Abstract
On 13 Dec 1994, two massive detonations leveled portions of an ammonium nitrate plant near Sioux City, IA. The primary explosion allegedly occurred in defectively-designed titanium sparger piping inside the neutralizer vessel. Investigation however, revealed the explosion occurred because of unsafe plant operations and poor maintenance procedures. Specifically, the ammonium nitrate within the 18,000 gal capacity neutralizer vessel had become contaminated and made highly acidic. The operators then injected superheated steam directly into the ammonium nitrate in the neutralizer vessel.

Abstract
Two 25 x 40 mm (1 x 1.5 in.) AISI 4150 hot-rolled steel bars that cracked during heat treatment were examined to determine whether the heat treating procedure had contributed to the failure. Metallographic examination of a cross section taken through the fracture revealed an oxide coating on both sides of the fracture surface. The oxide was also found on the top and bottom sides of the sample. Sawcut sides of the bar did not exhibit the oxide layer The presence of the oxide in the fracture, combined with its absence on all exterior surfaces, indicated that the fracture occurred as a result of an oxide seam in the original material rather than from oxide from heat treating. Nondestructive testing prior to machining and heat treatment was recommended.

Abstract
A steam turbine developed excessive noise and vibration during routine operation. It was found that the nut that locked the turbine disk In place had worked its way out from the threads and the disk had come of the shaft. Examination of the locking mechanism indicated that its design was responsible for the loosening of the nut. It was recommended that the locking mechanism be redesigned and changed in all existing turbines.

Abstract
A stainless steel flexible connector failed after a short period of service. Visual examination of the failed part revealed that a fracture had occurred in the thin-walled stainless steel bellows brazed into the flanges at each end. Surface examination by SEM fractography showed that failure of the bellows occurred via fatigue. The crack in the bellows had widened considerably after the fracture, and the bellows had been severely compressed on the fracture side prior to failure. Based on these observations, it was concluded that bellows had been damaged prior to installation. The damage resulted in high mean tensile stresses upon which were superimposed cyclic stresses, with fatigue failure the final result.

Abstract
A sprinkler head unit that was installed in a smoking lounge of a multi story office building in 1975 failed, causing substantial water damage. There was no fire in the building. A set of four sprinkler heads -- three that had been installed in 1975 (the failed unit, an unfailed unit from the same room, and an unfailed unit from another room) and an unused 1991 unit -- were examined. casting revealed no material defects or mechanical damage. Because of several environmental factors, it was suspected that the failed unit was exposed to temperatures much above the normal office environment. On this basis, it was concluded that creep of the solder alloy was the most probable cause of failure.

Abstract
Compound bow handle risers that had failed in service and during assembly along with an unassembled riser were submitted for analysis. The risers were die cast from magnesium-base alloy AM60A. Inspection of the failed risers and metallurgical investigations conducted on the stock riser revealed the presence of cold shuts at the same site in all specimens. It was recommended that all risers be thoroughly inspected and that the bow company work with their die casting shop to design a mold with acceptable filling characteristics.

Abstract
During routine quality control testing, small circuit breakers exhibited high contact resistance and, in some cases, insulation of the contacts by a surface film. The contacts were made of silver-refractory (tungsten or molybdenum) alloys. Infrared analysis revealed the film to be a corrosion layer that resulted from exposure to ammonia in a humid atmosphere. Simulation tests confirmed that ammonia was the corrodent. The ammonia originated from the phenolic molding area of the plant. It was recommended that fumes from molding areas be vented outside the plant and that assembly, storage, and calibration areas be isolated from molding areas.

Abstract
A type 430Ti stainless steel flue gas expansion joint cracked because of caustic-induced stress-corrosion cracking. Energy-dispersive X-ray spectroscope analysis of the fracture surface deposits revealed the presence of sodium and potassium—caustics in hydroxide form. Primary fracture surfaces were all similar in appearance, and a primary crack origin could not be identified. A secondary crack brought to fracture in the laboratory showed brittle, cleavage features rather than classic, tensile overload features. This suggested that the material was embrittled.

Abstract
A cracked 356 mm (14 in.) diam slip-on flange (Ni-Cr-Mo-V steel) was submitted for failure analysis. Reported results and observations indicated that the flange was not an integral forging or a casting, as specified. It had been fabricated by welding and machining a ring insert within a flange with a larger internal diameter. The flange cracked because the welds between the flange and the insert were inadequate to withstand the bolting pressures. A warning was issued to end users of the flanges, which are being inspected nondestructively for conformance to specifications.

Abstract
Failure occurred in a type 304 stainless steel leaf spring attached to the undercarriage assembly of an airport shuttle train. Failure analysis showed that the fracture was caused by low-cycle, reversed bending fatigue. The stresses leading to failure were imposed by poor alignment. It was recommended that improved assembly procedures be used and that, if another failure occurred, a steel of higher fatigue strength be used.

Abstract
A white cast iron water-line plug in a fire sprinkler systems split during leak repair. Examination revealed no material flaws, fatigue, or excessive corrosion. The plug head exhibited signs of excessive loads used in attempts to force the plug farther into the pipe. The evidence obtained indicated that the failure resulted from human error.

Abstract
A crane long-travel worm drive shaft was found to be chipped during unpacking after delivery. Chemical analysis showed that the steel (EN36A with a case depth of 1 mm, or 0.04 inch did not meet specifications. Magnetic particle inspection revealed a crack on the side of the shaft opposite the chip. Metallographic examination indicated that the case depth was approximately 2 mm (0.08 in.) and that a repair weld of an earlier chip had been made in the cracked area. The chipping was attributed to excessive case depth and rough handling. It was recommended that the shaft be returned to the manufacturer and a replacement requested.

Abstract
Two hot water reheat coil valves from a heating/ventilating/air-conditioning system failed in service. The values, a 353 copper alloy 19 mm (3/4 in.) valve and a 360 copper alloy 13 mm (1/2 in.) valve, had been failing at an increasing rate. The failures were confined to the stems and seats. Visual examination revealed severe localized metal loss in the form of deep grooves with smooth and wavy surfaces. Metallographic analysis of the grooved areas revealed uniform metal loss. No evidence of intergranular or selective attack indicating erosion-corrosion was observed, Recommendations included use of a higher-copper brass, cupronickel, or Monel for the valve seats and stems and operation of the valves in either the fully opened or closed position.

Abstract
The rear outside wheel of an over-the-road 18-wheel tractor-trailer failed at its bolt holes, permitting the tire and wheel to separate from the hub. Failure analysis was conducted using photographic examination of actual fracture surfaces and SEM examination of fracture replicas. The examinations indicated that fatigue cracks had originated at the rim backside and propagated to the rim midsection. Catastrophic failure occurred at a final overland. Fatigue fracture of the wheel rim was attributed to cyclic loading created by improper wheel mounting of a spare tire.

Abstract
Three radiant heating element tubes from an aluminum holding furnace failed after a few months of service. One side of each of the tubes had disintegrated, leaving large holes and thinned cross sections. Microstructural analysis showed that the surface of the tube had been oxidized along the grain boundaries and had extensive precipitation inside the grains. Chemical analysis indicated that the steel used for the tubes was AISI type 316 stainless steel Specifications for the tubes had called for AISI type 310S to be used. It was recommended that other tubes made from the same batch of steel sheet be checked.

Abstract
The failure during use of a seat on a heavy-duty swing set at an elementary school was investigated. The seat contained a perforated reinforcing sheet metal (galvanized type 430 stainless steel) insert covered by an elastomeric material. Specimens of the reinforcing sheet from the failed seat were examined using SEM fractography, tensile and ductility tests, and spectrographic chemical analysis. The test results showed that the steel used did not meet the manufacturer's specifications for ductility (elongation). In addition, the small-diameter punched holes caused a stress concentration factor that aggravated the brittleness of the steel.

Abstract
Although field corrosion tests had indicated that type 316L stainless steel would be a suitable material for neutralization tanks, the vessels suffered severe corrosion when placed in service. Welded coupons of type 316L had been tested along with similar Alloy 20Cb® (UNS NO8020) specimens in a lead-lined tank equipped with copper coils that had served in this function prior to construction of the new tanks. Both materials exhibited virtually no corrosion and no preferential weld attack. Type 316L was selected for the project. The subsequent corrosion was the result of the borderline passivity of type 316L in hot dilute sulfuric acid (about 0.1%). Inaccuracy of the testing was attributed to the presence of cupric ions in the lead-lined vessel fluids, which had been released by corrosion of the copper coils. Careful control of both temperature and pH was recommended to reduce the corrosion to an acceptable limit.

Abstract
Replacement sprockets installed on chain drive shafts for winding fibers exhibited excessive wear. Metallographic and chemical analyses conducted on the original and replacement sprockets showed that the material of the replacement sprocket was 1020 low-carbon steel, whereas the original (and specified) material was medium-carbon 1045 steel. The low-carbon steel also had lower hardness because of a lower pearlite fraction in the microstructure. It was recommended that replacement sprockets be made of normalized 1045 steel. It was further suggested that wear resistance could be improved by through hardening or induction surface hardening of the teeth.
