Abstract
Metal-framed polycarbonate (PC) ophthalmic lenses shattered from acetone solvent-induced cracking. The lenses exhibited primary and secondary cracks with solvent swelling and crazing. A laboratory accident splashed acetone onto the lenses. The metal frames gripped approximately two-thirds of the lenses' periphery and introduced an unevenly distributed force on the lenses. To prevent future failures, it was recommended to protect PC from service environments with solvents, such as acetone; or from marking pens, adhesives or soaps which contain undesirable solvents; and to not apply excessive stress on ophthalmic lenses in the form of working or residual stresses.

Abstract
Reliability-centered maintenance (RCM) is a systematic methodology for preventing failures. This article begins by discussing the history of RCM and uses Society of Automotive Engineers (SAE) all-industry standard JA1011 as its model to describe the key characteristics of an RCM process. It then expands on questions involved in RCM process, offering definitions when necessary. Next, the article describes the approach of RCM to failure modes and effects analysis (FMEA), the failure management policies available under RCM, and the criteria of RCM for deciding when a specific failure management policy is technically feasible. Then, after discussing the ways that RCM classifies failure effects in terms of consequences, it describes how RCM uses failure consequences to identify the best failure management policy for each failure mode. Next, the building blocks of RCM are put together to create a failure management program. The article ends with a discussion on some practical issues pertaining to RCM that lie outside the scope of SAE JA1011.

Abstract
Reliability-centered maintenance (RCM) is a systematic methodology for preventing failures. This article discusses the history of RCM and describes the key characteristics of an RCM process, which involves asking seven questions. The first four questions comprise a form of failure modes and effects analysis (FMEA), and therefore, the article explains the approach of RCM to FMEA and the failure management policies available under RCM. It reviews the ways that RCM classifies failure effects in terms of consequences and details how RCM uses failure consequences to identify the best failure management policy for each failure mode. The article concludes with a discussion on some practical issues pertaining to RCM that lie outside the scope of SAE JA1011.

Abstract
This article focuses on failure analyses of aircraft components from a metallurgical and materials engineering standpoint, which considers the interdependence of processing, structure, properties, and performance of materials. It discusses methodologies for conducting aircraft investigations and inspections and emphasizes cases where metallurgical or materials contributions were causal to an accident event. The article highlights how the failure of a component or system can affect the associated systems and the overall aircraft. The case studies in this article provide examples of aircraft component and system-level failures that resulted from various factors, including operational stresses, environmental effects, improper maintenance/inspection/repair, construction and installation issues, manufacturing issues, and inadequate design.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. The article also describes the role of the materials engineer in the design and materials selection process. It provides information on the significance of materials selection in both the prevention and analysis of failures.

Abstract
The side supporting flange of the bottom platen of an 800 ton hydraulic press fractured after 9 x 10's cycles under a maximum load of 530 tons. The platen material specified in the design was cast steel 52. Metallographic examination of the fracture surface indicated that the platen had failed in fatigue as a result of a high stress concentration in a sharp 0.6 mm (0.02 in.) radius fillet. Stress analysis and fracture mechanics predictions revealed that there was also danger of fatigue failure for platens with the design radius of 10 mm (0. 4 in.) if the press operates at 800 tons. It was recommended that the remaining life of similar presses be assessed periodically controlling the cracks, their dimensions, and their propagation rates. An increase in the radius of the fillet was also recommended.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

This article discusses the fundamental variables involved in fatigue-life assessment, which describe the effects and interaction of material behavior, geometry, and stress history on the life of a component. It compares the safe-life approach with the damage-tolerance approach, which employs the stress-life method of fatigue life assessment. The article examines the behavior of three different metallic materials used in the design and manufacture of structural components: steel, aluminum, and titanium. It also reviews the effects of retardation and spectrum load on component life. The article concludes with case studies of fatigue life assessment from the aerospace industry.

Abstract
Nuclear power plants typically experience two or three high-cycle fatigue failures of stainless steel socket-welded connections in small bore piping during each plant-year of operation. This paper discusses fatigue-induced failure in socket-welded joints and the strategy Texas Utilities Electric Company (TU Electric) has implemented in response to these failures. High-cycle fatigue is invisible to proven commercial nondestructive evaluation (NDE) methods during crack initiation and the initial phases of crack growth. Under a constant applied stress, cracks grow at accelerating rates, which means cracks extend from a detectable size to a through-wall crack in a relatively short time. When fatigue cracks grow large enough to be visible to NDE, it is likely that the component is near the end of its useful life. TU Electric has determined that an inspection program designed to detect a crack prior to the component leaking would involve frequent inspections at a given location and that the cost of the inspection program would far exceed the benefits of avoiding a leak. Instead, TU Electric locates these cracks by visually monitoring for leaks. Field experience with fatigue-induced cracks in socket-welded joints has confirmed that visual monitoring does detect cracks in a timely manner, that these cracks do not result in catastrophic failures, and that the plant can be safely shut down in spite of a leaking socket-welded joint in a small bore pipe. Historical data from TU Electric and Southwest Research Institute are presented regarding the frequency of failures, failure locations, and the potential causes. The topics addressed include 1) metallurgical and fractographic features of fatigue cracks at the weld toe and weld root; 2) factors that are associated with fatigue, such as mechanical vibration, internal pulsation, joint design, and welding workmanship; and 3) implications of a leaking crack on plant safety. TU Electric has implemented the use of modified welding techniques for the fabrication of socket-welded joints that are expected to improve their ability to tolerate fatigue.

Abstract
This article reviews the mechanical behavior and fracture characteristics that discriminate structural polymers from metals. It provides information on deformation, fracture, and crack propagation as well as the fractography involving the examination and interpretation of fracture surfaces, to determine the cause of failure. The fracture modes such as ductile fractures and brittle fractures are reviewed. The article also presents a detailed account of various fracture surface features. It concludes with several cases of field failure in various polymers that illustrate the applicability of available analytical tools in conjunction with an understanding of failure mechanisms.

Abstract
This article describes the methodology for performing a failure modes and effects analysis (FMEA). It explains the methodology with the help of a hot water heater and provides a discussion on the role of FMEA in the design process. The article presents the analysis procedures and shows how proper planning, along with functional, interface, and detailed fault analyses, makes FMEA a process that facilitates the design throughout the product development cycle. It also discusses the use of fault equivalence to reduce the amount of labor required by the analysis. The article shows how fault trees are used to unify the analysis of failure modes caused by design errors, manufacturing and maintenance processes, materials, and so on, and to assess the probability of failure mode occurrence. It concludes with information on some of the approaches to automating the FMEA.

Abstract
Corrosion in potable and nonpotable water systems has been well documented in the past, and new research discusses innovations in water treatment and materials that are designed to enhance the quality of a water system, whether commercial or residential. This paper is a collection of five case histories on the failure of copper and steels as used in potable and non-potable water systems. The case histories cover a range of applications in which copper and steel products have been used. Copper and steel pipes are the two most commonly used materials in residential, commercial and industrial applications. The projects that are discussed cover these three important applications. The purpose of presenting this information is to allow the reader to gain an understanding of real life corrosion issues that affect plumbing materials, how they should have been addressed during the design of the water system, and how a water system should be maintained during service. We share this information in the hope that the reader will gain some limited knowledge of the problems that exist, and apply that knowledge in designing or using water systems in day-to day life.

Abstract
Metal-induced embrittlement is a phenomenon in which the ductility or the fracture stress of a solid metal is reduced by surface contact with another metal in either the liquid or solid form. This article summarizes some of the characteristics of liquid-metal- and solid-metal-induced embrittlement. This phenomenon shares many of these characteristics with other modes of environmentally induced cracking, such as hydrogen embrittlement and stress-corrosion cracking. The discussion covers the occurrence, failure analysis, and service failures of the embrittlement. The article also briefly reviews some commercial alloy systems in which liquid-metal-induced embrittlement or solid-metal-induced embrittlement has been documented and describes some examples of cracking due to these phenomena, either in manufacturing or in service.

Abstract
Failure analysis is generally defined as the investigation and analysis of parts or structures that have failed or appeared to have failed to perform their intended duty. Methods of field inspection and initial examination are also critical factors for both reconstruction analysts and materials failure analysts. This article focuses on the general methods and approaches from the perspective of a reconstruction analyst. It describes the elements of accident reconstruction, which have conceptual similarity with the principles for failure analysis of material incidents that are less complex than a large-scale accident. The approach presented is that the analysis and reconstruction is based on the physical evidence. The article provides a brief review of some general concepts on the use and limitations of advanced data acquisition tools and computer modeling. Legal implications of destructive testing are discussed in detail.

This article offers an overview of fatigue fundamentals, common fatigue terminology, and examples of damage morphology. It presents a summary of relevant engineering mechanics, cyclic plasticity principles, and perspective on the modern design by analysis (DBA) techniques. The article reviews fatigue assessment methods incorporated in international design and post construction codes and standards, with special emphasis on evaluating welds. Specifically, the stress-life approach, the strain-life approach, and the fracture mechanics (crack growth) approach are described. An overview of high-cycle welded fatigue methods, cycle-counting techniques, and a discussion on ratcheting are also offered. A historical synopsis of fatigue technology advancements and commentary on component design and fabrication strategies to mitigate fatigue damage and improve damage tolerance are provided. Finally, the article presents practical fatigue assessment case studies of in-service equipment (pressure vessels) that employ DBA methods.

Abstract
This article describes concepts and tools that can be used by the failure analyst to understand and address deformation, cracking, or fracture after a stress-related failure has occurred. Issues related to the determination and use of stress are detailed. Stress is defined, and a procedure to deal with stress by determining maximum values through stress transformation is described. The article provides the stress analysis equations of typical component geometries and discusses some of the implications of the stress analysis relative to failure in components. It focuses on linear elastic fracture mechanics analysis, with some mention of elastic-plastic fracture mechanics analysis. The article describes the probabilistic aspects of fatigue and fracture. Information on crack-growth simulation of the material is also provided.
