Abstract
An open electrical circuit was found between plated through-holes in a six-layer printed circuit board after thermal cycling. The copper plating was very thin in the failure area but did make an electrical contact during initial testing. During thermal cycling, differential z-expansion between the epoxy board and copper caused the thin plating to crack. During electrical testing of a four-layer circuit board, an open electrical circuit was found between the plated through-holes. Plating discontinuity was caused by poor drilling using a dull drill with improper speed (rpm) and/or feed rate as was observed by nonuniform plating and nodule formation in the plated layer. In a third example, an open electrical circuit was found in a six-layer board between two adjacent plated through-holes. A plating void was on one side of the conductor joining the two holes. Continuity was found when tested from one side of the board but lost when tested from the other. In a fourth case, an open circuit found between a plated through-hole and contact pad on a six-layer printed circuit board was caused by an etching defect.

Abstract
Electroless nickel plating separation from copper alloy CDA175 retaining clips used on printed circuit boards was caused by a copper oxide layer that reduced adhesion of the nickel plating on the clips. Stresses that developed during module insertion caused flaking to occur at the oxidized copper surface. Electroless nickel plating separation from OFHC copper leads was caused by improper handling rather than a plating anomaly per se. Tin plating separation from copper underplating on a hybrid package lid occurred because of a four-week delay between the copper plating and tin plating steps. It was recommended that tin plating should follow the copper underplating within 24 h and a cleaning step of bright dipping after copper plating be performed.

Abstract
One of five underground drain lines intended to carry a highly acidic effluent from a chemical-processing plant to distant holding tanks failed in just a few months. Each line was made of 304L stainless steel pipe 73 mm (2 in.) in diam with a 5 mm (0.203 in.) wall thickness. Lengths of pipe were joined by shielded metal arc welding. Soundness of the welded joints was determined by water back-pressure testing after several lengths of pipe had been installed and joined. Before completion of the pipeline, a pressure drop was observed during back-pressure testing. An extreme depression in the backfill revealed the site of failure. Analysis (visual inspection, electrical conductivity, and soil analysis) supported the conclusions that the failure had resulted from galvanic corrosion at a point where the corrosivity of the soil was substantially greater than the average, resulting in a voltage decrease near the point of failure of about 1.3 to 1.7 V. Recommendations included that the pipelines be asphalt coated and enclosed in a concrete trough with a concrete cover. Also, magnesium anodes, connected electrically to each line, should be installed at periodic intervals along their entire length to provide cathodic protection.

Abstract
During routine quality control testing, small circuit breakers exhibited high contact resistance and, in some cases, insulation of the contacts by a surface film. The contacts were made of silver-refractory (tungsten or molybdenum) alloys. Infrared analysis revealed the film to be a corrosion layer that resulted from exposure to ammonia in a humid atmosphere. Simulation tests confirmed that ammonia was the corrodent. The ammonia originated from the phenolic molding area of the plant. It was recommended that fumes from molding areas be vented outside the plant and that assembly, storage, and calibration areas be isolated from molding areas.

Abstract
Three instances involving the failure of aluminum wiring at the service entrance to single-family homes are discussed. Arcing led to a fire which severely damaged a home in one case. In a second, the failure sequence was initiated by water intrusion into the service entrance electrical box during construction of the home. In the third, failure was caused by a marginal installation. Strict adherence to all applicable electrical codes and standards is critical in the case of aluminum wiring. Electrical components not specifically designed for aluminum must never be used with this type of wiring. All doors, panels and similar portions of electrical boxes should be secured to prevent damage to surroundings in the event of an electrical fault. If symptoms of arcing are observed, professional service should be sought. The latest designs of connectors for use with aluminum wiring are less susceptible to deviations in installation practice.

Abstract
Two cracks were discovered in a deck plate of an aircraft during overhaul and repair after 659 h of service. The cracks were on opposite sides of the deck plate in the flange joggles. The plate had been formed from 7178-T6 aluminum alloy sheet. Analysis (visual inspection, 0.2x/2x/2.3x electron microscope fractographs, hardness testing, and electrical conductivity testing) supported the conclusions that the failure was caused by fatigue cracks originating on the inside curved surface of the flanges. The cracks had initiated in surface defects caused by either corrosion pitting or forming notches, acting in combination with lateral forces evidenced by the moderate distortion of the fastener holes. Recommendations included eliminating the surface defects by revised cleaning and/or forming procedures. Revised design and installation should also alleviate the lateral forces.

A substantial number of copper alloy C27000 (yellow brass, 65Cu-35Zn) ferrules for electrical fuses cracked while in storage and while in service in paper mills and other chemical processing plants. The ferrules, made by three different manufacturers, were of several sizes. One commonly used ferrule was 3.5 cm long by 7.5 cm in diam and was drawn from 0.5 mm (0.020 in.) thick strip. Investigation (visual inspection, metallographic examination, and a mercurous nitrate test, which is an accelerated test used to detect residual stress in copper and copper alloys) of both ferrules from fuses in service and storage in different types of plants, and ferrules from newly manufactured fuses, supported the conclusion that the ferrules failed by SCC resulting from residual stresses induced during forming and the ambient atmospheres in the chemical plants. The atmosphere in the paper mills was the most detrimental, and the higher incidence of cracking of ferrules there was apparently related to a higher concentration of ammonia in conjunction with high humidity. Recommendations included specifying that the fuses meet the requirements of ASTM B 154.

Abstract
Coaxial cable connectors made of brass were failing at a high rate after less than one year of service in an outdoor industrial environonment. The observed failures, which consisted of cracks in the body and end cap, were analyzed and found to be brittle fractures due to stress-corrosion cracking. Two common stress-corrosion cracking tests for copper materials were conducted on new connectors from the same manufacturing lot, confirming the initial determination of the fracture mode. Additional testing as was done in the investigation is often helpful when analyzing corrosion failures.

Abstract
Housings (being tested as part of a material conversion) from an electrical appliance failed during an engineering evaluation. They had been injection molded from a commercial polycarbonate/PET blend. Parts produced from the previous material, a nylon 6/6 resin, had consistently passed the testing regimen. Grease was applied liberally within the housing assembly during production. Investigation included visual inspection, 24x SEM images, micro-FTIR in the ATR mode, and analysis using DSC. No signs of material contamination were found, but the thermograms showed a crystallization of the PET resin. The grease present within the housing assembly, analyzed using micro-FTIR, was composed of a hydrocarbon-based oil, a phthalate-based oil, lithium stearate, and an amide-based additive. The conclusion was that the appliance housings failed through environmental stress cracking caused by a phthalate-based oil that was not compatible with the PC portion of the resin blend. Thus, the resin conversion was the root cause of the failures. Additionally, during the injection molding process the molded parts had been undercrystallized, reducing their mechanical strength. More importantly, the resin had been degraded, producing a reduction in the molecular weight and reducing both the mechanical integrity and chemical-resistance properties of the parts.

While a chain sling was being used to lift a casting one of the links ruptured. The sling, reputed to be of the electrically-welded steel type, was at least eight years old and had been overhauled several times during its working life. Examination showed the links were scarf-welded. Furthermore, the welds were at the ends and not at the sides as is usual in the case of electrically-welded chains. A transverse section from one side of a link was examined microscopically. This showed the material to be wrought-iron of satisfactory quality. It was concluded this chain sling had been made from wrought-iron, forge welded in the usual manner, and that it was not electrically-welded steel as had been supposed. Failure was attributed to embrittlement in service of the surface material of the links. If it had been realized that the sling was made from wrought-iron then it would doubtless have been subjected to periodical annealing in accordance with Statutory requirements, which would have restored the ductility of the surface material.

Abstract
Ball bearings made of type 440C stainless steel hardened to 60 HRC and suspected as the source of intermittent noise in an office machine were examined. A number of spots on the inner-ring raceway were revealed by scanning electron microscopy. The metal in the area around the spot was evidenced to have been melted and welded to the inner-ring raceway. It was revealed by randomly spaced welded areas on the raceways that the welding was the result of short electrical discharges between the bearing raceways and the balls. The use of an electrically nonconductive lubricant in the bearings was suspected to have caused the electric discharge by accumulation and discharge of static charge. The electrical resistance between the rotor and the motor frame lubricated with electrically conductive grease and the grease used in the current case was measured and compared to confirm the fact the currently used grease was nonconductive. It was concluded that the pits were formed by momentary welding between the ball and ring surfaces. The lubricant was replaced by electrically conductive grease as a corrective measure.

Following three similar failures of load chains on manually operated geared pulley-blocks of 1-ton capacity, a portion of one of the chains was obtained for examination. The chain was made of mild steel and the links had been electrically butt-welded at one side. In the case of the sample obtained, the failure in service had resulted from fracture of one of the links in the plane of the weld. Six of the other links in the vicinity showed cracks in the welds in various stages of development. Microscope examination showed a crack in an early stage of development and also from an apparently sound link, the prepared surfaces lying in the planes of the links. This examination revealed that the welds were initially defective. Discontinuities were present in both cases adjacent to the insides of the links, of a type indicative of either inadequate fusion or incomplete expulsion of oxide, etc., at the time of the upset, i.e. the pressing together of the ends of the links to complete the welding. It was evident from the examination that the service failures were due to the use of chain that was initially defective.

Within the first few months of operation of an 8 km (5 mile) long 455 mm (18 in.) diam high-pressure steam line between a coal-fired electricity-generating plant and a paper mill, several of the Inconel 600 bellows failed. The steam line operated at 6030 kPa (875 psi) and 420 deg C (790 deg F). Metallographic sections, energy-dispersive x-ray spectra, chemical analyses, tensile tests, and Auger microscope analyses showed the failed bellows met the specifications for the material. However, investigation also showed entire oxide thickness was contaminated with relatively large amounts of sodium, calcium, potassium, aluminum, and sulfur, alkali, alkali earth, and other contaminants that completely permeated even the thin oxides on the fracture surfaces. Additional investigation of the purity of the steam itself as reported by the power plant showed that corrosion and cracks were ultimately caused by the steam. While under normal operation, the steam's purity posed no problem to the material, during boiler cleaning operations, the generating plant had allowed contamination to get into the steam line.

Abstract
Immediately after installation, leakage was observed at the mounting surface of several rebuilt hydraulic actuators that had been in storage for up to three years. At each joint, there was an aluminum alloy spacer and a vellum gasket. The mounting flanges of the steel actuators had been nickel plated. During assembly of the actuators a lubricant containing molybdenum disulfide had been applied to the gaskets as a sealant. The vellum gasket was found to be electrically conductive, and analysis (visual inspection, 500x unetched micrographs, galvanic action testing, and x-ray diffraction) supported the conclusions that leakage was the result of galvanic corrosion of the aluminum alloy spacers while in storage. The molybdenum disulfide was apparently suspended in a volatile water-containing vehicle that acted as an electrolyte between the aluminum alloy spacer and the nickel-plated steel actuator housing. Initially, the vellum gasket acted as an insulator, but the water-containing lubricant gradually impregnated the vellum gasket, establishing a galvanic couple. Recommendations included discontinuing use of molybdenum disulfide lubricant as a gasket sealer, and assembling the actuators using dry vellum gaskets.

Abstract
Two failures of AP15A grade J-55 electric resistance welded (ERW) tubing in as our gas environment were investigated. The first failure occurred after 112 days of service. Replacement pipe failed 2 days later. Surface examination of the failed tubing indicated that fracture initiated at the outside surface. Metallographic analysis showed that the fracture originated in the upturned fibers adjacent to the ERW bond line. Cross sections of the weld were removed from three random locations in the test sample. At each location, the up turned fibers of the weld zone contained bands of hard-appearing microstructure. Hardness measurements confirmed these observations. The cracks followed these bands. It was concluded that the tubing failed from sulfide stress cracking, which resulted from bands of susceptible microstructure in the ERW zone. The banded microstructure in the pipe suggested that chemical segregation contributed to the hard areas. Postweld normalized heat treatment apparently did not sufficiently reduce the hardness of these areas.

A segment from a premium-quality H13 tool steel die for die casting of aluminum failed after only 700 shots. The segment was subjected to visual, macroscopic, hardness, and metallographic testing. The investigation revealed that failure occurred as a result of fatigue at an electrical-discharge-machined surface where the resulting rehardened layer had not been removed. This rehardened layer had cracked, providing a source for fatigue initiation.

A section of pipe in a hydrocarbon pipeline was found to be leaking. The pipeline was installed several decades earlier and was protected by an external coating of extruded polyethylene and a cathodic protection system. The failed pipe section was made from API 5L X46 line pipe steel, approximately 22 cm (8.7 in.) OD x 0.5 cm (0.2 in.) wall thickness, which was electric resistance welded along the longitudinal seam. The pressure at the time and location of the failure was 2760 kPa, which corresponds to 20% of the specified minimum yield strength. The cause of failure (based on visual inspection, magnetic particle inspection, stereoscopic analysis, scanning electron microscopy, tensile and hardness testing, and chemical analysis) was attributed to damage resulting from a lightning strike.

Abstract
An 1100 aluminum alloy connector of a high-tension aluminum conductor steel-reinforced (ACSR) transmission cable failed after more than 20 years in service, in a region of consider able industrial pollution. The steel core was spliced with a galvanized 1020 carbon steel sheath. Visual examination showed that the connector had undergone considerable plastic deformation and necking before fracture. The steel sheath was severely corroded, and the steel splice was pressed off-center in the axial direction inside the connector. Examination of the fracture surface and micro-structural analysis indicated that the failure was caused by mechanical overload, which occurred because of weakening of the steel support cable by corrosion inside the fitting. The corrosion was ascribed to defective assembly of the connector which allowed moisture penetration.

Abstract
A shrunk-fit 18 Mn-5Cr steel retaining ring failed without warning during normal unit operation of a 380 MW electrical generator. The cause of the ring failure was determined to be intergranular stress-corrosion cracking (IGSCC) because of the high strength of the ring material and the presence of moist hydrogen used to cool the ring. Factors which promoted the failure were higher than normal strength levels in the ring material, lower than normal ring operating temperatures, possible moisture in the lubrication oil system, periodic poor performance of the hydrogen dryers, and a ring design which allowed water to become trapped in a relief groove. These factors caused pitting in the ring in an estimated 100 hours of operation. The ring had been inspected previously 18 months prior to the failure and no defects or pitting were found. Calculations showed that a 0.127-cm (0.050-in.) deep pit could grow to a critical size in 3000 to 4000 hours of operation. To prevent further failures, it was recommended that the ring be replaced with an 18 Mn-18Cr alloy with superior resistance to IGSCC. A program of periodic inspection and replacement of other retaining rings in the system was also recommended.
