than conventional wire ropes with fiber cores and therefore are more susceptible to the adverse effects of bending stresses. The reasons for premature in-service wire rope failures are rather complex but frequently may be attributed to inappropriate wire quality and/or abusive operating environment. In either case, a systematic investigation to diagnose precisely the genesis of failure is desirable. This article provides a microstructural insight into the causes of wire breakages on the outer layer of a 40 mm diam locked coil wire rope during service. The study reveals that the breakages of Z-profile wires on the outer rope layer were abrasion induced and accentuated by arrays of fine transverse cracks that developed on a surface martensite layer.

Accelerated aging tests on detonator assemblies, to verify the compatibility of gold bridgewire and Pd-In-Sn solder with the intended explosives, revealed an unusual form of corrosion. The tests, conducted at 74 deg C (165 deg F) and 54 deg C (130 deg F), indicated a preferential attack of the gold. To investigate the problem, a matrix of test units was produced and analyzed. Scanning electron microscopy, EDX analysis, and x-ray diffraction techniques were used to determine the extent of the corrosion and identify the corrosion products. The results indicated that the preferential attack of the gold was due to HCN formed by decomposition of the explosive powder at high temperatures. Other associated reactions were also observed including the subsequent attack of the solder by the gold corrosion product and degradation of the plastic header.

Abstract
This article reviews the analytical techniques most commonly used in plastic component failure analysis. These include the Fourier transform infrared spectroscopy, differential scanning calorimetry, thermogravimetric analysis, thermomechanical analysis, and dynamic mechanical analysis. The descriptions of the analytical techniques are supplemented by a series of case studies that include pertinent visual examination results and the corresponding images that aid in the characterization of the failures. The article describes the methods used for determining the molecular weight of a plastic resin. It explains the use of mechanical testing in failure analysis and also describes the considerations in the selection and use of test methods.

Cracking occurred within the plastic jacket (injection molded from an impact-modified, 15% glass-fiber-reinforced PET resin.) of several assemblies used in a transportation application during an engineering testing regimen which involved cyclic thermal shock (exposing the parts to alternating temperatures of -40 and 180 deg C (-40 and 360 deg F)). Prior to molding, the resin had reportedly been dried at 135 deg C (275 deg F). The drying process usually lasted 6 h, but occasionally, the material was dried overnight. Comparison investigation (visual inspection, 20x SEM views, micro-FTIR, and analysis using DSC and TGA) with non-failed parts supported the conclusion that that the failure was via brittle fracture associated with the exertion of stresses that exceeded the strength of the resin as-molded caused by the disparity in the CTEs of the PET jacket and the mating steel sleeve. The drying process had exposed the resin to relatively high temperatures, which caused substantial molecular degradation, thus limiting the part's ability to withstand the stresses. The drying temperature was found to be significantly higher than the recommendation for the PET resin, and the testing itself exposed the parts to temperatures above the recognized limits for PET.

A 1-in. diam pump spindle fractured within the length covered by the boss of the impeller which was attached to the spindle by means of an axial screw. The pump had been in use in a chemical plant handling mixtures of organic liquids and dilute sulfuric acid having a pH value of 2 to 4 at temperatures of 80 to 90 deg C (176 to 194 deg F). The fracture was unusual in that it was of a fibrous nature, the fibers-which were orientated radially-were readily detachable. The surface of the spindle adjacent to the fracture had an etched appearance and the mode of cracking in this region suggested that failure resulted from an intergranular attack. Subsequent microscope examination confirmed the generally intergranular mode of failure. A macro-etched section near the fracture revealed a radial arrangement of columnar crystals, indicating that the spindle was a cast and not a wrought product as had been presumed. Spectroscope examination showed this particular composition (Fe-23Cr-18Ni-1.8Mo-1.2Si) did not conform to a standard specification and is apparently a proprietary alloy. It was evident that the particular mode of failure was related to the inherent structure of the material.

The quantitative characterization of fracture surface geometry, that is, quantitative fractography, can provide useful information regarding the microstructural features and failure mechanisms that govern material fracture. This article is devoted to the fractographic techniques that are based on fracture profilometry. This is followed by a section describing the methods based on scanning electron microscope fractography. The article also addresses procedures for three-dimensional fracture surface reconstruction. In each case, sufficient methodological details, governing equations, and practical examples are provided.

To determine the effect of severe service on cast 357 aluminum pistons, a metallurgical evaluation was made of four pistons removed from the engine of the Hawk-Offenhauser car which had been driven by Rich Muther in the first Ontario, California 500 race. The pistons were studied by visual inspection, hardness traverses, radiography, dye penetrant inspection, chemical analysis, macrometallography, optical microscopy, and electron microscopy. The crown of one piston had a rough, crumbly deposit, which was detachable with a knife. Two pistons had remains of carbonaceous deposits. The fourth was severely hammered. It was concluded that the high temperatures developed in this engine created an environment too severe for 357 aluminum. Surfaces were so hot that the low-melting constituent melted. Then, the alloy oxidized rapidly to form Al2O3, an abrasive which further aggravated problems. The temperature in much of the piston was high enough to cause softening by overaging, lowering strength.
